류마티스관절염환자에서 동맥경화증의 독립적인 예측인자 규명: KARRA 코호트 연구 기반

Objective. This study sought to investigate independent predictive factors for subclinical atherosclerosis in Korean patients with rheumatoid arthritis (RA). Methods. We used high-resolution B-mode ultrasonography to measure the carotid artery intima-media thickness (IMT) and carotid plaque in 367 patients with RA. Detailed information on the demographic characteristics, cardiovascular (CV) risk factors, and RA disease characteristics were collected on all subjects. The relationship of the carotid artery IMT and carotid plaque to relevant clinical and laboratory variables were examined. Results. Old age and male sex had the most significant association with increased IMT and presence of plaque than other factors. Erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and mKHAQ (Korean version of modified health assessment questionnaire) were significantly associated with both increased IMT and presence of plaque after univariate analysis adjusting for age and sex. A multivariable logistic regression analysis revealed that ESR and TJC68 were independent factors associated with the presence of plaque (p<0.001 and p=0.019, respectively). There was a significant linear correlation between the number of plaques and ESR (p<0.001 and R2=0.07). Conclusion. Our results indicated that markers of systemic inflammation contributed significantly to subclinical atherosclerosis in patients with RA. We emphasize the need for aggressive control of RA disease activity in patients who persistently demonstrate highly elevated ESR levels.
평막을 이용한 정수처리용 정밀여과 막분리 시스템에서 최적 운전인자 도출에 관한 연구(1)

The permeability of membrane filtration system depends on characteristics of the raw water and operating conditions. The purpose of this study was to determine the optimum operation parameters for water treatment using plate type MF membrane(pore size 0.22㎛). The pilot microfiltraion(MF) membrane separation system had been operated in a dead-end filtration mode and applied to actual Pal-dang raw water. The experimental results are shown as follows; (1)permeate flux 30LMH was determined the optimum operation condition, (2)the chemical cleaning efficiency was only found to be 100% using H₂SO₄+NaOCl, (3)the removal efficiencies of SS and turbidity were found to be almost 100%. However, The removal efficiencies of UV_(260) and DOC were 30% and 13%, respectively. Also, in order to improve the permeate flux and the removal efficiency of dissolved organic matter, granular activated carbon system should be added to the MF membrane System.
대피인원수에 따른 지하철 승강장 유형별 피난성능 비교 및 분석

현재 국내 지하철 역사는 지하상가와 연계로 인한 복합 문화공간으로 그 용도의 범위가 넓어짐에 따라 지하철 이용객 수 및 체류시간이 증가하였으며 이와 함께 안전사고에 대한 우려도 증가하게 되었다. 이에 국내에서는 지하철 승강장 유형별 피난안전에 대한 연구들이 진행되고 있으며 대부분의 연구에서 섬식 승강장이 상대식 승강장 보다 피난에 소요되는 시간이 적은 것으로 나타났다. 기존의 연구들에서는 고정된 피난인원수 대한 피난효율에 대한 결과만이 보고되었으며 현재 지하 역사를 이용하는 사람들의 수는 유동적으로 바뀌기 때문에 인원수변화에 따른 피난효율에 대한 연구가 필요하다고 사료된다. 본 연구에서는 피난시뮬레이션을 활용하여 대피인원수가 승강장 유형별로 피난시간에 미치는 영향을 분석하고 승강장 유형별 피난시간에 대한 증가폭을 비교하였다. 그 결과 Case 1의 경우 상대식 승강장이 섬식 승강장 보다 피난시간이 더 빠른 것으로 나타났으며, Case 3의 경우 섬식 승강장이 상대식 승강장보다 피난시간이 더 빠른 것으로 나타났다. 대피인원수에 따른 피난시간의 증가폭은 섬식 승강장 보다 상대식 승강장에서 더 크게 증가하였으며 많은 인원이 승강장을 이용할 경우 섬식 승강장이 피난에 있어 더 유리하다 판단된다.

