A Case of Multiple Piloleiomyoma on the Shoulder, which is Mistaken for Keloid

A 30-year-old male presented with multiple tender erythematous nodules, varying in size from 0.5 cm to 2 cm on the right shoulder of the body. He gave a history of pain associated with these lesions, especially on exposure to cold and touch. These lesions showed multi-segmental distribution; each began as a papule, which gradually increased both in size and number to form a nodule. The lesions, which were misdiagnosed as keloids at another hospital, were managed with numerous intralesional steroid injections. This treatment did not improve the symptoms. Based on the history and cutaneous findings during the physical examination, we considered a diagnosis of a benign tumor. Among the lesions, some of the larger ones were completely excised, followed by excision biopsy. We then made a diagnosis of cutaneous leiomyoma, more specifically, piloleiomyoma. The excisional lesions were covered by skin grafts and closed by primary repair.

Cutaneous piloleiomyomas are benign smooth muscle tumors arising from the arrector pili muscles. Piloleiomyomas appear as firm dermal papules of skin color or with a reddish to brown surface, and are commonly located on the extremities. Histologically, these lesions are composed of interlacing bundles of smooth muscle cells in the reticular dermis. Our case presented with an unusually large nodule on the forehead that was accompanied by intermittent pain. Histological analysis was compatible with piloleiomyoma and the lesion showed haphazardly arranged bundles of smooth muscle in the dermis. We describe herein an interesting case of a giant piloleiomyoma occurring on the forehead. (Ann Dermatol 23(S2) S144~S146, 2011)

Wolf``s isotopic response implies that the development of a secondary dermatoses having no association with the primary dermatoses which are usually herpes zoster or simplex. The secondary dermatoses could be various with neoplastic (breast carcinoma, squamous cell carcinoma, basal cell carcinoma, leukemic or lymphomatous infiltration and etc.), infectioous (warts, dermatophytosis, molluscum contagiosum, and etc.) inflammatory (lichen planus, granuloma annulare, and etc.,) and other conditions (reactive perforating collagenosis and etc). Though piloleiomyoma, which is derived from arrector pili muscle, is not rare condition, its segmental form is quite rare. A 51-year-old man presented with a three-year history of multiple scattered erythematous papules on left middle back that developed in the healed site of herpes zoster which had occurred nine years before. The distribution was zosteriform at the dermatomes of T9-10. Histopathologically, well-defined bundles of smooth muscle were arranged in dermis. The bundles were stained as red in Masson``s trichrome and positivity in smooth muscle actin. The final diagnosis were segmental piloleiomyomas as Wolf``s isotopic response after herpes zoster. Various dermatoses as Wolf``s isotopic response after herpes zoster have been reported in the literature. However, to our best of knowledge, segmental piloleiomyomas as Wolf``s isotopic response has never been reported in the literature.

Piloleiomyoma is an unusual benign tumor of smooth muscle derived from the arrector pilorum muscle, which may be solitary or multiple and generally present as pink, red or brown, firm nodules of varying sizes. The histologic examination reveals interlacing bundles of the smooth muscle fibers with centrally located nuclei in the dermis. We present a case of multiple piloleiomyoma in a 49-year-old female with multiple, pinhead to pea sized, skin colored, firm papules and nodules on face. About 15 years ago, multiple skin colored papules developed on both cheeks and eyebrow area. About 5 years after the onset of skin lesion, she as performed hysterectomy for uterine myoma. Skin lesions were gradually increasing in number and size. Some large nodular lesions became painful by cold or pressure. The histologic examination showed typical piloleiomyoma findings.

Cutaneous piloleiomyoma is a benign, often painful, smooth muscle tumor, originating from arrector pili muscle. It is usually present as red-brown nodules on the extensor extremities, and it can be multiple in 80% of patients. A 34-year-old man with multiple brownish papules and nodules on his left side of face and neck visited our department. Papules and nodules were dispersed with segmental distribution, and some papules coalescence to form plaque. There was one satellite brownish papule on left temporal area apart from main lesions. Histopathological examination showed a neoplastic aggregation composed of irregular interlacing bundles of smooth muscle cells. Immunohistochemical staining with smooth muscle actin was positive in tumor cells. To our knowledge, this case shows peculiar clinical manifestation of segmental piloleiomyoma with satellite lesion that has not reported before.

Piloleiomyoma is a benign neoplasm arising from the erector pili muscle in the skin. It occurs as linear or dermatomal arrangements of firm, red to brown intradermal nodules, which are fixed to the skin but not to the deeper tissues. Although various treatments have been attempted, they have shown limited success and several complications remain. A 21-year-old male presented with 3-year history of multiple, erythematous, firm 4 mm to 3 cm sized nodules on the chest, The histopathological examination was compatible with piloleiomyoma, The lesions were removed using the dermal shaving method with a suction-assisted cartilage shaver, Each lesion were markedly flattened. This treatment was quicker and caused less subjective pain to the patient compared with those of classic surgical excision. No signs of adverse events or recurrence have been observed.

We report a case multiple piloleiomyoma in a 50-year-old man. About 30 years ago, multiple red colored firm papules with pain developed on the left lower leg. The skin lesions gradually increased in number and in and size arranging in a group. Some, lesion became more painful after cold exposure. About a few years ago, new lesions developed on the left back and flank abdomen, The histologic findings showed poorly demarcated and were composed of interlacing bundles of smooth muscle fibers with varying amounts of collagen bundles being intermingled. Masson's trichrome and smooth muscle actin stain showed typical leiomyoma, and s-100 stains revealed an increased amount of nerve fibers interlacing within muscle fibers and the surrounding tissue. We diagnosed our case as unilateral multiple piloleiomyoma arising from arrector pilorum muscle. Pain was ameliorated with oral nifedipine alone.

BCG is a live attenuated vaccine derived from Mycobacterium bovis. It can give rise to several cutaneous complications including benign and malignant tumors. There are many reports about tumors arising in vaccination sites, especially those of small pox and BCG vaccinations. Reported tumors are basal cell carcinoma, dermatofibroma, melanoma, pilomatricoma and so on. Piloleiomyoma is a rare soft tissue tumor arising from arrector pili muscles. Clinically, piloleiomyomas appear as firm dermal papules and nodules with skin colored, pink, or reddish-brown surfaces. We experienced a case of piloleiomyma arising in a BCG vaccination site, which has not been reported in previous literature. Although several hypotheses have been propsed, there are no identified causes of leiomyoma. Trauma by the vaccination, persistent inflammation, wound healing process, or antigenic stimulation of the innoculation agent could be the possible causes in this case.
