        A hypertension risk score for adults: a population-based cross-sectional study from Dubai Household Survey 2019

        OBJECTIVES: The aim of this study was to develop a risk score model for predicting hypertension specific to the population of Dubai in the United Arab Emirates (UAE) to facilitate prevention and early intervention. METHODS: A retrospective analysis of data from the Dubai Household Health Survey 2019 was conducted. Demographic and physical parameters, as well as blood glucose levels, were included in the data. The risk factors for hypertension were identified using bivariate analysis. A risk score model was developed using the enter method, where all significant predictors of hypertension in bivariate analyses were entered in a single step with the primary outcome of hypertension status (yes/no). The model was validated internally by splitting the data into Emirati and non-Emirati populations. RESULTS: A total of 2,533 subjects were studied. The significant risk factors for hypertension identified were male sex, older age (≥ 40 years), education level, body mass index, diabetes mellitus, and dyslipidaemia. The model showed a high discrimination ability between individuals with and without hypertension, with an area under the curve of 0.77 (95% confidence interval [CI], 0.75 to 0.79), excellent sensitivity (81.0%; 95% CI, 71.9 to 88.2) and moderate specificity (56.0%; 95% CI, 45.7 to 65.9). CONCLUSIONS: The model developed by this study is simple, convenient, and based on readily available demographic and medical characteristics. This risk score model could support initial hypertension screening and provide an effective tool for targeted lifestyle counselling and prevention programs.

      • Effects of Tobacco Smoking on the Dorsum of the Tongue and Buccal Epithelium

        Objective: The aim of this study was to assess the effects of tobacco smoking on the dorsum of the tongue and buccal epithelium. Methodology: This case control cross-sectional study was conducted with 174 smoking and non-smoking volunteers living in the city of Hail, Northern KSA. Cytological Materials were obtained from buccal mucosa and dorsum of the tongue, and assessed using cytopathological methods. Results: In buccal smears, cytological atypia was observed in 17 out of 101 (16.8%) smoker cases but only 3/73(4.1%) of the controls. For cytological atypia in buccal and tongue smears, the adjusted odd ratio (OR) and the 95% confidence interval (CI) were found to be 4.7 (1.3-16.8), P < 0.016)) and 4.3 (0.93- 20.2), P <0.06)), respectively, in the two sites. Conclusion: Tobacco smoking is a major risk factor for occurrence of cytological atypia, which might subsequently develop into oral precancerous and cancerous lesions. Oral exfoliative cytology is an easy and cheap non-invasive procedure which appears highly suitable for screening populations at risk of developing oral cancer.

        Herbivore and phytohormone induced defensive response in kale against cabbage butterfly, Pieris brassicae Linn

        The constitutive and induced resistance were studied in two varieties (Khanyari and Kawdari) of kale, Brassicaoleracea var. acephala in response to cabbage butterfly, Pieris brassicae infestation and exogenous application ofjasmonic acid (JA) and salicylic acid (SA). Phenols, condensed tannins, flavonoids and proteins were measuredat six days after JA (1 mM) and SA (1 mM) application and/or insect infestation. Plant damage and larvalweights were also recorded. Khanyari variety showed highest amounts of phenols (208.23 μg/g FW), condensedtannin (347.76 μg/g FW), flavonoids (175.61 μg/g FW) and proteins (0.71 mg/g FW) in plants pre-treated withJA and infested with insects. The PAL activity was high in response to JA application followed by insect infestation. Insects reared on Khanyari and Kawdari plants pre-treated with JA prior showed significantly reducedlarval weights (97.88 and 102.46 mg, respectively). Damage was low in plants pre-treated with JA in Khanyari at3, 6 and 9 days after treatment (10.52%, 8.52%, and 5.30%, respectively). Thus, JA can play an important role inplant defense in kale against P. brassicae.

        Augmented Reality to Improve Surgical Workflow in Minimally Invasive Transforaminal Lumbar Interbody Fusion – A Feasibility Study With Case Series

