      • 생비골 성장판 이식술을 이용한 선천성 다발성 골연골증에서 전완부 변형의 치료

        It is difficult to manage the growing deformity of forearm bone caused by hereditary osteochondromatosis in children, because deformity and discrepancy of limb length is progressive. The are many treatment methods of these problems including excisio of osteochondroma, lengthening of ulna, shortening of radius, corrective osteotomy with or without lengthening apparatus. Among many treatment methods, we tried free vascularized epiphyseal transplantation with the proximal fibular epiphysis in 3 patients of hereditary osteochondromatosis for inducement of continuous bone growth and deformity correction. The average duration of follow up was 7 years and 1 month, the shortest duration being to 4 years and 5 months and the longest 10 years and 8 months. Serial radiologic and clinical evaluation were carried out during follow up and there were satisfactory length gain, deformity correction and improvement of adjacent joint motion in 2 cases. According to our follow up evaluation, free vascularized epiphyseal transplantation is valuable procedure in forearm deformity of hereditary osteochondromatosis although it needs skillful and experienced operative technique.

        Ossifying fibroma is one of a group of fibro-osseous lesions which arises typically within the jaw bones and only rarely affects the long bones. Ossifying fibroma of the long bones almost involve exclusively the tibia but may also involve the fibula. Ossifying fibroma of the long bones is distinct from fibrous dysplasia, adamantinoma and nonossifying fibroma with regard to age of the patient, site, radiographic appearance, histological features, and clinical course. We are reporting the cases of seven patients with a tumor-like lesion that named osteofibrous dysplasia. It is most commonly found in the tibia and fibula of a child ten years of age or younger. Of the seven cases reported in this study, only one patient was younger than ten years. In all cases, the lesions were usually located in the tibial diaphysis. The average duration of clinical manifestation was 5.2 years. The clinical symptoms were anterior bowing of the tibia in 2 cases, buldging of the tibia in 2 cases, and mass overlying the tibia in 3 cases. On the roentgenography, it shows multiple radiolucent lesion with intervening sclerotic rim of the tibial diaphysis. In seven patients, 6 cases were confirmed with biopsy. We had done curettage and bone graft in three cases, VFG was done in one case. The other three cases underwent conservative management.

        IP 네트워크 상의 다양한 서비스 복구 알고리즘

        네트워크 상에 장애가 발생 시에는 서비스들의 Qos 요구사항을 만족시킬 수 있는 또 다른 백업 경로를 사용하여 서비스해야 한다. 따라서 본 논문에서는 IP 네트워크 상에 발생된 단일 회선 장애를 위해 simple PFC(5-PFC) 기법과 다중 회선 장애를 위한 iterative PFC(i-PFC) 그리고 라우터와 같은 노드 장애를 복구하기 위한 fault_node Avoidance PFC(a-PFC) 기법들을 제안하고 이를 기반으로 Direct_destination Rerouting(DR) 기법을 최소 재라우팅 비용 문제를 통해 성능 비교 분석하기로 한다. In case of failure in the IP network services should be provided via a backup path which fully meets QoS requirements. In this regard, we in this paper suggest simple-PFC(5-PFC) to restore single link failure, iterative-PFC(i-PFC) for multiple link failures, and fault_node Avoidance-PFC(a-PFC) for failures of nodes such as routers. Taking the minimum cost rerouting problem into account, we compare and analyze performance of those three mechanisms carrying out Direct_destination Rerouting(DR).

