        Current Opinion on Endovascular Therapy for Emergent Large Vessel Occlusion Due to Underlying Intracranial Atherosclerotic Stenosis

        For recanalization of emergent large vessel occlusions (ELVOs), endovascular therapy (EVT) using newer devices, such as a stent retriever and large-bore catheter, has shown better patient outcomes compared with intravenous recombinant tissue plasminogen activator only. Intracranial atherosclerotic stenosis (ICAS) is a major cause of acute ischemic stroke, the incidence of which is rising worldwide. Thus, it is not rare to encounter underlying ICAS during EVT procedures, particularly in Asian countries. ELVO due to underlying ICAS is often related to EVT procedure failure or complications, which can lead to poor functional recovery. However, information regarding EVT for this type of stroke is lacking because large clinical trials have been largely based on Western populations. In this review, we discuss the unique pathologic basis of ELVO with underlying ICAS, which may complicate EVT procedures. Moreover, we review EVT data for patients with ELVO due to underlying ICAS and suggest an optimal endovascular recanalization strategy based on the existing literature. Finally, we present future perspectives on this subject.

        Comparison of The Muscle Activity in The Normal and Forward Head Postures Based on Pressure Level during Cranio-Cervical Flexion Exercises

        Purpose: This paper proposes proper and effective neck exercises by comparing the deep and superficial cervical flexor muscle activities and thickness according to the pressure level during cranio-cervical flexion exercises between a normal posture group and forward head posture group. Methods: A total of 20 subjects (8 males and 12 females) without neck pain and disabilities were selected. The subjects’ craniovertebral angles were measured; they were divided into a normal posture and a forward head posture group. During cranio-cervical flexion exercises, the thickness of the deep cervical flexor neck muscle and the activity of the surface neck muscles were measured using ultrasound and EMG. Results: The results showed that the thickening of the deep cervical flexor was increased significantly to 28 and 30mmHg in the forward head posture group. The sternocleidomastoid muscle activity increased significantly to 24, 26, 28, and 30mmHg in the forward head posture group. The anterior scalene muscle activity increased significantly to 26, 28, and 30mmHg in the forward head posture group. A significant difference of 26, 28, and 30mmHg in the sternocleidomastoid and anterior scalene muscles was observed between two groups. Conclusion: To prevent a forward head posture and maintain proper cervical curve alignment, the use of the superficial cervical flexor muscles must be minimized. In addition, to perform a cranio-cervical flexion exercises to effectively activate the deep cervical flexor muscles, 28 and 30mmHg for normal posture adults and 28mmHg for adults with forward head postures are recommended.

      • 운동성능을 고려한 부유식 해상 풍력 발전기 설계에 관한 연구

        자원고갈과 지구 환경문제에 관한 관심으로 풍력 에너지는 상용화에 주목받는 신재생에너지로 평가된다. 이에 대응하여 풍력 발전기의 시장규모는 급격하게 성장할 것으로 예상된다. 최근 풍력 발전기는 점점 대형화 되고 있으며 장소의 제약과 소음 문제 때문에 해상으로 옮겨지고 있는 추세다. 본 연구에서는 서해안 칠발도의 파랑과 풍속을 분석하여 5MW급 TLP형식 해상 풍력 발전기의 하부구조물에 대한 설계 대안을 설정하였다. 그리고 설정한 대안에 대해 운동성능 해석을 하여 파레토 최적화를 통해 최적의 설계 대안을 선택했다.

        Prediction of Cochlear Implant Outcomes in Patients With Prelingual Deafness

        Objectives. To evaluate the factors that limit post-cochlear implantation (CI) speech perception in prelingually deaf children. Methods. Patients with CI were divided into two groups according to Category of Auditory Performance (CAP) scores 3 years post-CI: the poor performance group (poor performance group, CAP scores≤4, n=41) and the good performance group (good performance group, CAP scores≥5, n=85). The distribution and contribution of the potential limiting factors related to post-CI speech perception was compared. Results. Perinatal problems, inner ear anomalies, narrow bony cochlear nerve canal (BCNC), and intraoperative problems was significantly higher in the poor performance group than the good performance group (P=0.010, P=0.003, P=0.001, and P=0.045, respectively). The mean number of limiting factors was significantly higher in the poor performance group (1.98±1.04) than the good performance group (1.25±1.11, P=0.001). The odds ratios for perinatal problems and narrow bony cochlear nerve canal in the poor performance group in comparison with the good performance group were 4.878 (95% confidence interval, 0.067 to 0.625; P=0.005) and 4.785 (95% confidence interval, 0.045 to 0.972; P=0.046). Conclusion. This study highlights the comprehensive prediction of speech perception after CI and provides otologic surgeons with useful information for individualized preoperative counseling of CI candidates.