        Objective: Minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) is a highly reproducible procedure for the fusion of spinal segments. We recently introduced the concept of “total navigation” to improve workflow and eliminate fluoroscopy. Imageguided surgery incorporating augmented reality (AR) may further facilitate workflow. In this study, we developed and evaluated a protocol to integrate AR into the workflow of MISTLIF. Methods: A case series of 10 patients was the basis for the evaluation of a protocol to facilitate tubular MIS-TLIF by the application of AR. Surgical TLIF landmarks were marked on a preoperative computed tomography (CT)-scan using dedicated software. This marked CT scan was fused intraoperatively with the low-dose navigation CT scan using elastic image fusion, and the markers were transferred to the intraoperative scan. Our experience with this workflow and the surgical outcomes were collected. Results: Our AR protocol was safely implemented in all cases. The TLIF landmarks could be preoperatively planned and transferred to the intraoperative imaging. Of the 10 cases, 1 case had additionally a synovial cyst resection and in 2 cases an additional bony decompression was performed due to central stenosis. The average procedure time was 160.6 ± 31.9 minutes. The AR implementation added 1.72 ± 0.37 minutes to the overall procedure time. No complications occurred. Conclusion: Our findings support the idea that total navigation with AR may further facilitate the workflow, especially in cases with more complex anatomy and for teaching and training purposes. More work is needed to simplify the software and make AR integration more user-friendly.

        Safety and Feasibility of Augmented Reality Assistance in Minimally Invasive and Open Resection of Benign Intradural Extramedullary Tumors

        Objective: Surgical resection of benign intradural extramedullary tumors (BIETs) is effective for appropriately selected patients. Minimally invasive surgical (MIS) techniques have been described for successful resection of BIET while minimizing soft tissue injury. Augmented reality (AR) is a promising new technology that can accurately allow for intraoperative localization from skin through the intradural compartment. We present a case series evaluating the timing, steps, and accuracy at which this technology is able to enhance BIET resection. Methods: A protocol for MIS and open AR-guided BIET resection was developed and applied to determine the feasibility. The tumor is marked on diagnostic magnetic resonance imaging (MRI) using AR software. Intraoperatively, the planning MRI is fused with the intraoperative computed tomography. The position and size of the tumor is projected into the surgical microscope and directly into the surgeon's field of view. Intraoperative orientation is performed exclusively via navigation and AR projection. Demographic and perioperative factors were collected. Results: Eight patients were enrolled. The average operative time for MIS cases was 128 ± 8 minutes and for open cases 206 ± 55 minutes. The estimated intraoperative blood loss was 97 ± 77 mL in MIS and 240 ± 206 mL in open procedures. AR tumor location and margins were considered sufficiently precise by the surgeon in every case. Neither correction of the approach trajectory nor ultrasound assistance to localize the tumor were necessary in any case. No intraoperative complications were observed. Conclusion: Current findings suggest that AR may be a feasible technique for tumor localization in the MIS and open resection of benign spinal extramedullary tumors.

        EEIRI: Efficient Encrypted Image Retrieval in IoT-Cloud

        One of the best means to safeguard the confidentiality, security, and privacy of an image within the IoT-Cloud is through encryption. However, looking through encrypted data is a difficult process. Several techniques for searching encrypted data have been devised, but certain security solutions may not be used in IoT-Cloud because such solutions are not lightweight. We propose a lightweight scheme that can perform a content-based search of encrypted images, namely EEIRI. In this scheme, the images are represented using local features. We develop and validate a secure scheme for measuring the Euclidean distance between two descriptor sets. To improve the search efficiency, we employ the k-means clustering technique to construct a searchable tree-based index. Our index construction process ensures the privacy of the stored data and search requests. When compared with more familiar techniques of searching images over plaintexts, EEIRI is considered to be more efficient, demonstrating a higher search cost of 7% and a decrease in search accuracy of 1.7%. Numerous empirical investigations are carried out in relation to real image collections so as to evidence our work.

        Initiation, proliferation, and improvement of a micropropagation system for mass clonal production of banana through shoot-tip culture

        The present study aimed to evaluate the initiation, proliferation potential, and mass clonal production ability of a micropropagation system for banana through tissue culture. A total of 60 explants were cultured on basal media supplemented with various concentrations of BAP and NAA. Banana plants regenerated on MS basal medium (control) without the addition of BAP + NAA showed a significantly (P < 0.05) lower survival rate with no signs of shoots up to the end of the experimental period. The results further revealed that the performance in MSS-XI medium was almost 89%, followed by MSS-IX and MSS-X media, both of which showed performance up to 88%. In contrast, the performance in the MSS-XVI medium was less than 60%, at the less duration of time and highly shoot induction detected at MSS-XIII medium. The maximum number of shoots (4.9) was observed in the medium supplemented with growth adjuster MSS-XI, followed by the MSS-XII medium (4.5). Surprisingly, the best performance was observed for the MSR-VII medium approximately 16 days after initiation, while the lowest performance was observed with MSR-XI (approximately 31 days). The maximum rooting percentage (98%) was observed in the MSR-V to MSR-VIII media (98%), while the minimum rooting percentage was observed in MSR-XI (approximately 45%)

