N/A There are few published reports of septate gallbladder although it is generally stated that they are not uncommon at necropsy and in surgical specimens. In present study, all cases showed differential concentration of contrast material in the proximal and distal portions of the gallbladder. Pathologically, chronic inflammatory changes with cholelithiasis were most common findings, especially in the distal compartment. The more common occurrence of inflammatory changes with cholelithiasis on distal portion maybe due in part to the relatively slower exchange of bile across the stricture There is obvious difficulty in deciding to operate on a patients gallbladder where stones are not found. Rut, if septation with cholelithiasis in gallbladder is confirmed, we suggest that cholecystectomy is the best treatment

N/A The records of 39 patients with extrahepatic bi]e duct (EHBD) carcinoma who received operation at the Department of Surgery, Yonsei University Wonju College of Medicine from Mar., 1982 to Feb., 1991 were reviewed. Twenty-seven of 39 patients were men with and average age of 57. 5 years (26 to 78 years). Jaundice was the most frequent clinical finding (33/39) followed by RUQ abdominal pain (22/39) and pruritus (17/39), and palpable abdominal mass was noted on only 2 of 39. The carcinoma was located at the distal 1/3 of the EHBD in 59% of the patients and in 5 1%, 35. 9% at the middle and proximal thirds, respectively. All cases were adenocarcinoma. Overall resectability was 69.2%, Twenty-seven of 39 underwent a resection (13 pancreaticoduodenectomy, 13 segmental resection, I pancreatoduodenectomy with trisegmentectomy of liver). Postoperative complications were wound infection (12 8%, n=5), anastomotic leakage (12.8%, n= 5), and UGI bleeding (2.6%, n=1). Operative motality was 15.4%(6/39). One, 2, 3, 5-year survival rates in the curative group (92%, 73 %, 61 %, 40%) were,superior to those in the noncurative group (50%, 0%). The result of this study suggest that early diagnosis and surgica] exploration with resectiron provides the only meaningful chance for cure and increased survival in these patiets.

Melanin is major factor that determines skin color as well as one of the defense systems that prevent the UV-induced damage. White and clean skin is very important part of the satisfaction of appearance and life quality. The purpose of this clinical study was to investigate the anti-melanogenesis activity for whitening product containing aerial part of Puer-aria lobata extract (APPL) can be utilized based on the increase in demands for cosmetics, particularly natural products. A total of 23 subjects who visited Dermapro Skin Research Center were included in this study. Volunteers were irradiated at a test site with UVA+B to induce pigmentation then applied 3% APPL at the test site for 8 weeks. Changes in visual eval-uation on pigmentation, average melanin content were observed, and subjective skin lightness and color improvements were analyzed. In all parameters measured in this clinical study, APPL treated group showed significant improvement com-pared with the control group. After 4 weeks, subjects with 3% APPL treatment started to improve melanin content and visual evaluation on pigmentation and skin lightness. Skin colors have improved from 8 weeks of treatment with 3% APPL. The safety of product containing 3% APPL was also confirmed. Indexes of subjective irritation(itching, prickling, tickling, burning, stinging, stiffness, tightening etc.) and objective irritation(erythema, edema, scale, papule etc.) proved that there was no adverse reaction on skin upon treatment of products. Considering all the results obtained, 3% APPL product can be used as a safe and effective skin-whitening agent.

N/A The present study was undertaken to document operative mortality and morbidity and cumulative survival in patiients who underwent Whipple's operation due to periampullary cancer and benign lesions including trauma from January 1980 throught Apeil 1991. Patients fell one of six categories based upon pathologic diagnosis: adenocarcinoma at the head of the pancreas (n=9), carcinoma of the distal common bile duct (n =11), adenocareinoma of the ampulla of vater (n=9), duodenal carcinoma (n=9), other cancer not considered part of the periampullary region (n=3), and benign disease (n=12). Of the 19 patients alive at the time of this study, ten were patients with benign disease. Eight of the 53 patients were lost to follow-up. In malignant group, overall mean survival time was 23. 7 months until present study: 1 year survival rate, 75%, 2 year survival rate, 62.5%, 3 year survival rate, 38.5%, and 5 year survival rate, 25%. Postoperative death was developed in 6 (11.3%) of 53 patients: malignant group, 4 patients; non-maligant group, 2 patients. Its causes were sepsis (n=2), postoperative bleeding (n=2), hepatorenal syndrome (n=l), and long-standing anesthesia (n=l).
백반증환자의 광선에 노출된 피부에서 Tenascin의 면역조직화학적 변화

