        침습, 비침습 혈관레이저에 의한 고콜레스테롤혈증의 치료 효과 비교

        Objectives : The purpose of study was to compare and analyze the effect of the Intravascular Laser Irradiation Therapy and the Exterior vascular Laser Irradiation Therapy on Hypercholesterolemia. Methods : The 73 Sam-se-han-bang hospital patients who were irradiated to the Intravascular Laser and the 57 Sam-se-han-bang hospital patients who were irradiated to the Exterior vascular Laser were the study group. They had the treatment two or three times a week, and had checked total cholesterol before and after the ten times irradiation therapy. Results : Both the Intravascular Laser Irradiation Therapy and the Exterior vascular Laser Irradiation Therapy were effective on decreasing total cholesterol. The effect on decreasing total cholesterol was more definite at the abnormal group than the border line group, and the border line group than the normal group. The effect on decreasing total cholesterol was more definite at the Exterior vascular Laser Irradiation Therapy than the Intravascular Laser Irradiation Therapy, but this result was unavailable in the statistics. Conclusion : Both the Intravascular Laser Irradiation Therapy and the Exterior vascular Laser Irradiation Therapy had available effect on decreasing total cholesterol.

        혈관레이저 치료와 관련한 생혈액 검사의 진단 의미 고찰

        Objectives: The purpose of this study was to investigate the usability of live blood analysis on interior and exterior vascular laser irradiation therapy. Methods: We had analyzed the changing forms of the live blood sample with microscope before and after vascular laser irradiation therapy of Blood. The live blood analysis was operated on Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals, red crystals, protoplasts. First, we analyzed all patients on each item, then did same thing classified two groups, interior and exterior. Results: Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals, red crystals, protoplasts were decreased significantly, after interior and exterior vascular laser irradiation therapy. Exterior vascular laser irradiation therapy was more effective than interior on Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals. Interior vascular laser irradiation therapy was more effective than exterior on red crystals, protoplasts. Conclusions: This study suggests that live blood analysis has the usability on vascular laser irradiation therapy. Then according to interior and exterior vascular laser irradiation therapy, the result has some different on each item. So it is better that choose the method, interior or exterior, for more effective therapy.

        A Face-Split Study to Evaluate the Effects of Microneedle Radiofrequency with Q-Switched Nd:YAG Laser for the Treatment of Melasma

        Background: Laser toning using a low-fluence 1,064 nm Q-switched Nd:YAG laser is one of the most frequently used treatment modalities for melasma. However, this therapy is time consuming because it requires a lot of treatment sessions. Recently, it has been reported that transdermal radiofrequency (RF) is effective for the treatment of melasma. Objective: To determine whether microneedle RF conduction could be an adjunct therapy for melasma, we have studied the effect of simultaneous treatments with laser toning and RF for melasma. Methods: Fifteen patients with melasma underwent five sessions of laser toning and microneedle RF on the right side of the face, and only laser toning on the left side. Responses to treatments were evaluated using the Mexameter<sup>Ⓡ</sup> (Courage Khazaka, Germany) score, the pigmentation and severity index (PSI) score, and the patient’s overall assessment. Additionally, an electron microscopic study of a skin biopsy was performed. Results: Both laser toning and combination therapy showed significant decreases in the Mexameter<sup>Ⓡ</sup> and PSI score after five treatment sessions. Combination therapy showed a more significant improvement of melasma than laser toning. No remarkable side effects were reported. Electron microscopic analysis showed a greater number of vacuolar changes and increased loosening of melanocytes and adjacent epidermal cells after combination therapy. Conclusion: The combination treatment of laser toning and microneedle RF therapy showed a better therapeutic effect for melasma than laser toning alone. Therefore, the microneedle RF technique could be a new and safe adjunct therapy for the treatment of melasma. (Ann Dermatol 31(2) 133∼138, 2019)

