      • Disorders of Liver : Is Endoscopic Variceal Ligation Effective For The Treatment Of Bleeding From Gastroesophageal Varices (GOV)?

        Background/Aims: The optimal management of bleeding from gastroesophageal varices (GOV) remains controversial. The aim of this study was to investigate whether endoseopic variceal ligation (EVL) is effective and safe for the management of bleeding from GOV. Methods: Forty-one cirrhotic patients receiving EVL due to a history of variceal bleeding from GOV were analyzed. In twenty-two patients of them, serial endoseopic ultrasonography (EUS) were performed. GOV were grade using the system suggested by Sarin et al: type I (GOVI) and type 2 (GOV2). Results: Twenty-eight patients were GOVI and thirteen patients were GOV2. Successful treatment for controlling acute bleeding were achieved in 26/28 (93%) in GOVI and 9/13 (85%) in GOV2. Rebleeding was 15% in GOVI, while 40% in GOV2. Multivariate Cox regression indicated that type of GOV, variceal size and residual variceal size after EVL were independent factors predictive of GOV re-bleeding. Serial EUS findings demonstrated that the size of GOVI became decreased after EVL in 10/15 (67%) patients, while GOV2 did not change. Conclusions: EVL was safe and effective for the emergency treatment of acutely variceal bleedings from GOV. For the preventing variceal rebleeding, EVL was more effective in GOVI than GOV2.

      • The clinical manifestations of stasis dermatitis and analysis of association with other vascular diseases

        Background: Stasis dermatitis is a common dermatologic disorder. It is caused by venous hypertension results from venous backward flow. However, research on the association with specific vascular disease is insufficient. Objectives: This study was intended to investigate in clinical manifestations of stasis dermatitis and analysis of association with other vascular diseases in our hospital outpatients. Methods: In this study, we investigated 37 patients with a diagnosis of stasis dermatitis in our hospital from 2014 to 2019. We reviewed clinical manifestations and relation with other vascular disease. Results: In total 37 cases, 20 (54.1%) patients were female and 17 (45.9%) patients were male. Twelve two (59.5%) patients were presented as erythema, followed by 14 (37.8%) as purpuric lesions and 11 (29.7%) as brownish changes. There were 4 (10.8%) patients showed crust, 3 (8.1%) as edema, 2 (5.4%) as scale, 2 (5.4%) as ulcer. For symptoms, 15 (40.5%) patients complained itching, 4 (10.8%) as tenderness, 3 (8.1%) as pain, 1 (2.7%). In relation with other vascular disease, 16 (43.2%) patients represented with vein related disease such as deep vein thrombosis, varicose vein and thrombophlebitis. However, there was no arterial disease such as arteriosclerosis obliterans. Conclusion: The result shows clinical manifestations of stasis dermatitis and association with other specific vascular diseases. By understanding these findings, it is expected that better treatment outcome will be obtained.

      • Nitrogen - incorporated (Ba, Sr)TiO₃ thin films fabricated by r.f. - magnetron sputtering

        In this study, two kinds of barium strontium titanate (BST) samples were prepared. One is a conventional BST film that is sputtered in a mixture of argon and oxygen. The other is a nitrogen-incorporated BST film that is sputtered in a mixture of oxygen and intentionally added nitrogen instead of argon gas. The structural properties of both of the BST films had not changed significantly with the species of sputtering gas. However, the leakage current of BST films sputtered at (N₂+O₂) atmosphere was lower than those sputtered at (Ar +O₂) atmosphere: 1.9×10^(-8) A/㎤ at 2V for the films prepared at (Ar +O₂) atmosphere and 8.6×10^(-9) A/㎤ for the films at (N₂+O₂) atmosphere. From an XPS analysis, it has been found that nitrogen atoms are incorporated in BST films with a concentration of 1.92 at% and form a certain oxynitride phase. It is proposed that nitrogen atoms are able to fill the oxygen vacancies of BST films during sputtering process, and then the leakage current reduces due to a decrease in the vacancies. The BST films sputtered at (N₂+ O₂) atmosphere have superior electrical properties to the films sputtered at (Ar +O₂), without any significant structural changes.

