갱년기장애 상열감을 호소하는 내측 연수 경색 환자에 대한 치험 1례

This case study reports on the effectiveness of Korean medicine treatment for menopausal hot flashes in stroke patients. During hospitalization, a female patient with medial medullary infarction was diagnosed with menopausal hot flashes and received treatment with traditional Korean medicine and acupuncture. The assessment was performed by monitoring daily habits to relieve hot flashes. The hot flash index, menopause rating scale (MRS), menopause-specific quality of life (MENQoL) questionnaire, and Kupperman index (KI) were also used. After 28 days of treatment, the hot flash index improved from 27 to 2, MRS improved from 11 to 2, MENQoL improved from 40 to 21, and KI improved from 22 to 6. Thus, this case suggests that Korean medicine treatment may be effective for menopausal hot flashes in stroke patients.

The purpose of this case report is to describe the effectiveness of Korean medicine in the treatment of gait disturbance in advanced stage multiple system atrophy-cerebellar type (MSA-C). This inpatient was treated with herbal medicine, acupuncture, pharmaco-acupuncture, moxibustion, cupping, and chuna therapy. The gait disturbance was assessed by Unified Multiple System Atrophy Rating Scale(UMSARS), Tinnetti test, and 16m walking spatial features. After treatment, UMSARS Part I score was enhanced, decreasing from 29 to 24, Part II score from 34 to 18. Tinetti test score increased from 7 to 20. Gait stride length increased and width decreased. This study suggests that Korean medical treatment could be an effective treatment for delaying the progress of gait disturbance in advanced-stage MSA-C patients.

Objective: The purpose of this case study is to report the effectiveness of Baliao acupoints electro-acupuncture treatment on a neurogenic overactive bladder induced by stroke. Method: A patient suffering from middle cerebral artery infarction was diagnosed with a neurogenic overactive bladder and treated by Korean medicine, including electro-acupuncture on Baliao acupoints six times per week. The evaluation was performed by monitoring urinary frequency and using the Korean versions of the Overactive Bladder Symptom Score (OABSS) and Urinary Incontinence Quality of Life (I-QoL). Results: After 27 days of electro-acupuncture treatment, the OABSS score improved from 6 to 3. The I-QoL total score improved from 60 to 87. The patient also showed a decrease in nocturia frequency. Conclusion: This case suggests that electro-acupuncture on Baliao acupoints can be effective for overactive bladders due to supra-pontine lesions.

Persistent hiccups persist for more than 48 hours. Those can often occur in cancer patients after chemotherapy or high dose of corticosteroids. A 52-year-old male patient undergoing Non-small cell lung cancer(NSCLC), recently suffered presistent hiccups and dyspnea. He was treated with herbal medicine for 56days and acupuncture for 9days. No side effects were observed during treatment. The severity of hiccups was measured in every hospital visit and psychometric symptoms were evaluated periodically through the Edmonton Symptom Assessment System(ESAS). The severity of hiccups and psychometric symptoms were alleviated and the effect persisted during the treatment. The patient took no other nervous system drugs in the end of the treatment. In conclusion, Korean medicine like Hyeongbangjihwang-tang and Jeongcheonhwadamgangki-tang can be effective for patients with persistent hiccups and dyspnea who cannot use alternative drugs or treatment because of other complications.
보행장애를 호소하는 약인성 파킨슨 증후군 환자의 한방복합치료 1례

Objective: This case study reported the effectiveness of adjuvant Korean therapy on gait disturbances induced by drug-induced Parkinsonism. Method: A patient suffering from frontotemporal lobe dementia was diagnosed with drug-induced Parkinsonism and treated with adjuvant Korean therapy, including herbal medicine and pharmaco-acupuncture. The evaluation was performed by monitoring the length of time and number of steps during an 8 m gait, using the Unified Parkinson’s Disease Rating Scale (UPDRS). Results: After 17 days of adjuvant Korean therapy, the UPDRS score improved from 32 to 16. The length of time for the 8 m gait improved from 20 seconds to 14 seconds. The patient also showed a decrease in the number of steps during the 8 m gait from 43 to 22. Conclusion: This case suggests that adjuvant Korean therapy can be effective for drug-induced Parkinsonism.
고혈압의 침치료에 대한 문헌고찰 : 국내 임상 연구를 중심으로

