      • Gastroprotective Effect of Selenium on Ethanol-Induced Gastric Damage in Rats

        <P>In the present study, we examined the gastroprotective effect of selenium against ethanol-induced gastric mucosal lesions in rats. The gastric mucosal lesions were produced by oral administration with various concentrations of ethanol for three days, and 80% ethanol treatment was determined to be the optimal condition for induction of gastric damage. To identify the protective effect of selenium on ethanol-induced gastric damage, various doses of selenium were given as pretreatment for three days, and then gastric damage was induced by 80% ethanol treatment. Selenium showed a protective effect against ethanol-induced gastric mucosal lesions in a dose dependent manner. Specifically, 100 μg/kg selenium showed the highest level of gastroprotection. In addition, selenium markedly attenuated ethanol-induced lipid peroxidation in gastric mucosa and increased activities of radical scavenging enzymes, such as superoxide dismutase (SOD), catalase, and glutathione peroxidase in a dose-dependent manner. Histological data showed that 100 μg/kg selenium distinctly reduced the depth and severity of the ethanol induced gastric lesion. These results clearly demonstrate that selenium inhibits the formation of ethanol-induced gastric mucosal lesions through prevention of lipid peroxidation and activation of enzymatic radical scavenging.</P>

      • Does treatment of Helicobacter Pylori Infection Reduce Gastric Precancerous Lesions?

        Background: Treatment of Helicobacter pylori (H. pylori) decreases the prevalence of gastric cancer, and may inhibit gastric precancerous lesions progression into gastric cancer. The aim of this study was to determine the effect of treatment on subsequent gastric precancerous lesion development. Materials and Methods: We prospectively studied 27 patients who had low grade dysplasia at the time of enrollment, in addition to dysplasia atrophic gastritis and intestinal metaplasia observed in all patients. All were prescribed quadruple therapy to treat H. Pylori infection for 10 days. Patients underwent endoscopy with biopsy at enrollment and then at follow up two years later. Biopsy samples included five biopsies from the antrum of lesser curvature, antrum of greater curvature, angularis, body of stomach and fundus. Results of these biopsies were compared before and after treatment. Results: Overall, the successful eradication rate after two years was 15/27 (55.6%). After antibiotic therapy, the number of patients with low grade dysplasia decreased significantly (p=0.03), also with reduction of the atrophic lesions (p=0.01), but not metaplasia. Conclusions: Treatment of H. pylori likely is an effective therapy in preventing the development of subsequent gastric premalignant lesions.

