Background/Aims: Respiratory viruses (RVs) are considered to be important respiratory pathogens in adult patients, and the multiplex reverse transcription- polymerase chain reaction (RT-PCR) test is used frequently in adult patients with respiratory infections. However, clinical data regarding utilization of the multiplex RT-PCR test for RVs are lacking. Methods: We investigated the utilization of the multiplex RT-PCR test for RVs at Chung-Ang University Hospital in Seoul, Korea, between January 2012 and April 2013. Results: During the study period, the multiplex RT-PCR test was performed for 291 adult patients. The test frequency was 4.9% of rapid influenza antigen detection tests and 0.8% of respiratory bacterial culture studies. A turnaround time of < 48 hours was observed in 25.9% of positive tests. Most of the tests were performed for admitted patients (97.9%) with a community-acquired infection (84.2%) during the flu season (82.5%). RVs were detected in 81 of 291 cases (27.8%). The RV positivity rates for community- and hospital-acquired infections did not differ (28.6% vs. 23.9%, p = 0.52). Of 166 patients with pneumonia, 44 (26.5%) had a viral infection. Among the patients with RV-associated pneumonia, an RV other than influenza was detected in 20 patients (45.4%). Conclusions: The multiplex RT-PCR test for RVs was infrequently performed at a tertiary care center, and the test results were often reported late. The test was most often performed for admitted adult patients with community-acquired infections during the flu season. The utilization of multiplex RT-PCR testing for RVs in current clinical practice should be improved.

DW-166HC (^(166)Holmium (^(166)Ho)-Chitosan complex) is a new radiopharmaceutic anticancer agent with a broad anti-tumorigenic spectrum, especially against human hepatic cancer. DW-166HC was evaluated for the appearance of micronucleus in polychromatic erythrocytes (PCEs) of mouse bone marrow cells after subcutaneous arid intravenous single administration. Bone marrow cells were prepared at 24 hr and 48 hr after DW-166HC-I (^(165)Ho-Chitosan complex : cold compound) administration and at 24 hr, 72 hr and 2 weeks after DW-166HC (^(166)Ho-Chitosan complex : hot compound) administration. The results showed there was no statistically significant increase of the numbers of PCEs with micronucleus in all DW-166HC-I administered groups compared with a negative control group but there was statistically significant increase of the numbers of PCEs with micronucleus at 24 hr arid 72 hr in all DW-166HC administered groups, which was recovered after 2 weeks from the drug administration. The results also showed the ratio of normochromatic erythrocytes (NCEs) to PCEs of all DW-166HC-I administered groups was not significantly different from that of a negative controi group but there was significant difference of this ratio at 24hr and 72 hr in all DW-166HC administered groups compared with that of negative group, which was also recovered after two weeks from the drug administration. These results suggested that DW-166HC-I may not cause any chromosomal damage but DW-166HC has in vivo mutagenic potential because of its radioactivity.

Background/Aims: CYP2C19 has an important role in the catabolism of several proton pump inhibitors. However, the relative contribution of CYP2C19-mediated metabolism varies among the different proton pump inhibitors. We investigate to determine the effect of CYP2C19 genotype status on intragastric pH during dosing with omeprazole. Methods: The subjects were 16 healthy volunteers. Their CYP2C19 genotype status was determined by a polymerase chain reaction-restriction fragment length polymorphism method. Twenty-four-hour monitoring of intragastric acidity were performed on the day without medication and day 4 after omeprazole administration. Results: A single dose of omeprazole significantly decreased 24 hour intragastric acidity (1.7±0.3 vs. 5.1±0.4). Subjects were divided into three groups on the basis of their CYP2C19 genotype status: homozygous extensive metabolizers (Ho-EMs, n=5), heterozygous extensive metabolizers (Ht-EMs, n=6), and poor metabolizers (PMs, n=5). Median 24 hour intragastric pH in the Ho-EM group was 3.1 compared with 5.5 in Ht-EM group and 5.9 in PM group(P<0.05). The median pH during omeprazole administration was influenced by CYP2C19 genotype. On the other hand, the Helicobacter pylori infection did not influence the median intragastric pH during omeprazole administration (p= NS). Conclusions: The effects of omeprazole on intragastric acidity is influenced by the CYP2C19 polymorphism. (Korean J Gastroenterol 2001;38:399-404)

Objectives : The purpose of this study is to explore the trends of Korean medical study on stress in the industrial worker. Methods : We received numbers of the studies about stress in the industrial worker from 6 Korean web databases, using key words such as ‘Industry AND Stress’, ‘Oriental medicine AND Stress’, ‘Worker AND Stress’, ‘Workplace AND Stress’. We received 12 research papers and they were analyzed and the journals, the authors, the contents of 3 classification of those research papers. Results : Among them, we had 6 reviews of Dept. of preventive medicine and Dept. of third medicine, college of Korean Medicine, Won-kwang University, and the rest of them were 6 reviews. We had 4 reviews of Korean Journal of Oriental Preventive Medical Society, 2 reviews of the Journal of Traditional Korean Medicine, 2 reviews of Journal of Oriental Neuropsychiatry and so on. According to the contents of reviews, we had 7 reviews of correlation of industrial worker and stress, 4 reviews of cause on the industrial worker’s stress and 1 case report. Conclusions : We reviewed the trends of Korean medical study on stress in the industrial worker. The stress in the industrial workers were related with the type of occupation, social psychology, drinking, smoking, the satisfaction of job, the harmony of organization.And we knew that the musculoskeletal pain was related to the industrial worker’s stress. In the future, abundant case studies, follow-up trials and RCTs should be done about the industrial worker’s stress.