        Neurosurgical Approaches to and through the Frontal Sinus using Osteoplastic Frontal Sinusotomy

        Objective : The frontal sinus is frequently a troublesome anatomical obstacle to gain access to the medial anterior cranial base. Surgical approaches to and through the frontal sinus using osteoplastic frontal sinusotomy provide significant advantages to the treatment of lesions of the medial anterior cranial base in addition to the frontal sinus itself. However, appropriate management is necessary to avoid postoperative complications such as cerebrospinal fluid leakage, infection, mucocele formation, and deformity of the forehead. Methods : The advantages and shortcomings of the approach along with the surgical technique are reported based on our clinical experience with pertinent literature review. The approach using the osteoplastic frontal sinusotomy was applied to two cases of osteoma in the frontal sinus, seven cases of craniofacial tumors, a case of chordoma in the sphenoid and clivus, and two cases of intradural lesions in the anterior cranial fossa. The frontal sinus was managed in such a way as to prevent the postoperative complications. Results : All patients underwent gross total resection of the tumors. With a mean follow-up of 26 months, there were no postoperative complications related to frontal sinus violation. Conclusion : The neurosurgical approaches via the frontal sinus using osteoplastic frontal sinusotomy are versatile for various lesions of the anterior cranial base in patients with large frontal sinuses. In situations that the frontal sinus have to be violated to approach medial anterior cranial base, the osteoplastic frontal sinusotomy provides such advantages as optimal frontal sinus control to prevent postoperative complications; increases viewing angle with superior trajectory from nasofrontal suture; lower incidence of pnemocephalus due to minimal dural exposure; and excellent cosmesis without frontal burr holes.

        나노재료 기반 페인트형 발열코팅 기술의 동절기 철도현장 적용성 평가

        The snow ice that is generated at around entrances of railway vehicle in winter season can be caused malfunction of entrances, which can occur difficulties of railway operation. Even though fundamental methods are needed to overcome the problems, conventional manual methods depended on manpower are still used. So, in order to fundamentally overcome the problems generated by snow ice, paint-type surface exothermic coating technology which has advantages such as self-heating and application to large-scale area structures has been developed in this study. However, for effective application of the technology to railway site, it needs to be evaluated on effects of temperature environment in winter season, external impact damages, waterlogged condition, de-icing and snow removal performance and so on considering characteristics of practical environments on site. Thus, exothermic performance of specimen based on exothermic coating has been investigated in external impact as well as waterlogged conditions. From the test results, defects on surface of specimen from external impact has limited effect on exothermic performance of specimen, and it is not affected even in waterlogged condition. Additionally, from the evaluation of de-icing and snow removal performance considering environment of practical sites, it has verified that the developed exothermic coating technology can be effectively applied for removing the snow and ice. 동절기 철도 차량 출입문 주변에 생성되는 설빙은 출입문의 오작동을 유발하여 철도 운용의 어려움으로작용하고 있다. 하지만, 이에 대한 근본적인 해결방안이 없어 현재 인력 의존적인 방법으로 문제에대처하고 있는 실정이다. 따라서, 본 연구에서는 설빙에 의한 문제를 근본적으로 해결하기 위해 자체발열이 가능하고 대면적으로 적용할 수 있는 페인트형 면상발열코팅 기술을 개발하였다. 하지만, 본기술의 철도 현장 적용을 위해서는 실 적용 환경 특성을 고려하여 동절기 온도 환경, 외부 충격에 의한손상, 침수 환경, 제빙 및 제설 성능 등에 대한 영향 평가가 필요하다. 이를 위해, 본 연구에서는 발열코팅이적용된 시편을 이용하여 동절기 온도 환경, 외부 충격, 침수 환경에서 시편의 발열 특성을 평가하였다. 실험 결과, 외부 충격에 의해 생성된 발열면 손상이 전체적인 발열 성능에는 제한적인 영향을 주었으며침수 환경에서도 발열 코팅의 발열 성능은 영향을 받지 않음을 확인하였다. 마지막으로, 실 적용 환경을고려한 제설 및 제빙 성능 평가를 통해 본 연구에서 개발한 발열코팅 기술이 눈과 얼음 제거에 매우효율적으로 적용 가능함을 확인하였다.