백반증으로 진단 받은 환자에서 일광 노출 부위인 얼굴과 위팔의 피부 조직의 tenascin의 분포를 면역조직화학적 방법 을 이용하여 분석하였다. 실험군인 백반증 환자의 병변부에서 tenascin의 발현 부위는 대조군과 일치하나, 발현양은 증가되어 있었다. 실험군에서 는 특히, 표피 바닥층과 바닥판을 포함한 진피유두 부위에서 염색 정도가 현저히 증가되었다. Wilcoxon rank sum test를 시행한 결과 P = 0.0108로서 대조군에 비해 실험군이 유의하게 증가함을 알 수 있었다. 광선에 노출된 백반증조직에서 tenascin의 증가는 광손상에 의한 바닥층 각질세포에서의 tenascin 합성의 결과와 관련이 있는 것으로 추정되며, tenascin이 항부착작용을 통해 멜라닌세포의 이주 및 접착의 이상에 영향을 미칠 것으로 생각된다. 유병기간과 tenascin과의 관계 및 멜라닌의 존재와의 관계는 추후 더 연구되어야 할 과제이나, 본 연구에서는, 특히 피부탈색질환의 치료로서 현재도 많이 이용되고 있는 광선치료 (phototherapy)가 백반증의 유발원인과 과정에 따라서는 환자에게 유해할 수도 있음을 고려해야 한다고 생각된다. Tenascin is suggested the one of cause of vitiligo by interfering melanocyte adhesion and migration. The distribution and expression levels of tenascin were examined by semiquantitative immunohistochemistry on skin biopsies from vitiligo patients with varying area of photodamage. The level of tenascin on adult skin is severely restricted but we had observed the increase on vitiligo skin lesions. Although it is uncertain that the increased tenascin expression is the cause or the result of disease, vitiligo and increased tenascin expression is thought to be related each other. This study has shown that tenascin increased on photodamaged vitiligo skin lesions. So we shoud consider choosing phototherapy to vitiligo.
졸-겔 방법으로 합성된 TiO<sub>2</sub> 상안정성에 대한 pH 및 열처리 온도의 영향

TiO<sub>2</sub>의 rutile 상은 anatase 상에 비해서 고온에서 안정한 상을 형성하지만 anatase의 안정한 온도 영역은 합성조건에 따라 변화된다. 본 연구에서는 TiOSO<sub>4</sub>와 에탄올과 증류수의 혼합용매를 사용하여 Sol-gel법으로 나노 사이즈의 TiO<sub>2</sub>를 합성하여 pH 및 열처리 온도를 따른 anatase와 rutile의 상변화에 대하여 조사하였다. 제조한 TiO<sub>2</sub>의 pH(3, 5, 7, 9)와 열처리 온도(500, 600, 700, 800, 900℃) 조건을 변화시켜 anatase와 rutile의 비율 변화를 관찰하였다. XRD, FE-SEM 분석을 통해 이와 같은 변화를 관찰한 결과, 500℃에서는 anatase TiO<sub>2</sub>, 900℃에서는 rutile TiO<sub>2</sub>가 관찰되었다. 이 중간 온도인 600, 700, 800℃에서는 pH에 따라 anatase와 rutile의 비율이 변화한다. 700℃를 기준으로 pH = 3, 5는 anatase TiO<sub>2</sub>의 비율이 더 크고, pH = 7, 9는 rutile TiO<sub>2</sub>의 비율이 더 크다는 결론을 얻었다. The rutile phase of TiO<sub>2</sub> forms a stable phase at high temperatures compared to anatase phase, but the stable temperature range of anatase changes depending on the synthesis conditions. In this study, nano-sized TiO<sub>2</sub> was synthesized by the Sol-gel method using TiOSO<sub>4</sub> and a mixed solvent of ethanol and distilled water, and the phase change of anatase and rutile according to pH and heat treatment temperature was investigated. Changes in the ratio of anatase and rutile were observed by changing the pH (3, 5, 7, 9) and heat treatment temperature (500, 600, 700, 800, 900℃) conditions of the prepared TiO<sub>2</sub>. As a result of observing these changes through XRD and FE-SEM analysis, anatase TiO<sub>2</sub> at 500℃ and rutile TiO<sub>2</sub> at 900℃ were observed. According to the pH, at these intermediate temperatures of 600, 700 and 800℃, the ratio of anatase and rutile changes. At 700℃, it was concluded that pH = 3~5 had a larger ratio of anatase TiO<sub>2</sub>, and pH = 7~9 had a larger ratio of rutile TiO<sub>2</sub>.
