        약국서비스 만족에 영향을 미치는 요인 분석 : 환자체감시간과 실 조제시간 비교를 중심으로

        Purpose : To shorten processing time for variety of medical affairs of the patient at the outpatient clinic of a big hospital is very important to qualify medical care of the patient. Therefore, patient's waiting time for drug delivery after doctor's prescription is often utilized as a strong tool to evaluate patient satisfaction with a medical care provided. We performed this study to investigate factors influencing patient satisfaction related with waiting time for drug delivery. Methods : The data were collected from July 21 to August 12, 1998. A total 535 patients or their families who visited outpatient clinics of Inha University Hospital were subjected to evaluate the drug delivery time and the level of their satisfaction related, which were compared with those objectively evaluated by Quality Improvement Team. The reliability of the scale was tested with Cronbach's alpha, and the data were analyzed using frequency, t-test, ANOVA, correlation analysis and multiple regression. Results : The mean drug delivery time subjectively evaluated by the patient (16.1 13.0 min) was longer than that objectively evaluated (10.9 7.6 min) by 5.2 min. Drug delivery time objectively evaluated was influenced by the prescription contents, total amount or type of drug dispensed, etc, as expected. The time discrepancy between two evaluations was influenced by several causative factors. One of those proved to be a patient's late response to the information from the pharmacy which the drug is ready to deliver. Interestingly, this discrepancy was found to be more prominent especially when waiting place for drug delivery was not less crowded. Other factors, pharmaceutical counseling at the pharmacy, emotional status or behavior of a patient while he waits for the medicine, were also found to influence the time subjectively evaluated. Regarding the degree of patient satisfaction with the drug delivery, majority of patients accepted drug delivery time with less than 10 min. It was also found to be influenced by emotional status of the patient as well as kindness or activity of pharmaceutical counselor. Conclusion : The results show that, besides prescription contents, behavior pattern or emotional status of a patient, environment of the waiting place, and quality of pharmaceutical counseling at the pharmacy, may influence the patient's subjective evaluation of waiting time for drug delivery and his satisfaction related with the service in the big hospital. In order to improve patient satisfaction related with waiting time for drug delivery, it will be cost effective to qualify pharmaceutical counseling and information system at the drug delivery site or waiting place rather than to shorten the real processing time within the pharmacy.

        일 병원의 외래진료대기시간 지연요인 분석

Therefore, patient’s waiting time for medical examination is often utilized as a strong tool to evaluate patient satisfaction with a medical care provided. We performed this study to investigate factors delaying related with waiting time for medical examination. Methods : The data were collected from June 26 to July 30, 1999. A total 275 case of medical treatment and 5,634 patients who visited outpatient clinics of a tertiary hospital were subjected to evaluate the waiting time. The data were analyzed using frequency, t-test, ANOVA, X^(2)-test by SPSS Windows 7.5 program. Results : The mean patient’s waiting time objectively evaluated(30.9±33.9min) was longer than that subjectively by patient evaluated(25.1±26.2min).Patient waiting time objectively evaluated was influenced by the starting time of medical examination, consultation hours, patients arriving time etc, as expected. The time discrepancy between two evaluations was influenced by several causative factors. Regarding the degree of patients accepted waiting time with the medical examination is 20 min. Conclusion : The results show that, besides the starting time of medical examination, consultation hours and patients arriving time, influence the patient’s subjective evaluation of waiting time for medical examination and his satisfaction related with the service in the big hospital. In order to improve patient satisfaction related with waiting time for medical examination, it will be effective examination rather than to shorten the real processing time within the consultation room.

        임상진료지침 개발과정의 장애요인 : 심폐소생술을 중심으로

        Background : Despite favorable effects of guidelines on patient care, guidelines often fail to achieve their objectives. Poorly implemented medical practice guidelines can produce only weak effects on the process of health caredelivery. Therefore, we performed this study to investigate barriers related development of clinical practice guidelines. Methods : Cardiopulmonary resuscitation was selected as a target problem. Self questionnaires about management of cardiopulmonary resuscitation were developed by a researcher with advisory 8 experts. The questionnaires were designed as a unstructured methods. The data were collected from March I to May 31, 1999. A total 50 death case admitted inpatient to Inha University Hospital were subjected to evaluate the development and application of clinical practice guidelines for cardiopulmonary resuscitation. The data were examined by frequency, t-test with SPSS. Result : The article reviewed several common barriers that might limit successful implementation of guidelines in clinical practice, as illustrated by-evaluating recommendations for cardiopulmonary resuscitation clinical practice guidelines. Some major problems with guidelines were characterized as follows ; (1) ethical problem : euthanasia, (2) occurrence on various emergency event and setting, (3) non-medical problems (4) unreliable of medical record etc. Conclusion : Careful analysis of guideline attributes, anticipated effect on medical care, and organizational factors revealed several barriers to successful guideline- implementation that should be addressed in the design of future guideline-based interventions.

