Hypoxic-ischemic brain injury (HIBI) after cardiac arrest (CA) is a leading cause of mortality and long-term neurological disorders in survivors. Targeted temperature management (TTM) has been rigorously studied as a way to improve results compared to a normal body temperature for preventing secondary damage after HIBI. We report a case of successful TTM in a patient who was suspected to have HIBI after resuscitation from cardiovascular collapse due to respiratory failure during elective surgery under brachial plexus block with dexmedetomidine and remifentanil infusion. A 27-year-old male patient developed CA due to apnea during orthopedic surgery. TTM was performed in the surgical intensive care unit for 72 hours after resuscitation, and the patient recovered successfully. TTM application immediately after resuscitation from CA in patients with suspected HIBI may be an appropriate treatment.
Impact of institutional case volume on intensive care unit mortality

The primary aim of this review is to explore current knowledge on the relationship between institutional intensive care unit (ICU) patient volume and patient outcomes. Studies indicate that a higher institutional ICU patient volume is positively correlated with patient survival. Although the exact mechanism underlying this association remains unclear, several studies have proposed that the cumulative experience of physicians and selective referral between institutions may play a role. The overall ICU mortality rate in Korea is relatively high compared to other developed countries. A distinctive aspect of critical care in Korea is the existence of significant disparities in the quality of care and services provided across regions and hospitals. Addressing these disparities and optimizing the management of critically ill patients necessitates thoroughly trained intensivists who are well-versed in the latest clinical practice guidelines. A fully functioning unit with adequate patient throughput is also essential for maintaining consistent and reliable quality of patient care. However, the positive impact of ICU volume on mortality outcomes is also linked to complex organizational factors, such as multidisciplinary rounds, nurse staffing and education, the presence of a clinical pharmacist, care protocols for weaning and sedation, and a culture of teamwork and communication. Despite some inconsistencies in the association between ICU patient volume and patient outcomes, which are thought to arise from differences in healthcare systems, ICU case volume significantly affects patient outcomes and should be taken into account when formulating related healthcare policies.

<P>Three new isoflavonoid glycosides, termisoflavones A-C (1-3), and eight isoflavonoids (4-11) were isolated from termite-associated Streptomyces sp. RB1 recovered from the cuticle of the South African termite, Macrotermes natalensis. The structures of new compounds were determined by spectroscopic methods including ID and 2D NMR and HR-MS analysis, as well as comparison of their NMR data with those of related isoflavonoid glycoside derivatives. The absolute configurations of the sugar moieties were clarified by chemical reactions. None of the isolates (1-11) displayed antifungal or antimicrobial activities (MICs > 100 mu g/mL), whereas compounds 6 and 11 ameliorated cisplatin-induced kidney cell damage to 80% of the control value at a cisplatin dose of 25 mu M.</P>

Solid organ transplantation is distinguished from other high-risk surgical procedures by the fact that it utilizes an extremely limited and precious resource and requires a multidisciplinary team approach. For several decades, institutional experience, as quantified by center volume, has been shown to be strongly associated with patient outcomes and graft survival after solid organ transplantation. The United States has implemented a minimum case volume requirement and performance standards for accreditation as a validated transplantation center. Solid organ transplantation in Europe is also governed by the European Union, which monitors patient outcomes and organ allocation. The number of solid organ transplantation cases in Korea is increasing, with patient outcomes comparable to international standards. However, Korea has outdated regulations regarding hospital facilities, and performance indicators including patient outcomes after transplantation are not monitored. Therefore, centers perform solid organ transplantation with no meaningful oversight. In this review, data regarding the impact of institutional case volume of kidney, liver, lung, and heart transplantation are summarized, followed by a description of current transplantation center regulations in the United States and Europe. The basis for the necessity of adequate transplantation center regulations in Korea is presented.

Involving issues of environmental, consumer safety and health, and/or social concerns, a morally controversial action refers to an activity that does not have a single standard of ethics for all people, making it challenging to draw a clear line between what is ethical versus unethical. Despite its ambiguity, any firm can be involved in it, especially when the firm operates in the global marketplace where a variety of social, health, and environmental consequences surrounding a given action are questioned by consumers. If consumers come to know of a familiar brand’s morally controversial behavior, it can create approach-avoidance conflicts in their minds; they may see the brand’s action as mismatched with their own moral standards (“avoidance” manifested through a measurable construct––moral incongruence), while being tempted to justify the action (“approach” manifested through moral rationalization).
Joint BLAST-STTC for MIMO-OFDM System

This study focuses on improving MIMO-OFDM systems by combining a wireless communication architecture known as vertical BLAST(bell laboratories layered space-time) or V-BLAST and STTC(space time trellis coding). In this paper, the combination is done by introducing STTC in each V-BLAST layer. Moreover, this architecture uses multiple antennas that are grouped into small number of antennas which makes it less complex to decode by decoding every group. Whereas, in traditional V-BLAST, all the antennas form one group and they are decoded together at the receiver, therefore, this increases the complexity as the number of antennas is getting high. We compare the bit error rate performance of this system with MIMO-OFDM that uses convolutional coding instead of STTC. Under the same spectral efficiency, the simulation results prove that joining V-BLAST with STTC improves MIMO-OFDM systems performance.

Multiple RBC transfusions inevitably lead to a state of iron overload before and after highdose chemotherapy and autologous stem cell transplantation (HDCT/autoSCT). Nonetheless, iron status during post-SCT follow-up remains unknown. Therefore, we investigated post-SCT ferritin levels, factors contributing to its sustained levels, and organ functions affected by iron overload in 49 children with high-risk neuroblastoma who underwent tandem HDCT/autoSCT. Although serum ferritin levels gradually decreased during post-SCT follow-up, 47.7% of the patients maintained ferritin levels above 1,000ng/mL at 1 yr after the second HDCT/autoSCT. These patients had higher serum creatinine (0.62 vs 0.47 mg/mL, P = 0.007) than their counterparts ( < 1,000 ng/mL). Post-SCT transfusion amount corresponded to increased ferritin levels at 1 yr after the second HDCT/autoSCT (P < 0.001). A lower CD34+ cell count was associated with a greater need of RBC transfusion, which in turn led to a higher serum ferritin level at 1 yr after HDCT/autoSCT. The number of CD34+ cells transplanted was an independent factor for ferritin levels at 1 yr after the second HDCT/autoSCT (P = 0.019). Consequently, CD34+ cells should be transplanted as many as possible to prevent the sustained iron overload after tandem HDCT/autoSCT and consequent adverse effects.

3D technologies have received much attention recently in various research area. Among them, 3D printer and 3D scanner are fast and easy techniques to manufacture its products in one simple process. Due to its several benefits, such as low cost fabrication and simple process, these technologies can manufacture products based on consumption or our needs. Here, our studies are focused on making customized mask for yellow dust using 3D printing and 3D scanning methods. Our goal is to design a process to manufacture functional mask for yellow dust using cost-effective 3D technologies. Using this 3D scanner, one’s face will be scanned so that this mask for yellow dust will be customized to fit one’s own face for complete protection from the dust. Moreover, it can be reusable with numerous washings since flexible and biocompatible filaments are used to fabricate masks where conventional masks are disposable, resulting in polluted waste. Mechanical properties as well as microstructure of this fabricated mask for yellow dust will be studied for reusability. Therefore, this 3D printing and 3D scanner can bring a new era in the health market.
