코로나-19 긴급틈새돌봄에 관한 탐색적 연구 - 서비스 제공자와 이용자의 경험을 중심으로 -

본 연구는 코로나-19 확산 상황에서 취약계층의 생존권을 확보해내기 위해 고안된 긴급틈새돌봄 사업에서 제공자와 이용자들의 경험을 귀납적으로 포착하여 돌봄서비스의 개선방안을 모색하고자 수행되었다. 이를 위해 긴급틈새돌봄 서비스의 제공자와 이용자를 각각 심층 면접하고 내용분석 하였다. 분석결과 서비스 제공자의 경험을 총 6개의 대범주와 16개의 중범주로 제시하였으며 서비스 이용자의 경험을 총 4개의 대범주와 12개의 중범주로 제시하였다. 결과를 통해 향후 팬데믹 등의 상황에서 긴급돌봄 사업 이용자의 체감도를 높이고 서비스 제공자의 인권과 처우 등을 개선할 수 있는 돌봄서비스 개선 방향을 제언하였다. This study was conducted to find ways to improve the care service through the experiences of providers and clients who used the ‘emergency care project’ designed to secure the life of the vulnerable in the COVID-19 situation. For this purpose, ‘in-depth interview’ and ‘content analysis’ were conducted with providers and Clients of ‘emergency care service’ respectively. As a result of the analysis, the experience of service providers was presented in a total of 6 major categories and 16 medium categories. The experience of service users was presented in a total of 4 major categories and 12 medium categories. Based on the results, it was suggested to improve the care service that can improve the satisfaction of emergency care service users and improve the human rights and treatment of service providers in the future, such as a pandemic.

<P>In a phase I/IIa open-label and nonrandomized controlled clinical trial, we sought to assess the safety and neurological effects of human neural stem/progenitor cells (hNSPCs) transplanted into the injured cord after traumatic cervical spinal cord injury (SCI). Of 19 treated subjects, 17 were sensorimotor complete and 2 were motor complete and sensory incomplete. hNSPCs derived from the fetal telencephalon were grown as neurospheres and transplanted into the cord. In the control group, who did not receive cell implantation but were otherwise closely matched with the transplantation group, 15 patients with traumatic cervical SCI were included. At 1 year after cell transplantation, there was no evidence of cord damage, syrinx or tumor formation, neurological deterioration, and exacerbating neuropathic pain or spasticity. The American Spinal Injury Association Impairment Scale (AIS) grade improved in 5 of 19 transplanted patients, 2 (A → C), 1 (A → B), and 2 (B → D), whereas only one patient in the control group showed improvement (A → B). Improvements included increased motor scores, recovery of motor levels, and responses to electrophysiological studies in the transplantation group. Therefore, the transplantation of hNSPCs into cervical SCI is safe and well-tolerated and is of modest neurological benefit up to 1 year after transplants. 대량 맞춤 야구복 하의 생산 시스템을 설계하기 위하여, 효율적인 생산이 가능한 표준 생산 공정을 개발하고, 개별 맞춤 시스템과 대량생산 시스템의 단점을 보완하여 장점을 융합한 대량 맞춤을 위한 새로운 생산 시스템을 개발하고자, 기준 야구복 하의의 기준표준공정과 작업시간을 설정하였다. 본 연구는 설정된 표준공정에 근거하여, 작업시간, 작업인원, 기기에 따라 대량 맞춤생산관리가 가능한 혼합형 유니트 생산 시스템을 개발하였다. 대량 맞춤 하의 생산 시스템 구성을 위해, 기준 야구복 하의 본즈형을 개별 생산 공장 2곳의 기준공정과 JUKI사에서 제시하는 바지봉제의 공정 순서 중 작업 단위별로 참고, 분석하여 총 50공정의 기준공정을 편성하였다. 야구복의 다양한 디자인, 개별 맞춤의 특성에 맞는 다품종 소량생산과 샘플단위의 생산 시스템을 접목하여 다양한 생산요소에 대응할 수 있는 유사 공정에 따라 작업 단위를 묶어 4개의 유니트로 구성하고, 본봉 재봉기, 2본침 오버록, 인터록, 삼봉 재봉기, 2본침 재봉기, 다리미, 단추 구멍기기, 단추달이 기기 등 총 8종의 최소기종으로 구성하였다. 작업인원은 13명으로 94%의 편성효율을 나타내는 시스템으로, 작업시간, 작업인원, 기기를 고려하여 맞춤생산관리가 가능한 혼합형 유니트 생산 시스템을 개발하였다. The standard production process and working time were defined to develop standard production process designed to ensure efficiency in production and also develop new customized clothes production system that improves the shortcomings of individual customized production and mass production systems and incorporate the advantages of both systems with an aim to design efficient baseball uniform pants customized mass production system. This study developed the mixed-type unit production system enabling the customized mass production management, depending on the number of operators and equipment based on the defined standard process. For the configuration of customized mass production of the pants, a total of 50 reference processes were planned by referring to and analyzing the unitary operation among the reference processes of two individual production plants manufacturing the basic baseball uniform pants bonds type and the pants sewing process presented by JUKI Corporation. The operation units were grouped into 4 units based on similar processes that can cope with various production elements by converging the sample-based production system and the multiple variety-small quantity production system suitable for various designs of baseball uniform and characteristics of individual customized production, and a total of 8 devices were set, such as Lock stitch sewing machine, Overlock machine, Interock machine, Top and botton covering sitich machine, Overedge stitch machine, Iron, Button hole machine, Button sewing machine, etc. The number of operators was 13, and the system had 94% formation efficiency. A mixed-type unit production system was developed which enabled the customized production management, considering the working hours, number of operators, and equipment.

N/A Background: There is increasing interest in the use of a very low calorie diet (VLCD) as a treatment regimen for weight loss in obese subjects. This study was designed to investigate the effect s and safety of a traditional Korean very-low-calory diet Methods: Twelve hospitalized obese patients at Dong Eui hospital in Busan city from May 1998 to December 1998 were selected. Height, body weight, blood lipids, blood insulin level, body fat, lean body mass, subcutaneous fat and visceral fat were measured. A traditional Korean VLCD was supplied for 14 days. Results: Patient's body weight was significantly decreased from 83.8 kg to 78.6 kg, body fat from 32.2 kg to 28.6 kg, lean body mass from 32.2 kg to 28.6 kg, and BMI from 31.4 to 29.5, respectively (p <0.05). Total fat was significantly decreased from 807 cc to 659 cc, subcutaneous fat from 567 cc to 473 cc, visceral fat from 273 cc to 185 cc, respectively. However there were no significant changes in minerals. After VLCD intervention, total cholesterol was significantly decreased from 199 mg/dL to 166mg/dL, HDL-cholesterol from 42 mg/dL, to 35mg/dL, triglyceride from 158mg/dL, to 75mg/dL (p<0.05). The insulin area under the curve was also significantly decreased from 205μIU/mL·2hr to 168 μIU/mL·2hr(p <0.05). Conclusion: Above findings suggested that a traditional Korean VLCD is effective and safe for short term use in terms of reducing body fat and improving insulin resistance in obese patients.
