        This study has been conducted to achieve the following objectives: First, in order to understand the horizontal propagation and attenuation characteristics for the railroad traffic noise, we selected actively developed areas within 100 meters away from the road to measure the level of traffic noise near a road. To measure the noise near a railroad, we selected areas within 100 meters away from the railroad and then selected Saemaul-ho and Mugoongwha-ho as the subjects for our experiment, In this way, we analyzed the horizontal propagation and attenuation characteristics for the traffic noise occurring in diversified areas. Second, in order to understand the vertical propagation and attenuation characteristics for the railroad traffic noise, we measured and analyzed the distributional characteristics of vertical sound pressure levels on each floor of multi-storied apartment buildings according to changes of traffic load and types, and the existence or nonexistence of soundproof walls. For the case of the railroad traffic noise, we also selected Samaul-ho and Mugoongwha-ho as the subjects for our experiment, and we measured and analyzed the different noise levels on each floor of multi-storied apartment buildings from the soundproof wall. The results of Horizontal propagation and attenuation characteristics for the railroad traffic noise are as follows: In cases of the flat land, cutting land, and bridge area, as distance increases, the sound pressure level steadily decreases. The cutting land has an especially high distance decrease compared to the other areas, and the sound pressure level for the bridge area is higher than that of the flat land with a measurement of 9.7~17.5㏈(A). Vertical propagation and attenuation characteristics for the railroad traffic noise are as follows: The amount of sound pressure level decrease is 13.9~15.4㏈(A) for Samaul-ho and 10.9~11.5㏈(A) for Mugoongwha-ho when measuring the vertical sound pressure levels at heights lower than 4.5m, which indicates a fairly large decrease. At 6m, the amount of decrease is 7.3㏈(A) for Samaul-ho and 5.0㏈(A) for Mugoongwha-ho, which indicates a small decrease.

      • Clinical effects of tocilizumab in patients with active rheumatoid arthritis

        Objective: The purpose of this study is to evaluate the clinical and hematological effects of tocilizumab in active rheumatoid arthritis (RA) patients. Methods: Fourteen patients with active RA were enrolled in this study. The patients received tocilizumab 8 mg/kg intravenously every four weeks for 6 months. Disease activity, anemia-related factors including serum hepcidin-25, and hematological parameters were monitored at baseline and at 1, 3, and 6 months after the initiation of treatment. Results: Significant reductions in tender joint count, swollen joint count, visual analogue scale, erythrocyte sedimentation rate, C-reactive protein plus reductions in a 28-joint disease activity score were seen within one month after the first tocilizumab treatment. These effects lasted throughout the six-month study period. In addition, significant improvements in anemia related factors such as hepcidin- 25, ferritin, iron, hemoglobin, red blood cell counts and mean corpuscular volume were observed during the treatment period. Moreover, hematological parameters were improved reductions in counts for leukocytes, monocytes, neutrophils, and platelets. The lymphocyte counts and their subset numbers were unchanged. Changes in hepcidin levels significantly correlated with changes in C-reactive protein, erythrocyte sedimentation rate, ferritin, hemoglobin and counts for red blood cells, leukocytes and neutrophils during the treatment period. Conclusions: This study demonstrates that tocilizumab significantly and meaningfully reduces disease burden in patients with active RA patients. In addition, tocilizumab diminishes the levels of inflammatory anemia in the patients by inhibiting hepcidin production. These clinical data provide evidence of a favorable outcome from the tocilizumab in RA.

        A Case of Drastically Aggravated Erythema Induratum due to Co-existing Peripheral Arterial Occlusive Disease

        A 72-year-old woman presented with recurrent painful erythematous nodules and ulcers on both legs. Latent tuberculosis was proven by a positive interferon-gamma release assay, and a histopathology examination revealed septolobular panniculitis with vasculitis. The initial diagnosis was erythema induratum associated with tuberculosis, but the leg ulcers became worse despite anti-tuberculosis medication and wound dressing. Computed tomography angiography showed occlusion of the superficial femoral and popliteal arteries bilaterally, supporting that the vascular event contributes to the ulcers according to the vascular territories. Under the diagnosis of peripheral arterial occlusive disease, she was treated with percutaneous transluminal angioplasty and antiplatelet medication. The skin ulcers were resolved. Elderly patients with erythema induratum have a risk of coincidental peripheral arterial occlusive disease, therefore dermatologists should be aware of the possibility of underlying vascular disease, so even minor trauma like skin biopsy can evoke serious condition shown in this patient. Here, we report a case of drastically aggravated erythema induratum due to co-existing peripheral arterial occlusive disease, which resolved with vascular intervention after not responding to antituberculosis medication.       • FC 2-14 : A study for reliability of articles about alopecia in newspaper

