        Micro gadolinium oxide dispersed flexible composites developed for the shielding of thermal neutron/gamma rays

        In this study, a series of flexible neutron/gamma shielding composites are fabricated through the doping of Gd2O3 into the matrix of SEBS with (MGd2O3: MSEBS) % from 5% to 100%. Neutron transmittance test shows an exponential attenuation with the increase of areal density of Gd, in which the transmittance T ranges from 59.1440% to 35.3026%, with standard deviation less than 2.2743%, mass attenuation coefficient mm from 0.3194 cm2 /g to 0.4999 cm2 /g, and half value layer-HVL value from 2.4530 mm to 1.1313 mm. Shielding efficiency of the Gd2O3/SEBS composites is basically improved in comparison with that of B4C/SEBS. The transmittance T, mass/linear attenuation coefficient mm and m, HVL and effective atomic number Zeff for the shielding of g rays (39 keV, 59 keV and 122 keV) are measured and calculated with XCOM as well as MCX programs. Finally, plots of the three dimensional relationships between transmittance, doping amount and thickness are provided to the guidance for engineering shielding design. In summary, the Gd2O3/SEBS composite is proved to be an effective flexible neutron/low energy g rays shielding material, which could be of potential applications in the field of nuclear technology and nuclear engineering.

        Ginsenoside Rg1 enhances the healing of injured tendon in achilles tendinitis through the activation of IGF1R signaling mediated by oestrogen receptor

        Background: During the pathogenesis of tendinopathy, the chronic inflammation caused by the injury and apoptosis leads to the generation of scars. Ginsenoside Rg1 (Rg1) is extracted from ginseng and has anti-inflammatory effects. Rg1 is a unique phytoestrogen that can activate the estrogen response element. This research aimed to explore whether Rg1 can function in the process of tendon repair through the estrogen receptor. Methods: In this research, the effects of Rg1 were evaluated in tenocytes and in a rat model of Achilles tendinitis (AT). Protein levels were shown by western blotting. qRT-PCR was employed for evaluating mRNA levels. Cell proliferation was evaluated through EdU assay and cell migration was evaluated by transwell assay and scratch test assay. Results: Rg1 up-regulated the expression of matrix-related factors and function of tendon in AT rat model. Rg1 reduced early inflammatory response and apoptosis in the tendon tissue of AT rat model. Rg1 promoted tenocyte migration and proliferation. The effects of Rg1 on tenocytes were inhibited by ICI182780. Rg1 activates the insulin-like growth factor-I receptor (IGF1R) and MAPK signaling pathway. Conclusion: Rg1 promotes injured tendon healing in AT rat model through IGF1R and MAPK signaling pathway activation.

      • Innovative Research on Auditory Experience Design Using Binaural Technology in Tourist Interactive Guide Desk

        In recent years, the tourism industry has witnessed multifaceted development trends, including digitalization, intelligence, and sustainable practices. Faced with the transformative shifts in the tourism landscape, epitomized by the emergence of smart tourism, nations globally have launched initiatives to leverage advanced information technology for enhancing the tourism experience and improving service efficiency. In order to present tourists with a more authentic and expressive urban landscape,this article employed research methodologies such as As-Is Analysis and Affinity Diagramming to analyze the travel needs of ten tourists, utilized Binaural, the core technology for achieving immersive audio effects, and innovatively designed the auditory experience in the interactive guide desk. We aim to create a more immersive and unique touring experience, offering a novel technological path for the South Korean tourism industry, thus contributing pioneering concepts to the advancement of smart tourism.

        Incremental Fuzzy Clustering Based on a Fuzzy Scatter Matrix

        For clustering large-scale data, which cannot be loaded into memory entirely, incremental clustering algorithms are very popular. Usually, these algorithms only concern the within-cluster compactness and ignore the between-cluster separation. In this paper, we propose two incremental fuzzy compactness and separation (FCS) clustering algorithms, Single-Pass FCS (SPFCS) and Online FCS (OFCS), based on a fuzzy scatter matrix. Firstly, we introduce two incremental clustering methods called single-pass and online fuzzy C-means algorithms. Then, we combine these two methods separately with the weighted fuzzy C-means algorithm, so that they can be applied to the FCS algorithm. Afterwards, we optimize the within-cluster matrix and betweencluster matrix simultaneously to obtain the minimum within-cluster distance and maximum between-cluster distance. Finally, large-scale datasets can be well clustered within limited memory. We implemented experiments on some artificial datasets and real datasets separately. And experimental results show that, compared with SPFCM and OFCM, our SPFCS and OFCS are more robust to the value of fuzzy index m and noise.

        SINR loss and user selection in massive MU‐MISO systems with ZFBF

        Separating highly correlated users can reduce the loss caused by spatial correlation (SC) in multiuser multiple‐input multiple‐output (MU‐MIMO) systems. However, few accurate analyses of the loss caused by SC have been conducted. In this study, we define signal‐to‐interference‐plus‐noise ratio (SINR) loss to characterize it in multiuser multiple‐input single‐output (MU‐MISO) systems, and use coefficient of correlation (CoC) to describe the SC between users. A formula is deduced to show the accurate relation between SINR loss and CoC. Based on this relation, we propose a user selection method that utilizes CoC to minimize the average SINR loss of users in massive MU‐MISO systems. Simulation results verify the correctness of the relation and show that the proposed user selection method is very effective at reducing the loss caused by SC in massive MU‐MISO systems.

