        신혈관성 고혈압쥐에서 Atrial Natriuretic Peptide가 혈관 기초장력에 미치는 영향

        Purpose: Hypertension may be involved an alteration of intrinsic basal tone in vascular smooth muscle. The purpose of this study was to investigate the vasorelaxant effect of atrial natriuretic peptide (ANP) on isolated non-contracted aorta from two-kidney, one clip (2K1C) renovascular hypertensive rats. Methods: 2K1C hypertension was induced by clipping the left renal artery and were used 6 weeks later. Age-matched rats receiving a sham treatment, which served as controls. The thoracic aortae were mounted in tissue baths to measure the isometric tension. Results: ANP diminished basal tone in previously unstimulated thoracic aortic rings from 2K1C hypertensive rats, while it had no effect in the control rats. Endothelial destruction potentiated the vasorelaxant effect of ANP on basal tone in 2K1C rats. A similar potentiation of the ANP response was observed by pre-treatment with Nω-nitro-L-arginine methyl ester (L-NAME) or methylene blue in aortic rings with endothelium. Treatment with calcium-free Krebs decreased basal tone and abolished ANP-response. These effects were observed only in aortic rings from 2K1C rats. Similarly, staurosporine and calphostin C, inhibitors of protein kinase C (PKC), lowered basal tone and abolished ANP-response in hypertensive rats. Conclusion: These results demonstrate that ANP has a vasorelaxant effect on basal tone in 2K1C renovascular hypertension. Inhibition of ANP effects on basal tone by calcium-free Krebs and PKC antagonists suggests that altered Ca2+-active tone is involved in hypertension, that modifies the response of vascular smooth muscle to the ANP.

      • 자기공명영상을 이용한 만성 족관절통의 원인 분석

        Purpose : Chronic ankle pain is one of the most popular problems with orthopedic clinicians, but it has complex etiologic factors in diagnosis. Recently, magnetic resornance imaging has used as common diagnostic modality in ligaments and tendon problems of ankle sprain. The purpose of this article is to determine the appearances and frequency of chronic ankle sprain by MRI, and to determine the value of MRI as a diagnostic tool. Materials and Methods : We have analysed MRI findings of 84 chronic sprained ankles over 6 months duration from September 1995 to January 1997. All cases had been past history of ankle injury. Among them, sports players ie, soccer, basket ball, competitive runner and ballet dancer are 51 cases( 60.7% ). Average age of patients is 27.06( 13 to 54 ) years old and sex ratio is 23:5. For the confirmative diagnosis, We adopted variable methods, ie, ultrasonogram, bone scan, CT scan, arthroscopic examination and surgical intervention. Results : The Etiologic factors of chronic ankle sprain by MRI includes 39 Lateral ankle instability due to Anterior Talofibular and Calcaneofibular ligamentous tear, 18 Peroneal tendon insufficiency ie, synovitis, longitudinal tear, subluxation and abnormal tendon insertion, 15 Osteochondral fracture of talus or tibia, 10 Anterior Impingement syndrome due to anterior bony spur, 9 Deltoid tear, 6 Os subfibulare, 4 Distal tibiofibular ligament abnormality, 3 Os subtibiale, 2 Ankle synovitis, 2 Intraarticular loose body. Conclusion : MRI study employed in hazy chronic ankle sprain is useful diagnostic modality. But, Diagnostic result using MRI was not correlated the clinical prognosis, and proper physical examination is important in the point of technical aspect to get the exact dissecting angle of MRI.

        축구 선수의 족관절 후방 삼각골 증후군 : 유병률, 진단 및 치료 Incidence, Diagnosis & Treatment

        Os trigonum is developed from an accessory ossification center of the posterior aspect of the talus. Chronic impingement force between posterior lip of tibia and calcaneous, increase the prevalence of Os trigonum. In soccer, a drop shoot is most commonly used skill in all over a game, and affect Os trigonum syndrome cause of the posterior ankle pain, sometimes. From Jan. 1994 to Dec. 1996, the lateral ankle roentgenogram had been obtained in 50 soccer players to determine the incidence of os trigonum. Average age of the soccer players was 25.5 years. All 71 cases obtained from 50 ;layers had no symptom in posterior ankle at the time of X-ray check up. We differentiated the findings of lateral X-ray as isolated os trigonum, large posterior tubercle of the talus and normal. Of the 71 cases, 46.5% had the os trigonum, 26.8% large posterior tubercle of the talus, 26.8% normal. Average incidence of the os trigonum is reported about 8∼13% in normal population, but incidence of soccer players is significantly high.
