      • 축구선수에서 발생한 상치골지(Superior Pubic Ramus) 스트레스 골절에 대한 임상적 결과

        Stress fractures are common injuries that begin with repetitive and excessive stress on the bone. It is very rare that stress fracture was occurred in pubis superior ramus in athletics, and the report concerning the exercise therapy for this fracture was not sufficient in the literature. We report of a case of a 17-year-old male soccer player who suffered from left inguinal pain due to the stress fracture of pubis superior ramus. He was conducted an exercise program that consisted of muscle strength, stretching, aerobic and functional exercise for 2 months. After this intervention, all muscle strength was significantly improved (17%) compared to baseline data, and also flexibility and range of motion were improved. This case shows that exercise therapy considered the cause and cyclic formation of bone remodeling is benefit to improve the exercise performance for a soccer player with superior ramus stress fracture.

        좌심실 보조장치를 삽입한 환자에게 적용된 개별화된 운동 변인의 효과

        Left ventricular assist devices (LVADs) are a treatment option for patients with severe chronic heart failure. These patients are referred to an inpatient cardiac rehabilitation after implantation to improve an aerobic capacity and quality of life (QOL). Several studies have reported that an exercise therapy, which is a component of cardiac rehabilitation, improves exercise capacity and QOL. The LVADs were implanted successfully in a destination therapy in two Korean patients, and these patients were enrolled in the cardiac rehabilitation. After an individualized intervention, they were discharged from improved exercise functional capacity and QOL. This is the first report showing a benefit of the individualized exercise therapy using different parameters after LVADs implantation in Korea.

        PECVD법에 의해 증착된 SiO<sub>2</sub> 후막의 광학적 성질 및 구조적 분석

        저온($320^{\circ}$C)에서 $SiH_4$와 $N_2O$ 가스의 혼합을 통해 플라즈마화학기상증착(PECVD)법을 이용하여 실리카 광도파로의 클래딩막으로 사용되는 $SiO_2$ 후막을 제조하였으며, 공정변수로는 $N_2O/SiH_4$ 유량비와 RF power에 변화를 주었다. 증착된 시편은 $N_2$ 분위기의 열처리로에서 $1050{\circ}$에서 2시간동안 열처리하였다. $N_2O/SiH_4$ 유량비가 증가함에 따라 증착속도는 $9.4~2.9{\mu}m /h$까지 감소하였으며, RF power가 증가함에 따라 증착속도는 $4.7~6.9{\mu}m /h$까지 증가하였다. 두께 및 굴절률은 Prism Coupler를 이용하여 분석하였다. 화학적 성질 및 구조적 성질은 X-ray Photoelectron Spectroscopy(XPS)와 Fourier Transform-Infrared Spectroscopy(FT-IR)를 이용하여 분석하였으며, Scanning Electron Microscopy(SEM)를 이용하여 시편의 단면을 관찰하였다. Silicon dioxide thick film using silica optical waveguide cladding was fabricated by Plasma Enhanced Chemical Vapor Deposition(PECVD) method, at a low temperature ($320^{\circ}$C) and from $(SiH_4+N_2O)$ gas mixtures. The effects of deposition parameters on properties of $SiO_2$ thick films were investigated by variation of $N_2O/SiH_4$ flow ratio and RF power. After the deposition process, the samples were annealed in a furnace at $1150^{\circ}$C, in N2 atmosphere, for 2h. As the $N_2O/SiH_4$ flow ratio increased, deposition rate decreased from 9.4 to 2.9 ${\mu}m/h$. As the RF power increased, deposition rate increased from 4.7 to 6.9 ${\mu}m/h$. The thickness and the refractive index measurements were measured by prism coupler. X-ray Photoelectron Spectroscopy(XPS) and Fourier Transform-infrared Spectroscopy(FT-IR) were used to determine the chemical states. The cross-section of films was observed by Scanning Electron Microscopy(SEM).

