        얼굴피부염 환자에서 피부접촉검사 결과에 대한 고찰

        Background: Patch test is usually performed in order to differentiate allergic contact dermatitis from other facial skin diseases; however, there are only infrequent comparative studies regarding the positive allergens patch tested between allergic contact dermatitis and other facial skin diseases. Objective: The purpose of this study was to investigate the positivity of patch test in patients with facial dermatitis and to analyze the valid positive allergen patch tested in allergic contact dermatitis compared to other facial skin diseases. In addition, we evaluated the difference of positive allergen according to clinical features, such as age, sex, site of the lesion and duration of disease, in patients with allergic contact dermatitis. Methods: A total of 409 patients tested with facial dermatitis patch were retrospectively reviewed for medical recordings, including clinical features, occupational history, clinical photos and the results of a Korean standard series of patch tests. Results: Compared with other facial skin disease, patients diagnosed with allergic contact dermatitis showed higher rate in the patch test, responding to 2 or more allergens. They also had more statistically valid positive allergens, such as phenylendiamine base, cobalt, colophony, thimerosal and isothizolinone. In addition, there were a few allergens which revealed significant differences in the positivity of the patch test according to the clinical features in patients diagnosed with allergic contact dermatitis. Conclusion: Based on these results, we could receive help in order to classify the positive allergens as relevant allergens in the patch test for the differentiation of allergic contact dermatitis from other facial skin disease.         알레르기 접촉피부염 환자에서 피지오겔(R) 크림의 임상적 적용에 관한 연구

        Background: A allergic contact dermatitis has been increasing in the world. The first choice of treatment for the allergic contact dermatitis is topical steroids, but application of topical steroids may lead to many side effects. Physiogel(R) cream, made of ceramide, triglyceride, squalene and others is a moisturizer without any fragnance or preservatives which has effects of normalizing the damaged skin barrier and soothing troubled skin. Objectives: We carried out this study to evaluate the effects of the Physiogel(R) cream in the treamemt of the allergic contact dermatitis. Methods: Among the patients clinically suspected to have allergic contact dermatitis due to fragnance or perservatives, patients already diagnosed with allergic contact dermatitis were included. On the skin lesions of the right side of individual patients the Physiogel(R) cream was applied twice a day. No topical agents were applied to the lesions of the left side. Two weeks later the clinical improvement of skin lesions was compared. Results: In fourteen (61%) out of 23 patients the Physiogel(R) cream-applied lesions showed significant clinical improvement compared with the control lesions. In cases of atopic dermatitis or seborrheic dermatitis, the ratio of patients in whom the Physiogel(R) cream was efficacious, was 80% or 70% respectively. The skin lesions of all patients of 2nd and 3rd decades were significantly improved by the Physiogel(R) cream. In patients whose skin lesions were not responsive to the Physiogel(R) cream, patch test results showed the highest positive response rate against nickel (20%) and formaldehyde (20%). Conclusion: Physiogel(R) cream could be used as well as topical steroids in the treatment of allergic contact dermatitis reducing the risk of side effects due to long-term use of topical steroids.         백서(白鼠)에 유도된 알러지성 피부염에 대한 초감탕(椒甘湯)의 효과

        Background and Objectives: Allergic contact dermatitis is a common environmental health issue and seriously affect the patient's quality of life. The more our environment industrialized, the number of material that could cause the allergic contact dermatitis has been increased, consequently the prevalence of allergic contact dermatitis has been increased. In oriental medicine, clinically Chogam-Tang has been used fur the treatment of allergic dermatitis, eczema, atopic dermatitis etc. The objective of this study is to investigate the effects of Chogam-Tang on allergic contact dermatitis Meterial and Methods: Fifteen Sprague-Dawley rats were divided into three groups: normal, control, experimental group. Control and experimental group were induced allergic contact dermatitis, by DNCB. Experimental group was orally administered the Chogam-Tang. Each group was observed after 24, 48 and 72 hours. Contact hypersensitivity assay, melanin-erythema measurement, pH measurement skin moisture measurement and biopsy were performed. Results: 1. In contact hypersensitivity assay, experimental group showed decreased ear swelling compared with control group at 48hours. 2. ln melanin measurement there was no difference in three groups. 3. In erythema measurement experimental group showed reduction at 48. 72 hours. 4. In pH measurement, experimental group and control group showed increase in pH but there was no statistical significance. 5. In skin moisture measurement, experimental group showed higher skin moisture level than control group at 24 hours and showed lower skin moisture level at 72 hours, but there was no statistical significance. 6. In biopsy, experimental group showed decrement of Iymphocyte as time goes by, and regeneration of keratin layer was increased compared with normal group. Conclusions: Chogam-Tang shows anti-inflammatory effect in biopsy, improves hydration levels of skin, decreases erythema level on allergic contact dermatitis.

      • 사암침법으로 호전된 만성화된 알레르기성 접촉피부염 환자 치험 1례

        Objectives : The purpose of this study is to report that SAAM Acupuncture and herbal medicine were effective for Chronic Allergic Contact Dermatitis. Method : A patient with Chronic Allergic Contact Dermatitis suffering side effects for 1 year was treated with SAAM acupuncture and herbal medicine. Improvement of vesicles, itching, erythema and erosions was evaluated. Result : After treatment, symptoms of Chronic Allergic Contact Dermatitis such as vesicles, itching, erythema and erosions were improved. Conclusions : The SAAM acupuncture and herbal medicine can be applied to Chronic Allergic Contact Dermatitis.

