      • Semi-Markov Process를 이용한 MBR의 위치등록 성능 분석

        본 논문에서는 이동기준 위치등록 방식(MBR)의 성능을 수리적으로 계산할 수 있는 정확한 방법을 제안한다. MBR에서는 이동국이 이동임계치 이상의 셀을 통과하면 위치등록을 수행한다. 제안하는 방식의 분석은 발신호를 고려한 MBR인 MBIR을 대상으로, 이전의 수리적 방법과 제안하는 수리적 방법의 비교를 통하여 수행하였다. 수리적 분석 결과, 제안하는 방식은 Semi-Markov Process를 이용하여 이전 연구에서 계산되어지지 않던 MS의 state sojourn time을 반영함으로써 더 정확한 성능이 계산되어짐을 알 수 있었다.

        <P><B>Abstract</B></P> <P>Multistate resistive switching device was demonstrated with organic/inorganic nanocomposite layer of polyvinylphenol (PVP) embedded with zinc oxide nanoparticles (ZnO NPs). Both cap (ITO/ZnO NPs+PVP/Al) and crossbar (Pt/ZnO NPs+PVP/Al) device formats were examined, respectively. Extending range of voltage sweep could show adaptive memory effect, which could roughly emulate analog resistive switching device. In addition, potentiation and depression behaviors were also examined with 50 time consecutive pulsed bias (+2.0V and −2.0V respectively) having 10ms width measured at read voltage of −0.2V. A new possibility of analog resistive switching can be further pursued with the ZnO NPs+PVP nanocomposite.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Objectives: To evaluate the efficacy of the hyaluronic acid (HA)-based microneedle patches containing ascorbic acid, tranexamic acid, niacin amide and resveratrol on melamsa patients. Methods: A twelve week, randomized, double blind study was performed. Twenty four Korean patients with mild to moderate melasma, according to Melasma Area and Severity Scale (MASI) were enrolled and completed the study. Patients used two different types of patches on each side of the designated area every other day for 8 weeks. Patients were requested for an additional 12-week follow up visit. Pigmentary improvements were assessed subjectively with MASI, patient satisfaction score, and photographic analysis. Additionally, quantitative analysis was performed with mexameter and corneometer (Courage & Khazaka, Cologne, Germany). Results: No patient complained of irritation or other side effects regarding the use of microneedle patch. Compared to control area without microneedle and active ingredients only, microneedle patch applied area showed statistically significant clinical improvement in degree of pigmentation. Conclusion: Results indicates that microneedle patch with active ingredients can be used safely and effectively for melasma treatment.

        <P><B>Abstract</B></P> <P>In this study, a resistive switching (RS) memory characteristic of zinc oxide (ZnO) nanoparticles (NPs) multiple layer through a neurotransmitter and its corresponding antibody’s specific binding was demonstrated in Al/ZnO NPs/ITO device format. Particularly, a layer-by-layer (LbL) assembly was enabled through a specific immune binding between a stress hormone, cortisol, and its fittable cortisol monoclonal antibody (c-Mab). The assembly formation was accomplished using sequential LbL deposition, which could accomplish alternating self-assembly monolayers (SAMs) of ZnO NPs as the RS active layer. Here, the current–voltage (I–V) curve exhibited disparate nonvolatile RS characteristics depending on sweep polarity showing consecutively evolved hysteresis. In this study, analog-typed RS function of the ZnO NPs LbL device was demonstrated and characterized.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Lower limb compartment syndrome by reperfusion injury after treatment of arterial thrombosis post-laparoscopic radical hysterectomy and pelvic lymph node dissection for cervical cancer

        Compartment syndrome is a clinical condition associated with decreased blood circulation that can lead to swelling of tissue in limited space. Several factors including lithotomy position, prolonged surgery, intermittent pneumatic compressor, and reperfusion after treatment of arterial thrombosis may contribute to compartment syndrome. However, compartment syndrome rarely occurs after gynecologic surgery. In this case, the patient was diagnosed as compartment syndrome due to reperfusion injury after treatment of arterial thrombosis, which occurred after laparoscopic radical hysterectomy and pelvic lymph node dissection for cervical cancer. Despite its rarity, prevention and identifying the risk factors of complication should be performed perioperatively; furthermore, gynecologist should be aware of the possibility of complications.

      • Impact of different adjuvant treatment approaches on survival in high-risk endometrial carcinoma: a multicenter study

        Objective: The purpose of this study is to compare the oncological outcomes according to adjuvant treatment in patients with high risk endometrial cancer. Methods: A retrospective study was conducted using data between January 1, 2000, and December 31, 2020 at two tertiary centers in Korea (Samsung Medical Center) and Taiwan (National Taiwan University Hospital). Patients with high risk endometrial cancer were eligible. The definition of high risk referred to when final pathology showed grade 3 endometrioid or non-endometrioid with any stages or grade 1-2 endometrioid with stage III or IV. In terms of adjuvant therapy, we categorized it into systemic chemotherapy (CT), radiotherapy (RT), or combination of chemotherapy and radiotherapy (CRT). Propensity score matching was done based on the pathologic stage and cell type after surgery. Results: Total 519 patients with high risk endometrial cancer were eligible in which 176 patients, 200 patients, and 143 patients received RT, CT, and CRT, respectively. In all patient groups, disease free survival and overall survival were statistically significantly higher in the RT group. Advanced stage (stage III-IV) patients who underwent the CRT were improved survival. After propensity score matching, 83 patients in each treatment group were finally analyzed. There was no difference in survival with adjuvant therapy after matching in all patients group, early stage and advanced stage patients. Although statistical significance was not reached after matching, a high survival rate was achieved when CRT was performed in advanced stage patients. CRT was further analyzed by dividing it into concurrent CT and RT, sequential (RT followed by CT, or vice versa), PORTEC3, and Sandwich method, and there was no difference in the survival curve. Conclusion: Although all high-risk endometrial cancer patients were included, survival gains were no different for each stage and treatment group. In stage III and IV groups, it is considered that there is a survival gain in the CRT.

