Endoscopic Findings and Treatment of Gastric Neoplasms in Familial Adenomatous Polyposis

Purpose Gastric neoplasia is a common manifestation of familial adenomatous polyposis (FAP). This study aimed to elucidate the clinical characteristics, endoscopic features including fundic gland polyposis (FGPsis), and treatment outcomes of gastric neoplasms (GNs) in patients with FAP. Materials and Methods A total of 35 patients diagnosed with FAP, including nine patients from four pedigrees who underwent esophagogastroduodenoscopy (EGD), were investigated regarding patient characteristics, GN morphology, and treatment outcomes. Results Twenty-one patients (60.0%) had 38 GNs; 33 (86.8%) and 5 (13.2%) were histologically diagnosed with adenocarcinoma and adenoma, respectively. There were no specific patient characteristics related to GNs. Nodule-type GNs were more prevalent in patients with FGP than without (52.2% vs. 0.0%, P=0.002) in the upper body of the stomach. Conversely, depressed-type GNs were fewer in patients with FGPsis than in those without (13.0% vs. 73.3%, P<0.001). Slightly elevated-type GNs were observed in both groups (34.8% vs. 20.0%, P=0.538). Even within pedigrees, the background gastric mucosa and types of GNs varied. In total, 24 GNs were treated with endoscopic submucosal dissection (ESD) and eight with endoscopic mucosal resection (EMR). EMR was selected for GNs with FGPsis because of the technical difficulty of ESD, resulting in a lower en bloc resection rate (62.5% vs. 100%, P=0.014). Conclusions Our study indicates the necessity of routine EGD surveillance in patients diagnosed with FAP. Notably, the morphology and location of GNs differed between patients with and without FGPsis. Endoscopic treatment and outcomes require more attention in cases of FGPsis. This study investigated the influences of the addition of lactic acid bacteria inoculants and an enzyme during the crush processing of com silage on the nutritive value, nutrient intake and aerobic deterioration. The fermentation quality of the silages showed that although all of the silages were of good quality, the V-score of U was 91.0 points and significantly lower than those of other silages (P<0.01). On the chemical composition of silages, the crude protein concentration of U was 7.1%DM and significantly lower than those of other silages (P<0.05). On the digestibility, the nutritive value and nutrient intake did not significantly differ among the silages examined. After silo opening, a rapid rise in heat was seen from the 6th after opening for 11C38 and SLA, and from the 9th in U and SL, respectively.
DT Neutronics Benchmark Experiment on Lead at JAEA-FNS

Lead is one of the most important candidate materials for nuclear fusion reactor blankets. We have carried out an integral benchmark experiment on lead at the DT neutron source facility of JAEA, FNS. A cubic lead assembly on a side of 45.3 cm was set up and was irradiated with the DT neutron source. Reaction rates of the ^(27)Al(n,α)^(24)Na, ^(93)Nb(n,2n)^(92m)Nb, ^(90)Zr(n,2n)^(89)Zr, and ^(115)In(n,n')^(115m)In reactions and neutron spectra above 2 MeV were measured inside the assembly with activation foil and a small NE213 spectrometer, respectively. A Monte Carlo code, MCNP5, was adopted to calculate the reaction rates and neutron spectra. The latest nuclear data libraries, JENDL-3.3 ENDF/B-VII.0, JEFF-3.1 and FENDL-2.1, were used in the calculation. The calculation results with the three libraries except for JENDL-3.3 agreed with the measured reaction rates and neutron spectra. On the other hand, that with JENDL-3.3 underestimated the measured ones with the depth. We found out that the inappropriate evaluation of the (n,2n), elastic scattering and inelastic scattering reactions in the lead isotopes of JENDL-3.3 caused the disagreements.

The objective of this study was evaluated the influences of the nitrogen fertilization level on timothy silage fermentation quality, nutritive value, and nutrient intake. In both the first and second crops, high level nitrogen fertilization (H) led to a lower WSC content and higher CP content than standard nitrogen fertilization (S). The silage fermentation quality was good in the presence of a Lactobacillus inoculant (SI and HI) than uninoculant (SC and HC). In the first crop silage, the TDN content and intake were significantly higher (P<0.01) for HI than for SI. In the second silage, the DM, DE, and TDN intakes did not significantly differ among the 4 treatments.

patients and institutions. We are developing a glucose area under the curve (AUC) monitoring system without blood sampling using a minimally invasive interstitial fluid extraction technology (MIET). Here we evaluated the accuracy of interstitial fluid glucose (IG) AUC measured by MIET in patients with diabetes for an extended time interval and the potency of detecting hyperglycemia using CGM data as a reference. Methods: Thirty-eight inpatients with diabetes undergoing CGM were enrolled. MIET comprised a pretreatment step using a plastic microneedle array and glucose accumulation step with a hydrogel patch, which was placed on two sites from 9:00 AM to 5:00 PM or from 10:00 PM to 6:00 AM. IG AUC was calculated by accumulated glucose extracted by hydrogel patches using sodium ion as standard. Results: A significant correlation was observed between the predicted AUC by MIET and CGM in daytime (r=0.76) and nighttime (r=0.82). The optimal cutoff for the IG AUC value of MIET to predict hyperglycemia over 200 mg/dL measured by CGM for 8 hours was 1,067.3 mg·hr/dL with 88.2% sensitivity and 81.5% specificity. Conclusion: We showed that 8-hour IG AUC levels using MIET were valuable in estimating the blood glucose AUC without blood sampling. The results also supported the concept of using this technique for evaluating glucose excursion and for screening hyperglycemia during 8 hours in patients with diabetes at any time of day.

An experiment is proposed to study a novel technique of muon phase rotation for storage rings using pulsed alpha-particle injection from a standard radioactive 241Am source into an ultra-high field gradient RF cavity of a storage ring. The 6-sector PRISM-FFAG ring is specically considered. The specifications of the pulser, in ector and electrodes for guiding the 241Am-source to a sector are described based on simulation studies. The method of RF-particle TOF measurement demonstrates the validity of the proposed method.