This experiment was conducted to breed a very early maturing variety of Italian ryegrass (Lolium multiflorum Lam.) in Grassland and Forage Crops Division, National Institute of Animal Science, RDA, Cheonan 2015-2017. The new variety of Italian ryegrass, 'Green call 2ho' is a diploid variety with green in leaf color and has semi-erect growth habit in late autumn and erect growth habit in early spring, 'Green call 2ho' was in heading date as a early-maturing variety April 24. Also 'Green call 2ho' was narrower by 2 mm in flag leaf width, longer by 2.5 cm in flag leaf length and shorter by 3 cm in plant height than those of the control variety, 'Florida 80', respectively. 'Green call 2ho' was also thicker by 0.33 mm in stem thickness and strong in winter hardness. Dry matter (DM) yield (11,688 kg/ha) of 'Green call 2ho' was 7% higher than that of 'Florida 80'. Total digestible nutrient (TDN), crude protein (CP) and relative feed value (RFV) of 'Green call 2ho' were 61.3 %, 9.8 % and 98.2, respectively which are 2.6, 0.6, and 8.4 % higher, respectively than those of 'Florida 80', respectively. Acid detergent fiber (ADF) and neutral detergent fiber (NDF) of 'Green call 2ho' were 34.9 and 58.5 % which are 3.3 and 2.7 % lower than those of 'Florida 80', respectively.

Purpose: The present study is aimed at examining the drying characteristics of sea tangle through a combination of microwave and far-infrared drying experiment and finding the optimal drying conditions. Methods: Sea tangle was cleaned and cut into fine pieces (5mm) before they were subjected to combinational drying by microwave and far-infrared ray. The amount of specimen per drying is 2 kg. The finely cut pieces of sea tangle were preheated in a microwave dryer for three different lengths of time (10, 15, and 20 min). Subsequently, they were dried using a far-infrared dryer at tow temperatures (90℃ and 100℃) at an air velocity of 0.8 m/s until the final moisture content reduced to 10%. Results: Sea tangle dried under the condition of 20 min of preheating in the microwave dryer and drying at 100℃ by the far-infrared dryer. Of the drying models verified in this study, the logarithmic model showed high accuracy with the coefficient of determination R 2>0.7825 and RMSE <0.1095. The rehydration ratio of sea tangle was the highest (12.87 g water/g dry matter) under the condition of 15 min of preheating in the microwave dryer and drying at 100℃ by the far-infrared dryer. The energy consumption for the combination of microwave and far-infrared drying was the lowest (4.78 kJ/kg water) under the condition of 20 min of preheating in the microwave dryer and drying at 100℃ by the far-infrared dryer. Conclusions: Considering the drying time, discoloration during drying, rehydration ratio, and energy consumption for the drying of sea tangle, the optimal drying conditions for high-quality sea tangle are 15 min of preheating in a microwave dryer and drying at 100℃ by a far-infrared dryer.

A leukemoid reaction is usually associated with malignancies of the lung, stomach, and thyroid. In contrast, urothelial cell carcinoma is rarely associated with leukemoid reactions, with few cases reported over the past 30 years. Here, we describe a patient with urothelial cell carcinoma who exhibited a leukemoid reaction. The patient had an elevated white blood cell count and experienced a rapid and aggressive clinical course, terminating in death. For urothelial cell carcinoma patients exhibiting a leukemoid reaction, removal of the inciting tumor is the definitive treatment. However, considering the aggressive nature of these tumors, if the patient is unsuitable for radical surgical management, palliative chemotherapy should be considered.

Occupational lung diseases are caused by several toxic substances including heavy metals; however, the exact pathologic mechanisms remain unknown. In the workplace, dental technicians are often exposed to heavy metals such as cobalt, nickel, or beryllium and occasionally develop occupational lung diseases. We described a case of occupational lung disease in a patient who was employed as a dental technician for over a decade. A 31-year-old, non-smoking woman presented with productive cough and shortness of breath of several weeks duration. Chest computed tomography revealed a large number of scattered, bilateral small pulmonary nodules throughout the lung field, and multiple mediastinal lymph nodes enlargement. Percutaneous needle biopsy showed multifocal small granulomas with foreign body type giant cells suggestive of heavy metals inhalation. The patient's condition improved on simple avoidance strategy for several months. This case highlighted the importance of proper workplace safety.