        골반 안정화 운동이 엉치엉덩관절 기능부전 환자의 관절가동범위, VAS 및 근 기능에 미치는 영향

        연구목적: 본 연구는 엉치엉덩관절 기능부전 여성 환자를 대상으로 12주간 골반 안정화 운동과 근 에너지 기법 프로그램을 적용한 후 관절가동범위, VAS, 그리고 근 기능 변화에 미치는 영향을 알아보는 것이다. 연구방법: 연구 대상자는 이학적, 자기 공명 검사 상 엉치엉덩관절 기능부전 진단을 받은 엉치엉덩관절 통증 성인 여성 환자 20명을 근 에너지 기법집단(MEG, n=10), 골반 안정화 운동 집단(PSG, n=10)으로 분류하여 12주간 각각 적용한 후 처치 전, 처치 후의 결과를 관찰하여 다음과 같은 연구결과를 도출하였다. 연구결과: 관절가동범위의 바깥 돌림 변화는 시기, 상호작용, 집단에서 유의한 변화와 Paired t test 결과 MEG(P<.05), PSG(P<.001), Independent t test 결과 PSG에서 더 유의한 변화가 나타났다(P<.001). 안쪽 돌림 변화는 시기(P<.001), 상호작용(P<.003)의 유의한 변화와 Paired t test 결과 MEG(P<.05), PSG(P<.01), Independent t test 결과 PSG에서 더 유의한 변화가 나타났다(P<.05). VAS는 시기(P<.001), 상호작용(P<.001)의 유의한 변화와 Paired t test 결과 MEG(P<.01), PSG(P<.001), Independent t test 결과 PSG에서 더 유의한 변화가 나타났다(P<.01). 근 기능의 변화는 전방, 후방, 우측, 좌측 기울임 모두 시기, 상호작용에서만 유의한 변화가 나타났고, Paired t test 결과 MEG와 PSG 모두 유의한 차이가 나타났고, Independent t test 결과, PSG에서 더 유의하게 개선된 것으로 나타났다. 결론: 이상의 결과, 엉치엉덩관절 기능부전으로 통증을 호소하는 여성 환자의 증상 개선을 위한 중재 방법 중 PSG에서 MEG에 비해 더 긍정적인 결과를 나타냈다. 이후 연구에서는 다양한 연령대를 포함한 많은 환자가 연구에 참여하여 임상 연구가 진행되기를 기대한다. Purpose: The aim of this study is to confirm effects of different type of exercise on range of motion (ROM), visual analogue scale (VAS) and myofunction in patients with sacroiliac joint dysfunction (SID). Method: Twenty patients with SID participated in this study. They were allocated to two different exercise groups according to the order of enrollment; muscle energy technique group (MEG, n=10) and pelvic stabilization exercise group (PSG, n=10). The exercise program was conducted with 3 times per week, 1 hour for 12 week. ROM of external rotation (ER) and internal rotation (IR), VAS, and myofunction in hip joint was evaluated before and after exercise intervention. Result: In ROM of ER, there was a significant increase in MEG and PSG but PSG showed more increase. A significant increase in IR showed in MEG and PSG. VAS was a significant increase in MEG and PSG but PSG showed more increase. In change of myofunction, all variable showed a significant difference. Independent t test results for ROM, VAS, and myofunction showed more significant changes in PSG than MEG. Conclusion: This study demostrated that MEG and PSG showed a positive result to improve ROM, VAS, and myofunction and pelvic stabilization exercise is more beneficial exercise intervention than muscle energy technique in patients with SID. Further study with large sample is needed to confirm the result of this study.

        안정시 및 운동 강도에 따른 고교축구선수의 뇌혈류 속도 및 저항지수의 변화

        The purpose of this research is to prepare exercise stability/safety by predicting possible dangerous situations during competitions in advance and presenting appropriate exercise intensity based on TCD(Transcranial Doppler ultrasound) analysis of Systolic, Diastolic and Mean Blood Flow Velocity, Pulsatility Index(PI) and Resistive Index(RI) from MCA according to exercise intensity as well as at rest. The subjects were 10 soccer players from high school over 5 years in experience. To analyze Cerebral Blood Flow, PI and RI Changes according to exercise intensity, treadmill test was performed among single group at rest and with moderate, high and maximal intensity. In comparison with the velocities at rest, Systolic, Diastolic and Mean Blood Velocity were increased at moderate and high intensity but they were decreased at maximal intensity(p<0.05). On the other hand, Pulsatility and Resistive Index were increased at all the other intensities(p<0.05). In a repeated measured One-way ANOVA, there were significant differences between the each measured values-blood velocities and indexes-and the various exercise intensities (p<0.05). In a post Hoc test, there were significant differences between rest and the other intensities in Systolic Blood Velocity ; rest and moderate/high intensity, moderate and maximal intensity, and high and maximal intensity in Diastolic Blood Velocity ; rest and moderate/high/maximal intensity, and high and maximal intensity in Mean Blood Velocity ; rest and moderate/maximal intensity, and moderate and high intensity in Pulsatility and Resistive Index (p<0.05).