        뇌신경질환에 사용된 레이저 및 광선치료에 대한 임상 연구 고찰

        Objectives: Recently, many studies have reported beneficial effects from the application of laser and light-emitting diode (LED) therapy for cerebral nervous disease. Transcranial laser therapy and LED therapy may be an effective method to treat diseases of the cerebral nervous system. This study aims to discuss the possibility of laser and LED therapy for cerebral nervous disease by reviewing literature about its effectiveness. Methods: We searched papers using PubMed, Science Direct, CINAHL, KTKP, Oasis and NDSL, using the keywords "Laser therapy, low-level", "Transcranial laser", "Transcranial light emitting diode" and "stroke", "traumatic brain injury", "dementia", "anxiety", "cognitive", "emotional effects", "psychiatric disorder", "multiple sclerosis", "Parkinson's disease". The search range included randomized controlled trials (RCTs) and clinical case series. Reviews and animal experiments were not included. Studies not matched with inclusion criteria were excluded. Results: A total 1,119 studies were found. 1,100 were excluded from scanning titles and abstracts and finally 9 articles were selected. Among the 9 articles, 5 were RCTs, one was a controlled study, and the other 3 were case reports. They reported that transcranial laser therapy and LED treatment had beneficial effects from photobiomodulation to the cerebral nervous system. Clinical evaluation factors showed favorable trends. Conclusions: Transcranial laser therapy and LED therapy seem to be effective to the cerebral nervous system and they may be a favorable choice for cerebral nervous disease.

        Enhancement of Contour Using Laser Scar Treatment in the One- Stage Nasal Ala Reconstruction with Nasolabial Fold Flap

        Postoperative scarring around the nasolabial area and alar rim poses very high cosmetic concerns, leading to patient dissatisfaction. Several strategies, like laser therapy, have been developed to improve its clinical aspects. However, to date, little is known about when in the early wound healing phases laser exposure would have the most optimal results. This paper introduces a remarkable case of effectively treating a 32-year-old male patient with scarring on the alar rim and nasolabial fold using early active laser treatment. We started carbon dioxide (CO2) laser therapy early in the second month after surgery. In the course of laser treatment, we performed a combined treatment using an ablative fractional CO2 laser device and the combination of pulsed-dye laser and neodymium: yttrium-aluminum-garnet laser. A total of 7 sessions of CO2 laser therapy and 3 sessions of cynergy laser therapy were performed, and the patient was followed-up for evaluation. These early laser treatments resulted in both patient and clinical satisfaction. In this case, we confirmed that early treatment with CO2 laser and cynergy laser can have a positive effect on scarring of the alar rim and nasolabial fold.

        초기 성문암에서 레이저 성대절제술과 방사선치료 결과

        Background and Objectives:Early glottic cancer can be efectively treated with conservation laryngeal surgery, radiation therapy, and endoscopic laser surgery. The aim of this study was to compare the clinical results between laser cordectomy and Materials and Method:From 1988 to 1998, 89 patients with T1-T2/N0 glottic cancer were treated initially with radiation therapy or laser cordectomy. There were 67 T1 and 22 T2 tumors. Fifty-two patients were treated by radiation therapy (RT), and thirty-seven patients were treated by endoscopic laser cordectomy. The method of primary treatment, local control rate, survival rate and larynx preser-vation were retrospectively evaluated. Results:With the median follow-up period of 48.2 months, the local control rates in laser cordectomy and radiation therapy were 88.9%, 89.7% for T1, and 90.0% and 61.5% for T2 tumors, respectively. The 3-year survival rate was 88.9% and 87.2% for T1 and 80.0% and 61.5% for T2. Larynx preservation rate was 83.4% in T1 and 70.0% in T2 patients. These results of laser cordectomy were superior to those treated by radiation therapy. Conclusion:In T1b glottic cancer, radiation therapy gave better results than laser cordectomy, whereas for T2 glottic cancer, laser cordectomy greater likelihood of larynx preservation and more options for further treatment in case of failure. We conclude that the laser cordectomy is a good surgical alternative for properly selected early glottic cancer. (Korean J Otolaryngol 2002;45:159-63)