      • 거대세포종의 보조 요법으로의 전기소작술

        목적: 거대세포종의 치료에서 보조요법으로 전기소작술을 시행한 환자들을 추시하여 종양학적 및 임상적 결과를 알아보고자 하였다. 대상 및 방법: 1989년부터 2004년까지 두 곳의 대학병원에서 치료한 94례의 거대세포종 중 보조요법으로 전기소작술을 시행하였던 47례를 대상으로 하였다. 수술방법은 종양부위의 피질골에 창을 충분한 크기로 내고, 큐렛과 speed bur를 이용한 철저한 소파술 후 Bovie로 전기 소작술을 시행하였고, 추시 환자의 의무기록과 방사선 사진들을 조사하여 임상적 및 종양학적 결과를 조사하였다. 결과: 평균 44개월의 추시 기간 중 국소재발은 8례(17%)에서 있었고, 폐전이는 1례에서 발견 되었다. Bovie를 이용한 전기소작술로 인한 합병증 또한 1례(화농성 슬관절염)에서 있었다. 결론: Bovie를 이용한 전기소작술은 거대세포종의 수술 중 사용하는 다른 보조요법에 비하여 비교적 간편하면서도, 재발율이 다른 방법에 비하여 높지 않다는 점에서 유용한 방법이라고 사료된다. Purpose: This study was designed to investigate the clinical and oncological results of giant cell tumor, treated with electrocautery as an adjuvant. Materials and Methods: We evaluated 47 giant cell tumors treated in both institution from 1989 to 2004, retrospectively. We treated all 47 cases with intralesional curettage and speed burring and followed electrocautery as an local adjuvant. Results: The mean follow up periods was 44 months and ranged from 12 to 180 months. The local recurrence occurred in 8 patients (17%) and one case lung metastasis was found. There was one case complication (septic knee) associated with electrocautery as an adjuvant. Conclusions: We thought electrocautery was feasible adjuvant treatment method of giant cell tumor, because it is very convenient technique, and the local recurrence rate was similar to any other methods.

      • 개선된 SARM을 이용한 객체지향 부품 재사용 시스템

        In this paper, we made software components reusable through syntax-analysis method, designed a Viewer for understanding component information, and retrieved similar components by using Enhanced SARM. Because SARM requires a lot of computation time, it was enhanced by reducing unnecessary activation value. Also GUI was designed for component-query relationship and Viewer represents hierarchy diagram of a retrieved component. This system supports facilities which can insert and delete components on diagram. For a component modification, this system supports a editor to rebuild class inheritance relationship. In this paper, SCRS (software components reuse system) is consisted of syntax-analysis method, component-query relationship interface, retrieval (Enhanced SARM), diagram viewer, reuse on diagram, and a editor.

        Rerouting기법들 간에 Orthogonal 관계를 통한 차별적인 서비스 제공에 관한 연구

        사용자가 요구하는 서비스들은 그들의 QoS에 따라 다양하게 분류되어질 수 있으며, 이렇게 각기 다른 특성을 갖는 서비스들이 네트워크 상에 발생할 수 있는 장애 상황에 대처하여 안전하고 효율적인 서비스를 제공하기 위해서는 이들 연결에 대한 재라우팅(rerouting) 기법이 사용되어야 한다. 본 논문은 장애가 발생할 수 있는 IP 네트워크 상에서 차별화 된 서비스 제공을 위해 각 서비스들의 QoS에 따라 이들의 우선순위를 분류하고 이에 맞는 재라우팅 기법을 연구하고자 한다. 이를 위해서 재라우팅을 시작하는 시작점에 따라 Source Rerouting, Link Rerouting 기법을, 재라우팅을 수행하는 시간에 따라 Immediate Rerouting, Random Rerouting을 연구하여 이들에 대한 효과적인 orthogonal 관계에 대한 연구를 통해 서비스의 우선순위에 따라 보다 높은 성능을 갖는 DRIT와 DRDT 재라우팅 기법을 연구하고자 한다 결과적으로 차별화 된 서비스를 지원하기 위해 각 재라우팅 기법을 차별적으로 적용한 DRDT 보다 높은 성능을 보이고 있다. Rerouting mechanisms must be used by connections in order to provide QoS (Quality of service) characterization of services, which provides mean for reliable and efficient transfer of services under fault generating network. Also, user's services can classily according to their QoS characterizations. In this paper, we study classification of user services according to their characterization for providing differentiated services, and propose rerouting mechanisms under fault generating network. For this, we study various rerouting mechanisms including rerouting locus of start (Source Rerouting, Link Rerouting), rerouting timing of start (Immediate Rerouting, Random Rerouting) and their orthogonal relationship, eventually we propose new rerouting mechanisms such as DRIT, DRDT which show higher performance according to priority of services than others. Our simulation shows that rerouting mechanism (DRDT), applied differentiated mechanisms is better performance to provide differentiated service.

