        중첩증후군:만성 폐쇄성 폐질환을 가 진 폐쇄성 수면무호흡-저호흡 증후군

        Overlap syndrome can be defined as a coexistence of chronic obstructive pulmonary disease (COPD) and sleep apnea-hypopnea syndrome (SAHS). The association of COPD and SAHS has been suspected because of the frequency of both diseases. Prevalence of COPD and SAHS is respectively 10 and 5% of the adult population over 40 years of age. However, a recent study has shown that the prevalence of SAHS is not higher in COPD than in the general population. The coexistence of the two diseases is only due to chance. SAHS does not affect the pathophysiology of COPD and vice versa. Prevalence of overlap syndrome is expected to occur in about 0.5% of the adult population over 40 years of age. Patients with overlap syndrome have a more profound hypoxemia, hypercapnia, and pulmonary hypertension when compared with patients with SAHS alone or usual COPD patients without SAHS. To treat the overlap syndrome, nocturnal noninvasive ventilation (NIV) or nasal continuous positive airway pressure (nCPAP) can be applied with or without nocturnal oxygen supplement.

        폐쇄성 수면 무호흡 증후군과 폐동맥 고혈압에서 엔도텔린-1의 역할

        Obstructive sleep apnea syndrome is associated with significant cardiovascular morbidity and increased mortality. However, it was controversial whether obstructive sleep apnea syndrome could cause pulmonary hypertension. The controversy was resolved by several studies that have shown pulmonary hypertension in 20% to 40% of patients with obstructive sleep apnea syndrome without underlying other cardiopulmonary diseases and reductions in pulmonary arterial pressure in patients with obstructive sleep apnea syndrome after treatment with nocturnal continuous positive airway pressure. Recent studies provide strong evidence for endothelial dysfunction in obstructive sleep apnea syndrome and pulmonary hypertension. Endothelin-1 is a 21 amino acid peptide with diverse biologic activity such as highly potent vasoconstrictor and mitogen regulator that may play a key role in obstructive sleep ap-nea syndrome and pulmonary hypertension. Continuous positive airway pressure therapy is moderately effective in reducing pulmonary arterial pressure. Further researches are needed to assess the therapeutic efficacy of pharmacologic therapy with agents that inhibit the action of endothelin-1 in obstructive sleep apnea syndrome patients with pulmonary hypertension.

        BCG osteomyelitis is rare comlication and the incidence rate has been estimated to be 1/milion vaccinated neonate and infants. BCG osteomyelitis is also reported as a complication of intravesical BCG treatment for bladder carcinoma. We had experienced a 14 months old infant who presented swelling, tenderness and redness on left upper arm suspicious due to BCG vaccination. The MRI finding showed $2{\times}4{\times}4cm$ subcutaneous abscess with cortical defect on proximal humerus and axillary lymph adenopathy. The histologic finding showed diffuse caseous necrosis and Langhans type giant cell. We report A case of BCG osteomyelitis.

        폐쇄성 수면 무호흡 증후군과 상기도 저항 증후군의 진단적 및 임상적 차이

        It has been controversial whether upper airway resistance syndrome (UARS) is a distinct syndrome or not since it was reported in 1993. The International Classification of Sleep Disorders classified UARS under obstructive sleep apnea syndrome (OSAS) in 2005. UARS can be diagnosed when the apnea-hypopnea index (AHI) is fewer than 5 events per hour, the simultaneously calculated respiratory disturbance index (RDI) is more than 5 events per hour due to abnormal non-apneic non-hypopneic respiratory events accompanying respiratory effort related arousals (RERAs), and oxygen saturation is greater than 92% at termination of an abnormal breathing event. Although esophageal pressure measurement remains the gold standard for detecting subtle breathing abnormality other than hypopnea and apnea, nasal pressure transducer has been most commonly used. RERAs include phase A2 of cyclical alternating patterns (CAPs) associated with EEG changes. Symptoms of OSAS can overlap with UARS, but chronic insomnia tends to be more common in UARS than in OSAS and clinical symptoms similar with functional somatic syndrome are also more common in UARS. In this journal, diagnostic and clinical differences between UARS and OSAS are reviewed.

        인프라그램을 활용한 "식물의 건강 모니터링" STEAM 수업이 초등학생의 창의적 문제해결력, 과학 탐구 능력 및 정의적 영역에 미치는 영향

        Digital technology has adopted in classroom for meaningful and relevant learning. In this paper, the development and application of the STEAM program, whose main theme is ‘Monitoring of plants health’, are introduced. The STEAM program also contained a series of activities to use Infragram, an online based infrared camera tool. This study focused on the effects of the program on creative problem solving ability, scientific process skills and affective domain in elementary student’s level. The study subjects were composed of two groups of fifth grade students in J province. The experimental group was involved to learn in the integrated education program class, while the comparative group participated in traditional science lessons. As the result, the developed STEAM program contributed to the significant improvement of elementary students’ creative problem solving ability, scientific process skills and affective domain. The authors propose that STEAM program for plants health monitoring using Infragram is a powerful approach to facilitate learners’ development of these skills.

