        Association of a Provisional New emm Type Opacity Factor-Negative Group A Streptococci Strain ST4529 with Septicemia

        Group A Streptococcus strain ST4529 is a provisional new ems type which has been recently reported in Malaysia (Jomal, et al. 1999. Energ. Infect . Dis. 5,10-14). This strain was found to be opacity factor (OF) negative with a Tl phenotype. Usually, OF negative strains with T1 phenotypes are associated with acute rheumatic fever. However, strain ST4529 was isolated from the blood of a patient with septicemia. Comparison of the deduced amino acid sequence of the mature hypervariable N-terminus of ST4529 showed only 43% identity with that of M5, the closest matched OF negative strain with a T1 phenotype. Thus, ST4529 most probably encodes a new serospecifically unique M protein which is associated with septicemia rather than pharyngitis infections. The strains with these phenotypes are very important because their sequences should be considered for developing any anti-streptococcal vaccines.

      • Clinical Impact of Overexpression of FOXP3 and WT1 on Disease Outcome in Egyptian Acute Myeloid Leukemia Patients

        Background: In the last decade, it has become clear that change of gene expression may alter the hematopoietic cell quiescent state and consequently play a major role in leukemogenesis. WT1 is known to be a player in acute myeloid leukemia (AML) and FOXP3 has a crucial role in regulating the immune response. Objectives: To evaluate the impact of overexpression of WT1and FOXP3 genes on clinical course in adult and pediatric AML patients in Egypt. Patients and methods: Bone marrow and peripheral blood samples were obtained from 97 de novo non M3 AML patients (63 adult and 34 pediatric). Real-time quantitative PCR was used to detect overexpression WT1 and FOXP3 genes. Patient follow up ranged from 0.2 to 39.0 months with a median of 5 months. Results: In the pediatric group; WT1 was significantly expressed with a high total leukocyte count median 50X109/L (p=0.018). In the adult group, WT1 had an adverse impact on complete remission induction, disease-free survival and overall survival (p=0.02, p=0.035, p=0.019 respectively). FOXP3 overexpression was associated with FAB subtypes AML M0 +M1 vs. M2, M4+M5 (p =0.039) and the presence of hepatomegaly (p=0.005). Conclusions: WT1 and FOXP3 overexpression has an adverse impact on clinical presentation, treatment response and survival of pediatric and adult Egyptian AML patients.

        The rate of delayed hydride cracking (DHC), V, has been measured in cold-worked and stress-relieved Zircaloy-4 fuel cladding using the Pin-Loading Tension technique. At 250 °C the mean value of V from 69 specimens was 3.3(±0.8)x10⁻⁸ m/s while the temperature dependence up to 275 °C was described by Aexp(-Q/RT), where Q is 48.3 kJ/mol. No cracking or cracking at very low rates was observed at higher temperatures. The fracture surface consisted of flat fracture with no striations. The results are compared with previous results on fuel cladding and pressure tubes.

      • Multi-model-based interactive authoring environment for creating shareable medical knowledge

        Objective: Technologically integrated healthcare environments can be realized if physicians are encouraged to use smart systems for the creation and sharing of knowledge used in clinical decision support systems (CDSS). While CDSSs are heading toward smart environments, they lack support for abstraction of technology-oriented knowledge from physicians. Therefore, abstraction in the form of a user-friendly and flexible authoring environment is required in order for physicians to create shareable and interoperable knowledge for CDSS workflows. Our proposed system provides a user-friendly authoring environment to create Arden Syntax MLM (Medical Logic Module) as shareable knowledge rules for intelligent decision-making by CDSS. Methods and materials: Existing systems are not physician friendly and lack interoperability and shareability of knowledge. In this paper, we proposed Intelligent-Knowledge Authoring Tool (I-KAT), a knowledge authoring environment that overcomes the above mentioned limitations. Shareability is achieved by creating a knowledge base from MLMs using Arden Syntax. Interoperability is enhanced using standard data models and terminologies. However, creation of shareable and interoperable knowledge using Arden Syntax without abstraction increases complexity, which ultimately makes it difficult for physicians to use the authoring environment. Therefore, physician friendliness is provided by abstraction at the application layer to reduce complexity. This abstraction is regulated by mappings created between legacy system concepts, which are modeled as domain clinical model (DCM) and decision support standards such as virtual medical record (vMR) and Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT). We represent these mappings with a semantic reconciliation model (SRM). Results: The objective of the study is the creation of shareable and interoperable knowledge using a user-friendly and flexible I-KAT. Therefore we evaluated our system using completeness and user satisfaction criteria, which we assessed through the system- and user-centric evaluation processes. For system-centric evaluation, we compared the implementation of clinical information modelling system requirements in our proposed system and in existing systems. The results suggested that 82.05% of the requirements were fully supported, 7.69% were partially supported, and 10.25% were not supported by our system. In the existing systems, 35.89% of requirements were fully supported, 28.20% were partially supported, and 35.89% were not supported. For user-centric evaluation, the assessment criterion was 'ease of use'. Our proposed system showed 15 times better results with respect to MLM creation time than the existing systems. Moreover, on average, the participants made only one error in MLM creation using our proposed system, but 13 errors per MLM using the existing systems. Conclusion: We provide a user-friendly authoring environment for creation of shareable and interoperable knowledge for CDSS to overcome knowledge acquisition complexity. The authoring environment uses state-of-the-art decision support-related clinical standards with increased ease of use.

