        급성 경막하 혈종에서 응급 두개골 천공의 위치

        The motality rate of acute subdural hematoma(ASDH) associated with closed head injury is high in spite of recent advances in neurosurgery. Many variables in regard to outcome of ASDH have been assessed. But among them, intracranial pressure(ICP) control and the time interval between injury and operative evacuation are the only things that can be affected by doctor. We introduced a simple method to the management of ASDH for reducing the time interval between injury and operation. When the immediate decompressive operation of ASDH was impossible by any causes, we made a burr hole at the center of hematoma, usually on 2-3cm above temporal squama and 1-2cm behind coronal suture under local anesthesia before main operation. Partial hematoma evacuation was achieved through the burr hole and it was effective in preventing further worsening of patients neurological status before main operation. Prompt hematoma evacuation through the burr hole seemed to be effective in delaying secondary ischemic brain damage and made easy to closing the dura opening and replacement of the bone flap at the end of main decompressive operation. This easy method may reduce the time interval between injury and operation. We represent surgical technique with two cases of ASDH managed with this simple method.

        독일 여성운동의 시원, 루이제 오토

        Louise Otto-Peters was widely acknowledged as the mother of the German women’s movement. She was the editor of the “Frauen- Zeitung”, the first political women’s newspaper in Germany, which was founded in the aftermath of the democratic revolution of 1848/49 and the founder of “Allgemeiner Deutscher Frauenverein”, the first “German” women’s rights association. Germany in the second half of the 19th century was pretty much a conservative society. Hence, her campaigns were under serious surveillance. The government of the federal state of Germany, Sachsen which was her home state, issued “Lex Otto” which prohibited women from being the editors of newspaperssince it was concerned that Louise Otto- Peters would mobilize the women against the government. As a result, the “Frauen-Zeitung” was relatively short-lived: it had been published only between 1849~1951. She restarted her campaigns by organizing the “Allgemeiner Deutscher Frauenverein” in 1865. Her association did not turn into true suffrage, but focused on female education and increasing job opportunities for women. Louise Otto- Peters grounded her feminist cause on the women’s difference from males, in other words, the maternity. Her relational approach, rather than individualist one, was widely viewed or even criticized as the symbol of the passivity of German feminism. However, it was not uncommon that feminists grounded their campaigns not only on the individual’s rights and personal autonomy but also equality-in-difference, and many sought entitlement as citizens in terms of sexual difference. In that sense, Otto’s feminism cannot be dismissed simply as a deviation from the “right” course of feminism. Although many feminist activists of the first generation firmly believed that their suffrage should be the first step or gateway to the achievement of the genuine feminist goals, it turned out that there could not be one decisive solution on the way to achieving gender equality.         정기(鄭琦)의 시 세계