        Effectiveness of worksite-based dietary interventions on employees' obesity: a systematic review and meta-analysis

        BACKGROUND/OBJECTIVES: This study was designed to provide scientific evidence on the effectiveness of worksite-based dietary intervention to reduce obesity among overweight/obese employees. MATERIALS/METHODS: Electronic search was performed using Ovid Medline, Embase, Cochrane Library, and CINAHL databases. The keywords used were "obesity," "nutrition therapy," and "worksite." The internal validity of the randomized controlled trials (RCTs) was assessed using the Cochrane's Risk of Bias. Meta-analysis of selected studies was performed using Review Manager 5.3. RESULTS: A total of seven RCTs with 2,854 participants were identified. The effectiveness of dietary interventions was analyzed in terms of changes in weight, body mass index (BMI), total cholesterol, and blood pressure. The results showed that weight decreased with weighted mean difference (WMD) of -4.37 (95% confidence interval (CI): -6.54 to -2.20), but the effectiveness was statistically significant only in short-term programs < 6 months (P = 0.001). BMI also decreased with WMD of -1.26 (95% CI: -1.98 to -0.55), but the effectiveness was statistically significant only in short-term programs < 6 months (P = 0.001). Total cholesterol decreased with WMD of -5.57 (95% CI: -9.07 to -2.07) mg/dL, demonstrating significant effectiveness (P = 0.002). Both systolic (WMD: -4.90 mmHg) and diastolic (WMD: -2.88 mmHg) blood pressure decreased, demonstrating effectiveness, but with no statistical significance. CONCLUSIONS: The worksite-based dietary interventions for overweight/obese employees showed modest short-term effects. These interventions can be considered successful because weight loss was below approximately 5-10 kg of the initial body weight, which is the threshold for the management of obesity recommended by the Scottish Intercollegiate Guideline Network (SIGN).

        Relaxation Therapy for Irritable Bowel Syndrome: A Systematic Review

        Purpose: This study investigated evidence for the use of relaxation therapies as interventions to decreaseirritable bowel syndrome (IBS) symptoms and severity as well as reduce anxiety and improve quality oflife in IBS patients. Methods: A search of electronic bibliographic databases (e.g., Medline Ovid, Embase, KoreaMed, andNational Discovery for Science Leaders) was conducted to identify randomized controlled trials thatincluded relaxation exercise programs for adults (>18 years old) with IBS. Of the 486 publicationsidentified, 8 studies met inclusion and exclusion criteria, and all studies were used in the meta-analysis. We used Cochrane's risk of bias to assess study quality. Results: The results showed that IBS symptoms decreased significantly, 6.19 (95% confidence interval[2.74, 14.02]) and there was no heterogeneity. Symptom severity and anxiety decreased by 0.38 (95%confidence interval [ 1.41, 0.65]) and 0.08 (95% confidence interval [ 0.38, 0.23]) due to relaxationtherapies, but theses scores were not statistically significant. Conclusion: This review revealed positive effects of relaxation therapy on IBS symptoms in adult patientswith IBS. However, these results should be interpreted with caution due to the small number of studiesexamined and the associated methodological problems. Further studies are needed to ascertain the longtermeffects of relaxation therapy and the underlying psychosocial mechanisms leading to anxietyreduction and improved quality of life.

      • Biological Activity of Recombinant Human Erythropoietin (EPO) In Vivo and In Vitro

        The hematopoietic growth factor erythropoietin (EPO) is required for the maintenance, proliferation, and differentiation of the stem cells that produce erythrocytes. To analyse the biological activity of the recombinant human EPO (rec-hEPO), we have cloned the EPO cDNA and genomic DNA and produced rec-hEPO in the CHO cell lines. The growth and differentiation of EPO-dependent human leukemic cell line (F36E) were used to measure cytokine dependency and in vitro bioactivity of rec-hEPO. MIT assay values were increased by survival of F36E cells at 24h or 72h. The hematocrit and RBC values were increased by subcutaneous injection of 20 IU (in mice) and 100IU(in rats) rec-hEPO. Hematocrit values remarkably increased at $13.2\%$ (in mice) and $12.2\%$ (in rats). The pharmacokinetic behavior with injection of 6 IU of rec-hEPO remained detectable after 24 h in all mice tested. The highest peat appeared at 2h after injection. The long half-life of rec-hEPO is likely to confer clinical advantages by allowing less frequent dosing in patients treated for anemia. These data demonstratethat ree-hEPO produced in this study has a potent activity in vivo and in vitro. The results also suggest that biological activity of ree-hEPO could be remarkably enhanced by genetic engineering that affects the potential activity, including mutants with added oligosaccharide chain and designed to produce EPO-EPO fusion protein.

      • FLOW FIELD COMPUTATION BY UNPWIND MESHFREE METHOD FOR SIMPLIFIED HIGH VOLTAGE GAS CIRCUIT BREAKER MODEL

        We consider the computational problems related to high voltage (HV) circuit breakers. To effectively design HV circuit breakers, a meshfree method which was proposed by the authors is tested. The meshfree method is implemented to a simplified model for the HV circuit breaker and showed a robust behavior even in a severe boundary condition. A good shock capturing is shown in the computations and a delicate gas diffusion through a slit is also computed, which is usually difficult to catch. Vector splitting method is employed to exploit the merit of mesh free methods in the hyperbolic problems.
