        Endoscopic Findings and Treatment of Gastric Neoplasms in Familial Adenomatous Polyposis

        Purpose Gastric neoplasia is a common manifestation of familial adenomatous polyposis (FAP). This study aimed to elucidate the clinical characteristics, endoscopic features including fundic gland polyposis (FGPsis), and treatment outcomes of gastric neoplasms (GNs) in patients with FAP. Materials and Methods A total of 35 patients diagnosed with FAP, including nine patients from four pedigrees who underwent esophagogastroduodenoscopy (EGD), were investigated regarding patient characteristics, GN morphology, and treatment outcomes. Results Twenty-one patients (60.0%) had 38 GNs; 33 (86.8%) and 5 (13.2%) were histologically diagnosed with adenocarcinoma and adenoma, respectively. There were no specific patient characteristics related to GNs. Nodule-type GNs were more prevalent in patients with FGP than without (52.2% vs. 0.0%, P=0.002) in the upper body of the stomach. Conversely, depressed-type GNs were fewer in patients with FGPsis than in those without (13.0% vs. 73.3%, P<0.001). Slightly elevated-type GNs were observed in both groups (34.8% vs. 20.0%, P=0.538). Even within pedigrees, the background gastric mucosa and types of GNs varied. In total, 24 GNs were treated with endoscopic submucosal dissection (ESD) and eight with endoscopic mucosal resection (EMR). EMR was selected for GNs with FGPsis because of the technical difficulty of ESD, resulting in a lower en bloc resection rate (62.5% vs. 100%, P=0.014). Conclusions Our study indicates the necessity of routine EGD surveillance in patients diagnosed with FAP. Notably, the morphology and location of GNs differed between patients with and without FGPsis. Endoscopic treatment and outcomes require more attention in cases of FGPsis.         Utility of Over-the-Scope Clipping for Closure of a Persistent Post-Percutaneous Endoscopic Gastrostomy Fistula under Long-Term Steroid Therapy

        A 50-year-old woman had a percutaneous endoscopic gastrostomy (PEG) tube placed after surgery for pharyngeal cancer. After 21 months, the PEG tube was removed due to improvement of per-oral ingestion. She had taken prednisolone for 31 years for systemic lupus erythematosus. The post-PEG fistula did not close spontaneously. The cause of the fistula was slow wound healing and gastrostomy site inflammation due to long-term steroid therapy. We were able to close the fistula with an over-the-scope clipping (OTSC) system. This case suggests that OTSC is useful for closing persistent post-PEG fistulas in patients receiving long-term prednisolone therapy.

        Effects of Heat Stress and Dietary Tryptophan on Performance and Plasma Amino Acid Concentrations of Broiler Chickens

        Two experiments were conducted to investigate the impact of high temperature and dietary tryptophan (Trp) on performance, selected organ weights and plasma free amino acid (AA) concentrations in broiler chickens. In Experiment 1, exposure to $27-33^{\circ}C$ of chickens for 2 weeks from 2 weeks of age did not affect growth and plasma free AA concentration except for a decrease in the concentration of plasma tyrosine (Tyr). In Experiment 2, 2-week-old birds were allocated to one of three temperature treatments; $24^{\circ}C$ (control), $36^{\circ}C$ (heat stress, HS) and $24^{\circ}C$ pair-fed (24PF) for 2 weeks and fed on diets containing 50, 100 and 300% of NRC requirement for Trp. Heat stress caused a reduction of weight gain and feed intake irrespective of dietary Trp levels compared with control counterparts, while feeding of 300% Trp diet did not attenuate the reduced performance by HS exposure. In groups fed the 100% Trp diets, plasma aromatic AA (AAA) and Tyr concentrations were decreased in the HS birds compared with the 24PF group. Plasma concentrations in most of AA groups were increased by HS in chickens fed the 50% Trp diet, while those were not changed by HS in chickens fed the 300% Trp diet, compared with 24PF counterparts. The plasma Trp/LNAA (LNAA=large neutral AAs, which are comprised of BCAA, AAA and Trp) ratio was increased by HS in chickens fed the 100% Trp diet, while it was decreased in chickens fed on 50% Trp diet as compared with 24PF group. From these results, it is suggested that performance and plasma amino acid profile deranged by heat stress are modulated, at least, to be relieved from the heat stress by feeding 50% Trp diet but not at all by feeding 300% Trp diet. The involvement of altered plasma AA profiles, in particular plasma Tyr concentrations and Trp/LNAA ratio, is discussed in association with the performance characteristics of HS chickens.