        원발병소 및 전이병소에서 배양된 위선암세포의 체외 항암제 감수성 비교

        Background : Recently, attention has been focused on the possibility of predicting the effectiveness of anticaner drugs against individual tumors through chemosensitivity tests. The MTT assay is a preferable, accurate, and convenient colorimetric assay which is based on the premise that the mitochondria of living cells reduce the tetrazolium salt to formazan. It has been suggested that malignant neoplasms contain multiple cell populations exhibiting tremendous biologic heterogeneity. Particularly the detailed differences in chemosensitivity between cells from primary and from metastasis lesions remain unsolved. This study was designed to assess the usefulness of in vitro chemosensitivity tests in the tratment of gastric cancers and differences in chemosensitivity between gastric cancer cells cultured from primary and metastatic lesions. Method : Primary cancer cells of 138 patients with gastric cancer (group A), obtained through the endoscopic biopsy or surgical open biopsy, and metastatc cancer cells of 26 patients (group B : 27 lesions), obtained through the paracentesis or pleural centesis, were cultured using culture midium supplemented with 20% fetal bovine serum. The colorimetric MTT assay was applied for in vitro chemosensitivity tests. Results : 1) The success rates of short-term culture of gastric cancer cells were 14.5% (20/138) in group A and 66.7% (18/27) in group B, respectively. 2) The morphology of the cultured cells observed through phase-contrast and light microscopy showed no significant difference between group A and B. 3) The mean IC_50 of ADR and MMC were significantly higher in group B than group A (p<0.05) and that of VOㆍ16 significantly higher in group A (p<001) 4) The two-drug combination regimens having thd statistically significant synergistic effects were ADR+MMC (p<0.05) and ADR+DDP (p=0.01) in group a, ADR+VPㆍ16 (p<0.05), 5ㆍFU+VPㆍ16 (p<0.05), and MMC+VPㆍ16 (p<0.05) in group B, and the three-drug combination regimens were 5ㆍFU+ADR+DDP (p<0.05) and MMC+DDP+5ㆍFU (p=0.01) in group A, 5ㆍFU+ADR +VPㆍ16 (p<0.01) and VPㆍ16+ADR+DDP (p<0.01) in group B. 5) The two-drun combination regimens having the supraaddictive effects over 70% of the cases were ADR+MMC and ADR+DDP in group A, ADR+VP-16, 5-FU+VP-16, and DDP+VP-16 in group B, and the three-drug combination regimens 5-FU +ADR+DDP and MMC+DDP+5-FU in group A, 5-FU +ADR +VP-16 and VP-16 +ADR+DDP in group B. 6) The combination regimens having synergistic effects were different between group A and group B. 5-FU +ADR+MMC and 5-FU +ADR+DDP combination had synergistic effects in group A, but antagonistic effects in group B. 5-FU +ADR+VP-16 and VP-16+ADR+DDP combination synergistic effects in group B, but antagonistic effects in group A. Conclusion : The chemosensitivities of gastric cancer cells were significantly different between the primary and metastatic lesions from which the cultured cells were derived. Thus, the individual chemosensitivity test using short-term culture system may be useful for the selection of the most effective anticancer drugs.

        위 조직편 접촉세포검사의 진단적 의의

        N/A Background/Aims: Currently, the endoscopic biopsy technique is the most accurate diagnostic modality detecting the ga'tric carcinoma. But, it is thought importantly to evaluate the significance of touch cytology in diagnosing the gastric lesion because the representative area of gastric lesion is possible to be removed during the process making the specimen. Methods. After l,333 cytologic specimens from 1,235 patients were made by touch of the biopsied tissue on the slide glass and were stained by Papanecolaous method, the tissue specimens were obtained by the usual specimen making technique. The results of the cytologic diagnosis were compared to the biopsy results. Results: The accuracy of the cytologic diagnosis was 99.0%(1,108/1,119 cases) in the gastritis or benign ulcer, 20.4%(11/54 cases) in the atypical epithelial lesions, and 75.0%(120/160 cases) in the malignancy. The overall accuracy of the cytologic diagnosis was 92.9%(1,239/1,333 cases). False positivity of the cytologic diagnosis was 0.4%(5/1,177 cases) and false negativity was 16.9%(27/ 160 cases). All of the 4 cases, which were diagnosed class IV or V in the cytologic diagnosis and gastritis or benign ulcer in the biopsy diagnosis in the first study, were found to be benign lesions in the follow up cytologic and biopsy studies. Conclusion: This study indicates that touch cytologic examinations supplementary to biopsies are not so significant enough to performed spending many efforts since biopsied specimen represent the majority of gastric lesion, if they were sampled at correct site of lesion at all.         향사육군자탕(香砂六君子湯)의 Indomethacin 유발 위점막 손상에 대한 효과

