Cancer, a leading mortality disease following cardiovascular disease worldwide, has high incidence as one out of every four adults in Korea. It was known to be caused by several reasons including somatic mutation, activation of oncogene and chromosome aneuploidy. Cancer cells show a faster growth rate and have metastatic and heterogeneous cell populations compared to normal cells. Cancer stem cells, the most invested field in cancer biology, is a theory to explain heterogeneous cell populations of cancer cells among several characteristics of cancer cells, which is providing the theoretical background for incidence of cancer and treatment failure by drug resistance. Cancer stem cells initially explain heterogeneous cell populations of cancer cells based on the same markers of normal stem cells in cancer, in which only cancer stem cells showed heterogeneity of cancer cells and tumor initiating ability of leukemia. Based on these results, cancer stem cells were reported in various solid cancers such as breast cancer, liver cancer, and lung cancer. Breast cancer stem cells were first reported in solid cancer which had tumor initiating ability and further identified as anti-cancer drug resistance. There were several identification methods in breast cancer stem cells such as specific surface markers and culture methods. The discovery of cancer stem cells not only explains heterogeneity of cancer cells, but it also provides theoretical background for targeting cancer stem cells to complete elimination of cancer cells. Many institutes have been developing new anticancer drugs targeting cancer stem cells, but there have not been noticeable results yet. Many researchers also reported a necessity for improvement of current concepts and methods of research on cancer stem cells. Herein, we discuss the limitations and the perspectives of breast cancer stem cells based on the current concept and history.
Managing Complications Related to Multiple Supernumerary Teeth

임상가가 주로 접하게 되는 과잉치는 주로 1~2개인 경우가 거의 대부분을 차지한다. 드물게 보고되는 다수 과잉치는 1~2개의 과잉치의 경우보다 교정적인 합병증을 유발할 가능성이 높아지며, 인접치아의 손상가능성 또한 증가하게 된다. 상악 전치부에 호발 하는 과잉치는 상악 전치의 치간 이개, 맹출 장애, 전위, 인접 치아의 치근흡수 그리고 함치성 낭종의 형성 등 다양한 문제점들을 일으킬 수 있다. 따라서 조기에 발견하고 인접 구조물에 위해를 주지 않는 다면 이른 시기에 외과적 적출술을 시행해 주는 것을 권한다. 최근에는 정기 검진과 방사선적 진단 장비의 발달로 이러한 대응이 원할 해 질 수 있는 여건이 마련되었다. 본 증례는 상악 전치부에 발생한 3개의 과잉치를 가능한 인접치의 손상을 줄이고, 유치의 잔존을 목표로 두 번에 걸쳐 발거를 시행하였다. 개인 치과에서 시술을 하였기에 CBCT (Cone-Beam Computed Tomography) 와 같은 진단 장비의 도움을 얻지 못하는 상황에서 브라스 와이어를 이용한 과잉치의 심도 확인법을 응용하였다. One or two mesiodens are mostly common to the clinician. However, three mesiodens is rarely found and can cause complications such as orthodontic problems or interruptions of the adjacent teeth with more than 1 or 2 mesiodens. Many factors cause irregularities in the permanent dentition. Mesiodens, especially in the anterior maxilla, can disturb the eruption of adjacent permanent teeth and cause diastemas, ectopic eruptions, root resorption, or the formation of dentigerous cysts. The early diagnosis of mesiodens is important for preventing such complications, and the timing of intervention should be based on their location and number. Periodic checkups and improved diagnostic devices make it easy to find mesiodens and associated complications. In this case, 3 supernumerary teeth in the maxillary anterior region were affecting the eruption of the adjacent permanent incisors. To minimize complications and preserve the deciduous teeth, the three supernumerary teeth were extracted in 2 steps. Since cone-beam computed tomography was not available, a brace wire was used to measure the depth of the mesiodens.