■Objectives This study aimed to collect and analyze clinical studies on the significance of acupuncture in the treatment of hypertension among Korean patients. ■Methods Among Korean patients with a blood pressure (BP) of 120 mmHg or higher and a diastolic BP of 80 mmHg or higher, those, treated with acupuncture only, were included. A literature search was conducted through 'Embase', 'Medline', 'Science and Technology Information Integration Service (NDSL)', 'Traditional Korean Knowledge Portal (OASIS)', 'PubMed', and 'Scopus'. The search keywords were (hypertension OR 'blood pressure') AND acupuncture. The papers, published before the day of the search (02. January 2022), were included in this study. ■Results Among the 12 selected papers, seven involved randomized controlled trial (RCT), four before-and-after studies, and one case series. The number of RCTs has increased yearly. The treatment methods used in the studies included needle acupuncture in eight studies, auricular acupuncture in two, pharmacopuncture in one, and si-acupuncture in one. The average numbers of patients enrolled in RCTs, before-and-after studies, and case series were 21.1, 30, and 23 respectively. Six studies were conducted on prehypertensive patients, two on stage 1 hypertension patients, and four on stage 2 hypertension patients. The involved acupoints have been reported to be ST36 in eight papers, LI11 in four papers, and PC6 in three papers. The treatment period lasted for < 1 week in eight studies and 8 weeks in four studies. Acupuncture successfully lowered blood pressure in nine out of 12 studies ■Conclusions Acupuncture is a viable alternative treatment option for prehypertensive patients, who are not taking medications. Additionally, it is also useful in further lowering the BP of patients with stage 1 and stage 2 hypertension in the short term. Large-scale and long-term studies on acupuncture for hypertension should be conducted.

Objective: This study examined the effectiveness of Korean medicine treatments in two patients with Parkinson’s disease complaining of discomfort stemming from postural instability and gait disturbance (PIGD). Methods: Two patients were treated for 3 months. They visited the clinic once a week for the first month and thereafter once every 2 weeks. The Unified Parkinson’s Disease Rating Scale (UPDRS) and a three-dimensional gait analysis were performed at the first visit and at 1, 2, and 3 months thereafter. Results: In Case 1, gait speed, stride length, and swing phase increased. Double support decreased until 2 months after treatment but increased slightly after 3 months. Among the kinematic parameters, tilt and rotation increased. The total UPDRS Part III score decreased from 51 points to 29 points after 3 months of treatment. In Case 2, gait speed, stride length, and swing phase increased, but double support decreased. Among the kinematic parameters, tilt, rotation, and obliquity decreased. The total UPDRS Part III score decreased from 11 points to 7 points after 3 months of treatment. Conclusions: This study suggests that Korean medicine can be an effective treatment for patients with Parkinson’s disease who experience discomfort due to PIGD.
양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례

Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.
교뇌경색 이후 배뇨장애가 악화된 소양인 환자의 한의 치료 1례 보고

■Objective The purpose of this case report is to report the effect of Korean medicine treatment on a patient with pontine infarction complaining of discomfort due to aggravated voiding dysfunction after the onset of the stroke. ■Methods The patient was hospitalized for 44 days and treated with herbal medicine, acupuncture, electroacupuncture, moxibustion, and interferential current therapy(ICT). We checked the amount of self-voided volume and postvoid residual urine volume(PVR), and asked the patient to subjectively evaluate the degree of discomfort due to voiding dysfunction. ■Results After 44 days of hospitalization, the patient responded that the subjective discomfort due to residual urine remained 30% and delayed urine remained 50% compared to the day of admission. The maximum amount of PVR was 234 ml on the 3<sup>rd</sup> day, and the minimum amount of PVR was checked on 25<sup>th</sup> and 35<sup>th</sup> day as 0 ml. ■Conclusions This study suggests that Korean medicine treatment may be effective in the treatment of voiding dysfunction after stroke.

Objective: This study investigated the effectiveness of bee venom acupuncture for a diabetic neuropathy patient. Methods: A diabetic neuropathy patient received twenty-one bee venom acupuncture treatments for a total of 99 days. The Neuropathy Pain Scale (NPS) was used to evaluate the weekly peak pain during the treatment period. Results: After the treatments, the NPS scores improved, and the frequencies of peak pain decreased. Conclusions: This clinical case indicates that bee venom acupuncture combined with Korean medicine may be effective for treating diabetic neuropathy patients.
