        Comparison of EGFR Mutation Status in Malignant Pleural Effusion and Plasma from Lung Adenocarcinoma Patients

        Malignant pleural effusion (MPE) and blood samples can be used as practical sources for the detection of epidermal growth factor receptor (EGFR) mutations in advanced non-small cell lung cancer patients. We compared EGFR mutation status of cell blocks, cell-free fluid of MPE, and plasma from lung adenocarcinoma patients. We obtained paired samples of MPE and plasma from 14 pathologically-confirmed lung adenocarcinoma patients. EGFR mutation status was determined by peptide nucleic acid (PNA)-mediated real-time polymerase chain reaction (RT-PCR) clamping. EGFR mutations were detected in five (35.7%) cell blocks of MPE, which showed identical results as the corresponding cell-free fluid, whereas mutations were detected in the plasma of only two (40.0%) of the five patients. Of the seven patients treated with EGFR tyrosine kinase inhibitors (TKIs), EGFR mutations were detected in cell blocks, cell-free fluid of MPE, and plasma for only one of the four patients who responded to EGFR TKIs, while mutations were detected only in cell blocks of MPE and cell-free fluid of the three remaining patients. Our results suggest that it may be feasible to detect EGFR mutations in cell-free pleural fluid from lung adenocarcinoma patients by using highly sensitive methods, but that analysis of free plasma may lead to undetected mutations and misdiagnosis.

        Regulation of glucose metabolism from a liver-centric perspective

        Glucose homeostasis is tightly regulated to meet the energy requirements of the vital organs and maintain an individual’s health. The liver has a major role in the control of glucose homeostasis by controlling various pathways of glucose metabolism, including glycogenesis, glycogenolysis, glycolysis and gluconeogenesis. Both the acute and chronic regulation of the enzymes involved in the pathways are required for the proper functioning of these complex interwoven systems. Allosteric control by various metabolic intermediates, as well as post-translational modifications of these metabolic enzymes constitute the acute control of these pathways, and the controlled expression of the genes encoding these enzymes is critical in mediating the longer-term regulation of these metabolic pathways. Notably, several key transcription factors are shown to be involved in the control of glucose metabolism including glycolysis and gluconeogenesis in the liver. In this review, we would like to illustrate the current understanding of glucose metabolism, with an emphasis on the transcription factors and their regulators that are involved in the chronic control of glucose homeostasis.

        비정상적 질 출혈로 발현한 단독 자궁 전이의 재발성 췌장선암 - 증례보고 -

        췌장선암은 악성도가 높고 불량한 예후를 갖는 암종으로서, 흔한 전이 부위는 간, 복막, 원격 림프절 등이 다. 드물게 여성 생식기로 전이될 수 있으나, 난소가 아닌 자궁전이는 매우 드물다. 우리는 비정상적인 질 출혈로 발견된 63세 여성의 단독 자궁 전이로 재발한 췌장선암의 증례를 보고하는 바이다. 환자는 3년전 췌장선암 진단 후 항암화학요법 및 방사선요법 후 췌장선암에 대한 수술적 절제를 시행받은 기왕력이 있었 다. 재발은 자궁 내막과 경부에서 확인되었으며, 췌장선암 고유의 조직학적 특징을 보여 전이성 췌장선암으 로 진단되었다. 치료는 자궁절제술 후 항암화학요법을 시행받았고 이후 추가적인 재발성 병변 및 전이는 관찰되지 않았다. 골반 외 악성종양에서의 자궁 전이는 매우 드물지만, 악성종양의 병력이 있는 환자에서 비정상적인 질 출혈을 보이는 경우 자궁 전이의 가능성을 고려해야 하며, 이에 대한 조직학적 확인을 면밀 하게 시행하여 원발 및 전이성 자궁암을 구별하여야 한다. 전이성 자궁암일 경우 여러 상황을 고려한 개별 맞춤형 치료 전략을 면밀하게 세우는 것이 환자의 예후를 향상시킬 수 있다. Pancreatic ductal adenocarcinoma (PDAC) is known for its aggressiveness and poor prognosis and often metastasizes to common sites, including the liver, peritoneum, and distant lymph nodes. PDAC may metastasize to the female genital tract, particularly to the uterus; however, this is extremely rare. We report a case of a 63-year-old woman with recurrent PDAC presented as an isolated uterine metastasis, initially presented with abnormal vaginal bleeding. Despite preoperative chemotherapy followed by radiotherapy and surgical resection for PDAC, the patient developed recurrence manifested in the uterineendometrium and cervix, and diagnosis was confirmed based on histopathological evaluation, whichshowed characteristics of PDAC. We performed hysterectomy followed by chemotherapy, which resultedin a complete response. This case report highlights the importance of considering uterine metastasis inthe differential diagnosis in patients with unusual vaginal bleeding and a history of extra pelvic malignancies. Additionally, we have outlined the challenges in distinguishing between primary and metastatic uterine tumors and the need for tailored therapeutic strategies.