        Objective: This study was carried out to investigate the effects of Hyangsayukgunja-tang extract on indomethacin-induced gastric mucosal lesions of mice. Methods: To evaluate the effects of Hyangsayukgunja-tang extract and Misoprostol, the morphology of gastric mucosa, and the distribution of mucose cells, PNA(Peanut Agglutinin), ICAM(intercellular adhesion molecule), and apoptotic cells were observed. Hyangsayukgunja-tang extract and Misoprostol were intragastric injected to the test groups at hour 72 before and just before indomethacin treatment(HYT-J, HYT-72, M-J, M-72), while the INDO group was injected only with indomethacin and the control group was subcutaneously injected only with saline. Results: The gastric mucosal lesions incresed in the fundus and body of INDO group, but softened in HYT group and M group, the effects were more excellent in the HYT-72, M-72 groups than the HYT-J, M-J groups and in the HYT group than M group. The disappearance of surface and neck mucose cells were shown in INDO group, but softened in HYT group and M group. The mucosal configuration of HYT-72 group was the same as control group. The numerical increase of PNA positive reaction in cytoplasm of perietal cells were appeared in INDO group. The PNA positive reaction in HYT group and Miso-group were shown in surface mucous cells and microvilli of apical surface in chief cells as control group, and were the same as control group in all mucosa of HYT-72 group. The distribution of ICAM positive cells, increased in INDO group, but decreased in M-72 group, and were the same as control group in HYT-72 group. The apoptotic cells, increased noticeably in gastric mucosa of INDO group, decreased in HYT group and M group, and decreased noticeably in HYT-72 group. Conclusions: Hyangsayukgunja-tang extract had excellent effects on indomethacin-induced gastric mucosal lesions.

        두릅나무 근피 추출물의 약물학적 연구 -흰쥐의 위염 및 웨궤양에 대한 효과-

        In a preliminary screening of the plant extracts for the antigastritic action in rats, the extract of Aralia elata(Araliaceae) showed positive activity in HCI plus ethanol induced gastric lesion. Systematic fractions with hexane, chloroform, ethyl acetate and butanol resulted in the most patent activity with the butanol fraction: This butanol fraction at the oral dose of 200 mg/kg exhibited significant inhibition of absolute alcohol induced gastric lesion which was more potent than 100 mg/kg of cimetidine and had significant stimulation of mucus secretion. The butanol fraction showed significant decreases in the ulcer indices of Shay ulcers and inhibition of gastric juice secretion with acid output in pylorus-ligated stomachs of rats. It also suppressed the acetic acid induced gastric ulcer. These results might suggest that the butanol fraction had inhibitory action in gastric lesion and ulceration through inhibition of gastric acid secretion and stimulation of mucin secretion in the stomachs of rats.

        Extra-gastric Cyst Lesions Mimicking Gastric Submucosal Tumor

        Gastric cancer remains still the most frequent type of cancer despite its declining incidence in Korea. As a result of the health promotion policy of the Korean government and increase in concern for individuals’ health, screening endoscopy for detecting early gastric cancer and general physical exams have become widespread. Thereby, the incidental detection of gastric submucosal tumors (SMTs) are now occasionally diagnosed by screening endoscopy. Since endoscopic examination gives little information on SMTs, a variety of studies have been conducted to determine the etiology of SMTs and to distinguish them from extra-luminal compressive lesions. Here, we report one clinical case of cavernous hemangioma on the left lateral section of the liver and one case of omental cyst, which was preoperatively mistaken as gastric SMT.

        The Feasibility and Safety of the Endoscopic Submucosal Dissection of Superfi cial Gastric Neoplastic Lesions in Patients with Compensated Liver Cirrhosis: A Retrospective Study

        Background/Aims: When undergoing endoscopic submucosal dissection (ESD), patients with liver cirrhosis (LC) may suffer from a high risk of bleeding, bacteremia and tissue vulnerability. There have been few reports evaluating the effi cacy and safety of ESD in patients with LC. Methods: From January 2004 to March 2010, 23 patients with LC (cirrhosis group) underwent ESD for superficial gastric neoplastic lesions. The number of patients with a liver function in the Child-Pugh classes A and B were 20 and 3, respectively. The clinical outcomes and complications were compared with 69 patients without LC (control group) that were matched for age and sex. Results: The en bloc resection, R0 resection and en bloc plus R0 resection rates of the cirrhosis group were 82.6%, 91.3%, and 82.6%, respectively, and did not show significant differences from the rates of the control group. No local recurrence was found in either group during the follow-up period. The procedure length of time (41.0 vs 39.0 minutes), rate of bleeding (4.3% vs 7.2%) and rate of perforation (0.0% vs 1.4%) in the cirrhosis group were also comparable to the results from the control group. Conclusions: ESD was safely performed in patients with LC, and satisfactory outcomes were achieved with high en bloc and R0 resection rates for superfi cial gastric neoplastic lesions.