        저단계 레이저 치료의 한방안이비인후피부과 활용에 대한 논문 경향 분석

        Objective : Low level laser therapy(LLLT) is conjugated many different diseases such as skin disease, nervous system disease, cardiovascular disease, etc. This study was tried to survey the conjugation of LLLT in field of ophthalmology, otolaryngology and dermatology of Korean Medicine. Methods : In this study, LLLT deal with the field of ophthalmology, otolaryngology and dermatology was searched at Pubmed and NDSL. We searched Pubmed and NDSL with the title "Laser Therapy and Low-Level" for the last 10 years and analyzed disease, laser type, sample size, effect, journal and year. Results : Using Pubmed and NDSL, 8 treatises were researched. About 8 treatises, 7 treatises were papers in field of dermatology, 1 treatise was ophthalmology. And about 8 treatises, 6 treatises were foreign papers and 2 treatises were domestic papers on clinical application of laser status. Mainly used type of laser was a He-Ne laser and Diode laser. The diseases were ocular hypertension, acne, ulcers in patients with leprosy sequelae, freckles, segmental-type vitiligo lesions, diabetes-linked skin lesion, facial skin melanin and elasticity, atopic dermatitis symptom. As a result, we found that treat group had more improvement than control group in 7 of the 8 treatises, that treat group had no difference control group on effect significantly only 1 treatise. Conclusions: As therapeutic tool, LLLT's application range is very wide in the field of ophthalmology, otorhinolaryngology, and dermatology. Keep up with the increasing interest of domestic and foreign about laser therapy, clinical tries on the use of laser will be studied steadily. It is necessary to have sustained attention and research on the point of view of the Korean Medicine.

      • A comparison of efficacy in ablative fractional laser assisted photodynamic therapy according to ablative depth for actinic keratosis

        Background: Er:YAG ablative fractional laser-assisted photodynamic therapy (AFL-PDT) has shown significant benefit for the treatment of actinic keratosis(AK). However, no comparison of the PDT efficacy with ablative laser parameters has been completed. Objectives: To investigate whether the use of increased laser depth affects the efficacy, side effects, cosmetic outcomes, and protoporphyrin IX(PPIX) accumulation of AFL-PDT for facial AK. Methods: 309 AK lesions of 36 patients were included. Patients were randomly allocated to the 150-μm, 350-μ m, and 500-μm laser depth groups. After AFL was performed with either different laser depth, all patients underwent one session of PDT. Patients were followed up with at 1week, 1month, and 3months post treatment. Efficacy, side effects, cosmetic outcomes, and accumulated levels of PPIX were assessed. Results: Increasing laser depth did not enhance PPIX fluorescence. However, three months after treatment, the treatment of the higher laser depth groups(500μm) appeared to be significantly more effective than that of the lower laser depth groups(150μm) in terms of the management of Olsen grade III AK lesions. No significant differences were found with respect to side effects, cosmetic outcomes and safety. Conclusion: Higher laser depth does not increase PPIX accumulation, but does improve the treatment effect of AFL-PDT for high-grade AK lesions. We recommend AFL-PDT using higher laser depth parameters in cases of high-grade AK lesions.

      • A case of malignant melanoma presenting after laser therapy of pigmented lesion

        Laser therapy of a pigmented lesion is a common cosmetic procedure. However, pigmented skin lesions include diverse diagnosis; few of the contraindicated lesions, including dysplastic nevi and malignant melanoma, are being mistreated with laser. Thus, the use of laser therapy in the treatment of pigmented lesions is a controversial issue as it can delay melanoma diagnosis and the possibility of laser as a promoting factor of malignancy cannot be denied. We report a case of a 43-year-old woman with a black colored nevus that was treated several times with laser for the past ten years. Even after laser treatments, she still presented with an ovoid brownish black pigmented plaque on dorsum of left thumb base. Skin biopsy revealed malignant melanoma. Wide excision operation was performed with a 5mm margin. No additional treatments were required. There are several reports of malignant melanoma occurring after laser treatment, however it is hard to discuss the causality. In this case, the patient was evaluated by dermoscopy for the first time, and the findings corresponded to the histopathologic results. It emphasizes that precise evaluation of pigmented lesion is necessary. Also in our case, the pigmented lesion existed for a long time and this indicates that if the nevus was initially malignant, it had slowly progressed. Otherwise, we can think that recent malignant transformation occurred following previous laser treatments.

        Combination Treatment of Nd:YAG Picosecond-domain Laser and Fractional CO2 Laser for Contracted Neck Scar with Hyperpigmentation

        There is growing interest in minimizing postoperative scarring after a thyroidectomy. Among the many treatment types, laser therapy, especially picosecond-domain laser therapy, is accepted as a standard method. In the present case, a patient with a pigmented, contracted scar was treated using the combination of a picosecond laser and ablative fractional (AF) CO2 laser. After 15 sessions of 1,064-nm picosecond with micro lens array (MLA) and AF CO2 laser application, the patient showed significant improvement in their pigmented lesions and scar contracture with no noticeable side effects for 16 months. These results suggest that a combination of picosecond laser with MLA and AF CO2 laser can treat pigmented, contracted scars safely and effectively.
