Acute kidney injury (AKI) is characterized by tubular cell death and interstitial inflammation. TWEAK promotes experimental kidney injury and activates the transcription factor NF-κB, a key regulator of genes involved in cell survival and inflammatory response. In search of potential therapeutic targets for AKI, we compared a transcriptomics database of NF-κB-related genes from murine AKI-kidneys with a transcriptomics database of TWEAK-stimulated cultured tubular cells. Four out of twenty-four (17%) genes were significantly upregulated (false discovery rate, FDRo0.05), while nine out of twenty-four (37%) genes were significantly upregulated at FDR o0.1 in both databases. Bcl3 was the top upregulated NF-κB-related gene in experimental AKI and one of the most upregulated genes in TWEAK-stimulated tubular cells. Quantitative reverse transcription PCR (qRT-PCR), western blot and immunohistochemistry confirmed Bcl3 upregulation in both experimental conditions and localized increased Bcl3 expression to tubular cells in AKI. Transcriptomics database analysis revealed increased Bcl3 expression in numerous experimental and human kidney conditions. Furthermore, systemic TWEAK administration increased kidney Bcl3 expression. In cultured tubular cells, targeting Bcl3 by siRNA resulted in the magnification of TWEAK-induced NF-κB transcriptional activity, chemokine upregulation and Klotho downregulation, and in the sensitization to cell death induced by TWEAK/TNFα/interferon-γ. In contrast, Bcl3 overexpression decreased NF-κB transcriptional activity, inflammatory response and cell death while dampening the decrease in Klotho expression. In conclusion, Bcl3 expressed in response to TWEAK stimulation decreases TWEAK-induced inflammatory and lethal responses. Therefore, therapeutic upregulation of Bcl3 activity should be explored in kidney disease because it has advantages over chemical inhibitors of NF-κB that are known to prevent inflammatory responses but can also sensitize the cells to apoptosis.

Background: Sacral nerve stimulation is a therapeutic option with demonstrated efficacy for conditions presenting with perineal pain caused by different etiologies. We aimed to assess whether a sacral electrode ($Interstim^{(R)}$, Medtronic, Minneapolis, MN, USA) inserted through the caudal pathway is able to offer an acceptable level of sacral stimulation and rate of catheter migration. Methods: We present 12 patients with pelvic pain who received sacral neuromodulation via the sacral hiatus with the InterStim electrode. We evaluated patient satisfaction as well as migration and removal of the electrode, if necessary. Results: Our experience included 12 patients, 10 women and two men, with a mean age of 60 years. In eight of the 12 patients, the initial therapy was effective, and the final system implantation was performed. During subsequent follow-up, patient satisfaction was good. To date, there have been no cases of electrode displacement or migration. Conclusions: The caudal insertion of the InterStim electrode, with its own fixation system, and initially designed for transsacral insertion, appears in our experience to be a satisfactory option which can minimize electrode displacements, achieving similar results in therapeutic efficacy and causing no difficulties in removal.

The aim of this work was to provide further confirmation of the possible use of non-linear ultrasonic techniques for detecting the cracking due to corrosion of steel reinforcements in concrete. To this end accelerated steel corrosion tests have been conducted on model reinforced cement mortar specimens, while monitoring the appearance and width evolution of visible surface cracks, and performing non-linear ultrasonic measurements based on the phenomena of harmonic distortion and intermodulation. A new parameter, based on the difference between the amplitude of the fundamental frequency and the sum of the amplitudes of all the first-order and second-order intermodulation products, has been proposed in this work. The results confirm that the appearance of visible surface micro-cracks are preceded and accompanied by the observation of strong non-linear features in the received signal. Furthermore, the new parameter proposed in this work is as efficient as the relative non-linearity parameters, classically used in harmonic distortion non-linear ultrasonic studies, for detecting the non-linear features associated with the critical events of the cracking of cement mortar due to embedded steel corrosion. A hypothesis has been developed considering the possible effect of the filling of the void space by liquid containing rust products after the formation of new cracks or the enlargement of its width. This filling process, which might be particularly enhanced by net convective transport of liquid, would explain the evolution of the values of all the parameters used for putting in evidence the non-linear elastic features after the critical events of the cracking process.
Corns with Different Nutritional Profiles on Growing and Finishing Pigs Feeding (30 to 90 kg)

Three experiments were carried out to determine the nutritional value and to verify the pig performance in growing and finishing phases (30 to 90 kg) fed on diets containing common corn (CC), high-lysine corn (HLC) and high-oil corn (HOC). In the total digestibility trial (Exp. I) 12 barrows were used. Values of digestible energy (DE) and metabolizable energy (ME) as-fed basis for CC, HLC and HOC, were: 3,396 and 3,275 kcal/kg; 3,248 and 3,139 kcal/kg; 3,445 and 3,308 kcal/kg, respectively. In order to determine the apparent and true ileal digestibility coefficients of amino acids, as well as the values of true digestible amino acids of the CC, HLC and HOC, an ileal digestibility trial was done (Exp. II) with T-cannulated barrows ("T" simple). The treatments consisted of three diets, with one of them as the sole source of protein (CC, HLC and HOC). In the performance experiment (Exp. III), 36 crossbred pigs, allotted in a completely randomized design with three treatments and 12 replications were used. Treatments consisted of three diets: 1 - CC; 2 - HLC and 3 - HOC. It was observed no difference for performance and carcass variables among the corns with different nutritional profiles. Results of the three experiments highlighted the importance of segregating corns in their real chemical and energetic composition as well as the values of true digestible amino acids for formulating diets for growing and finishing pigs.

A 36 year-old man was referred to our hospital for the evaluation of an abnormal shadow on the cardiac apex on a chest X-ray fi lm.He denied systemic disease such as hypertension or diabetes mellitus and had no history of specifi c conditions, such as Kawasaki`s disease, chest trauma or connective tissue disorder. He had no subjective symptoms. His blood pressure was 120/75 mmHg and pulse rate was 70 beats/min. On physical examination, continuous murmur of a grade II/VI was audible at 4th left intercostal space. The results of other physical examination and blood test were normal. A Chest X-ray showed a radio-opaque shadow at the apex. An echocardiography was arranged and a round sac of 4x4 cm located in the apex, as well as a double-chamber right ventricle was observed Contrast echocardiography with Sonovue showed the fi lling of the vascular sac with contrast then partial fi lling of the distal RV apex. The abnormal vascular sac was connected from the left side to a vascular structure that looks to be coronary artery and from the right side to the distal part of a double right ventricle apex with a bi-directional shunt (fi stula). It also demonstrates the high velocity and centrifuge nature of blood fl ow in the vascular sac. Chest CT showed a LAD ectasia with a 4x4 cm saccular aneurysm at the distal portion of LAD Cardiac MRI revealed a giant left descending coronary artery distal aneurysm attached to a double apex right ventricle Coronary angiography revealed a dilated LAD with a huge saccular coronary aneurysm at its distal portion. The patient refused surgical correction for the coronary aneurysm and fi stula. He was discharged and has been followed up for more than 1 year without cardiovascular events.