        율계 정기(栗溪 鄭琦, 1879~1950)의 시는 총 445편으로, 형식에 따라 분류하면 5언시는 133편, 7언시가 299편이고 장시인 「화동역대가」(華東歷代歌)등 기타가 13편이다. 1편의 시가 여러 수(首)로 이루어진 것도 있어, 수로따진다면 500수가 훨씬 넘는다. 그의 시 331편을 처음으로 번역하고 내용별로 분류해 보았다. 그러나 시의 종합성과 복합성 때문에 하나의 분류 기준으로 나누기가 어려웠다. 정기 시를 제재·소재에 따라 편의상 대별해 보면 기행시(紀行詩)가 149편, 교유시가 233편, 자연시가 38편, 영사시(詠史詩) 등기타가 총 25편이다. 정기는 위정척사 사상을 지녀 일제의 침략에 맞서 저항하려 하였으며 광복 후의 혼란과 6·25 전쟁을 겪었다. 그의 시에는 일제에 대한 저항의 흔적이나 난세에 대한 고민이 담겨 있기도 하다. 일본 제국주의를 직설적으로 비판한 작품이나 시대나 역사를 본격적으로 노래한 시가 많은 편은 아니었다. 그러나 기행시, 교유시 등에 역사적 사실과 연관지어 곤고한 현실을 노래한 시들이 상당 수 있었다. 정기는 여행과 자연을 좋아하여 그와 관련된 시편을 많이 남겼다. 중국에세 번 들어갔다 오면서 역사 유적과 명승지 등에서 느끼는 감회를 시로 읊었다. 특히 임진왜란이나 병자호란과 관련된 인물이나 사적지, 고려의 유적지에서 당대 망국의 현실을 중첩시켜 노래한 것이 여럿이다. 경상도, 전라도는 물론 전국을 유람하면서 누정이나 산사에 들러 지은 시들도 상당하다. 유학자이지만 불교를 이단시하여 배척하지 않고 호의를 갖고 회통하였음을 확인할 수 있다. 친구·제자들과 산행하면서 지은 시에서는 소탈하고 따뜻한 인간적 풍모나 풍류를 느낄 수 있다. 동문수학한 친구, 자신이 아끼는 효당 김문옥, 고당 김규태 등 제자들과 빈번하게 교유하며, 혹 눈에 거슬리는 언행을 하는 제자가 있으면 따끔하게 타이르고 권계하기도 하였다. 지인들의 죽음이나 이별을 당하여서는 추모와 석별의 정을 곡진하게 노래하였다. 비둘기나 복숭아를 보내준 지인, 헤어진 후 편지를 보내준 사람이나, 늙고 병든 자기를 불원천리 찾아오는 사람에게는 감사의 정을 아낌없이 드러내었다. 생신이나 회갑을 맞은 분들에게는 진심으로 축하를 보내며 건강하게 장수할 것을 빌었다. 정기의 시는 형식적 기교나 멋보다 내용의 표현을 더 중시하였고, 오언시보다 칠언시가 많았다. 교유시와 여행시는 물론 고요하고 깨끗한 자연에 대한 노래, 생활과 밀착(密着)된 시도 적지 않았다. 그의 시를 통해 정기가 49세에 지리산 아래 구례 토지면 오미리로 이주하여 강학을 하며, 자연을 유람하고 지인들과 교유하며 유유자적한 삶을 산 궤적을 확인할 수 있었다. The poems of Yulgye, Jeonggi(1879~1950) are 445 in all. Classified by style, there are 133 poems of 5 characters per line, and 299 poems of 7 characters per line. There is a long poem, Hwadong Yeokdaega (a historical poem about the Joseon Dynasty) and there are 13 other poems of assorted styles. Some poems are composed of many stanzas, altogether, his poems contain a total of more than 500 stanzas. Among them, I have taken 331 poems of his poems and translated them into modern Korean for the first time, and then classified them by subject. However, because of the complexity and overlapping themes of his poems, it was difficult to classify them by topic. If I divide them broadly into 4 groups according to topic and subject matter, there are 149 travel poems, 233 friendship poems, 38 nature poems and 25 others including historical poems. Yulgye held personal beliefs against foreign influence in Korea, especially against Japanese imperialism, so his poems contain traces of resistance against Japanese imperialism. He experienced the chaotic state of affairs after the restoration of independence and the Korean War and his agony over these troubling times is apparent in his poems. Though there are not many poems which directly criticize Japanese imperialism or which discuss the history of that era, there are a considerable number of poems expressing his grief over that era in his travel poems and friendship poems. He liked travel and nature so he wrote many poems on these subjects. Having been to China three times, he wrote about his feelings and thoughts at the historic sites and scenic attractions. Notably, there are some poems about characters and historic sites related to the Japanese invasion of Korea in 1952 or the Manchu War of 1636. There are also some travel poems about historic sites of the Goryeo era, and woven into these poems are references to the reality of Korea’s ruination. A considerable number of travel poems were written at gardens or mountain temples while he was sightseeing around Gyeongsangdo, Jeollado and all around the country. Though Yulgye was a Confucian philosopher, he didn``t exclude Buddism from his thinking. A warm-hearted mood and a humanistic outlook are apparent in his poems written while hiking in the mountains with friends and disciples. In his interactions or writings with his friends who had studied under the same teacher, and his cherished disciples, such as Hyodang Gim Mun Ok and Godang Gim Kyu Tae, he frequently offered wise words of guidance and sound advice. At the deaths or partings of his acquaintances, he wrote ardent and earnest poems. He showed his gratitude and affection generously to those who sent him peaches or letters and to those who visited him from a long distance when he was old and sick. He also wrote some poems to celebrate his friends` special birthdays or occasions wishing for their health and longevity. In his poems, he valued on content more highly than technique, form, or style. There are more 7-character poems than 5-character ones. There are several friendship poems and travel poems extolling the serenity and beauty of nature and everyday life. Through his poems, we can confirm the traces of his easy and carefree life of teaching, sightseeing in nature, and associating with other scholars after he moved to Omiri, Togi Myen, Gurye at the foot of Jiri mountain at the age of 49.

        태반속 융모막암종에 의한 태아모체출혈

        Intraplacental choriocarcinoma is rare, and occasionally results in massive feto-maternal hemorrhage. We describe a case of an intraplacental choriocarcinoma diagnosed postpartum after a preterm cesarean delivery of a severely anemic newborn. The microscopic examination showed that clusters of malignant trophoblasts arose from residual normal chorionic villi and infi ltrated into the intervillous spaces, confirmed as intraplacental choriocarcinoma. Fetomaternal hemorrhage is a rare complication of choriocarcinoma but its presence should always warrant detailed examination of placenta, mother, and infant.

        The reversible cerebral vasoconstriction syndromes (RCVS) is a group of conditions, which is characterized by a reversible segmental constriction, typically associated with recurrent thunderclap headaches, and is often complicated by ischemia or hemorrhagic stroke. Clinical situations, associated with the development of RCVS, include pregnancy or the postpartum period, as well as various medications and illicit drugs. However, vasoconstriction syndromes remain poorly characterized, under-recognized, and difficult to diagnose, because of the lack of specific diagnostic tests or diagnostic criteria. Therefore, we present a first case of reversible cerebral vasoconstriction syndrome of pregnancy complicated with preeclampsia, diagnosed by a magnetic resonance image and angiography, which results in chronic cerebral infarction, with a brief review of the relevant literature.