        편평배부 증후군 환자에 대한 교정운동 프로그램이 척추 구조, 목 장애지수 및 균형 능력에 미치는 영향

        Purpose: This study aims to compare and analyze the effect of a 12-weke corrective exercise program on the structural variables of spinal curvature, neck disability index and balance ability in patients with flat back. Method: In this study, 24 patients with flat back participated, and they were randomly divided into three group; corrective exercise group (CEG: n=8), resistance exercise group (REG; n=8), and physical therapy group (PTG: n=8). The CEG and REG performed corrective exercise3 times per week and 70 minutes at a time for 12 weeks and REG conducted the exercise with thera-band. The variables of spinal curves included cervical lordosis, thoracic kyphosis, and lumbar lordosis were measured by X-ray test. The function of the neck was checked using a questionnaire with a neck disability index. The balance ability was measured by a balance machine and the test was conducted before and after exercise intervention for 12 weeks. Results: The results of this study are as follows. The cervical lordosis angle showed significant differences in both group and period, interactions p(<.001). Significant differences were found in CEG p(<.001), at each group period, (p<.05) in the groups (p<.001), as a result of paired t-test, REG and PTG, and in CEG and PTG (p<.001) post-testing. Thoracic kyphosis angle showed significant differences in both group and period, interactions (p<.001). Significant differences were found in CEG (p<.001), at each group period (p<.01) in the groups (p<.05), as a result of paired t-test, REG and PTG, and in CEG and PTG (p<.001) post-testing. The lumbar lordosis angle showed significant differences in both group and period, interactions (p<.001). Significant differences were found in PTG (p<.001) and CEG (p<.001) levels in each group, and (p<.001) levels between groups. As a result of paired t-test, REG and PTG, and in CEG and PTG (p<.001) post-testing. The neck disability index showed significant differences in both group and period, interactions(p<.001). At the time of each group, there were significant differences in PTG (p<.05), REG (p<.01), and CEG (p<.001) levels. Between groups (p<.001), there was a significant difference between CEG and PTG as a result of post-mortem examination (p<.05). The balance ability was found to be significant in the interaction between group and timing, timing (p<.001), and group main effect (p<.05). In each group period, significant differences were found in REG (p<.01), among groups (p<.001), compared with REG and PTG, and compared with CEG and REG (p<.01). Conclusion: The corrective exercise program is a more effective exercise intervention than resistance or physical therapy on the spinal curvature and the improvement of spinal curvature was associated with improving neck function and the ability of body balance although resistance exercise showed improvement of these variables. Therefore, corrective exercise is recommended as the most effective intervention to improve spinal curve, neck function, and the ability of body balance in patients with flat back.

        하지 운동프로그램이 직장 여성의 하지 부종과 통증에 미치는 영향

        The aim of this study was to evaluate the effects of 8 weeks of lower extremity exercise program on lower extremity edema and pain in women with working in office. Twenty-woman officers worked in sitting posture participated in this study and were randomly assigned into the exercise group (n=10) and the control group (n=10) according to order of registration for this study. The lower extremity exercise program was consisted of strengthening and stretching exercise and conducted using the participant"s weight at workplace. The exercise group performed the modified exercise program with three times a week for 8 weeks. In results, there was a significant different in muscle mass (p<.01), edema in calf (p<.05) and ankle (p<.01), and pain of calf (p<.001) after exercise intervention. This result suggests that lower extremity exercise program is a beneficial intervention to reduce lower extremity edema and pain in women officers with longstanding sitting position.
