      • Single-Port Robotic Cholecystectomy: Early Experience from 8 Cases

        Aims: Single-port laparoscopic cholecystectomy (SPLC) is a technical concept to reduced pain and improve cosmetic results when compared to multi-port cholecystectomy. However, SPLC is associated with technical limitation due to the enhanced complexity of the approach and limited number of specialized instruments or platforms. On the other hand, using a robotic platform may overcome these problems and enable more precise surgical actions by increasing freedom of movement and by restoring intuitive instrument control. In this presentation, we report the early clinical experience of our first 8 sing-port robotic cholecystectomy (SPRC) cases. Methods: Between November 2016 and February 2017, eight patients underwent SPRC with the da Vinci Xi robot and single-site instrumentation. We retrospectively reviewed clinical data on those patients. Results: All of 8 patients had completion of SPRC. Seven patients were female and one was male. Average patient age was 43.3±11.8 years and BMI was 22.4±1.4 kg/m2. Three patients (37.5%) were diagnosed with chronic calculous cholecystitis. Three patients (37.5%) underwent operation for polypoid lesions of the gallbladder. One patient (12.5%) was diagnosed with acute calculous cholecystitis. The mean operation time (skin-to-skin) was 83.9±30.7 min, docking time was 13.25±8.1 min, and console time was 42.1±26.4 min. The intraoperative blood loss was negligible. The mean Visual Analog Pain Scale score 6hr after the surgery was 2.9±0.4. The mean length of hospital stay average postoperative hospital stay was 2.3±1.0 day. There were no intraoperative complication and one patient developed seroma on port site. Conclusions: Robotic single-port cholecystectomy appears feasible and safe in our early experience.

        Generative Adversarial Network-Based Image Conversion Among Different Computed Tomography Protocols and Vendors: Effects on Accuracy and Variability in Quantifying Regional Disease Patterns of Interstitial Lung Disease

        Objective: To assess whether computed tomography (CT) conversion across different scan parameters and manufacturers using a routable generative adversarial network (RouteGAN) can improve the accuracy and variability in quantifying interstitial lung disease (ILD) using a deep learning-based automated software. Materials and Methods: This study included patients with ILD who underwent thin-section CT. Unmatched CT images obtained using scanners from four manufacturers (vendors A-D), standard- or low-radiation doses, and sharp or medium kernels were classified into groups 1–7 according to acquisition conditions. CT images in groups 2–7 were converted into the target CT style (Group 1: vendor A, standard dose, and sharp kernel) using a RouteGAN. ILD was quantified on original and converted CT images using a deep learning-based software (Aview, Coreline Soft). The accuracy of quantification was analyzed using the dice similarity coefficient (DSC) and pixel-wise overlap accuracy metrics against manual quantification by a radiologist. Five radiologists evaluated quantification accuracy using a 10-point visual scoring system. Results: Three hundred and fifty CT slices from 150 patients (mean age: 67.6 ± 10.7 years; 56 females) were included. The overlap accuracies for quantifying total abnormalities in groups 2–7 improved after CT conversion (original vs. converted: 0.63 vs. 0.68 for DSC, 0.66 vs. 0.70 for pixel-wise recall, and 0.68 vs. 0.73 for pixel-wise precision; P < 0.002 for all). The DSCs of fibrosis score, honeycombing, and reticulation significantly increased after CT conversion (0.32 vs. 0.64, 0.19 vs. 0.47, and 0.23 vs. 0.54, P < 0.002 for all), whereas those of ground-glass opacity, consolidation, and emphysema did not change significantly or decreased slightly. The radiologists’ scores were significantly higher (P < 0.001) and less variable on converted CT. Conclusion: CT conversion using a RouteGAN can improve the accuracy and variability of CT images obtained using different scan parameters and manufacturers in deep learning-based quantification of ILD.

      • The role of second alkali or alkaline-earth ions in LAS glass-ceramics

        LAS glass-ceramics show very narrow compositional range with transparency and low thermal expansion. The relatively high melting temperature of parent glass limits wide range of applications. The addition of second alkali or alkaline-earth ions into the LAS glass composition was attempted and the effects on the melting, phase containing Ba²? showed transparency after crystallization as well as reduced melting temperature. The transparency of this glass-ceramic was discussed in terms of ionic field strength and ionic size.

        In Korea Maritime University facilities, environment, education and awareness about the Green Campus has been studied in the field. Based on this, domestic and foreign universities and the benchmarking of the environmental characteristics of our university campus environment, the composition of the plan were considered. In this study, based on what our universities have links with the community on what was discussed a lot about. In our university campus tour aimed at young people plan green, and sustainable management measures have been studied.

        Predictors of upper trapezius pain with myofascial trigger points in food service workers : The STROBE study

        <P><B>Abstract</B></P><P>Shoulder pain occurs commonly in food service workers (FSWs) who repetitively perform motions of the upper limbs. Myofascial trigger points (MTrPs) on the upper trapezius (UT) are among the most common musculoskeletal shoulder pain syndromes. This study determined the psychological, posture, mobility, and strength factors associated with pain severity in FSWs with UT pain due to MTrPs.</P><P>In this cross-sectional study, we measured 17 variables in 163 FSWs with UT pain due to MTrPs: a visual analog scale (VAS) pain score, age, sex, Borg rating of perceived exertion (BRPE) scale, beck depression inventory, forward head posture angle, rounded shoulder angle (RSA), shoulder slope angle, scapular downward rotation ratio, cervical lateral-bending side difference angle, cervical rotation side difference angle, glenohumeral internal rotation angle, shoulder horizontal adduction angle, serratus anterior (SA) strength, lower trapezius (LT) strength, bicep strength, and glenohumeral external rotator strength, in 163 FSWs with UT pain due to MTrPs.</P><P>The model for factors influencing UT pain with MTrPs included SA strength, age, BRPE, LT strength, and RSA as predictor variables that accounted for 68.7% of the variance in VAS (<I>P</I> < .001) in multiple regression models with a stepwise selection procedure. The following were independent variables influencing the VAS in the order of standardized coefficients: SA strength (β = −0.380), age (β = 0.287), BRPE (β = 0.239), LT strength (β = −0.195), and RSA (β = 0.125).</P><P>SA strength, age, BRPE, LT strength, and RSA variables should be considered when evaluating and intervening in UT pain with MTrPs in FSWs.</P>

