Objectives : To see if there were any differences in waist circumference reduction when certain variable such as age, sex, BFR(BFR; Body Fat Rate), PIBW(PIBW; Percent Ideal Body Weight), WHR(WHR; Waist Hip Ratio) and BMI(BMI; Body Mass Index) were given. Methods : Among outpatients who admitted to department of Oriental Rehabilitation Medicine, Jeon-Ju Oriental Medicine Hospital, Won-kwang University from August 2003 to september 2004, 89 obese patients were chosen and electroacupunture was practiced to these groups in order to treat abdominal obesity and waist circumference reduction. Results : There were no significant correlation between sex, age and waist circumference reduction. Compared to groups who had shown lower BMI, higher BMI groups have shown greater change in waist circumference reduction. As BFR increase waist circumference reduction tended to increase as well. Conclusions : From the above results, higher BMI groups had shown greater change in waist circumference reduction compared to lower BMI groups.

Laparoendoscopic single-site surgery (LESS) is helpful if not an evidence of suspicious malignancy in huge adnexa tumors. This approach might be reasonable alternative to conventional laparoscopic surgery. We report two cases with LESS for huge cystic adnexal tumors in whole abdomen. A 58 years old multigravida woman in menopause underwent removal of a huge cystic adnexal mass that occupied her entire abdomen by LESS.(ca.5200ml aspiration) A 15 years old single nulligravida woman had a huge cystic adnexal tumor in whole abdomen reaching to the level of the xyphoid process. A needle through the transumbilical single port was inserted directly under the mass. Subsequently, 8000 ml of cystic fluid was aspirated without spillage.

Background In general, external rotation (ER) exercises are commonly used in rotator cuff retraining programs to improve stabilization ability. ER exercises at 90° of shoulder abduction are common and effective in strengthening the infraspinatus (ISP). However, if the ISP is weak during ER at 90° shoulder abduction, the posterior deltoid muscle (PD) is overactivated, which can lead to anterior translation of the humeral head. Therefore, the selective activation of ISP muscles is an important factor in rehabilitation. Objectives We investigated the muscle activities of the ISP, PD, and middle trapezius (MT), the ratio of the ISP to PD muscle activities, and the anterior translation of the humeral head in response to three resistance intensities during sitting external rotation (SITER) exercises. Methods A total of 12 healthy women recorded changes in the anterior translation of the humeral head using a load cell, the muscle activities of the ISP, PD, and MT, the ratio of the ISP to PD muscle activities using surface electromyography (EMG) during SITER under three different intensities [low (10-20%) medium (45-55%) high (60-70%)]. Results The ISP, PD, and MT amplitudes were highest at high intensity, followed by medium and low intensities during SITER (p < 0.001). The muscle activity ratio was significantly highest at medium intensity compared with low and high intensities (p = 0.031). The anterior translation of the humeral head was highest at high intensity, followed by medium and low intensities during SITER (p < 0.001). Conclusions Medium intensity (45-55%) was the most appropriate intensity during the SITER exercise. However, anterior gliding of the humeral head was the lowest at low intensity. Therefore, during ER exercises, we should think about how to control the anterior gliding of the humeral head.

