모 제약회사 근로자들의 직업성 천식 유병률과 약제 분말 노출수준

목적: 제약회사에서 직업상 천식의 유병 규모를 파악하고 작업공정에 대한 작업환경 측정을 통해 제약회사의 약제 분말에 대한 노출수준을 평가하고자 하였다. 방법: 제약회사에서 약제 분말에 노출되는 근로자를 대상으로 천식을 선별하기 위하여 설문조사 및 진찰, 혈액검사, 폐기능 검사 그리고 피부단자시험을 실시하였고, 각 부서별 공정에 따라서 총분진과 항생제 또는 효소제 분말에 대한 작업환경 측정을 실시하였다. 직업상 천식이 의심되는 근로자에 대하여 기관지과민성에 대한 검사(메타콜린을 이용한 비특이 기관지유발시험)와 약제 분말 기관지유발시험을 실시하여 확진하였다. 결과: 설문조사 및 진찰에서 작업 관련 천식 증상이 있는 근로자는 11명(34.4%)이었고, 약제 분말을 이용한 피부단자시험에서 1+ 이상 반응을 보인 근로자 10명(31.2%)이었다. 비특이 기관지유발시험 결과 8명(25.0%)이 양성(PC20 ≤16 mg/ml)이었다. 약제 분말 기관지유발시험을 통해 직업상 천식으로 확진된 근로자는 5명(15.6%)이었다. 작업환경 측정에서 원료를 충진하거나 투입하는 작업 그리고 정립작업에서 약제 분말에 대한 노출이 많았다. 결론: 제약회사에서 직업상 천식은 주로 제조공정 중 약제 분말에 노출된 근로자에서 발생하여, 증상 및 직업성 천식의 유병률이 높았다. 따라서 원료를 충진 또는 투입하는 작업과 정립작업 등 제조과적에서 약제 분말 노출을 줄이기 위한 세심한 주의가 필요하여, 약제 분말을 취급하는 근로자에 대해서는 천식 증상 조사뿐 아니라 정기적으로 피부단자시험, 비특이 기관지유발시험 등을 실시하여야 할 것이다. Objectives: After the investigation of one worker with occupational asthma, we surveyed the preva- lence of occupational asthma and the exposure level of pharmaceutical dust of 32 workers in a pharmaceutical company. Methods: Thirty-two of the 90 employees participated in the survey which consisted of questionnaire, blood sampling, spirometry and skin prick tests with 8 common allergens as well as 9 antibiotics and 2 enzymes. Various indices of the working environment were also measured. Subjects who had a symptom suggestive of work-related asthma or positive skin prick test were further investigated by PC20 methacholine. Nine subjects who had a PC20 result of 16 mg/ml or less (n=7) or had work-related symptoms and positive skin prick test (n=2) were referred to undergo a specific bronchial provocation test to phar- maceutical dust in an academic allergic disease center. Results: Eleven of 32 workers (34.4%) had a work-related symptom suggestive of occupational asthma. Ten (31.2%) showed positive skin prick test. 8 (25.0%) had a PC20 result of 16 mg/ml or less (indicative of significant bronchial hyperresponsiveness), and 5 (15.6%) had a positive result on the specific bronchial provocation test. Exposure levels of stuffing, input of raw materials and screening process were relatively high. Conclusions: This survey showed that pharmaceutical workers have an increased risk of occupational asthma. Although pharmaceutical factories maintain a relatively good working environment, careful control of respiratory tract exposures, especially during stuffing, input or raw materials and screening process, is important to prevent occupational asthma. Pharmaceutical workers need to undergo regular skin prick and methacholine bronchial provocation tests, as well as asthmatic symptom survey, to ensure the early detection and prevention of occupational asthma.

This paper deals with determinig the proper friction welding condition and analyzing various mechanical properties of friction welded joints of sintered carbide tool materials(K20, P25 and SKDX5 for the blade part of drill or press punch) to alloy steel (SKH4, SCM440 for the shank part of drill or press punch), the alloy steel to aluminum(A6061 for the interlayer material between the blade part and the shank) and sintered carbide tool materials to aluminum. And also acoustic emission test will be carried out during fiction welding to evaluate the weld quality.

In-process quality control and high reliability of the weld are the major concerns in applying friction welding to the economical and qualified mass-production. Experimental examinations and quantitative analysis were performed for the optimization of similar friction welding of hydraulic valve spool steels(SNCM220, SCM435, SACM645, SCM415, ø 24). The quantitative correlations were found between the initial cumulative counts of acoustic emission(AE) occurring during plastic deformation periods of the welding and the fatigue life as well as weld strength and welding conditions. A real-time evaluation system was developed for the friction weld quality by AE.
흉막삼출액이 있는 폐암환자에서 CT소견과 세포검사와의 비교연구

Pleural effusion is often associated in lung cancer and divided into malignant effusion due to tumor involvement and paramalignant effusion due to a local or systemic effect. When malignant effusion is present the tumor stage is T4 and then, operation is not considered. Therefore, determining whether malignant effusion is or not is important for lung cancer staging and treatment of patients. We retrospectively compared CT findings with cell cytology in 37 patients with pleural effusion associated with lung cancer. Four- teen patients had proven malignant pleural effusion and 23 patients had proven paramalignant effusion by repeated cell cytology of pleural effusion. Comparing CT findings with cell cytology of pleural effusion, the pleural thickening at CT was 38% sensitivity and 64% specificity, and the enhancing pleural nodule at CT was 50% sensitivity and 78% specificity. The positive predictive value of enhancing pleural nodule at CT was 70%. In conclusion, although the result of cell cytology is negative for malignancy in pleural effusion associated with lung cancer, malignant effusion should be considered in the presence of enhancing nodules at CT of the pleura.

