We report the unusual case of multiple intraorbital and intracranial germinoma primarily arising from optic nerve, pineal gland extending to the splenium of corpus callosum. The patient was 27-year-old male who presented with progressinve bilateral visual loss for 2yrs and suddenly developed right painful exothalmus. The neuroimaging studies disclosed tumors in the right optic nerve, optic chiasm, pineal gland and posterior corpus callosum. The tumor markers of serum and cerebrospinal fliud were all negative. The intraorbital and intracranial optic nerve tumor was removed via fronto-orbitotomy and pterional approach. The histological diagnosis was germinoma. Follow up brain MRI demonstrated complete disappearance of the multiple masses after completion of three courses of combinant chemotheraphy with Ifosfamide-Cisplatin-Etoposide. This case is believed to be the first case report as multiple intraorbital and intracranial primary germinoma.

Objective The purpose of this study is to determine the feasibility of transsphenoidal approach (TSA) and staged or repeated TSA for large pituitary adenoma (LPA) and remnant or recurrent PA. Methods Eighteen LPAs underwent TSA surgery from February 2002 to December 2006. Eleven cases were non-functioning adenomas, 5 cases were growth hormone secreting adenomas and 2 cases were pituitary apoplexy. Mean size of tumor was 38x28x30mm. All patients were initially operated via TSA. Results Total removal was performed in 8 patients (44.4%). Subtotal removal was done in 8 patients (44.4%) by the initial TSA, two patients in which underwent repeated TSA due to postoperative tumor bleeding. The visual and hormonal symptoms were improved in 13 patients (72.2%) after the surgery. Six patients out of 8 with subtotal removal underwent the staged operation. Two patients with tumor recurrence underwent repeated TSA, 18 and 26 months respectively after the initial TSA. All patients with growth hormone-secreting PA achieved the normalization of growth hormone. Conclusion The staged and repeated TSA is one of the feasible option for the treatment of LPA and residual or recurrent PA with acceptable surgical morbidity and mortality.

Objective:While total resection is a desirable goal for the treatment of craniopharyngioma, it is not always accomplished, and is associated with permanent endocrinological and visual deficits. This knowledge has led us to perform intratumoral bleomycin injection as a primary therapy for cystic craniopharyngioma. Methods:Six patients(4 adult and 2 children) in whom cystic craniopharyngioma was diagnosed were treated primarily with a schedule of 3 mg of intracystic bleomycin administration, three times a week for 5 weeks. One patient was excluded due to bleomycin leakage in reservoir permeability test. Two out of remainders underwent subsequent resection and radiotherapy, and one was treated with ventriculoperitoneal shunt. Median follow-up period was 36 months. Results:There was a reduction(>90%) in cyst size in 5 patients with the preservation of endocrinological function. However, the median recurrence-free survival was only 22 months. Conclusion:Intracystic bleomycin administration could be a feasible option for the treatment of predominantly cystic craniopharyngioma in terms of symptom relief and pituitary function preservation. However, it is suggested that the efficacy of intraumoral bleomycin therapy is temporary and it has the risk of meningitis and neuropathy.

We report a case of brain metastases from esophageal carcinoma. A 53-year-old man presented with weakness and numbness in the left arm. Systemic chemotherapy had been performed for the treatment of stomach carcinoma and esophageal carcinoma. Brain magnetic resonance images revealed two solid mass in the right parietal and the left occipital lobe. The mass was totally resected through two craniotomies. Histological examination revealed squamous cell carcinoma, which was identical to the pathology of esophageal carcinoma. The postoperative course was uneventful and the patient`s motor weakness was recovered. Additionally, whole brain radiation was given. Although brain metastases from esophageal carcinoma are very rare, it should be considered in the differential diagnosis of brain metastases in a patient with gastrointestinal tract cancers.

Objectives: This study was performed to assess the quality of life (QoL) of the patients with petroclival meningioma treat-ed with surgery with/without radiotherapy. Methods: From 2005 to 2010, 12 patients with newly diagnosed petroclival meningioma underwent surgery at a single in-stitution. The status of preoperative and postoperative (6 months) QoL was assessed using EORTC QLQ-C30 and Brain Cancer Module (BN20). Results: The most frequent preoperative symptoms were headache and dizziness. Total, subtotal, and partial resections were achieved in five, two, and five patients, respectively. Radiation therapy was performed in four patients with subtotal or par-tial resection. The postoperative complications were cranial nerve palsy (n=3), hemiparesis (n=2), and meningitis (n=2). Delayed hydrocephalus was found in two patients treated with additional radiosurgery. Motor dysfunction, bladder control, nausea/vomiting, dyspnea, insomnia, appetite loss, and constipation deteriorated and worsened the status of patients` QoL. However, headache, future uncertainty, and visual disorder improved. Conclusion: Postoperative QoL of the patients with petroclival meningiomas could be associated with surgery with/with-out radiation-related morbidity. Besides the treatment-associated morbidity and mortality rates, QoL issues should be eval-uated in the research of skull base tumors in the future.
