        무지 근위지골 기저부의 골연골 병변: 증례 보고

        Although osteochondral lesions are well-described in previous literature, lesions located at the proximal phalangeal base of the hallux are extremely rare. Here, the authors report the case of a 21-year-old male who presented with chronic right great toe pain, which was eventually diagnosed as an osteochondral lesion at the proximal phalangeal base of the hallux. The diagnosis presented herein should be kept in mind and included in the differential diagnosis when patients complain of chronic pain in the great toe.

        원위 요골 골절에서 도수 정복 및 석고 고정 치료의 결과: 배측 골간단 분쇄가 방사선학적 및 기능적 결과에 미치는 영향

        Purpose: The purpose of this study was to determine the relationship between dorsal metaphyseal comminution and the radiographic and functional outcomes of patients with distal radius fractures treated by closed reduction and cast immobilization. Methods: Twenty-six patients with acute distal radius fractures were retrospectively reviewed. The mean age of this patient group was 62.8 years (range, 45-87years). Eighteeen cases were AO type-A3 and 8 were AO type-A2. Radiographic and functional parameters were analyzed and compared between the patients who presented with or without dorsal metaphyseal comminution on their initial radiographs in order to assess the clinical outcomes. The radiographic parameters included radial inclination, radial length, volar/dorsal tilt, and ulnar variance. In order to measure the functional outcomes, each patient’s range of motion,grip strength, Quick disabilities of the arm, shoulder, and hand (DSAH), visual analog scale (VAS), and Mayo score were determined. Results: Seventeen patients (65%) presented with dorsal metaphyseal comminution on the initial radiographs. Radial inclination, radial length, and volar/dorsal tilt were decreased and ulnar variance was increased on the final radiographs in comparison with the postreduction. However, there were no statistically significant differences between the two groups that presented with or without dorsal metaphyseal comminution (p>0.05). None of the functional parameters (i.e., range of motion, grip strength, DASH, Mayo, and VAS score)were significantly different between the two groups (p>0.05). Conclusion: Dorsal metaphyseal comminution seems to have no significant impact on radiographic and functional outcomes when closed reduction and cast immobilization was planned for the treatment of distal radius fracture. 목적: 배측 골간단 분쇄가 있는 원위 요골 골절 환자에서 분쇄여부와 방사선적, 기능적 결과와의 관계를 알아보고자 한다. 대상 및 방법: 2009년부터 2011 12월까지 본원에서 원위 요골 골절 진단 받은 후 도수 정복 및 석고 부목 치료를 받은 26명의 환자를 대상으로 하였다. 환자 45세부터 87세까지였고, 평균나이 62.8세였다. 환자들의 원위 요골 골절은 AO type A2 18예, AO type A3 8예였다. 임상적 결과 확인하기 위해 배측 골간단 분쇄가 있는 집단과 없는 집단을 나누고, 두 집단에서 방사선학적 변수와 기능적 변수를 분석하였다. 방사선적 변수에는 요골 경사각, 요골 단축, 요골 요배측 경사, 척골 변위를 포함하였고, 기능적 변수로 관절운동범위, 악력, disabilities of arm, shoulder & hand (DASH), visual analog scale (VAS) score, Mayo score를 측정했다. 결과: 총 17명 (65%)의 환자에서 초기 엑스레이 영상에서 배측 골간단 분쇄가 있었다. 방사선적 결과는 외래 추시 중 시행한 마지막 방사선 촬영과, 도수 정복 직후의 방사선 촬영에서 요골 경사각, 요골 단축, 요골 배측 경사, 척골 변위를 비교하였고, 두 집단 사이에 차이는 없었다(p>0.05). 두 집단 사이의 기능적 결과로 관절운동범위, 악력, DASH, VAS score,Mayo score를 비교하였고, 유의한 차이를 보이지 않았다(p>0.05). 결론: 원위 요골 골절 뒤 도수정복 및 석고부목 치료받은 환자에서 배측 골간단 분쇄는 방사선적, 기능적 결과에 영향을미치지 않는다.

        거골 골연골병변의 자연 경과

        Many articles on operative treatment strategies for osteochondral lesions of the talus (OLT) have been published. On the other hand, there are only a few reports on the conservative treatment of OLT. Understanding the natural history of OLT is essential to establishing the correct treatment. Conservative treatment might lead to comparable clinical outcomes compared to the current options of the operative treatment in isolated OLT lesions. OLT lesions can be divided into two (or possibly three) types. Hence, the prognosis and outcome of certain operative treatments should be evaluated further. The prognosis of OLT combined with instability or malalignment appears poorer than that of isolated OLT lesions. Therefore, concomitant pathologies should be evaluated together when treating patients with OLT.