<P>Tanshinone IIA (Tan IIA; 14,16-epoxy-20-nor-5(10),6,8,13,15-abietapentaene-11,12-dione), a phytochemical derived from the roots of <I>Salvia miltiorrhiza</I> B<SMALL>UNGE</SMALL>, has been reported to posses anti-angiogenic, anti-oxidant, anti-inflammatory and apoptotic activities. However, the cancer growth inhibitory/cytocidal effects and molecular mechanisms in prostate cancer cells have not been well studied. In the present study, we demonstrate that Tan IIA significantly decreased the viable cell number of LNCaP (phosphate and tensin homolog (PTEN) mutant, high AKT, wild type p53) prostate cancer cells more sensitively than against the PC-3 (PTEN null, high AKT, p53 null) prostate cancer cells. Tan IIA significantly increased TdT-mediated dUTP nick-end labeling (TUNEL) positive index and sub-G1 DNA contents of treated cells, consistent with apoptosis. Tan IIA treatment led to cleavage activation of pro-caspases-9 and 3, but not pro-caspase-8, and cleavage of poly (ADP ribose) polymerase (PARP), a caspase-3 substrate. Additionally, Tan IIA treatment induced cytochrome <I>c</I> release from the mitochondria into the cytosol and reduced mitochondrial membrane potential and suppressed the expression of mitochondria protective Bcl-2 family protein Mcl-1<SUB>L</SUB>. Tan IIA reduced the expression of phosphoinositide 3-kinase (PI3K) p85 subunit, and the phosphorylation of AKT and mammalian target of rapamycin (mTOR) in a concentration-dependent manner. Moreover, the combination of Tan IIA and LY294002, a specific PI3K inhibitor, enhanced PARP cleavage of LNCaP and PC-3, but not in MDA-MB-231 breast cancer cells which do not contain detectable active AKT. The findings suggest that Tan IIA-induced apoptosis involves mitochondria intrinsic caspase activation cascade and an inhibition of the PI3K/AKT survival pathway.</P>

Background: Stasis dermatitis is a common dermatologic disorder. It is caused by venous hypertension results from venous backward flow. However, research on the association with specific vascular disease is insufficient. Objectives: This study was intended to investigate in clinical manifestations of stasis dermatitis and analysis of association with other vascular diseases in our hospital outpatients. Methods: In this study, we investigated 37 patients with a diagnosis of stasis dermatitis in our hospital from 2014 to 2019. We reviewed clinical manifestations and relation with other vascular disease. Results: In total 37 cases, 20 (54.1%) patients were female and 17 (45.9%) patients were male. Twelve two (59.5%) patients were presented as erythema, followed by 14 (37.8%) as purpuric lesions and 11 (29.7%) as brownish changes. There were 4 (10.8%) patients showed crust, 3 (8.1%) as edema, 2 (5.4%) as scale, 2 (5.4%) as ulcer. For symptoms, 15 (40.5%) patients complained itching, 4 (10.8%) as tenderness, 3 (8.1%) as pain, 1 (2.7%). In relation with other vascular disease, 16 (43.2%) patients represented with vein related disease such as deep vein thrombosis, varicose vein and thrombophlebitis. However, there was no arterial disease such as arteriosclerosis obliterans. Conclusion: The result shows clinical manifestations of stasis dermatitis and association with other specific vascular diseases. By understanding these findings, it is expected that better treatment outcome will be obtained.

목적: Urinary concentrations of 4-(Methylnitrosamino)-1-(3-pyridyl)-1-butanol (NNAL) could be used as a reliable biomarker of exposure to tobacco smoke. We therefore developed a relatively simple high-throughput analytical method using ultra-performance liquid chromatography coupled with tandem mass spectrometry (UPLC-MS/MS) to quantify total urinary NNAL concentrations in the general population. 방법: We developed and validated the UPLC-MS/MS method to quantify total NNAL concentrations in urine. And we studied quantified this biomarker of exposure in n=232 first-trimester pregnant women, of which n=17 were smokers, n=34 non-smokers, and n=181 with second-hand smoke exposure. 결과: Standard curves for total NNAL concentrations in urine were linear between 20-1500 pg/mL, with coefficients of determination >0.95, with precision ranging from 2.2 to 8.6% (CV), and accuracy from -5.6 to 10.9% (percent error). The lowest limit of quantitation was 6.7 pg/mL, and 2.0 pg/mL the lowest limit of detection (LLOD). Total urinary NNAL concentrations in non-smoker subjects (phase 1) non-smoker pregnant women (phase 2) were <LLOD, whereas in smokers total NNAL concentrations were between 0-35.2 pg/mL. Second-hand smoke exposure in pregnancy resulted in total urinary concentrations varying between 0-7.7 pg/mL. NNAL and cotinine in non-smoker samples were not detected. Measurement of urinary cotinine could be up to 14 days after last smoking. Measurement of urinary total NNAL could be up to 35 days after last smoking. 결론: We successfully developed an UPLC-MS/MS analytical method to quantify total urinary NNAL concentrations in the pregnant women. NNAL is better biomarker than cotinine to detect smoker in early pregnancy.
