Te1misartan is an angiotensin II receptor antagonist and chlorthalidone is a thiazide-like diuret-ics. In this study, we report serious adverse events (SAEs) during clinical trial for pharmacokinetic interaction between telmisartan and chlorthalidone in healthy Korean subjects. Two separate, ran-domized, multiple-dose, two-period, one-sequence studies were conducted at Kyungpook National University Hospital. In part A, 43 volunteers received telmisartan for 7 days, and then chlorthali-done for 14 days (days 8-21). Telrnisartan was co-administered during day 15-21 to evaluate the effects of chlorthalidone on the pharmacokinetics of telmisartan at steady state. A healthy 36-year-old male in part A was referred to the emergency room due to severe nausea and vomiting developed about 3 h after administration of chlorthalidone on day 9. Hypokalemia and QT prolongation were observed during his initial laboratory examination and electrocardiogram (ECG) monitoring in the emergency wilt. Nausea and vomiting improved after conservative management with hos-pitalization for 9 days. We consider that the episodes of excessive nausea and vomiting resulted in hypokalemic state which was potentiated by chlorthalidone. And the hypokalemic state caused the lengthening of the QT interval on ECG.

Background: We investigated the long-term effect of AST-120, which has been proposed as a therapeutic option against renal disease progression, in patients with advanced chronic kidney disease (CKD). Methods: We performed post-hoc analysis with a per-protocol group of the K-STAR study (Kremezin study against renal disease progression in Korea) that randomized participants into an AST-120 and a control arm. Patients in the AST-120 arm were given 6 g of AST-120 in three divided doses, and those in both arms received standard conventional treatment. Results: The two arms did not differ significantly in the occurrence of composite primary outcomes (log-rank P = 0.41). For AST-120 patients with higher compliance, there were fewer composite primary outcomes: intermediate tertile hazard ratio (HR) 0.62, 95% confidence interval (CI) 0.38 to 1.01, P = 0.05; highest tertile HR 0.436, 95% CI 0.25 to 0.76, P = 0.003. The estimated glomerular filtration rate level was more stable in the AST-120 arm, especially in diabetic patients. At one year, the AST-120-induced decrease in the serum indoxyl sulfate concentration inversely correlated with the occurrence of composite primary outcomes: second tertile HR 1.59, 95% CI 0.82 to 3.07, P = 0.17; third tertile HR 2.11, 95% CI 1.07 to 4.17, P = 0.031. Furthermore, AST-120 showed a protective effect against the major cardiovascular adverse events (HR 0.51, 95% CI 0.26 to 0.99, P = 0.046). Conclusion: Long-term use of AST-120 has potential for renal protection, especially in diabetic patients, as well as cardiovascular benefits. Reduction of the serum indoxyl sulfate level may be used to identify patients who would benefit from AST-120 administration.

<P>Intratumoral hypoxia has been correlated with distant metastatic potential. Two hypoxia inducible factors (HIFs), HIF-1관 and HIF-2관, are induced by hypoxia, and high expression of these proteins has been correlated to angiogenesis and distant metastasis. Thymosin 관4 (T관4) is frequently highly expressed in cancer, and this overexpression correlates with malignant progression. The objective of this study was to investigate the clinical correlation of HIF-관 with T관4 and the intracellular functional roles of T관4 on HIF-관 activation. We examined HIF-1관, HIF-2관 and T관4 expressions in clinical human breast carcinoma (n=70) by immunohistochemistry. We show that high expression of HIF-1관 and HIF-2관 strongly correlates with T관4 expression (P??.0001) and overexpression of T관4 correlates significantly with patients with lymph node metastasis (P<0.05) of human breast cancer. Additionally, we demonstrate that hypoxia up-regulates intracellular T관4 protein, which then affects HIF-관 activity, which is the key in regulating VEGF expression. We confirmed that hypoxia-induced intracellular T관4 and HIF-관 activities were reduced by interference of T관4 expression using T관4 shRNA lentivirus. Vascular epidermal growth factor (VEGF)-A, a well-recognized lymphangiogenic cytokine, was also down-regulated, but VEGF-C and VEGF-D expressions were not affected. These findings suggest that the overexpression of T관4 is strongly associated with HIF-1관 and HIF-2관 expression and is also clinicopathologically involved with lymph node metastatic potential of breast cancer through the modulation of HIF-관activation and induction of VEGF-A. Ultimately, these results highlight T관4 as a potentially therapeutic target in malignant cancers.</P>

Effect of Dietary Monascus Pigment on the Liver Damage Induced with CCl₄ in Rats

In the biological world, there are a number of ecological fights for survival between each organism such as plants, animals and microorganism. In such events, an organism can use its natural bioactive products as defence agent against other organisms. Furthermore, natural bioactive products can be utilized for medicine or functional food. Recently, we investigate the effect of Monascus pigment extracted from a fungus, Monascus anke, on the alcohol metabolism and blood lipid profile. In the present study, it is observed that Monascus pigment supplemented dietary may have a hepatoprotective effect on rat’s liver damage induced with CCl₄. By treatment with CCl₄ (3 times, I.P), liver damage was reduced more in the rats fed 2% Monascus pigment extract supplemented diet than those fed standard diet, based on the serum levels of alanine aminotransferase, microsomal glucose-6-phosphatse activity and hepaic malondialdehyde content. On the other hand, oxygen free radical generating enzymes, hepatic P-450 dependent aniline hydroxylase, xanthine oxidase, and oxygen free radical scavenging enzymes, hepatic glutathione S-transferase, catalase, superoxide dismutase activities were generally higher both in CCl₄ treated group and control fed 2% Monascus pigment extract supplemented diet than those fed standard diet. In conclusion, the rats fed 2% Monascus pigment extract supplemented diet showed more reduced liver damage than those fed standard diet, which may be due to the acceleration of oxygen free radical metabolism.

Background/Aims: Seasonal variation may influence the de-velopment and exacerbation of inflammatory bowel disease (IBD). However, most epidemiologic studies on this topic have been conducted in Western countries. The purpose of this study was to determine whether birth dates and symptom flares follow a seasonal pattern in Korean patients with IBD. Methods: Patients with a diagnosis of IBD established between January 2003 and December 2010 were investigated at six university hospitals in Korea. The expected births and flares, with a uniform distribution during the year and consid-ering differences in the number of days in the months of 1 year, were calculated. Results: A total of 411 patients with ul-cerative colitis (UC) and 316 patients with Crohn disease (CD) were included in the study. Birth during the winter period, and especially in January and February, was associated with an increased risk of IBD, especially in UC patients. The symp-tom flares of CD patients occurred most frequently in the spring, with a nadir in the autumn. However, no disease flare seasonality was noted for UC patients. Conclusions: Our data suggest that seasonally varying environmental factors during pregnancy and the postpartum period are associated with a susceptibility to IBD later in life and that exacerbations of CD are influenced by seasonal factors.