        Influence of Iron Doping on Structural, Optical and Magnetic Properties of TiO2 Nanoparticles

        In this study, various concentrations of Fe doped TiO2nanoparticles have been successfully synthesized using the sol–gelmethod. A variety of characterization techniques as ultra-violet visible (UV–Vis) spectroscopy, X-ray diffractometer (XRD),vibrating sample magnetometry (VSM) and field emission scanning electron microscopy (FESEM) were employed to analyzethe prepared nanopowders. XRD measurement confirmed the substitution of Fe ion without disturbing the tetragonal crystalsystem of TiO2. The crystallite size was found to decrease and lattice strain increases upon doping estimated by WilliamsonHall plot. Furthermore, the average grain size calculated by FESEM found was between 10 and 30 nm for pure and dopedTiO2. UV–Vis spectroscopy showed an increase in absorption accompanied red shift and increase in band gap energies from3.36 to 3.62 eV with the addition of Fe. The FTIR spectroscopy was employed to confirm the presence of functional groupsin the fabricated nanopowders. Upon mixing the saturation magnetization (Ms) varying from (2.12 to 1.51)10−2 emu/g wasobserved.

        A Class of Multivalent Functions with Negative Coefficients Defined by Convolution

        For a given $p$-valent analytic function $g$ with positive coefficients in the open unit disk $\UD$, we study a class of functions $f(z)=z^p-\sum_{n=m}^\infty a_nz^n$\ ($a_n\geq 0$) satisfying \[ \frac{1}{p}\Re \left( \frac{z(f*g)'(z)}{(f*g)(z)}\right) > \alpha\quad (0\leq \alpha<1; z\in\Delta).\] Coefficient inequalities, distortion and covering theorems, as well as closure theorems are determined. The results obtained extend several known results as special cases.

        Effect of cathodic arc PVD parameters on roughness of TiN coating on steel substrate

        Titanium nitride which is widely used as a hard coating material was coated on tool steel, by physical vapor deposition method. Surface roughness was investigated as a function of deposition rate, substrate bias and temperature, nitrogen flow rate and metal ion etching. The study showed that increase in surface roughness mainly depends on the condition of sample preparation, surface treatment, macro-droplets,pitting defects, rise in compressive stress at higher coating thickness, growth defects and to a lesser extent selection of surface under testing. It was observed that chromium ion etching significantly reduced the surface roughness compared to titanium ion etching.

        Deformation prediction by a feed forward artificial neural network during mouse embryo micromanipulation

        In this study, a neural network (NN) modeling approach has been used to predict the mechanical and geometrical behaviors of mouse embryo cells. Two NN models have been implemented. In the first NN model dimple depth (w),dimple radius (a) and radius of the semi-circular curved surface of the cell (R) were used as inputs of the model while indentation force (f) was considered as output. In the second NN model, indentation force (f), dimple radius (a) and radius of the semi-circular curved surface of the cell (R) were considered as inputs of the model and dimple depth was predicted as the output of the model. In addition, sensitivity analysis has been carried out to investigate the influence of the significance of input parameters on the mechanical behavior of mouse embryos. Experimental data deduced by Flu¨ckiger (2004) were collected to obtain training and test data for the NN. The results of these investigations show that the correlation values of the test and training data sets are between 0.9988 and 1.0000, and are in good agreement with the experimental observations.
