In order to clarify carbon disulfide retinopathy in Korea, we anlazed fluorescein angiograms of 42 patients diagnosed as CS₂ poisoning. Retinal abnormality characterized by microaneurysm was found in 66.7% of patients diagnosed as CS₂poisoning. Microaneurysms were located at posterior pole of retina and all microaneurysms were saccular microaneurysms. There was a significant correlation between prevalence of microaneurysm and CS₂ concentration(P<0.05) but, no significant correlation of prevalence of microaneurysm with CS₂ exposure duration and age was found(P>0.05).
Purpose: To report a case of streptoococcus gordonii endophthalmitis after pneumaic retinopexy in a patient with rhegmatogenous retinal detachment. Case summary: A 40-year-old man presented with a right eye macula-on retinal detachment extending from 9 to 1 o’clock with one-clock-hour hole at 11 o’clock. After sterilizing with a Betadine solution, 0.6 cc 100% SF6 gas was injected into the vitreous through the pars plana at 11 o’clock. Two days after the injection, eyeball pain, cell and flare, and pupillary membrane developed. Under the diagnosis of endophthalmitis, vitreous tap and intravitreous vancomycin (1.0 mg/0.1 cc) and ceftazime (2.0 mg/0.1 cc) were administered. However the symptoms and signs worsened, so vitrectomy was performed, and intravitrous injections of silicone, vancomycin and ceftazime were administered. Streptococcus gordonii was identified from the excised vitreous. Visual acuity was light perception due to severe retinal necrosis. Conclusions: In cases of endophthalmitis after pneumatic retinopexy even with meticulous sterilization, a prompt operation is necessary to prevent extensive retinal damage and visual loss due to the possibility of pathogen growth other than conjunctival normal flora. Purpose: To report a case of bilateral endogenous endophthalmitis in a Streptococcus pneumoniae meningitis patient. Case summary: A 45-year-old woman with bacterial meningitis was referred to the ophthalmologic clinic with acute visual impairment in both eyes. The patient`s visual acuities were hand motion in both eyes. Ophthalmoscopy revealed inflammation in the anterior chamber and vitreous opacities in both eyes. Streptococcus pneumoniae was isolated in the cerebrospinal fluid sample, but not in vitreal, blood samples. Vision improved to 70/100 in the right eye and 2/100 in the left eye after 8 days of treatment. Conclusions: Endogenous endophthalmitis constitutes a rare complication of Streptococcus pneumoniae meningitis, and a prompt diagnosis and administration of empirical intravitreal antibiotics can lead to a more favorable visual prognosis.
`Purpose: To assess macular function by multifocal electroretinography after reattachment of macula-off retinal detachment and to evaluate relationship between duration of macular detachment (DMD) and functional recovery. Methods: Nineteen patients (19 eyes) with macula-off rhegmatogenous retinal detachment underwent pneumatic retinopexy or scleral buckling. Retinal reattachment was obtained successfully in all patients. Duration of follow-up of all patients is at least more than 12 months. Eyes were divided into 2 groups, corresponding to the DMD : Group A (within 7 days, n=9 eyes), Group B (>7 days, n=10 eyes). The amplitudes and implicit times of N1-wave and P1-wave of mfERGs in Area 1 (Ring 1), Area 2 (Ring 1+2) were compared between Group A and B and between Group A and control group. Results: The P1 amplitudes in Area 1 and 2 were significantly different between Group A and B (P<0.05). N1 amplitudes in Area 1 were significantly different between Group A and B (P<0.05), but no significant difference of P1 amplitudes in Area 2 between Group A and B (P>0.05). There were no significant difference of all data between Group A and control in Area 1 and 2 (P>0.05) The implicit times of N1, P1 were not significantly different between Group A and B. Conclusions: The DMD plays a key role in recovery of visual function after reattachment. The mfERG is a useful tool to evaluate the recovery of macular function after reattachment of macula-off retinal detachments.`