        위용종의 내시경적 생검과 내시경적 절제술 후 병리학적 진단의 불일치

        목적 : 위용종이란 위의 내강으로 돌출한 종괴를 의미한다. 거의 90%에서 조직학적으로 과형성 용종이며 이러한 경우 암으로의 진행은 매우 적은 것으로 알려져 있다. 그러나 위선종은 일부에서 국소적으로 암성 병변을 동반하거나, 동반하고 있지 않더라도 시간이 지남에 따라 암으로 진행하는 것으로 알려져 있다. 그러나 내시경적 겸자 생검과 전체 병변의 절제 후 조직 소견에서는 차이가 있는 것으로 알려져 있다. 저자들은 내시경검사에서 위용종이 발견된 후 내시경적 절제술을 시행받은 74명의 환자, 85예의 위용종을 대상으로 임상적 및 병리학적 특성을 알아보고 겸자 생검과 내시경적 절제술 후 병리학적 진단의 불일치를 알아보고자 하였다. 방법 : 1996년 4월부터 2003년 2월까지 내시경검사에서 위용종이 발견된 후 내시경적 생검과 내시경적 절제술 후 조직학적 진단이 모두 이루어진 74명의 환자를 대상으로 내시경 사진, 의무 기록 및 병리 기록을 분석하였다. 결과 : 74명의 환자에서 총 85개의 용종을 관찰할 수 있었다. 위용종은 전정부에서 가장 호발하였으며(58.8%) 그 다음으로는 체하부(18.8%)에 호발하였다. 내시경적 겸자 생검에서 관상 선종은 48.2%, 과형성 용종은 30.6%, 염증성 용종은 15.3%, 위저선 용종은 2.4%, 이형성증은 2.4%, 관상융모 선종은 1.1%였다. 내시경적 절제술 후 조직학적 검사상 관상 선종은 45.9%, 과형성 용종은 31.8%, 염증성 용종은 9.4%, 과오종은 3.5%, 위저선 용종은 2.4%, 관상융모 선종은 2.4%, 선암은 2.4%, 이형성증은 1.1%, 점막 위지방종증은 1.1%였다. 내시경적 겸자 생검 결과와 내시경적 절제술 후 조직학적 진단의 불일치율은 27.1%였다. 용종의 크기에 따른 진단 일치율의 차이는 없었다(p>0.05). 결론 : 위용종의 겸자 생검과 내시경적 절제술 후 조직검사 소견은 차이가 있을 수 있다. 용종의 크기는 진단 일치율에 영향을 미치지 못했다. 따라서 특히 선종이 의심되는 경우에는 크기에 상관없이 내시경적 절제술을 통한 전체 병변의 조직검사가 필요할 것으로 생각된다. Background : Gastric polyp is a descriptive term referring to mucosal prominence that protrudes beyond the flat lining of the stomach. Almost 90% of gastric polyps are hyperplastic polyps. Adenomatous polyps may contain focal carcinomatous foci or undergo carcinomatous changes. It is known that there is some degree of discordance between the results of endoscopic forceps biopsy and pathology of resected specimens. The aim of this study was to investigate the discordance in pathologic findings between endoscopic forceps biopsy and endoscopic resection specimen of gastric polyps. Methods : We reviewed endoscopic photographs and medical records of the patients who underwent endoscopic resection from April, 1996 through February, 2003. Results : A total of 85 cases of gastric polyps from 74 patients were reviewed. Male-to-female ratio was 1:1.96. Mean age was 59.9±10.8 years. Multiple polyps were observed in 10.8%. Gastric polyps occurred in the antrum most frequently (58.8%). Pathology results on resected specimens were as follows: tubular adenoma 45.9%, hyperplastic polyp 31.8%, inflammatory polyp 9.4%, hamartoma 3.5%, fundic gland polyp 2.4%, tubulovillous adenoma 2.4%, adenocarcinoma 2.4%, dysplasia 1.1%, and mucosal pseudolipomatosis 1.1%. Discordance rate between endoscopic biopsy and pathology of resected specimens was 27.1%. There was no relationship between the size of the polyp and concordance rate. Conclusions : There was considerable discordance in pathologic findings between endoscopic forceps biopsy and resected specimens. Approaches to review the histology of an entire polyp should be performed, especially when an adenoma is suspected.(Korean J Med 69:481-486, 2005)