        Comparison of the Thickness of the Neck Flexor Muscles of Subjects With and Without a Forward Head Posture on the Two Initial Head Positions During Cranio-Cervical Flexion Exercise

        This study compared the effects of the initial head position (i.e., a HHP versus a relaxed head position) of subjects with and without a FHP on the thickness of the deep and superficial neck flexor muscles during CCF. The study recruited 6 subjects with a FHP and 10 subjects without a FHP. The subjects performed CCF in two different head positions: a HHP, with the head aligned so that the forehead and chin formed a horizontal line, and a relaxed head position (RHP), with the head aligned in a self-selected comfortable position. During the CCF exercise, the thickness of the longus colli (LCo) and the thickness of the sternocleidomastoid (SCM) were recorded using ultrasonography. The thickness of each muscle was measured by Image J software. The statistical analysis was performed with a two-way mixed-model analysis of variance. The thickness of the SCM differed significantly (p<.05) between the subjects with and without FHP. According to a post hoc independent t-test, the change in thickness of the SCM increased significantly during CCF in the subjects with FHP while adopting a HHP compared to that in the subjects without FHP. The change in thickness of the SCM was not significantly different between the two positions in subjects without FHP, and there was no significant change in thickness of the LCo muscle during the CCF exercise according to the initial position in both subjects with and without FHP. The results suggest that CCF should be performed in RHP to minimize contraction of the SCM in subjects with a FHP.

        Emulsion Mapping in Pork Meat Emulsion Systems with Various Lipid Types and Brown Rice Fiber

        This study was conducted to evaluate emulsion mapping between emulsion stability and cooking yields, apparent viscosity, and hardness of reduced-fat pork emulsion systems. The reduced-fat emulsion systems were supplemented with different lipid types and brown rice bran fiber (BRF) concentrations. Compared to the control with 30% back fat, lower emulsion stability and higher cooking yield of meat emulsion systems were observed in T1 (30% back fat+1% BRF), T2 (30% back fat+2% BRF), T3 (30% back fat+3% BRF), T4 (30% back fat+6% BRF), and T15 (10% back fat+10% canola oil+2% BRF). Lower emulsion stability and higher apparent viscosity were observed in T1, T2, T3, T4, and T8 (20% back fat+3% BRF) compared to the control. Lower emulsion stability and higher hardness was detected in all treatments compared with the control, except T5 (20% back fat), T10 (10% back fat+10% canola oil+2% BRF), T11 (10% back fat+10% olive oil+2% BRF), T12 (10% back fat+10% grape seed oil+2% BRF), and T13 (10% back fat+10% soybean oil+2% BRF). This approach has been found particularly useful for highlighting differences among the emulsified properties in emulsion meat products. Thus, the results obtained with emulsion mapping are useful in making emulsified meat products of desired quality characteristics, partially replacing pork back fat with a mix of 10% back fat, 10% canola oil and 2% BRF was most similar to the control with 30% pork back fat.

      • HCV, Alcoholic : O-024 ; The association between IL28B polymorphism and the progression of liver fibrosis in patients with chronic hepatitis C

        Background: The genetic variation of IL28B gene on chromosome 19 has relevance to spontaneous seroclearance and virologic response to the interferon based standard therapy of chronic hepatitis C. Up to date, however, it is not clear whether these polymorphisms of IL28B are related with disease progression of chronic hepatitis C. In this study, we aimed to determine the association between IL28B genotype and the stage of fibrosis in patients with chronic hepatitis C. Methods: A total of 189 patients seropositive for anti-HCV were enrolled, 10 of whom had been spontaneously recovered from infection. Two SNP polymorphism of IL28B, rs8099917 and rs12979860 were investigated using RFMP genotyping and sequencing. The degree of liver fibrosis was determined by liver biopsy in 91 patients who had no definite evidence of cirrhosis in imaging studies. Results: In the whole series, rs12979860 CC, CT, TT genotype were found in 85.7%, 13.8%, 0.5% of patients, respectively, while rs8099917 genotype TT, TG, GG, in 86.2%, 13.2%, 0.5%, respectively. The rs12979860 CC genotype and reciprocative rs8099917 TT genotype were found in 154 of 179 patients with chronic HCV infection and in 9 of 10 spontaneously serocleared patients (86.0% vs. 90.0%, p=0.925). rs12979860 CC genotype was identified in 84.3% of patients with mild to moderate fibrosis (F0-F2) while in 87.7% of patients with advanced fibrosis (F3-4) (p=0.551). In rs8099917 TT genotype, each of them was 85.7% and 86.4% (p=0.901). In multivariate logistic regression analysis, the only significant factor associated with advanced liver fibrosis was age (OR 4.733, p<0.001). Conclusions: In the present study, IL28B polymorphisms (major alleles, rs12979860 CC and rs8099917 TT) were associated neither with spontaneous seroclearance nor with the progression of fibrosis. However, the small sample size of the present study warrants further prospective multicenter study to clearly the role of SNPs in the progression of chronic hepatitis C.