        Comparison of apical extrusion of intracanal bacteria by various glide-path establishing systems: an in vitro study

        Objectives: This study compared the amount of apically extruded bacteria during the glide-path preparation by using multi-file and single-file glide-path establishing nickel-titanium (NiTi) rotary systems. Materials and Methods: Sixty mandibular first molar teeth were used to prepare the test apparatus. They were decoronated, blocked into glass vials, sterilized in ethylene oxide gas, infected with a pure culture of Enterococcus faecalis, randomly assigned to 5 experimental groups, and then prepared using manual stainless-steel files (group KF) and glide-path establishing NiTi rotary files (group PF with PathFiles, group GF with G-Files, group PG with ProGlider, and group OG with One G). At the end of canal preparation, 0.01 mL NaCl solution was taken from the experimental vials. The suspension was plated on brain heart infusion agar and colonies of bacteria were counted, and the results were given as number of colony-forming units (CFU). Results: The manual instrumentation technique tested in group KF extruded the highest number of bacteria compared to the other 4 groups (p < 0.05). The 4 groups using rotary glide-path establishing instruments extruded similar amounts of bacteria. Conclusions: All glide-path establishment instrument systems tested caused a measurable apical extrusion of bacteria. The manual glide-path preparation showed the highest number of bacteria extruded compared to the other NiTi glide-path establishing instruments.

      • Quantification of Her-2/Neu Gene in Breast Cancer Patients using Real Time-Polymerase Chain Reaction (Q-PCR) and Correlation with Immunohistochemistry Findings

        Background: HER-2/neu is a proto-oncogene that encodes a transmembrane tyrosine kinase growth factor which is crucial for stimulating growth and cellular motility. Overexpression of HER-2/neu is observed in 10-35% of human breast cancers and is associated with pathogenesis, prognosis as well as response to therapy. Given the imperative role of HER-2/neu overexpression in breast cancer, it is important to determine the magnitude of amplification which may facilitate a better prognosis as well as personalized therapy in affected patients. In this study, we determined HER-2/neu protein expression by immunohistochemistry (IHC) concurrently with HER-2/neu DNA amplification by quantitative real time-polymerase chain reaction (Q-PCR). Materials and Methods: A total of 53 paired tissue samples from breast cancer patients were frozen-sectioned to characterize the tumour and normal tissues. Only tissues with 80% tumour cells were used in this study. For confirmation, Q-PCR was used to determine the HER-2/neu DNA amplification. Results: We found 20/53 (37.7%) of the tumour tissues to be positive for HER-2/neu protein overexpression using IHC. Out of these twenty, only 9/53 (17%) cases were in agreement with the Q-PCR results. The concordance rate between IHC and Q-PCR was 79.3%. Approximately 20.7% of positive IHC cases showed no HER-2/neu gene amplification using Q-PCR. Conclusion: In conclusion, IHC can be used as an initial screening method for detection of the HER-2/neu protein overexpression. Techniques such as Q-PCR should be employed to verify the IHC results for uncertain cases as well as determination of HER-2/neu gene amplification.

        Carbon nanotubes loaded N,S-codoped TiO2: Heterojunction assembly for enhanced integrated adsorptive-photocatalytic performance

        A visible-light active N,S-codoped TiO2 incorporated with carbon nanotubes (CNTs) was synthesized by amulticomponent heterojunction scheme. The synthesized photocatalysts was examined by several analysistechniques. The conventional TiO2, N-doped TiO2, TiO2/CNT and N,S-codoped TiO2 were also synthesized,characterized, and integrated adsorptive-photocatalytic degradation rates were subsequentlycompared and assessed, for comparative purposes as reference photocatalysts. The parametric studywas also carried out to investigate the effect of various operational factors, such as solution pH, the photocatalystloading and initial concentration of dye solution, on the adsorptive-photocatalytic performance. The best activity performance was attained with the N,S-TiO2/CNTs nanocomposite due to thesynergistic effect of ternary components caused by the incorporation of dopants and CNTs in TiO2 lattice,providing a high number of adsorption sites and facilitating the effective separation of photogeneratedcharge carriers and minimal recombination rates. N,S-TiO2/CNTs exhibited around three times higherphotocatalytic performance than that of the binary composites, i.e., N-TiO2, TiO2/CNTs, N,S-TiO2, andabout four times higher than the conventional TiO2. Furthermore, the superior nanocomposites were alsoinvestigated for their recyclability for five consecutive cycles.