        Comparison of changes in retentive force of three stud attachments for implant overdentures

        PURPOSE The aim of this study was to compare the changes in retentive force of stud attachments for implant overdentures by in vitro 2-year-wear simulation. MATERIALS AND METHODS Three commercially available attachment systems were investigated: Kerator blue, O-ring red, and EZ lock. Two implant fixtures were embedded in parallel in each custom base mounting. Five pairs of each attachment system were tested. A universal testing machine was used to measure the retentive force during 2500 insertion and removal cycles. Surface changes on the components were evaluated by scanning electron microscopy (SEM). A Kruskal-Wallis test, followed by Pairwise comparison, was used to compare the retentive force between the groups, and to determine groups that were significantly different (α<.05). RESULTS A comparison of the initial retentive force revealed the highest value for Kerator, followed by the O-ring and EZ lock attachments. However, no significant difference was detected between Kerator and O-ring (P>.05). After 2500 insertion and removal cycles, the highest retention loss was recorded for O-ring, and no significant difference between Kerator and EZ lock (P>.05). Also, Kerator showed the highest retentive force, followed by EZ lock and O-ring, after 2500 cycles (P<.05). Based on SEM analysis, the polymeric components in O-ring and Kerator were observed to exhibit surface wear and deformation. CONCLUSION After 2500 insertion and removal cycles, all attachments exhibited significant loss in retention. Mechanism of retention loss can only be partially explained by surface changes.

        이산화탄소 포집 및 저장기술(Carbon dioxide Capture and Storage, CCS)이 기후변화 대응 기술로서 국내외적으로 주목 받고 있다. CCS 기술은 발전소 및 제철소 등 대규모 발생원에 서 배출되는 CO2를 포집, 파이프라인이나 선박 등을 통해 수송, 그리고 이를 지중에 장기간 저장하는 저장공정으로 이루어진다. 본 연구에서는 CO2 해양지중저장 실용화를 위한 CO2 물성 특성, 수송·주입 공정설계, 선박 수송 안전성, 중간저장 개념설계 그리고 저류층 내 CO2 거동 안전성 평가를 위한 탄성파 모니터링 기술을 개발하고 있다. 본 논문에서는 이에 대한 연구내용을 정리하고 주요 결과를 소개하고자 한다. Carbon dioxide Capture and Storage(CCS) is regarded as one of the most promising options to response climate change. CCS is a three-stage process consisting of the capture of carbon dioxide, the transport of CO2 to a storage location, and the long term isolation of CO2 from the atmosphere for the purpose of carbon emission mitigation. To carry out demonstration and commercialization of CO2 marine geological storage, property analysis focused on the hydrate phase equilibrium, process design of transport and injection, safety analysis of CO2 carrier, design of offshore intermediate storage facility and seismic wave propagation modeling were studied. In this paper, major results of these studies were introduced and summarized.

      • A survey of counseling and behavior regarding skin cancer

        Background: As the incidence of skin cancer increases, more information and education on skin cancer are needed. Although skin cancer is not rare, most patients are little known about general awareness of skin cancer. Objectives: The purpose of this study is to investigate the post-education effects on skin cancer in our hospital outpaitents and carers. Methods: We reviewed a written questionnaire and patients’ self-checked records including patients’ basic medical information and knowledge of skin cancer protective modalities. Results: In total 340 cases, 215 (63.2%) patients were female and 125 (36.8%) patients were male. 6 people in their teens, 56 people in their 20s and 30s, 156 people in their 40s and 50s, 122 people in their 60s or older. Before education, 142 people said sunlight can cause a skin cancer and 178 did not. After education, 246 people said Yes and 94 people did not. When asked how you wanted to get more information on skin cancer, 104 (30.6%) people said that they were printed handout, 142 (41.8%) were visit to dermatologists, 52 (15.3%) were photos of skin symptoms, and 42 (12.3%) were phone calls. Conclusion: This result showed that people are little known about risk factor of skin cancer such as sunlight and artificial tanning. However, we found that through education, people's perceptions could change. The education on behavior for skin cancer prevention should be continued so that the awareness of the risk of skin cancer can be changed.