2007년 12월 7일 발생한 허베이스피리트호 원유유출 사고로 약 12,547 kL의 기름이 해상으로 유출되어 서해안의 해안선이 광범위하게 기름으로 뒤덮이는 결과가 초래되었다. 사고발생 며칠 후부터 타르볼이 서남해안에서 관측되었으며, 이에 대한 방제를 위해 해양경찰(KCG)은 해상에서는 뜰채 및 그물끌기 방법을 이용하였고, 해안에서는 그물치기, 주워내기, 쓸어 담기 등 3가지 방제방법을 이용하였다. 그물막 치기 방법은 해안이 타르볼로 오염되는 것을 예방하는데 효과적이었으며, 사고당시의 계절이 겨울이었던 점은 타르볼이 쉽게 굳어 자원봉사자나 주민들이 주워 내거나 쓸어 담는데 큰 이점이 될 수 있었다. Approximately 12,547 kL of oil from the tanker 'Hebei Spirit' released into the western sea of Korea, which subsequently reached and covered extensive areas of the western coastlines of Korea. In the following days great numbers of tar balls hit the southwestern coast. Three different cleanup methods were used to mediate the southwestern coastline tar ball pollution by Korea Coast Guard (KCG) net setting, manual pick up, and sweeping them up. Net setting was useful in protecting coastlines from being hit by tar balls. The cold weather in winter conditions helped the tar ball response efforts because it caused them to harden, allowing them to be swept up from beaches and to be gathered up by hand.
단열 갱폼과 단열 유로폼을 이용한 한중콘크리트 타설에 관한 연구

In this paper, a comparative cold weather concrete member test was conducted with gang form and euro form, which are generally used as forms for construction in Korea. Three members were fabricated for the comparative test: the member with the general gang form and euro form, the member with the existing insulation gang form on the outside and the general form on the inside, and the member with the existing insulation gang form on the outside and the newly developed insulation euro form on the inside. In this test, ready-mixed concrete with a nominal strength of 24 MPa that met a certain standard was cast in the cold weather concrete condition at a temperature of 0℃ (5℃ ~?5℃), which is lower than the cold weather concrete management standard temperature in Korea 4℃ mean daily temperature. The standard water curing compressive strength, core compressive strength and temperature history were measured and comparatively examined for the general gang form-general euro form and insulation gang form-general euro form members that were cured at a certain inside temperature, and for the insulation gang form-insulation euro form member that was cured without additional temperature control.
피난안전성 평가에서 연기확산이 대피자의 피난경로 선정에 미치는 영향

The purpose of performance-based design is to present specific performance to achieve the purpose of fire safety, and a generally used method to perform this is evacuation safety evaluation. However, when evaluating the current evacuation safety, only the simulation results measured at a specific point are compared to determine stability. This raises the question of trusting the evaluation results. Thus, in this study, the evacuation safety evaluation results in situations where smoke is not considered and the evacuation safety results that appear when they are reflected in the selection of evacuation routes were compared and analyzed. Accordingly, in the evaluation method that does not consider the impact of smoke, the existing evacuation safety evaluation is judged to be an unreliable evaluation method as evacuees continue to move in the direction of smoke diffusion. As for the evaluation method considering the effect of smoke, the evacuee tends to deviate from the direction of smoke diffusion, so such an evaluation method is considered more reasonable.
류마트스 : 항인지질 증후군을 동반한 전신홍반루푸스에서 진단된 May-Thurner 증후군

항인지질 증후군이 동반된 전신홍반루푸스 환자에서 May-Thurner 증후군이 진단된 예는 아직 세계적으로 보고가 없다. 본 증례와 같이 전신홍반루푸스를 가진 젊은 환자에서 좌측 하지에 심부정맥 혈전증이 발생한 경우 항인지질 증후군을 감별하여야 하며 동시에 May-Thurner 증후군도 감별진단에 포함시켜야 한다. 그리고 진단 이후의 적극적인 혈관중 재시술과 지속적인 항응고제 치료를 통해 재발을 방지하기 위한 전략이 필요할 것이다. May-Thurner syndrome, which refers to an iliofemoral venous thrombosis caused by chronic compression of the left common iliac vein by the right common iliac artery, usually manifests as left lower extremity pain and swelling. The syndrome is particularly evident in patients with health conditions including obesity, smoking, pregnancy, surgery, or prolonged immobility. Antiphospholipid syndrome consists of arterial or venous thrombosis and the association of lupus anticoagulant or anticardiolipin antibodies. Most common clinical manifestations include pregnancy loss and deep vein thrombosis. We experienced a rare case of May-Thurner syndrome concurrent with antiphospholipid syndrome, secondary to systemic lupus erythematosus. The patient was treated successfully by catheter-directed percutaneous thrombectomy, venous thrombolysis, and stent insertion, followed by oral anticoagulant therapy.