        Background: There is growing interest in alopecia among the general population. Many of them get information from easily-accessible media rather than from doctor, so media can play an important role in shaping public acknowledgement. Objectives: This study was aimed to evaluate the reliability of articles about alopecia in print and online newspaper. Methods: Articles including the information about alopecia for recent 3 years were selected from one print newspaper (group A, 36) and two online newspapers (one published by a publishing company with both print and online newspaper (group B, 131), the other by a company with online newspaper only (group C, 35)) which were the most widely circulated among each category. The contents and the reliability of the articles were analysed. Results: General information of alopecia (72.8%) was the most common theme. Treatment of alopecia was main topic in most articles (90.6%). Majority of articles were based on the consultation from expert; the percentage of dermatologist among expert was the highest in group A(65.5%) followed by group B and C. Percentage of articles including direct or indirect advertisement was highest in group C (62.9%). Articles with high accordance with textbook was most common in group A (38.9%). Conclusion: The reliability of the articles were too variable. It is suggestive that more attention is paid to check the reliability of information through media and to supply easily accessible, balanced, trustworthy informations.

      • P170 : A survey on understanding of acne vulgaris among Busan patients

        Background: Only two reports about the understanding of acne vulgaris have been reported in Korea. There seems to be regional differences among patient`s tendency. But there has been no study performed in Busan. Objectives: This study was performed in the patients o survey the knowledge of acne vulgaris, including patient`s attitude towards the disease as well as preference for treatment methods in Busan. Methods: A total of 170 patients with acne vulgaris from 1st, 2nd and 3rd grade hospital in Busan, with the consent, were surveyed. Results: Cause: Most patients thought that sebum formation (96%) is the cause of acne vulgaris. Stress (90%), eating habits (86%) followed. Attitude toward treatment: Patients believed that ointment and per oral medication prescribed by dermatologists are effective treatments (89%). Most patients believed per oral medication for acne vulgaris to be too strong. Treatment preference: The most patients visited dermatologic clinic for their acne scar(26%). They preferred acne extraction(31%) and laser therapy(31%). Cosmetic usage: Fifty-seven of total 114 patients of female patients used concealer(51%) for acne scar. Most patients(99%) recognized that this cosmetics could worsen the acne, but half of them(44%) answered to use it consistently. Conclusion: In this study, we could recognize patients` overall acknowledge about acne vulgaris. More education and doctor`s explanation on consultation should be necessary.

        Erlotinib-related Severe Hair Loss with Severe Exanthematous Drug Eruption: A Case Report

        Erlotinib inhibits the epidermal growth factor receptor and is used in patients with various cancers. However, it can affect the epidermis and hair because the receptor is expressed in normal skin cells. A 54-year-old woman with metastatic non-small-cell lung cancer presented with erythematous patches over her entire body and severe hair shedding 2 weeks after starting erlotinib. Histopathological examinations showed lymphocytic exocytosis; perivascular infiltration of lymphohistiocytes and eosinophils in the right arm; and marked infiltration of eosinophils, neutrophils, and lymphohistiocytes in the scalp. Erlotinib discontinuation improved hair loss and skin lesions. Hair loss has been reported in 5% of patients taking erlotinib. Our case was unusual in that there was complete baldness, and to our knowledge, no report of complete hair loss and exanthematous drug eruption after erlotinib treatment has been previously reported. Here, we report a case of severe hair loss with concurrent exanthematous drug eruption that may have been linked to erlotinib hypersensitivity. (Korean J Dermatol 2018;56(2):139∼142)

        침술 후 발생한 편평태선

        Lichen planus is a chronic inflammatory disease that mainly involves the skin and oral mucosa. We report the case of a 57-year-old female who presented with a 6-week history of localized violaceous papules on her back and both extremities. Prior to presentation, she had received acupuncture on her back and both extremities, and 2 weeks later skin lesions occurred along the treated area. From the histopathologic findings of the lesion, the patient was diagnosed with lichen planus and treated with intralesional and topical corticosteroids. Dermatologic diseases such as atypical mycobacterial infection, foreign body granuloma and contact urticaria associated with traditional Korean medicine treatments have been reported. To the best of our knowledge, there has been only one report of eruptive lichen planus triggered by acupuncture in English literature to date. Herein, we report this rare and interesting case of lichen planus related to acupuncture. (Korean J Dermatol 2015;53(8):628∼630)