        손목관절의 기능적 운동 특성의 동작 분석

        Injuries of wrist in upper extremity is common onset in industrialized world. The development of joint arthroplasty and mechanical joint is area of research for biomechanical engineer and surgeon for a decade. Therefore. the knowledge of characteristic of joint motion is essential to develop the artificial wrist joint. In this study. the joint motions of wrist required for activities of daily living (ADLs). including personal hygiene and care. and general home activity were measured using flexible electrogoniometer. Total of 25 different daily activities were separated into four groups and tested on 15 subjects who did not show any abnormality of their joint functions. The maximum functional range of motion required for ADLs were obtained and standardized for analysis and comparison. Also. a least functional range of motion for ADLs were investigated. Results revealed that any significant differences were not found in least functional range of motion between left and right wrist to perform ADLs. However. a significant difference was found in different ADLs. Therefore. least range of motion obtained in this study can be used as basic data to design artificial joint and set a goal for surgeon to achieve appropriate treatment from patients.

        골연골종은 주로 장골의 골간단에서 호발하는 종양으로, 골 구조와 연결성이 없는 연부 조직에 발생하는 골격골외 관절주변의 골연골종은 보고가 드물다. 저자들은 42세 여자 환자의 슬관절에서 발생한 골격골외 관절주변 골연골종을 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Osteochondromas typically arise from the metaphyseal ends of long bones. Extraskeletal paraarticular osteochondromas are unusual osteocartilaginous lesions that arise in the soft tissue adjacent to a joint with no bony continuity. We present a case of pathologically proven extraskeletal paraarticular synovial osteochondroma of the knee observed in 42-year-old female with a literature review.

      • 상완 신경총 손상에서 Steindler 주관절 굴곡 성형술

        Purpose: The purpose of this study was to analyze the clinical results after Steindler flexoplasty. Materials and Methods: We analyzed 6 cases who had nearly normal finger and wrist joint flexion function, but could not flex elbow joint actively because of upper arm type brachial plexus injury. We performed operation during the period from February 1997 to July 2003. There were 5 males and 1 female with mean age of 28 years (range: $19{\sim}51$ years) when Steindler flexoplasty was done. The average follow-up period was 3 years 11 months (range: 12 months${\sim}$7 years 4 months). We assessed active range of motion of elbow joint, muscle power and elbow function by Mayer & Green grade scale at last follow-up. We assessed how much they were favorable for Steindler flexoplasty and had improvement of upper extremity function and correction of deformity. Results: Postoperative, flexion range of elbow joint improved to average $111.7^{\circ}$ (range: $90{\sim}130^{\circ}$). $25.8^{\circ}$ (range: $15{\sim}45^{\circ}$) in flexion contracture and $16.6^{\circ}$ (range: $10{\sim}35^{\circ}$) in pronation contracture were remained. Range of motion of elbow joint improved to average $85.3^{\circ}$ (range: $45{\sim}105^{\circ}$). Flexion power of elbow joint improved to Grade 5 in all cases. Postoperatively on Mayer & Green grade scale, there were excellent in 3 cases(50%), good in 2 cases(33.3%), fair in 1 case(16.7%). On patient's own assessment of functional improvement, there were excellent in 4 cases(66.6%), good in 1 case(16.7%) and fair in 1 case(l6.7%). Conclusion: Steindler flexoplasty can reserve good clinical results with being improved to active flexion of elbow joint in cases who have functional hand and wrist, but paralysis upper arm muscle in brachial plexus injury.

        Fly-By-Wireless 비행제어 기술의 연구 동향

        This paper deals with recent research cases and directions of Fly-By-Wireless (FBWLS) flight control technology. FBWLS is a new type of flight control system technology with the aim of solving the problems mainly caused by the increasing amount of wires in aircraft to which Fly-By-Wire (FBW) technology applies. Therefore, in FBWLS flight control system the wired communication system is replaced with a wireless communication system. Currently the FBWLS flight control technology is at an initial development stage and thus this paper surveys deals with the cases in the viewpoint of technology feasibility. In this context, this paper analyzes technology that needs further studies to secure the reliability, stability and accuracy to the similar level of the corresponding FBW system. Since the major problems of FBWLS technology are packet losses and time delays so that this paper suggests the research direction of wireless communication protocol selection, optimization of wireless communication network and controller design considered communication environment.
