Purpose: We investigated the efficacy and safety of a combination of oxaliplatin, 5-fluorouracil (5-FU), and folinic acid (FA) as first-line palliative chemotherapy for elderly patients with metastatic or recurrent gastric cancer.Materials and Methods: The study patients were chemotherapy-naïve patients (〉 65 years old) with histologically confirmed, metastatic or recurrent gastric cancer. Chemotherapy consisted of oxaliplatin 100 mg/m2 and FA 100 mg/m2 (2-hour infusion), and then 5-FU 2400 mg/m2 (46-hour continuous infusion) every 2 weeks.Results: A total of 37 patients were studied between April 2004 and October 2006. Of the 34 evaluable patients, none achieved a complete response (CR) and 14 achieved a partial response (PR), resulting in an overall response rate of 41.2%. The median time to progression (TTP) was 5.7 months (95% CI: 4.2∼6.3 months) and the median overall survival (OS) was 9.8 months (95% CI: 4.4∼12.0 months). The main hematologic toxicities were anemia and neutropenia, which were observed in 56.7% and 32.4% of the patients, respectively. Grade 3/4 neutropenia was observed in 8.1% of the patients. None of the patients experienced febrile neutropenia. Peripheral neuropathy occurred in 35.1% of the patients and all were grade 1/2.Conclusion: This oxaliplatin/5-FU/FA regimen showed good efficacy and an acceptable toxicity profile in elderly patients with metastatic or recurrent gastric cancer. 무정형 전구체를 통한 도세탁셀 나노 결정의 제조 및 특성 연구

도세탁셀은 유방암, 비소세포폐암 등의 치료에 사용되는 탁산계 항암제로, 용해도와 투과성이 매우 낮은 특성을 보인다. 이러한 이유로 첨가제와 함께 알코올에 용해해서 정맥 주사제로 사용되고 있지만, 첨가제로부터 야기되는 부작용들이 문제가 되고 있다. 이를 해결하기 위한 여러 방법 중 나노결정 제조는 첨가제의 감소뿐만 아니라 용해속도 증가, EPR 효과 등의 장점이 있어서 주목받고 있다. 본 연구에서는 무정형 전구체를 통해 200 nm 이하의 안정한 도세탁셀 나노결정을 제조하였다. 지질을 용매로 사용하여 도세탁셀과 고분자 첨가제를 함께 녹인 뒤, 초임계 이산화탄소로 지질을 제거하여 전구체를 생성시켰다. 생성된 전구체를 고압균 질기를 이용해 수분산하여 최종 나노결정을 제조하였다. 나노결정의 생성 메커니즘을 공정 단계별 입자 분석을 통해 파악하였으며, ex vivo 실험을 통해 생물학적 특성을 연구하였다.

Yi Chung-hwan’s (1690-1756) T’aengniji has been read by Koreans over the centuries without knowledge of the original title or original content. Recently, Yi’s text in Nam Ha-haeng’s collection of scholars’ works on travel was announced to have the title of “T’aengniji.” After a close examination of Yi’s work in Wayurok, however, I find an evidence that the original title is not “T’aengniji” but rather “Sadaebu kagŏch’ŏ.” I also find “Sadaebu kagŏch’ŏ” in Wayurok not to be an abridged manuscript. It also has errors as well as unique features not found in most manuscripts. The errors in “Sadaebu kagŏch’ŏ” include the incorrect return date of a Qing emperor to China after waging a war in Korea “in 1636.” Yi Ik, Yi Chung-hwan’s grand-uncle, revealed in his letter in 1752 that the manuscript he received from his “family member” contained over ten errors and that the readers changed the title to “T’aengniji.” Based on Yi Ik’s letter and my cross-examination of “Sadaebu kagŏch’ŏ” with other manuscripts, this study concludes that “Sadaebu kagŏch’ŏ” in Wayurok, copied around 1766 must be either the original content or at least the closest to the original text of Yi Chung-hwan’s work, known as the T’aengniji.
