An experimental study on gtr technique using lyodura and microfilter in rat s osseous defects

구강악안면외과 영역에서 골 결손부에 대한 골재생유도에 관한 많은 연구를 통해 GTR technique에 이용되고 있는 Gore-tex membrane의 생물학적 친화성이나 골 결손부의 재생시 유도능력에 관한 확신올 제공하고 있다. 최근에는 치주치료에 이용되는 GTR과 구분을 위하여 골 조직의 선택적인 재생을 유도한다는 의미로 GBR(Guided bone regeneration)과 osteopromotion이라는 용어가 주로 사용이 되고 있고 협의의 치료개념이 아닌 광범위한 악안면의 결손부의 재건에 응용이 기대되고 있다. 그러나 재료의 값이 비싸고 비흡수성으로 제거가 필요하다는 단점을 갖고 있다. 이에 저자 등운 값이 싸며 흡수성을 갖는 lyodura와 비흡수성의 microfilter를 GTR에 이용하여 Gore-tex membrane을 대치하는 효과를 살펴보고자 하였다. 실험군은 lyodura와 microfilter group으로 나누어 백서의 좌측 하악 우각부에 인위적인 8×8mm의 전층골결손부를 형성하고 각기 lyodura와 microfiliter를 골결손부위에 기개하고 봉합하였으며 반대측은 각 실험군의 대조군으로 골 결손부를 피개하지 않고 방치하였다. 술후 1주, 3주, 5주, 8주 후에 각기 실험동물을 희생시키고 골결손부의 조직학적 검사를 시행하였다. 양 실험군은 대조군에 비해 골결손부에 새로운 골의 형성이 더 많이 되었고, Lyodura 군이 Micro filter꾼에 비하여 골형성이 더 이루어졌다. 본 실혐의 결과로 흡수성의 Lyodura가 GTR technique에 이용되는 Gore-Tex를 대치할 수 있는 매우 유용한 재료로 사료되었다. Many reports confirmed excellent biocompatibility and ability of bony regeneration of e-PTEE (Gore-tex membrane) in the Oral & maxillofacial field. At present, the term「Guided bone regeneration」and osteopromotion are mainly used in the literature for this membrane technique. Biologically membrane coverage of bone defects create a suitable environment for bone regeneration. However, disadvantages of high cost and necessity of membrane removal still remained. Recently, biodegradable membranes have been studied and used The present study was designed to consider the effect of lyodura and microfilter for replacement of Gore-tex membrane in rat s osseous defects. The experimental groups were divided into 2 groups. Group 1 is the lyodura group, Group 2 is the microfilter group. The control group was the contralateral angle defect without membrane coverage. Fullthickness bony defects to expose the marrow 8×8 mm in size were created in the right and left mandible angle of rats, and then lyodura and microfilter were covered on the left side and not covered in the right side as a control. Each group was sacrificed after 1 week, 3 weeks, 5 weeks, and 8 weeks after surgery and histologic examination was performed. Both experimental groups had earlier and more bone formation than the control group. The Lyodura group seemed to have more bone formation than the Microfilter group. These results suggest that Lyodura may be very useful in GTR for replacement of Gore-tex material.

There were 6 deaths in the last 4 years in our dental clinic. The patients died form squamous cell carcinoma, odontogenic infection, adult respiratory distress syndrome(ARDS), and general anesthesia. Each case is analysed and presented, for this subject is rarely dealt in the literature and have relation with a complex medicolegal problem.