      • Development of a Forest Fire Detection System Using a Drone-based Convolutional Neural Network Model

        Considering forest fires cause environmental destruction, ecosystem collapse, and severe damage to human lives and nature, developing a real-time, accurate, and stable forest fire detection system has become a critical issue in modern society. In this study, a drone-based forest fire detection system was developed using a convolutional neural network (CNN) model. Real-time forest fire detection models were developed using the CNN-based MobileNet algorithm, and their fire detection performance was evaluated. The main research results indicated that errors decreased and accuracy tended to increase during the model training and validation process as training progressed. Moreover, the V1 model exhibited the highest validation accuracy of 0.9466 among the MobileNet V1, V2, and V3 models and showed the highest accuracy of 0.9667 in evaluating the new test dataset during the model evaluation process.

      • The resistance of extensively drug-resistant tuberculosis (XDR-TB) to clofazimine and β-lactam antibiotics

        Background: Drug resistance is still the major threat to global tuberculosis (TB) control, and drug-resistant (DR) Mycobacterium tuberculosis strains have become the main challenge worldwide. Although clofazimine (CFZ) has been traditionally used to treat leprosy, there has recently been interest in the use of CFX (a Group 5 agent) to treat MDR-TB and its use might shorten MDR-TB treatment to 9 month. Meropenem (MPM) and amoxicillin (AMX) are the family of β-lactam antibiotics. M. tuberculosis has traditionally been considered resistant to the β-lactams, there is emergeing evidence that meropenem/clavulanate can have anti-tubercular activity relevant to the treatment of XDR-TB. However, the mechanisms of resistance to these drugs are poorly understood. Therefore, we investigated the molecular basis of clofazimine, meropenem, and amoxicilin resistance using clinical isolates. Methods: We have selected 54 XDR isolates from the TB Specimen Bio-bank, Masan National Tuberculosis Hospital (MNTH). We obtained minimal inhibitory concentration (MIC) of CFZ, MPM, and AMX by resazurin microtiter assay (REMA). Then, whole genome sequencing (WGS) or target sequencing to identify possible mutations associated with CFZ, MPM, and AMX resistance. Results: We found that one of 46 isolates (2.17 %) of CFZ-R mutant, 86.96 % (40/46) of MPM-R, and 93.48 % (43/46) of AMX-R mutants. Particularly, CFZ-R mutant had mutation in gene. Conclusions: Mutations in Rv0678 are a major mechanism of CFZ resistance. Although we were not detected previously reported mutation region of CFZ-R, we detected minor mutation region. Our findings expect useful information for rapid detection of drug-resistance or mechanism of clofazimine resistance.

      • Pre-treatment neutrophil-to-lymphocyte ratio is associated with improved overall survival in patients with gynecological cancer treated with immunotherapy

        Objective: The purpose of this study was to determine whether the pretreatment neutrophil-lymphocyte ratio at the time of initiation of immune checkpoint inhibitors (ICIs) in patients with recurrent gynecological cancer has an effect on progression free survival (PFS) and overall survival (OS). Methods: A retrospective, single-center cohort study was conducted using data between March 1, 2002, and February 18, 2021 at Samsung Comprehensive Cancer Center, Seoul. We identified patients who received ICIs for recurrent gynecological cancer (ovarian, endometrial, cervical, and other cancers). The absolute serum neutrophil and lymphocyte ratio (NLR) was calculated from the complete blood count laboratory results performed before the first cycle of ICIs. Based on the median value of the NLR ratio, analysis was performed by dividing into 2 groups as high or low NLR. Results: Among 250 patients, 207 patients were included in the analysis, excluding patients with unknown pre-treatment NLR ratio or follow-up loss. 103 patients were assigned to the group with a low NLR, and 104 patients were assigned to the group with a high NLR ratio. There were statistically significant differences between the two groups in terms of body weight, BMI, prior radiation therapy, ECOG, total number of ICIs, and prior antibiotics use before ICIs (p<0.05). In the univariate analysis according to NLR, the Kaplan-Meier curve showed significantly longer overall survival (OS) in patients with low NLR compared with high NLR (p=0.17). In Cox multivariable analysis, NLR, ECOG, and total number of ICIs were found to affect Especially, low NLR group demonstrated survival gain about 3 times compared to the high group (HR 3.26; 95% CI 1.89 5.62; p<0.001). Conclusion: Pre-treatment high NLR showed significant improved. OS in patients with ICIs for recurrent gynecological cancer. It is thought that pretreatment NLR can be used as a predictive marker to predict survival after ICIs in patients with recurrent gynecological cancer.