The aim of this article is to provide major announcements within the last decade or so about possible medical emergencies in dentistry. This would be helpful to improve the knowledge on first aid suitable for dental environment. Syncope was the most common medical emergencies in dentistry. Medical emergency situations can be divided into urgency and true emergency. Urgency situations can be solved well if proper treatment is taken. However, even in an urgency, if the correct treatment is not performed, it may soon turn into an emergency. With the joint efforts of the Korean Dental Anesthesiology Society and the Korean Cardiopulmonary Resuscitation Association, dental advanced life support (DALS) has been prepared since 2015. Through 17 training experiences until January 2020, scenarios and textbooks were developed. Dentists and dental care teams need to be prepared to strengthen their competence as professionals and to increase their team-level response capabilities..

ZnO thin film was grown on a sapphire single crystal substrate by plasma assisted molecular beam epitaxy. In addition to near band edge (NBE) emissions, both blue and green luminescences are also observed together. The PL intensity of the blue luminescence (BL) range from 2.7 to 2.9 eV increased as the amount of activated oxygen increased, but green luminescence (GL) was weakly observed at about 2.4 eV without much change in intensity. This result is quite unlike previous studies in which BL and GL were regarded as the transition between shallow donor levels such as oxygen vacancy and interstitial zinc. Based on the transition level and formation energy of the ZnO intrinsic defects predicted through the first principle calculation, which employs density functional approximation (DFA) revised by local density approximation (LDA) and the LDA+U approach, the green and blue luminescence are nearly coincident with the transition from the conduction band to zinc vacancies of $V^{2-}_{Zn}$ and $V^-_{Zn}$, respectively.

Crohn`s disease is characterized by chronic transmural inflammation of the bowel and is associated with serious complications, such as bowel strictures, abscesses, fistula formation, and perforation. As neither medical nor surgical therapy provides a cure for Crohn`s disease, the primary goals of therapy are to induce and maintain remission and prevent complications. As a biologic agent, infliximab, a monoclonal antibody to tumor necrosis factor, is indicated for refractory luminal and fistulizing Crohn`s disease that does not respond to other medical therapies or surgery. Infliximab has proven to be very effective for inducing and maintaining remission in Crohn`s disease; however, infliximab treatment has several potential complications. Here, we report a case of free perforation following a therapeutic response after an initial dose of infliximab for Crohn`s disease. Purpose: In patients with IgA nephropathy, more than 1 g/day of proteinuria is a risk factor to develop end-stage renal failure. Uncommonly, patients with IgA nephropathy exhibit negative proteinuria on routine dipstick test during follow-up examination, spontaneously or after use of ACEI or ARB. We evaluated whether no proteinuric patients have good prognosis or not. Methods: 41 patients who had no proteinuria on routine urinalysis for more than 6 months, were classified into spontaneous remission group (25 cases, SR) without any treatment and drug-induced remission group (16 cases, DR) with treatment of ACEI/ARB or both, were analyzed for clinical findings and renal function, retrospectively. We examined spot urine protein/creatinine ratio (Up/c) to evaluate exact amount of proteinuria and GFR was estimated by MDRD equation. Results: Twenty eight percent of cases in SR and 50 percent in DR showed spot Up/c 0.3-1 g/g. After follow-up of 58±41 (9-192) months in SR and 79±56 (35-192) months in DR, the stages of CKD shifted to advanced levels as follows; in SR group, 11, 10, 4 patients in stage 1 (GFR≥90 ml/min/ 1.73m2), 2 (GFR 60-89 ml/min/1.73m2), 3 (GFR 30-59 ml/min/1.73m2) to 7, 13, 5 patients, respectively; in DR, 7, 8, 1 patients in stage 1, 2, 3 to 3, 8, 5 patients, respectively. There was a tendency of slow decreasing GFR in both groups but no case progressed to CKD stage 4 and 5. Conclusion: Of IgA nephropathy patients with no proteinuria on routine urinalysis, 30-50% of patients have proteinuria 0.3-1.0 g/g on spot Up/c and there was also a risk of progression.