      • Association Analysis of Single Nucleotide Polymorphisms in miR-146a and miR-196a2 on the Prevalence of Cancer in Elderly Japanese: A Case-Control Study

        Background: Single nucleotide polymorphisms (SNPs) affecting microRNA (miR) sequences may influence carcinogenesis. Our current study primarily aimed to confirm previously conducted association studies between rs2910164 found on miR-146a, and rs11614913 located on miR-196a2 polymorphisms and cancer phenotypes in the Japanese elderly population. rs2910164 (G/C) and rs11614913 (T/C) polymorphisms were determined by genotyping on the samples collected from 1,351 consecutive autopsy cases registered in the Japanese SNPs for geriatric research (JG-SNP) data base. Cancer samples were systematically reviewed, pathologically verified and assessed with respect to miR-146a and miR-196a2 genotypic variation. The current study covered 726 males and 625 females with a mean age of $80.3{\pm}8.9$ years. The study included 524 subjects without cancer and 827 subjects with at least one type of cancer, such as gastric (n=160), lung (n=148), colorectal (n=116) or others. Males with cancers (n=467) were more numerous than females (n=360). Both rs11614913 (CT: TT adjusted odds ratio (OR) 95% confidence interval (95%CI)=0.98 (0.75-1.28), p=0.873, CC: TT adjusted OR (95%CI)=1.06 (0.76-1.47), p=0.737, CT+CC: TT, adjusted OR (95%CI)=0.99 (0.77-1.29), p=0.990), and rs2910164 (CG: CC adjusted OR (95%CI)=1.12 (0.87-1.44), p=0.383, GG: CC adjusted OR (95%CI)=1.03 (0.71-1.48), p=0.887, CG+GG: CC adjusted OR (95%CI)=1.10 (0.87-1.39), p=0.446) polymorphisms did not show significant association with overall cancer in all subjects. However, "CC" genotype in rs11614913 polymorphism was significantly associated with increased gastric cancer (n=160) in all subjects (CC: CT+TT, adjusted OR (95%CI)=1.50 (1.02-2.22), p=0.040). We found that rs11614913 and rs2910164 do not pose general cancer risk, but rs11614913 may influence gastric cancer in Japanese elderly population. Confirmation of our study results requires further investigations with larger subject populations.

        Relative Contribution of Physiological Hydrostatic Pressure and Fluid Shear Stress to Endothelial Monolayer Integrity

        Purpose Vascular endothelial cells (ECs) are continuouslysubjected to mechanical forces such as fluid shear stress,stretching and hydrostatic pressure. The effect of hydrostaticpressure on EC responses has not been fully understoodcompared to that of the other two stimuli. The purpose ofthis study is to assess mechanical responses of ECs to thesemechanical stimuli. Methods Bovine aortic ECs were exposed to hydrostaticpressure of 50, 100, and 150 mmHg and fluid shear stressof 3 Pa in simultaneous or successive fashion. Immunofluorescencestaining of actin filaments and VEcadherin wasthen performed to observe cell morphology and cell-celljunctions, respectively. Results The results showed that ECs subjected to 50, 100,and 150 mmHg for 24 h elongated without predominantorientation and exhibited multilayered structure, whereassimultaneous application of 50 and 100 mmHg and 3 Pa for24 h induced marked elongation and orientation of ECsparallel to the direction of flow maintaining monolayerintegrity. This monolayer integrity was lost in ECs subjectedto 150 mmHg together with 3 Pa. A successive applicationof 100 mmHg for 24 h followed by 100 mmHg and 3 Pa for24 h, indicated that the loss of monolayer integrity due tohydrostatic pressure could not be retrieved by the followingsimultaneous application. Conclusions It can be concluded that physiological shearstress of 3 Pa is dominant to physiological hydrostatic pressureup to 100 mmHg, importantly suggesting the relativecontribution of physiological hydrostatic pressure and fluidshear stress to endothelial monolayer integrity.
