        Usefulness of Oblique Lumbar Magnetic Resonance Imaging for Nerve Root Anomalies and Extraforaminal Entrapment Lesions

        Study Design: A retrospective review of prospectively evaluated magnetic resonance (MR) images. Purpose: Routine lumbar axial and sagittal MR images may not clearly demonstrate nerve root anomalies and entrapments in the extraforaminal region. Thus, lumbar extraforaminal lesions or nerve root anomalies may be underdiagnosed because of unfamiliar radiological anatomy. We aimed to investigate the clinical efficacy of our oblique magnetic resonance imaging (MRI) technique for diagnosing nerve root anomalies and entrapment lesions. Overview of Literature: Evaluation of clinical usefulness of oblique lumbar MRI for nerve root anomalies and extraforaminal entrapment lesions. Methods: In total, 162 patients (60 males and 102 females; mean age, 59.8±17.8 years) with suspected nerve root anomalies and entrapments in routine axial and sagittal MR images underwent unilateral or bilateral oblique lumbar T2-weighted MRI. The axial angle of the oblique image was parallel to the foramen. The oblique MRI findings of the symptomatic side were compared with those of the asymptomatic side in cases with unilateral pathologic lesions. Interobserver agreement was analyzed using kappa statistics. Results: The following abnormal findings were obtained: nerve root entrapment due to foraminal stenosis in 82 cases; extraforaminal disk herniation in 29; conjoined nerve root in six; and foraminal bony cyst in one. Forty-three of the 46 patients experienced unilateral lateralizing symptoms, unilateral nerve root entrapment due to foraminal stenosis, herniated disk, and extraforaminal bony cyst compared with the asymptomatic contralateral side (p <0.05). Conclusions: Our results suggest that oblique lumbar MRI is a simple and valuable modality for diagnosing anomalous lumbar nerve root lesions and entrapment.

        Comparison of the Infant and Adult Adipose-Derived Mesenchymal Stem Cells in Proliferation, Senescence, Anti-oxidative Ability and Differentiation Potential

        BACKGROUND: Infant adipose-derived mesenchymal stem cells (ADSCs) collected from excised polydactyly fat tissue, which was surgical waste, could be cultured and expanded in vitro in this study. In addition, the collecting process would not cause pain in the host. In this study, the proliferation, reduction of senescence, anti-oxidative ability, and differentiation potential in the infant ADSCs were compared with those in the adult ADSCs harvested from thigh liposuction to determine the availability of infant ADSCs. METHODS: Proliferation was determined by detecting the fold changes in cell numbers and doubling time periods. Senescence was analyzed by investigating the age-related gene expression levels and the replicative stress. The superoxide dismutase (SOD) gene expression, adipogenic, neurogenic, osteogenic, and tenogenic differentiation were compared by RTqPCR. The chondrogenic differentiation efficiency was also determined using RT-qPCR and immunohistochemical staining. RESULTS: The proliferation, SOD (SOD1, SOD2 and SOD3) gene expression, the stemness-related gene (c-MYC) and telomerase reverse transcriptase of the infant ADSCs at early passages were enhanced compared with those of the adults’. Cellular senescence related genes, including p16, p21 and p53, and replicative stress were reduced in the infant ADSCs. The adipogenic genes (PPARc and LPL) and neurogenic genes (MAP2 and NEFH) of the infant ADSC differentiated cells were significantly higher than those of the adults’ while the expression of the osteogenic genes (OCN and RUNX) and tenogenic genes (TNC and COL3A1) of both demonstrated opposite results. The chondrogenic markers (SOX9, COL2 and COL10) were enhanced in the infant ADSC differentiated chondrogenic pellets, and the expression levels of SODs were decreased during the differentiation process. CONCLUSION: Cultured infant ADSCs demonstrate less cellular senescence and replicative stress, higher proliferation rates, better antioxidant defense activity, and higher potential of chondrogenic, adipogenic and neurogenic differentiation.

