대퇴골 간부 골절 치료에서 확공성 금속정과 비확공성 금속성 사용시 임상적 차이 비교

목 적: 대퇴골 간부 골절의 치료에 있어 확공성 금속정을 사용한 경우와 비확공성 금속정을 사용한 경우, 그 임상적 차이를 비교 분석해 보고자 하였다. 대상 및 방법: 1997년 6월부터 2000년 4월까지 대퇴 간부 골절로 진단, 본원에서 확공성 금속정 또는 비확공성 금속정으로 치료한 105례의 환자 중에서 1년 이상 추시 가능했던 95례의 활자를 대상으로 하였다 이중 확공성 금속정 사용군이 74례, 비확공성 사용군이 21례이였다. 골절의 분류는Winquist-Hansen 분류를 사용하였으며, 확공성 금속정을 사용한 경우는 Ⅰ형 20례, Ⅱ형 37례, Ⅲ형 15례, Ⅳ형 2례이였으며 비확공성 금속정을 사용한 경우는 Ⅰ형 0례, Ⅱ형 7례, Ⅲ형 10례, Ⅳ형 4례였다. 연구 방법으로는 연구 대상의 추시 기간 동안의 의무기록과 방사선학적 검사를 토대로 수술 시간, 수술시 출혈량, 최초 가골 형성 시기, 골절 유합 시기 및 합병증 등을 후향적으로 비교하여 통계학적 분석을 시행하였다. 결 과: 수술시간은 확공성 금속정 사용군이 평균 107분, 비확공성 금속정 사용군이 94분으로 통계학적 유의성을 보였으며 (p<0.005) 수술이 출혈량의 경우 금속정 사용군이 400㎖, 비확공성 금속정 사용군이 250 ㎖로 통계학적 유의성을 보였다 (p<0.001). 그 외 최고 가골 형성 시기나 골절 유합 시기 등에서는 통계학적 유의성이 없었으나, 골절 유합 시기가 비확공성 금속정 사용군에서 전반적으로 늦는 경향을 보였다. 결 론: 대퇴골 간부 골절 치료에 있어 비확공성 금속정을 사용한 경우가 확공성 금속정을 사용한 경우에 비해 수술 시간 및 수술시 출혈량에 있어 유리함이 통계학적으로 유의하게 나타나 제한된 경우에서는 비확공성 금속정의 사용을 고려해도 좋을 것으로 사료된다. Purpose: The comparative analysis of clinical difference between the use of reamed nail and unreamed nail in treatment of femoral shaft fracture. Materials and Methods: In 105 patients with femoral shaft fracture who were treated with reamed nail or unreamed nail between June of 1997 and April of 2000, 95 patients who underwent more than a year of follow-up were selected. Winquist-Hansen criteria was applied for the classification of fracture. Based on the medical records and radiological examinations, conducted a retrospective, statistical analysis of the duration of operation, the amount of bleeding during operation, the first time of callus formation, union time, and complications. Results: The average duration of operation was 107 minutes for reamed nail group, and 94 minutes for unreamed nail group, and the difference was statistically significant (p<0.005). The amount of bleeding during the operation was 400㎖ for reamed nail group and 250㎖ for unreamed nail group, and the difference was statistically significant (p<0.001). There was no statistical difference in the first time of callus formation and union time between the two groups but, in general union time tend to be long in unreamed nail group. Conclusion: In the treatment of femoral shaft fracture, the use of unreamed nail was shown to have an advantage over the use of reamed nail in terms of the duration of operation and the amount of bleeding. We recommend restrictive cases.
HDAC11 Inhibits Myoblast Differentiation through Repression of MyoD-Dependent Transcription

Abnormal differentiation of muscle is closely associated with aging (sarcopenia) and diseases such as cancer and type II diabetes. Thus, understanding the mechanisms that regulate muscle differentiation will be useful in the treatment and prevention of these conditions. Protein lysine acetylation and methylation are major post-translational modification mechanisms that regulate key cellular processes. In this study, to elucidate the relationship between myogenic differentiation and protein lysine acetylation/methylation, we performed a PCR array of enzymes related to protein lysine acetylation/methylation during C2C12 myoblast differentiation. Our results indicated that the expression pattern of HDAC11 was substantially increased during myoblast differentiation. Furthermore, ectopic expression of HDAC11 completely inhibited myoblast differentiation, concomitant with reduced expression of key myogenic transcription factors. However, the catalytically inactive mutant of HDAC11 (H142/143A) did not impede myoblast differentiation. In addition, wild-type HDAC11, but not the inactive HDAC11 mutant, suppressed MyoD-induced promoter activities of MEF2C and MYOG (Myogenin), and reduced histone acetylation near the E-boxes, the MyoD binding site, of the MEF2C and MYOG promoters. Collectively, our results indicate that HDAC11 would suppress myoblast differentiation via regulation of MyoD-dependent transcription. These findings suggest that HDAC11 is a novel critical target for controlling myoblast differentiation.