Polymethylmethacrylate(PMMA) is currently commonly used material for the reconstruction of bone defects and fixation of joint prosthetics following congenital and acquired causes. Although PMMA has widespread use, it does not possess the ideal mechanical characteristics with osteoconductivity and osteoinductivity required. In order to overcome these problem, addition of bovine bone drived defatting demineralized bone(BDB) powders to a PMMA bone cement was done for improvement of physical property and bone forming characteristics of composite. In order to investigate the influence of BDB reinforcement on the PMMA, we measured physical property of compressive, tensile, flexural strength, and scanning electron microscopic examinations. The results were obtained as follows: 1. The PMMA forms a solid cellular matrix with open cells about $100{\mu}m$ in variable size and incorporating BDB. BDB aggregates inside the cells form a porous network that is accessible from the outer surface. 2. The physical properties were compressive strength of mean $22.74{\pm}1.69MPa$, tensile strength of mean $22.74{\pm}1.69MPa$, flexural strength of mean $77.53{\pm}6.93MPa$. Scanning electron microscopic examinations were revealed that there was DBD particles form a highly porous agglomerates. BDB can be added PMMA in the form of dried powders, the composites are applicable as bone substitutes. BDB and PMMA mixture is shown to produce a class of composites that due to their microstructure and improved mechanical properties may be suitable for application as bone subsitutes. The mechanical and material properties of the BDB-PMMA bone substitute composites are competitive with those properties of a porous ceramic matrix of other hydroxyapatite and with those of natural bones.

Angiolymphoid hyperplasia with eosinophilia is an unusual and controversial lesion that occurs primarily in the head and neck area. This lesion was usually confused with Kimura's disease. We present the case of a 32-year-old woman with massive soft movable mass in left facial area which was diagnosed preoperatively as a fasciitis nodular. The final histologic diagnosis of the excised mass was angiolymphoid hyperplasia with eosinophilia(ALHE). 호산구 증가증을 동반한 ANGIOLYMPHOID HYPERPLASIA는 주로 두경부의 표층에 발생되고 진단에 어려움이 있는 특이한 질환이다. 일본에서 처음 보고된 이래 주로 동양에서 종종 보고되어온 KIMUR'S DISEASE와 많은 논란이 되고 있다. 저자등은 좌측 안면부에 거대한 유동성의 종괴를 주소로 환자로서 술전 진단에 상당한 어려움이 있었고 술후 최종적으로 ALHE로 진단된 32세 여자환자를 문헌고찰과 더불어 보고하는 바이다.

The purpose of this investigation was to evaluate the relationship between the hydration time of demineralized freeze-dried bone(DFDB) and early new bone formation in rat calvarial defects filled with DFDB. Rats (n = 43) were divided into 4 experimental groups. Standard, transosseous circular defects of the calvaria were made midparietally. In experimental group 1, the defect was grafted immediately after soaking the DFDB. In experimental group 2, the defects were grafted with DFDB after soaking the DFDB for 10 minutes. In experimental groups 3 and 4, the defects were filled after soaking the DFDB for 30 and 60 minutes, respectively. Graft sites were analyzed histologically after healing periods of 1, 2, or 4 weeks. Each group showed similar bone regeneration at each time point by histological analysis. The results of this study were as follows: 1. After 1 week, a significant amount of inflammation, granulation tissue, and edema were found. A small amount of bone was seen, but the amount of bone did not differ between groups. 2. After 2 weeks, a small amount of new bone formation and DFDB resorption were observed. 3. After 4 weeks, a greater amount of new bone formation was observed. The greatest amount of bone formation occurred in experimental group 4 after 4 weeks. We conclude that the hydration time of DFDB does not affect new bone formation and that it is very important to control inflammation in bone grafting.

The ameloblastoma is considered to be a slowly growing locally invasive tumor with a high rate of recurrence if not removed adequately. Ameloblastoma generally dose not metastasize to distant places. In very rar case, ameloblastoma changed its histologic appearance to carcinoma or showed distant metastasie, these generally called malignant ameloblastoma. Inadequate surgical procedures, radiation therapy, and a long duration of this tumor seem to have significant relation to the development of metastasis. Therefore, adequate surgical treatment of the primary lesion plays an important role in the prevention of metastasis postoperatively. This report described an instance of malignant ameloblastoma and review of literature.