      • Investigation of Viscoelasticity in the Relationship Between Carotid Artery Blood Pressure and Distal Pulse Volume Waveforms

        <P>We investigated the relationship between carotid artery blood pressure (BP) and distal pulse volume waveforms (PVRs) via subject-specific mathematical modeling. We conceived three physical models to define the relationship: a tube-load model augmented with a gain (TLG), Voigt (TLV), and standard linear solid (TLS) models. We compared these models using PVRs measured via BP cuffs at an upper arm and an ankle as well as carotid artery tonometry waveform collected from 133 subjects. At both upper arm and ankle, PVR was related to carotid artery tonometry by TLV and TLS models better than by TLG model; when root-mean-squared over all the subjects, the systolic and diastolic BP errors between measured carotid artery tonometry waveform and the one estimated from distal PVR reduced from 4.3 mmHg and 4.6 mmHg (TLG) to 1.1 mmHg and 1.0 mmHg (TLS) for the upper arm (<I>p</I> < 0.0167), and from 2.1 mmHg and 1.7 mmHg (TLG) to 2.1 mmHg and 1.5 mmHg (TLV) for the ankle. Further, TLV and TLS models exhibited superior Akaike's Information Criterion (AIC) in both locations than TLG model. However, the difference between TLG versus TLV and TLS models associated with the ankle was not large. Therefore, the relationship of central arterial BP to arm PVR arises from both wave reflection and viscoelasticity while the relationship to ankle PVR mainly arises from wave reflection. These findings may imply that an effective subject-specific transfer function for estimating accurate central arterial BP from an arm PVR should account for the impact of viscoelasticity.</P>

        Malignant transformation of ovarian mature cystic teratoma into squamous cell carcinoma: a Taiwanese Gynecologic Oncology Group (TGOG) study

        Objective: The malignant transformation (MT) of ovarian mature cystic teratoma (MCT)to squamous cell carcinoma (SCC) is very rare. This study analyzed cases from multiplemedical centers in Taiwan to investigate the clinicopathologic characteristics, treatment, andprognostic factors of this disease and reviewed related literature. Methods: Pathological reports of 16,001 patients with primary ovarian cancer who weretreated at Taiwan medical centers from 1990 to 2011 were reviewed. In total, 52 patients withMT of MCT to SCC were identified. Results: Among all ovarian MCTs, the incidence of MT to SCC is 0.2%. The median age ofpatients was 52 years (range, 29–89 years), and the mean tumor size was 10.5 cm (range, 1–40cm). We analyzed the patients in our study and those in the literature and determined thatearly identification and complete surgical resection of the tumor are essential for long-termsurvival. In addition, adjuvant chemotherapy or concurrent chemoradiotherapy can be usedto treat this malignancy. Old age, large tumor size (≥15.0 cm), and solid components in MCTsare suitable indicators predicting the risk of MT of MCT to SCC. Conclusion: Similar to general epithelial ovarian cancers, the early detection of MT of MCTto SCC is critical to long-term survival. Therefore, older patients with a large tumor or those with a tumor containing a solid component in a clinically diagnosed MCT should beevaluated to exclude potential MT to SCC.

        For the promotion of community development, the first step is to investigate the specialty resources and human resources, to understand the process and the current appearance of community development, and to explore the future development vision or direction should be promoted. Secondly, the local smart people must have the abilities of cooperation, communication, cross-domain integration, and linking external support. It is one of the keypoints of adding-value workforce. Therefore, leaders of a community must find the local characteristics of the championship, and have the capacity to promote citizen participation and to integrate external support and cooperation, so that communities can acquire and integrate diverse and sufficient resources in order to succeed in community sustainability. Therefore, the main purpose of this paper is to establish a model of resource inventory and integration. Through the methods of literature analysis and focus group discussion, this paper explores the community cases of Guantu area in Taipei, Shennong Street and Jingliao community in Tainan how to inventory both natural and human resources, to seek community development and value-added directions, to promote civic participation, cooperation, and to link external groups, so as to achieve the success of internal and external resource integration.
