        Percutaneous vertebroplasty is a relatively recent procedure in the treatment of osteoporotic vertebral compression fractures. Bone cement embolism is a severe and potentially life-threatening complication of percutaneous vertebroplasty with cement. The risk for cement pulmonary embolism ranges between 3.5 and 23% for osteoporotic fractures. Bone cement leakage may be usually more frequent with vertebroplasty than with kyphoplasty. We experienced a 69-year-old female patient of an asymptomatic pulmonary embolus (length 8 cm) after kyphoplasty for an osteoporotic vertebral compression fracture.

        Primary Aneurysmal Bone Cyst in the Iliac Bone: A Case Report

        Symptomatic aneurysmal bone cysts with expansible lesions in the pelvis are rare in children. The managementof an aggressive vascular lesion in a female child is challenging. The standard treatment for aneurysmal bonecysts is accompanied by a high risk of local recurrence. A 12-year-old female presented with a history of pelvicpain for 5 months. Plain radiographs and magnetic resonance imaging showed a very large expansile lytic lesionarising from the right iliac bone. Intralesional curettage, electric cauterization, chemical sclerotherapy andallogeneic bone graft were performed through the window of the iliac crest. At a follow-up consultation 3.5years post-surgery, the child had painless full-range movement in the hip joint with no recurrence. Althoughmany treatment options are described, our patient was treated successfully using curettage and allogeneic bonegraft without recurrence.

        Pipkin 분류 I형 대퇴골두 전위 골절에 대한 관절 경하 골절편 절제술: 증례 보고

        고관절 탈구와 동반되는 대퇴골두의 골절은 Pipkin의 분류에 따라 다양한 치료방법이 시행되고 있다. 이 중 I형 골절은 전위가 적은 경우에는 보존적 치료를 시행하지만 전위가 있거나 동심원적 정복에 방해를 주는 경우에는 수술적 치료로 관혈적으로 골절편을 제거하거나 정복 후 고정하는 방법이 일반적인 치료 방법이다. 이 증례에서는 Pipkin 분류 I형의 전위된 골절이 발생한 62세 남자 환자에게 관절경적 절제술을 시행하였고, 수술 전 영상학적 검사에서 확인이 어려웠던 작은 골편을 추가로 제거하여 만족할 만한 결과를 얻어 이를 문헌 고찰과 함께 보고하고자 한다. There has been a variety of options for treatment of femoral head fracture with hip dislocation according to the Pipkin classification. Pipkin type I fractures with minimal displacement have been treated conservatively. However, in cases where the fracture was displaced or reduced incongruently, it has been treated by open fragment excision or fixation after reduction. In our case, the patient was a 62-year-old man who sustained a displaced fracture of Pipkin type I. We achieved a satisfactory outcome by arthroscopic excision of a displaced bony fragment and small bony fragments that could not be confirmed by pre-operative imaging study. Therefore, we report on the case with a review of the literature.