        Evaluation of TYLCV-resistant Tomato Germplasm Using Molecular Markers

        토마토 황화잎말림바이러스(tomato yellow leaf curly virus, TYLCV)병은 세계전역에 걸쳐 토마토 생산에 막대한 손실을 입히고 있으며, 국내에서도 피해 농가가 속출하고 있어, 강한 저항성을 지닌 국내 품종의 개발이 매우 시급하다. 본 연구에서는 현재까지 알려진 주요 TYLCV 저항성 유전자인 Ty-1, Ty-2, Ty-3, Ty-3a를 지니는 저항성 계통을 해외로부터 도입하여 계통이 지니는 각 저항성 유전자간 대립유전자의 조성을 DNA 분자표지를 이용하여 분석한 결과, 분자표지의 유전형과 저항성 표현형간의 완전한 일치를 관찰할 수 있었다. 또한 AFLP을 통한 유연관계분석에서 TYLCV 저항성 유전자원과 국내 시판 품종 및 육성계통간 유전적 배경이 상당히 유사한 것으로 드러나, 본 유전자원을 국내 육종재료로 활용하는 것이 매우 용이하리라 판단되었다. 본 연구에서 소개된 TYLCV 저항성 도입계통들과 분자표지를 이용하여 다양한 저항성 유전자들을 집적시킴으로써, 높은 수준의 TYLCV 저항성을 지닌 국내 품종개발이 단시간에 가능하리라 생각된다. 하지만, 보다 정확한 저항성 판별을 위해서는 효율적인 생물검정기술의 확보가 국내 품종육종에 절실하다. Tomato yellow leaf curl virus (TYLCV) infection causes serious economic losses in tomato production world-wide, and the development of new domestic cultivars with high levels of resistance to TYLCV is required. In this study, major advanced breeding lines carrying TYLCV-resistance loci, Ty-1, Ty-2, Ty-3, and Ty-3a were collected from overseas breeding programs, and their allelic composition of each resistance locus were confirmed by using public PCR-based DNA markers. Perfect matches between marker genotypes and the resistance indicated a feasibility of marker-assisted selection (MAS). In addition, genetic relationship of the TYLCV-resistant germplasm with commercial hybrids broadly grown in South Korea and other breeding lines were evaluated by AFLPs. A very close genetic background among these tomato germplasm revealed by the genetic similarity coefficient (0.87 to 1.00) and UPGMA implied the easiness of a rapid incorporation of the resistance genes. Phenetic tree showed a close relationship between TYLCV-resistant CLN lines and general type commercial hybrids, while Gc lines formed an independent group distinguished from both general and cherry type cultivars. Pyramiding TYLCV-resistance alleles using the resistant germplasm and DNA markers introduced in this report will greatly improve the breeding process for high levels of TYLCV-resistance. For accurate evaluation of the TYLCV-resistance, however, a reliable method of bio-assay still remains to be established.

        <여씨향약언해> 화산본의 성조 연구- <석보상절>, <월인천강지곡>의 성조와 비교를 중심으로-

        The purpose of this study has two aims. The one is to describe similarities and differences between the tone in <Seokbosangjeo>, <Worincheongangjigok> published in mid-15th century and it in <Yeossihyangakonhae> published in 16th century early, And the other to elucidate a change of the tonal system from mid-15th century to 16th century early, According to the study, the tone in <Yeossihyangyakonhae> inherit it in <Seokbosangjeo> and <Worincheongangjigok>. But there is a slight change of the tonal system in the 16th century early. The tonal system consists of three tones in the mid-15th century:pyeongcheuk patterns, departing tonal pattern and rising tonal pattern. The pyeongcheuk patterns in the mid-15th century are classified into two tones:the pyeonggeohyeong and the pyeongsanghyeong by the distinction between departing tone and rising tone behind plain tone. But the pyeongsanghyeong in the pyeongcheuk patterns has been seriously undermined, since the process to distinguish between departing tone and rising tone behind plain tone nearly disappeared in the 16th century early. So the pyeongsanghyeong in the tonal system may be confused by the process of disappearance. Therefore, there was no loss of the tone as well as the destruction of the tonal system in the 16th century early. However, there is a slight change of the tonal system due to the change of constraints on combination of the tonemes.

        타임랩스를 활용한 스마트미디어 기반의 "식물의 한살이" STEAM 프로그램 개발과 적용 효과

        The purpose of this study was to investigate whether the STEAM program related ‘Life cycle of plant’ using a photographic technology time-lapse with smart media affects on elementary students’ creative problem solving ability, scientific process skills and affective domain. The participants were divided into an experimental group which is consisted of 53 students and a comparative group comprised 51 students of fourth grade, and they were engaged in different teaching and learning methods during 8 class hours. For the experimental group, the developed STEAM program based on integrated education with smart media was applied, whereas the other group learned the same scientific learning contents in traditional lessons. The results of this study indicate that the students who participated in the STEAM education achieved high score of creative solving problem ability, but their process skills did not improve significantly than the comparison group did. Enhancing the students’ affective domain with a significant difference, the STEAM approach was effective also the learners satisfied to study in the STEAM lessons. The authors suggest that using timelapse with smart media in STEAM lessons integrated life science may contribute to the effects on elementary student’s creative problem solving ability and affective domain such as critical thinking, motivation, attitudes toward science or interest.