The Egr family of zinc finger transcription factors is rapidly induced by various mitogens and regulates cell growth, differentiation, and apoptosis. While it is clear that loss of Egr1 leads to anovulatory infertility due to LHβ deficiency in female mice, molecular function of Egr1 in male reproduction has not been clearly investigated. Here, we demonstrate that Egr1 acts as an intrinsic transcription factor in Leydig cells to regulate their proliferation and steroidogenesis in the testis as well as an extrinsic factor for male reproduction via LHβ transcription in the pituitary. Egr1 is predominantly expressed in spermatogonia and Leydig cells in immature testes and later detected in some of these cell types in mature testes. The fertility potential of Egr1(-/-) male mice is relatively deteriorated even at 2 month-old age and aggravated with aging. The incidence of abnormalities of seminiferous tubules such as Sertoli cell only was dramatically increased with aging. The number and mean size of Leydig cells were significantly reduced in Egr1(-/-) testes. The impairment of Leydig cells is consistent with significant reduction in levels of testosterone and expression of factors critical for steroidogenesis such as StAR in Egr1(-/-) testes. Exogenous administration of hCG rapidly and transiently induced Egr1 expression in Leydig cells culture in vitro. hCG could reinstate reduced mean size of Leydig cells but not reduced number of Leydig cells and aberrantly low StAR expression, suggesting that Egr1 has critical functions for Leydig cell proliferation and their steroidgenesis. In addition, daily sperm production and in vitro fertilization (IVF) competence were significantly reduced, and apoptosis was facilitated in these mice. Furthermore, hCG administration to compensate for relatively low LH levels in Egr1(-/-) males could not restore the compromised reproductive phenotypes such as IVF competence and apoptosis in these mice. Interestingly, expression of Egr2, a member of Egr family, is significantly elevated in Egr1(-/-) Leydig cells suggesting that genetic compensation of Egr2 may alleviate phenotypic aberration of Egr1(-/-) male testes. Collectively, these results suggest that Egr1 act as an intrinsic transcription factor required for proliferation and steroidogenesis of Leydig cells to govern spermatogenesis in the testis.

chloroform-soluble fraction of the vinegar treated small black soybean [Glycine max(Leguminosae)] showed antiproliferative activity against human myeloid leukemia HL-60 cells, in terms offraction resulted in the isolation of two isoflavonoid compounds, genistein and daidzein, as active principles.Genistein showed more potent antiproliferative efects against HL-60 cels. Treatment of HL-60 cels withgenistein induced apoptosis in a dose dependent manner. With existing illumination rendering system being used in domestic, internationally developed common programs are taking over the market more than 90%. However, since the programs are established with DB designed to be most suitable for their own products, accuracy in analysis and visualized image are hardly reliable. This raises the necessities of establishing DB for domestic developed products. A method of establishing artificial luminair DB, which was studied on DB establishment in consideration of optical characteristics of domestic products with using RADIANCE program was presented in this study, and it is considered as the program of simulating light movement most accurately among all rendering programs developed so far.

Background: Pseudoceramides are used in barrier repair therapy for atopic dermatitis (AD) and demonstrated to reduce the adverse effects of topical glucocorticoids (GC). Objectives: We investigated whether PC-9S, one of the pseudoceramides, can regulate the expression of PPARα and topical GC-induced 11ß-hydroxysteroid dehydrogenase type 1 (11βHSD1) in oxazolone(Ox)-induced AD mice model (Ox-AD mice). Methods: Ox-AD mice were topically treated with PC-9S- containing physiologic lipid mixture for 4 days, and PPARα expression was assessed by real-time PCR and immunohistochemistry. Then, Ox-AD mice were treated with topical GC for 4 days with or without PC-9S co-application, and 11βHSD1 expression was evaluated. Skin barrier functions, inflammation, and antimicrobial peptides (AMPs) expression were quantitated. Results: Topical PC-9S significantly increased PPARα expression, normalized TEWL and surface pH, and reduced mast cell infiltrates in Ox-AD mice. It also prevented the decline of AMPs expression in Ox-AD mice. PC-9S suppressed the topical GC-induced upregulation of 11βHSD1 and prevented the GC-induced epidermal atrophy, reduced loricrin expression, and impaired barrier recovery. Conclusion: Skin barrier repair activity and the anti-inflammatory activity of pseudoceramide PC-9S in AD mice model might be partly mediated by PPARα stimulation. Inhibition of 11βHSD1 by PC-9S might contribute to reducing adverse effects of topical GC therapy.