        불안정성 대퇴 전자간 골절에서 전내측 피질골 중첩에 의한 정복

        목적: 불안정성 대퇴 전자간부 골절 치료에서 골절의 원위부를 외측 전위시켜 전내측 피질골을 중첩 시키는 방법으로 정복 후 치료 결과에 대해서 알아보고자 하였다. 대상 및 방법: 2011년 8월부터 2012년 8월까지 불안정성 대퇴 전자간부 골절로 진단 받고 감마-3 정을 이용하여 치료한 후 최소 1년 이상 추시가 가능하였던 20예를 대상으로 하였다. 방사선학적 평가는 수술 후 및 최종 추시의 방사선 검사를 통하여 NSA, TAD, Cleveland index, 지연 나사의 활강 정도, 골유합 시기를 측정하였다. 골절 정복의 정도는 Baumgaertner 기준 변형된 방법을 따라 양호(good), 적당(acceptable), 불량(poor)로 분류하였다. 결과: 대퇴 경간각은 수술 후 평균 140。였으며, 최종 추시상 평균 135。로 감소되었다. TAD는 평균 11.3 mm였으며, 지연 나사의 위치는 정중앙에 12예, 중앙 하부에 8예가 위치하였다. 최종 추시시 지연 나사 활강 거리는 평균 5.5 mm였다. 평균 골유합 기간은 3.7개월이었으며, 수술 직후 골절의 정복 상태는 양호 15예, 적당 5예로 전예에서 만족할만한 정복을 얻었다. 최종 추시상 정복 소실 및 지연 나사의 골두 관통(cut out) 등의 고정실패 및 다른 합병증은 발생하지 않았다. 결론: 불안정성 대퇴 전자간부 골절 치료에서 골절의 원위부를 외측 전위시키고, 전내측 피질골 근위부를 중첩(overlap) 시키고, 전후면상 외반 정복, 측면상 전방 전위 각형성 정복은 정복 소실 및 지연 나사의 골두 관통 등의 고정실패를 줄일 수 있는 방법이라고 사료된다. Purpose: The purpose of this article is to evaluate the result of treatment of unstable intertrochanteric fractures by lateralization of distal fragment and antero-medial cortex contact of the distal shaft piece. Materials and Methods: We conducted an analysis of 20 cases of unstable intertrochanteric fractures treated using a gamma-3 nail from August 2011 to August 2012 and followed up for more than one year. Using postoperative and last follow-up radiographs, we measured NSA, TAD, Cleveland index, sliding length of the lag screw, and union time. Adequacy of reduction was assessed by a modification in the criteria of Baumgaertner and classified as good, acceptable, or poor. Results: The mean NSA was 140。postoperative and 135。at last follow-up. The mean TAD was 11.3 mm. The position of the lag screw was in center-center in 12 cases, center-inferior in eight cases. The mean distance of lag screw sliding was 5.5 mm at last follow-up. The mean union time was 3.7 months. The state of reduction postoperatively was good in 15 cases, and acceptable in five cases. There was no failure of reduction, lag screw cut-out, or other complications at last follow-up. Conclusion: The reduction method for lateralization of distal fragment and antero-medial cortex contact of the distal shaft piece in an unstable intertrochanteric fracture is very useful for prevention of collapse of the fracture site, lag screw cut-out, and mechanical failure.

        The aim of this project was to use 3D scanning data collected at incident scenes and various evidence to 1) develop surveying methods based on 3D data consisting of overall and detailed scene evidence, captured by long-range and micros-canner,which can be shared by personnel working in different fields such as forensic medicine,video analysis, physical analysis, traffic engineering, and fire investigation; 2)create digital storage for human skeletons and set the foundation for virtual anthropology;and 3) improve the credibility of 3D evidence by virtual remodeling and simulation of incident scenes and evidence to provide a basis for advanced and high-tech scientific investigation. Two complete skeletons of male and female were scanned using 3D micro-scan-ner. Each bone was successfully reproduced and assembled in virtual space. In addition, recreating evidence scheduled for invasive examination by creating RP (rapid prototype) was possible. These outcomes could play an important role in setting up the new field of virtual anthropology. Case-specific surveying methods were developed through analysis of 3D scanning data collected by long-range surface scanners at the scenes of vehicular accidents, falls, shootings, and violent crimes. A technique and recording method was also developed for detecting forged seals by micro-scanning the pressure exerted on the seal. Appraisal methods developed in this project could be utilized to secure 3D data of human skeletal remains and incident scenes, create a standard for application, and increase objectivity, reproducibility, and accuracy of scanning methods. We plan to develop case-specific 3D data analysis techniques to improve the credibility of analysis at the NFS and to establish a 3D data collection and analysis team.         공압 피로 시험기 구성을 위한 공압실린더의 고속/고정밀 제어기술

        A hydraulic cylinder is typically used in fatigue testing machines for generating reciprocal motion. Hydraulic cylinders adversely affect the environment because they consume high amounts of oil for maintenance. Pneumatic cylinders can serve as potential alternatives to overcome this drawback. However, it is difficult to control a pneumatic cylinder with high accuracy due to the compressibility of air. This study presents a new control method for high-speed and high-precision control of a pneumatic cylinder for a servo-pneumatic fatigue testing machine. Displacement and load controllers with proportional-proportional-integral feedback and feedforward algorithms are proposed. Furthermore, an auto-tuning algorithm of a feedforward controller gain is proposed to improve control performance. A servo-pneumatic fatigue testing machine is constructed, and the control performance is evaluated via displacement and load control experiments at the various conditions.

      • 정중 동맥에 의한 수근관 증후군에서 초음파 검사의 유용성: 증례 보고

        정중 동맥은 전완 및 완관절에서 정상적으로 태생 전에 사라져 성인에서 수근관 내에 존재하는 정중 동맥은 드물다. 저자들은 수근관 증후군 환자에 시행한 초음파 검사에서 수근관 내에 정중 동맥이 관찰 되어 보고한다. Median artery of the forearm and wrist is not very frequently observed because it normally involutes before birth. Persistent median artery of the carpal tunnel is not frequently observed in adult life. We report a case of persistent median artery in the carpal tunnel that was examed by ultrasonography.