        두경부 편평세포암종에서의 HuR 발현 : 양성 및 전암성 편평세포병소와의 비교

        HuR(human embryonic-Iethal abnormal vi sion-like protein, ELAV)은 최근 염증반응 및 세 포성장 조절의 중요 기전 으로 관심 을 받고 있는 전사후 유진자 발한의 조절기전 에 관여한다 HuR은 3' -untranslated regi on에 AU- rich ele rn ents(ARE) 를 지닌 일부 mRNA들(에‘ c- fos, VEGF. COX• 2 동)의 안정화에 기여하여 mRNA의 증가 및 부가적인 딘백발 한을 증가시킨디 HuR 단백 은 이 러한 염증 및 세포성장의 생리적 기전 외에 종양발생과도 관련될 수 있으며 ‘ 유방암 난소 암 및 뇌 종잉 둥에서 f-luR의 발한증가가 보고된 바 있다 두경부 편평 세 포암종 전암성 및 양성 편평세포 병소를 대상으로 HuR의 발현양상을 띤역 조직회학적으로 살펴봄으로써 HuR의 발현이 두경부 펀평세포암종의 발생과 관련될 수 있는지 실펴 보고자 힌다 본 인 구는 80여1 의 두경부 편평세포암종 14 예의 전암성 편평세포병소 및 32 예의 양성 면평세포벙소를 실험대상 으로 이용히였다 두경 부 편평세 포암종은 AJCC(Amcrican Joint Comrnittee on Cancer) 의 분류법에 의한 TNM분류 , 병리 조 직학적 분화정 도 및 발생부위 별로 구분히였다 면역조직화학적 염색은 mouse anti-HuR monoclonal antibody(ZymeCl Clone 3A2) 와 Envis ion kit (DAKO) 를 이용하였다 염색결괴는 2명의 병리의사가 독립적으로 결과를 평가한 후 x 2 test fol' trends (SPSS‘ ve l'si o n12) 로 통계분석을 행하였다 HuR 염색반응을 핵과 세포질 부위로 구분하여 평가하고 비 교한 결과, 핵 과 세 포질 염색 결 괴 모두 편핑세 포암종 전암성병소. 양성병소에서 유의한 차이경향을 보였으며‘ 편평 세 포암종의 경우 임상벙 기 및 발생부위에 따른 유의 한 치이경향을 보였다 특히 , 후두에서 발생힌 편평세포암종은 세포질애서 강한 양성반응을 보였 다 본 연구결과, mRNA 안정화 인자인 HuR의 발현 및 세포 내 분포가 두경부 펀평세포암종에서 이상조절 됨 을 보였으며 그 이싱 조절 양상이 두정 부의 부위별로 차이가 있음을 확인하였다 따라서 향후에는 발현이상의 downstream effec ts 및 이} 후와의 관련성에 대힌 연 구기- 추가되어야 할 것은 여져진다

        Weekly Paclitaxel and Trastuzumab as a First-Line Therapy in Patients with HER2-Overexpressing Metastatic Breast Cancer: Magnitude of HER2/neu Amplification as a Predictive Factor for Efficacy

        We evaluated the efficacy and safety of weekly paclitaxel plus trastuzumab as firstline chemotherapy in women with HER2-overexpressing metastatic breast cancer (MBC), and we investigated the prognostic factors including magnitude of HER2/neu amplification in this population. We analyzed 54 patients with HER2-overexpressing MBC that were treated with weekly paclitaxel plus trastuzumab as first-line chemotherapy from February 2004 to December 2006. At a median follow-up of 28 months, median time to progression (TTP) was 16.6 months (95% CI, 9.4 to 23.7 months) and median overall survival was 25.6 months (95% CI, 21.8 to 27.3 months). Therapy was generally well tolerated, although three patients (5.5%) experienced reversible, symptomatic heart failure. Of the 27 patients evaluable for the HER2 FISH, patients with a HER2/CEP17 ratio of ≤4.0 had significantly shorter TTP than those with a HER2/CEP17 ratio of >4.0 (10.8 vs. 23.2 months, P=0.034). A HER2/CEP17 ratio of >4.0 was identified as significant predictive factor of TTP by multivariate analysis (P=0.032). The combination of weekly paclitaxel plus trastuzumab as first-line chemotherapy is an effective regimen in patients with HER2-FISH-positive MBC. Furthermore, the magnitude of HER2 amplification is an independent predictive factor of TTP.

        The Usefulness of Electronic Activity Measurement for 24-hour Ambulatory Blood Pressure Monitoring

        Background and Objectives:The overriding influence of physical activity and the diurnal variation during ambulatoryblood pressure monitoring (ABPM) has been well demonstrated. We prospectively evaluated the usefulnessof electronic activity monitoring for deriving the actual physical activity and the diurnal variation of ABPM.Subjects and Methods:24-hour ABPM with using an electronic activity monitor was performed on ten normotensivevolunteers and fifteen hypertensive subjects. To interpret the ABPM results of the fifteen hypertensive subjects,we obtained the actual awake/sleep periods of every subject with using an electronic activity monitor. Results:The activity values obtained from the ten normotensive volunteers correlated well with the values of the bloodpressure (BP) and the heart rate. In the hypertensives, the nocturnal mean BP derived by the actual period wassignificantly lower than that derived by the arbitrary period (130±16/81±13 mmHg versus 124±13/77±12mmHg, respectively, p<0.05). The nocturnal BP fall derived by the actual period was significantly larger than thatderived by the arbitrary period (14.9±8.7/11.6±7.5 mmHg versus 21.1±8.6/16.2±7.4 mmHg, respectively,p<0.01). Four among the 7 non-dippers determined by the arbitrary period were re-classified as dippers when theactual period was used. One among the 13 hypertensives, as determined by the arbitrary period, was also re-classifiedas a non-hypertensive. Conclusion:The electronic activity monitor was able to determine the actual activitylevel. The interpretation of ABPM may be altered by the use of the electronic activity monitor. These results suggestthat the accuracy and reproducibility of the 24-hour ABPM will be improved by using an electronic activitymonitor. (Korean Circulation J 2006;36:91-98)