        Outcomes of Anti-CD19 CAR-T Treatment of Pediatric B-ALL with Bone Marrow and Extramedullary Relapse

        PurposeAnti-CD19 chimeric antigen receptor T-cell immunotherapy (19CAR-T) has achieved impressive clinical results in adult and pediatric relapsed/refractory (r/r) B-lineage acute lymphoblastic leukemia (B-ALL). However, the application and effect of CAR-T therapy in B-ALL patients with extramedullary relapse are rarely issued even disqualified in some clinical trials. Here, we examined the efficacy of 19CAR-T in patients with both bone marrow and extramedullary involvement.Materials and MethodsCAR-T cells were generated by transfection of primary human T lymphocytes with a lentiviral vector expressing anti-CD19 single chain antibody fragments (scFvs) with the cytoplasmic domains of 4-1BB and CD3ζ, and used to infuse patients diagnosed as having r/r B-ALL with extramedullary origination. Clinical responses were evaluated by the use of bone marrow aspiration, imaging, and flow cytometry. ResultsEight patients received 19CAR-T infusion and all attained complete remission (CR). Only one patient was bridged to hematopoietic stem cell transplantation (HSCT). Although three patients relapsed after infusion, they received 19/22CAR-T infusion sequentially and attained a second remission. To date, five patients are in continuous CR and all eight patients are still alive. The mean follow-up time was 21.9 months, while the 24-month estimated event-free survival is 51.4%. Conclusion19CAR-T therapy can lead to clinical remission for extramedullary relapsed pediatric B-ALL patients. However, the problem of CD19+ relapses after CAR-T remained to be solved. For patients relapsing after CAR-T, a second CAR-T therapy creates another opportunity for remission for subsequent HSCT.

      • Smoking, <i>Helicobacter Pylori</i> Serology, and Gastric Cancer Risk in Prospective Studies from China, Japan, and Korea

        <P>Smoking is an established risk factor for gastric cancer development. In this study, we aimed to assess prospectively the association of smoking with gastric cancer risk in 1,446 non-cardia gastric cancer cases and 1,796 controls from China, Japan, and Korea with consideration of <I>Helicobacter pylori</I> infection as a potential effect modifier. Applying logistic regression models stratified by study and adjusted for age and sex we found that current, but not former, smoking was significantly associated with gastric cancer risk [OR = 1.33; 95% confidence interval (CI), 1.07–1.65]. However, the association was significant only in <I>H. pylori</I> sero-positive individuals determined by 3 different sero-markers: overall sero-positivity, sero-positivity to the onco-protein CagA, and sero-positivity to the gastric cancer associated sero-marker HP0305 and HP1564. Specifically, a significant interaction was found when stratifying by HP0305/HP1564 (<I>P</I><SUB>interaction</SUB> = 0.01) with a 46% increased risk of gastric cancer among HP0305/HP1564 sero-positive current smokers (95% CI, 1.10–1.93) as opposed to no increased gastric cancer risk among HP0305/HP1564 sero-negative current smokers (OR = 0.93; 95% CI, 0.65–1.33). We confirmed that current smoking is associated with an increased gastric cancer risk, however, only among individuals that are simultaneously sero-positive for the leading causal factor for gastric cancer, <I>H. pylori</I>.</P>

        Posterior Apophyseal Ring Fracture in Adult Lumbar Disc Herniation: An 8-Year Experience in Minimally Invasive Surgical Management of 48 Cases

        Objective: Posterior apophyseal ring fracture (PARF) is an uncommon disorder that is usually accompanied by lumbar disc herniation (LDH). The aim of this study to describe the 8-year experience of performing minimally invasive treatment of PARF, giving particular attention to surgical technique and clinical outcome. Methods: We reviewed 1,324 consecutive patients with LDH seen in our department between 2013 and 2020. Forty-eight patients (3.63%) were enrolled who were diagnosed with PARF associated with LDH and underwent transforminal endoscopic lumbar discectomy (TELD). Mean duration of the final postoperative follow-up was 5.1 years. The control group was comprised of 50 patients diagnosed with LDH without PARF at the same facility. Data on clinical outcomes were analyzed. Results: The mean operation time in the PARF group was 105.4 minutes, which was longer than the mean operation time of the control group (83.9 minutes) (p = 0.001). Surgical complications, including dural tears (6.3%) and surgical instrument rupture (4.2%) were more common in the PARF group (p = 0.025). However, there was no significant difference in the proportion of excellent and good results and recurrence rates between the LDH patients with and those without PARF, respectively. Conclusion: TELD is a safe and effective minimally invasive approach for the treatment of PARF. However, minimally invasive techniques may require longer operation time and steeper learning curve for inexperienced surgeons. The separation and removal of bone fragments, a key step in the procedure, requires patience and care to prevent rupture, residual surgical instruments, and leakage of cerebrospinal fluid.
