저자들은 FDG-PET과 전산화 단층 촬영에서 인두, 복부, 흉부에 악성 종양이 의심되는 소견이 보여 점막 표피모양 암종으로 오인된 복강내 결핵, 인두 결핵, 기관지 결핵이 동반된 환자를 조직 검사를 통하여 결핵으로 진단하였고, 이 환자에게 항 결핵제 투여의 치료를 통하여 추적 전산화 단층 촬영에서 호전 경과를 보인 1예를 경험하였기에 이를 보고하는 바이다. Tuberculosis remains as a major public health problem worldwide. In addition to classic pulmonary tuberculosis, tuberculosis may sometimes present atypically. In the case of atypical tuberculosis, the unusual sites and properties that mimic other diseases can lead to a misdiagnosis and therapeutic delay. Abdominal and pharyngeal tuberculosis are uncommon extrapulmonary manifestations of tuberculosis. To the best of our knowledge, a combination of abdominal and pharyngeal tuberculosis with endobronchial tuberculosis has not been reported. We report a case of concurrent abdominal and pharyngeal tuberculosis in a patient with chronic endobronchial tuberculosis mimicking a metastatic malignancy on computed tomography and FDG-PET. The steady and dynamic process model for an internal reforming molten carbonate fuel cell power plant is discussed in this paper. The dominant thermal and chemical dynamic processes are modeled for the stack module and balance-of-plant, including cathode gas preparation, heat recovery, heat loss (Each heat loss amount for the stack and MBOP is obtained from real plant data) and fuel processing. Based on dynamic model and control demand, PID controllers are designed in the whole system. By applying these controllers we can obtain temperature balance of stack and control system depending on changing steam to carbon ratio, air feed amount, and transient condition.

발전용 연료전지의 보급이 점차 증가하면서 시스템의 효율에 대한 관심이 높아지고 있는 실정이다. 효율은 연료전지의 운전상태 및 수명을 판단하는 중요한 요소 중에 하나이다. 따라서 본 논문에서는 300㎾급 발전용 연료전지의 계통연계 시 효율계산 수식을 정의하며, 이론적인 효율이 아니라 실제 운전되고 있는 MCFC 발전용 연료전지의 효율과 단위장치의 동작이 시스템 효율에 미치는 영향을 분석하였다. 효율분석을 위해 제한된 측정 포인트를 이용하여 출력을 확인 할 수 있도록 수식을 정의하고, 검증하였으며, 출력별 단위장치의 동작분석을 통해 시스템의 운전 상태와 성능의 관계를 확인하였다.
90세 이상 초고령 환자에서 수술 전 심장 초음파 검사 분석

Background/Aims: Adverse cardiac events pose a significant perioperative risk to patients undergoing noncardiac surgery, especially geriatric patients. This study presents resting echocardiography data for a population older than 90 years. Methods: We performed transthoracic echocardiography in 108 patients who were older than 90 years and scheduled to undergo noncardiac surgery between November 1, 2004, and September 22, 2008, and evaluated their cal histories and electrocardiograms. Results: The 108 patients included 73 (67.3%) women and 35 (32.7%) men with a mean age of 92.05±2.77 years (range 90-104). Their medical histories included hypertension in 38.9% and diabetes mellitus in 15.7%. The average left ventricular end diastolic dimension and ejection fraction were 49.0±6.3 mm and 62.9±11.1%, respectively. Six (5.6%) patients had clinically significant aortic regurgitation and seven (6.5%) had mitral regurgitation as pathologic changes of aging. Six patients had mild to moderate aortic stenosis, found incidentally, that did not affect the clinical decision regarding the scheduled operation. The diastolic parameters showed that most of the patients (87%) had grade I relaxation abnormalities. The average mitral inflow E/A, deceleration time, and E/E`s were 0.69±0.33, 266±82 msec, and 11.15±3.88, respectively. Conclusions: Echocardiography cannot predict all of the postoperative cardiovascular complications and risks, but it is useful for screening for significant left ventricular dysfunction, valvular abnormalities, and as a guide for managing extremely old patients in the perioperative period.