        거골 내측 결절에 발생한 골연골종과 통풍 발작에 의한 발목 내측 충돌 증후군: 증례 보고

        Osteochondromas are benign bone tumors typically found in the metaphyseal region of long bones. These tumors are often asymptom- atic and detected incidentally. However, their occurrence in atypical sites such as the talus can pose significant diagnostic and treatment challenges. This report describes a rare case of osteochondroma of the medial tubercle of the talus, which is an unprecedented location based on a review of relevant literature. A 28-year-old male presented with worsening medial ankle pain and limping. Imaging revealed a lesion consistent with osteochondroma contributing to medial ankle impingement syndrome. Uniquely, this case also featured a coincid- ing gout attack in the ankle joint. Surgical removal of the lesion resulted in significant symptom relief and functional improvement. This case underscores the need to consider rare diagnoses, such as talar osteochondroma, when presented with persistent medial ankle pain and highlights the potential presence of concurrent conditions, such as gout.

        삼각 부골과 횡인대에 의해 도수정복 되지 않은 발목관절 전방 아탈구: 증례 보고

        Ankle joint is a stable mortise structure, therefore pure ankle dislocation without ankle fracture is rare. Several adjacent anatomical structures have been reported as a cause of irreducible ankle dislocation. Author report a case of successful treatment of irreducible anterior subluxation of the ankle joint due to os trigonum and transverse ligament by closed reduction under anesthesia, and a review of the literature as well.

        Melorheostosis is a very rare disease of sclerosing bone dysplasia. Patient typically presents with insidious pain, swelling, and in severe cases, stiffness and limitation of range of motion. A dense cortical bone formation along the axis of the bone resembling candle dripping appearance is a pathognomonic sign in plain radiograph. The author reports a rare case of melorheostosis of the fifth metatarsal bone, which was treated conservatively and a review of literature as well.

        Tarsal coalition is a rare congenital condition, of which two or more tarsal bones are bridged together in fibrous, chondral, or bony form. Talonavicular coalition is reported as very rare compared to other several tarsal coalition variants, and usually accompanied with orthopaedic anomalies. The author reports a case of bilateral talonavicular coalition with mild clinical symptoms, and a review of the literature as well.

        Repeatability of a Multi-segment Foot Model with a 15-Marker Set in Normal Children

        Background: The use of three-dimensional multi-segment foot models (3D MFMs) is increasing since they have superior ability to illustrate the effect of foot and ankle pathologies on intersegmental motion of the foot compared to single-segment foot model gait analysis. However, validation of the repeatability of the 3D MFMs is important for their clinical use. Although many MFMs have been validated in normal adults, research on MFM repeatability in children is lacking. The purpose of this study is to validate the intrasession, intersession, and interrater repeatability of an MFM with a 15-marker set (DuPont foot model) in healthy children. Methods: The study included 20 feet of 20 healthy children (10 boys and 10 girls). We divided the participants into two groups of 10 each. One group was tested by the same operator in each test (intersession analysis), while the other group was tested by a different operator in each test (interrater analysis). The multiple correlation coefficient (CMC) and intraclass correlation coefficient (ICC) were calculated to assess repeatability. The difference between the two sessions of each group was assessed at each time point of gait cycle. Results: The intrasession CMC and ICC values of all parameters showed excellent or very good repeatability. The intersession CMC of many parameters showed good or better repeatability. Interrater CMC and ICC values were generally lower for all parameters than intrasession and intersession. The mean gaps of all parameters were generally similar to those of the previous study. Conclusions: We demonstrated that 3D MFM using a 15-marker set had high intrasession, intersession, and interrater repeatability in the assessment of foot motion in healthy children but recommend some caution in interpreting the hindfoot parameters.

        두 족저압 측정장비의 비교 분석

        Purpose: Foot pressure measurement devices are used widely in clinical settings for plantar pressure assessments. Despite the availability of various devices, studies evaluating the inter-device reliability are limited. This study compared plantar pressure measurements ob- tained from HR Mat (Tekscan Inc.) and EMED-n50 (Novel GmbH). Materials and Methods: The study involved 38 healthy male volunteers. The participants were categorized into two groups based on the Meary’s angle in standing foot lateral radiographs: those with normal feet (angles ranging from –4° to 4°) and those with mild flatfeet (angles from –8° to –15°). The static and dynamic plantar pressures of the participants were measured using HR Mat and EMED-n50. The reliability of the contact area and mean force was assessed using the interclass correlation coefficient (ICC). Furthermore, the differ- ences in measurements between the two devices were examined, considering the presence of mild flatfoot. Results: The ICC values for the contact area and mean force ranged from 0.703 to 0.947, indicating good-to-excellent reliability across all areas. EMED-n50 tended to record higher contact areas than HR Mat. The mean force was significantly higher in the forefoot region when measured with EMED-n50, whereas, in the hindfoot region, this difference was observed only during static measurements with HR Mat. Participants with mild flatfeet exhibited significantly higher contact areas in the midfoot region for both devices, with no con- sistent differences in the other parameters. Conclusion: The contact area and mean force measurements of the HR Mat and EMED-n50 showed high reliability. On the other hand, EMED-n50 tended to record higher contact areas than HR Mat. In cases of mild flatfoot, an increase in contact area within the midfoot region was observed, but no consistent impact on the differences between the two devices was evident.