The online sales channel which can be able to supplement problems of offline sales channel at the hotel industry is recognized as important means to create more many value than existing conventional offline sales channel and to secure predominance in competition. The purpose of this study was to define variable about the sales channel by reviewing literature related to online sales channel of the hotel industry and commitment of relationship, and to investigate whether online channel can be utilized as applicable tool to practical sales function based on actual proof analysis about the effect of variables on the commitment of relationship and the loyalty of customers. For proof analysis, SPSS 15.0 was used for basic statistics, and AMOS 7.0 was utilized for structural model and hypothesis verification. The result of this study was that factor(distribution channel, brand, communication, and promotion) of sales channel is one of very important factors in hotel industry. It will be very important to estimate the success of hotel company that which channel is selected for marketing activity. And it was confirmed that diversification of sales channel, preference of hotel brand, and enhancement of communication factors are very important in online than offline sales channel, since generation frequently using online is increased. It was identified that these factors can influence greatly on loyalty only when it was formed based on trust and satisfaction, that is commitment of relationship. And it was considered that these factors can be important factor which should be studied in the point of further marketing method.

In cardiac myocytes, stimulation of α1-adrenoceptor(α1-AR) leads to hypertrophic phenomenon. The Gh protein is tissue type transglutaminase and transmits the intracellular signal with GTPase activity. Little is known about Gh-mediated intracellular signal transductions to develop the hypertrophy. The aim of this study was to elucidate how Gh mediates norepinephrine(NE)-stimulated intracellular signal transductions leading to the hypertrophic responses in neonatal rat cardiomyocytes. NE-induced protein/DNA ratio and ERKs activation were inhibited by a α1-AR blocker(prazosin), but not by a β,-AR blocker(propranolol). Gq mainly regulated protein/DNA ratio and genetic markers of hypertrophy but inhibition of Gq did not induce a decrease of hypertrophic response to the normal control. The treatment with siRNA for Gh suppressed NE-induced PKC/MEK1,2/ERKs activation. NE-induced β- MHC reorganization was inhibited by small interfering RNA targeting Gq or Gh. Cell surface area level of Gq inhibition was not the same as control but atrial natriuretic peptide(ANP), a cardiac hypertrophy marker, was inhibited by Gh and Gq suppression and coincidently inhibition of both Gh and Gq reduced the gene expression. These findings suggest that Gh is partially involved in the hypertrophic response induced by NE in cardiomyocytes through PKC/ MEK1,2/ERKs signal pathways via α1-AR/Gh.
증례 : 순환기 ; 수술 중에 경흉부 심초음파로 진단된 우심방 혈전을 동반한 폐색전증 1예

우심방 혈전은 비교적 드물며, 대부분 폐색전증과 관련되어 나타나고, 즉각적인 진단과 치료를 하지 않을 경우 사망률이 높다고 알려져 있다. 특히 심초음파에서 다발성 및 가동성을 나타내는 경우 급격한 폐색전증으로 진행되어 예후가 나쁜것으로 보고되어 있다. 국내에서는 우심방 혈전과 동반된 폐색전증으로 생존한 경우가 1예 보고되어 있으나, 수술 중에 경흉부 심초음파로 진단하여 성공적으로 치료한 경우는 국내뿐만 아니라 전세계적으로 본 증례가 유일하다. 저자들은 패혈성 슬관절로 전신마취하에 수술 도중 심인성 쇼크로 발현된 우심방의 다발성 및 가동성 혈전을 동반한 폐색전증을 응급 경흉부 심초음파로 진단하였고, 혈전용해 및 항응고요법으로 치료한 1예를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다. Right atrial (RA) thrombi are rarer than left atrial thrombi; they are frequently associated with major pulmonary embolism and carry a very high risk of mortality, requiring prompt diagnosis and treatment. In particular, multiple, mobile right atrial thrombi have a very poor prognosis because of the high incidence of massive pulmonary embolism. Echocardiography is useful in the diagnosis of RA thrombi. The treatment options for RA thrombi are anticoagulation, thrombolysis, and thrombectomy. Here, we report a case of multiple, mobile right atrial thrombi and pulmonary embolism that presented as cardiogenic shock during arthroscopic lavage of a septic knee. It was diagnosed using transthoracic echocardiography and treated successfully with thrombolytics and anticoagulants. (Korean J Med 78:624-629, 2010)

<P><B>Abstract</B></P><P><B>Introduction</B></P><P>Thrombogenic occlusion, at the anastomotic microvessels, contributed impaired blood flow to flap failure. The effect of an orally active low molecular weight heparin (LMWH) derivative conjugated with deoxycholic acid (DOCA) on the patency of anastomosis of the crushed rat artery was investigated, expecting its antithrombogenic effect.</P><P><B>Materials and Methods</B></P><P>60 Femoral arteries of 30 rats, divided into three groups of 20 each, were reanastomosed. LMWH-DOCA conjugate was orally administered prior to and after operation for 5 consecutive days. On the sixth day of operation, the patency of the anastomosed artery was evaluated.</P><P><B>Results</B></P><P>The patency of oral LMWH-DOCA (10mg/kg) group was significantly enhanced from 15% to 45%, compared to non-treated control group. On the other hand, when the dosage of LMWH-DOCA was reduced to 1mg/kg, its efficacy on anastomosis was not as efficacious in terms of patency. The intima of crushed artery was impaired and thrombus formation was examined in the control group. In the drug treated group, the patency was only compromised by a thin layer of thrombus that covered the inner layer of the vessel without causing any damage to the internal elastic lamina.</P><P><B>Conclusion</B></P><P>The medication of oral LMWH-DOCA conjugate has been vetted in microvascular anastomosis of the crushed artery. LMWH-DOCA was potentially useful for improving the patency in compromised vessels after microsurgery.</P>
