요양급여심사기준 완화에 따른 비스테로이드성 항염제 및 위장관 보호제 처방 변화 및 안정성 분석

Objective: The prevalence rate of osteoarthritis in Koreans aged 50 years or older is 14.3%, and the total amount of medical costsis more than KRW 1 trillion. Recently, the reimbursement guidelines for osteoarthritis treatment have changed. Methods: In thisstudy, we sought to describe prescription patterns of nonsteroidal anti-inflammatory drugs (NSAIDs) and gastro-protective agent(GPA) and analyze the clinical and economic impacts of the new policy using the national health insurance claims data. Theincidence of upper gastrointestinal adverse event by policy change was identified through the odds ratio, and changes in medicineand medical costs related to osteoarthritis through mean and median. Results: There were 204,552 patients before thereimbursement guidelines relaxation and 239,710 after it, a 17.2% rise. The prescription ratio was 3.3% for the patients prescribedwith COX-2 selective NSAIDs alone and 1.3% for those with both COX-2 selective NSAIDs and GPA combination before thereimbursement guidelines relaxation. The reimbursement guidelines relaxation significantly increased their ratios to 6.9% and 2.8%,respectively. Gastrointestinal adverse events significantly reduced by 1.21%p after reimbursement guidelines relaxation. The averagemedicine cost per person increased significantly to KRW 140,291 from KRW 137,323 after the reimbursement guidelines relaxation,while the average medical cost per person slightly decreased from KRW 311,605 to KRW 310,755 after the relaxation, showing nomeaningful difference. Conclusion: The reimbursement guidelines relaxation may influence on decreasing the upper gastrointestinaladverse event, increasing the medicine costs and maintaining the medical costs for osteoarthritis.

This study examined the cases of English education practices at home through the lenses of some language acquisition approaches, theories, and methods. The English educational practices were guided and supported by parents at home, and the parents claimed that their children could master English by being exposed to English from early on. This study suggested some implications of the importance of establishing and implementing English reading programs for children in the public education system. The findings in the current study show that input of English is the most crucial and essential part of acquiring English. Having children exposed to oral and written English, which is fundamental in English education, many parents shared their success stories of raising children who can communicate in English as a tool for having their voices in the world. The present study found that children exposed to English at home enjoyed using English as a useful tool in today’s modern world. This study implies that well-implemented reading programs would provide good quality English education for children.

원가족 부모의 부정적 양육태도가 성인초기 자녀의 친밀감 두려움에 미치는 영향: 수치심의 매개효과

The purpose of the present study was to investigate the association between negative parenting attitude in the family of origin on child's fear of intimacy in early adulthood, as well as the mediating role of shame. A total of 178 university students completed measures of parenting attitude, fear of intimacy, and internalized shame. Among the 8 sub-components of parenting attitude, neglect and over-interference were significantly related to fear of intimacy. And over-interference and affection were significant predictors of shame. Over-interference, the mutual predictor of fear of intimacy and shame was entered in hierarchical multiple regression to examine the mediating role of shame. The analysis indicated that shame fully mediated between parenting attitude and fear of intimacy. The implications and limitations of the study were discussed and suggestions for future researches were presented. 본 연구는 원가족 부모의 부정적 양육태도가 성인초기 자녀의 친밀감 두려움에 미치는 영향을 살펴보고, 그 과정에서 수치심의 매개효과를 밝히고자 하였다. 총 178명의 대학생을 대상으로 부모 양육태도, 친밀감 두려움, 내재화된 수치심 척도로 구성된 설문을 실시하였다. 부모 양육태도의 8가지 하위요인 중 방임과 과잉간섭이 성인초기 자녀의 친밀감 두려움을 유의하게 예측하였다. 또한 수치심에 유의미한 예언력을 가지는 부모 양육태도 하위요인은 과잉간섭과 애정인 것으로 나타났다. 그로부터 친밀감 두려움과 수치심을 공통적으로 예측하는 양육태도 변인으로 과잉간섭이 확인되었다. 이에 과잉간섭적 양육태도를 예측변인으로 하고 수치심을 매개변인으로 하여 친밀감 두려움을 설명하기 위한 위계적 회귀분석을 실시하였다. 그 결과, 부모의 과잉간섭이 성인초기 자녀의 친밀감 두려움에 영향을 미치는 과정을 수치심이 완전매개 하는 것으로 밝혀졌다. 즉 과잉간섭적인 양육을 받은 것이 수치심을 유발했을 때에 친밀감 두려움이 형성된다고 볼 수 있다. 끝으로 본 연구가 지니는 의의와 한계점을 논의하였고, 향후 연구를 위한 제안사항들을 제시하였다.

단순선형회귀모형에서 어떤 미지의 시점을 전후하여 회귀계수에 변화가 있었는지에 대한 통계적 가설을 검정하고 변화점의 추정방법을 논의한다. 이차형식의 통계량을 제안하고 그 근사분포 및 유의수준제어를 살펴보았다. 또한 하나의 예를 통하여 제안된 방법을 적용하고 우도비검정과 비교하였다. A testing and estimation procedure is considered for changes at unknown time point in simple linear regression model. A test statistic of quadratic form is suggested. We also discuss the asymptotic distribution and its level control. The proposed method is compared with the likelihood ratio test through a example.

위험률 변화점모형에 대해 변화점의 최우추정을 고려하였다. 추정량의 점근분포 및 붓스트랩 분포의 성질을 알아보고 변화점의 신뢰구간을 제안한다. 변화점의 위치 및 변화점을 전후하여 위험률의 값에 따라 모의실험을 수행하고 포함확률을 조사하였다. 추정량의 점근분포가 매우 복잡하기 때문에 이를 직접 이용한 변화점의 통계적 추론이 매우 어려운 점을 감안할 때 제안된 방법은 바람직한 대안이 될 수 있다.

Implication of Self-thinning in Salix Communities on Riverine Wetland Restoration

Self-thining was measured in Salix comunities on Bam Island in Seoul at various age stages.D2H was used to estimate tre biomas, where D is tem diameter at breast height or 10 cm height for plantswith eight <1.5 m, and H is height. A log-log plot of density versusD2H and corelation analysis indicated asignificant relationship betwen density and biomas with equation 'log D2H = .1.27 log N + 7.06'. This indicatesthat self-thining afects biomas in the Salix comunity with .1.27 as the thining coeficient. If we asumea thining exponent .3/2, then the alometric oeficient of the quation, log w = a log D2H + b, is 1.18. Thisis much igher than that for any other species tudied in Kore a. There were statisticaly significant relationshipsbetwen age and ensity and betwen age and basal area nd the se relationships ugest guidelines fortransplantation of wilows and for the asesment of Salix comunity restoration projects in riverine wetlandsbased on standard ensity, basal area, and age. The results o f this tudy may also increase understanding ofsucesion proceses in Salix comunity restoration in riverine wetlands.