        망고 접촉피부염 1예

        Allergic reaction to different fruits and vegetables has frequently been described, but an allergic reaction to mango fruit, including its sap, pericarp, stems and leaves, has rarely reported in the literature. Mango dermatitis, although not common, is the term for allergic contact dermatitis caused by mango. The Mango plant is found worldwide, and the plant shares chemical compounds that are similar to other substances of the Anacardiaceae plant. The substances uroshiol and cardol cause the allergic contact dermatitis. Any case of allergic contact dermatitis with eating mango fruit has not been reported in the Korean dermatologic literature. Here we report the first case of mango contact dermatitis in a 20-years-old female and this happened when she drank mango juice. (Korean J Dermatol 2009; 47(5):612~614)

        A Case of Allergic Contact Dermatitis to 'Aroma Oils' in an Aroma Therapist

        Allergic contact dermatitis from aroma oils has long been recognized^(1-4) and with increasing frequency^(5,6). Contact dermatitis of the allergic type of fragrances used for aromatherapy has been reported rarely, and only in patients exposed due to their occupation^(5-7). We report herein a case of allergic contact dermatitis to aroma oils in an aroma therapist. A 25-year-old woman, an aroma therapist, had been treated with topical steroids under the impression of contact dermatitis. She had had pruritic erythematous maculopapules on the arms extending to the hands for the previous 6 months. Patch tests showed strong reactions to aroma oils(3% fennel and 3% juniper). The skin lesions were cleared after she stopped her job. However, re-exposure to aroma oils caused eczematous lesions repeatedly at the sites previously involved. We predict that increasing popularity of aromatherapy will lead to increasing number of patients developing allergic contact dermatitis from aroma oils. This has to be taken into consideration when testing patients with suspected contact dermatitis.

        알레르기성 접촉피부염이 유발된 마우스 표피세포의 림프구 자극능력

        Allergic contact dermatitis is a prototype of delayed hypersensitivity reaction. Langerhans cells, keratinocytes and T lyrnphocytes play major roles in the pathogenesis of allergic contact dermatitis. We observed Langerhans cells and keratinocytes in contact sensitized epidermal sheets of mice and performed mixed epidermal cell lymphocytes reaction with normal epidermal cells and contact sensitized epidermal cells to determine the lymphocyte stimulating capacity of contact sensitized epidermal cell. We obtained following results : 1. The Langerhans cells were decreased in number and morphologically damaged in contact sensitized epidermis. 2. Ia antigen expression on keratinocytes was detected in almost all contact sensitized epidermal sheets. 3. The allogeneic lymphocyte stimulating capacity of contact sensitized epidermal cells was greater than that of normal epidermal cells. 4. The allogeneic lymphocyte stimulating capacity of contact sensitized epiderrnal cells was lost after treatment of epidermal cells with anti Ia antibody and complement. From these results, it is conceivable that the contact sensitized epidermal cells can amplify the immune reactions by stimulating the lynphocytes which are infiltrated in contact sensitized epidermis.

        Objectives : This study was carried out to analyze the trends of clinical studies of allergic contact dermatitis published in the Journal of Korean Medical Ophthalmology & Otolaryngology & Dermatology. Methods : We studied 10 research papers on allergic contact dermatitis published in JKOOD. We analyzed about experiment methods, pharmacodynamic effects, therapeutic effects in published papers. Results : 1. The number of searched journal is 10 papers. 2. All experiments was performed using DNCB primed mouse. To demonstrate therapeutic effect of drugs, previous researches observed dermatological and histopathogical conditions, erythema, melasma, white blood cell count, Lymphocyte ratio to white blood cell. 3. Most of drugs have heat-clearing and detoxifying effects. 4. Every drugs have positive effect in treating allergic contact dermatitis. Conclusions : Figuring out how experiment proceeded and tendency of drugs in researching allergic contact dermatitis is significance of this paper. Further research about drugs and their treating methods should be carried out to improve curative effectiveness for allergic contact dermatitis.

        A Study on Leather in Korea as a Cause of Cobalt Allergic Contact Dermatitis

        Background: Cobalt is an important contact allergen and is contained mainly in metal products. Recent studies have reported an association between leather exposure and cobalt allergic contact dermatitis. However, there is no study on this subject in Korea. Objective: The aim of this study is to examine cobalt content and release from leather sofas, shoes, watch straps, and gloves sold in Korea, and to investigate leather exposure in cobalt allergic contact dermatitis. Methods: We collected 38 leather samples of leather sofa, 15 leather shoes, 8 leather watch straps, and 10 leather gloves sold in Korea. The cobalt spot test and inductively coupled plasma optical emission spectrometry were used to confirm cobalt content and release from leather samples. Results: All 71 leather samples were negative for cobalt in the spot test. No cobalt was found in the 35 leathers tested by inductively coupled plasma optical emission spectrometry. Conclusion: Unlike previous studies, cobalt was not identified in leather in this study. Therefore, the possibility of cobalt allergic contact dermatitis caused by leather is relatively low in Korea. However, further studies with larger numbers of leather samples are needed to obtain more accurate results. (Korean J Dermatol 2021;59(7):534∼537)

      • 경피적외선 조사요법과 침치료로 병행 치료한 알레르기성 접촉 피부염 환자 치험 1례

        The number of allergic contact dermatitis patients is steadily increasing but a proper reliability treatment guide is not prepared in Oriental Medicine. Also, the number of clinical cases on allergic contact dermatitis is not sufficient in Oriental medicine. As such, the facial flushing of allergic contact dermatitis which has been improved by infra-red therapy is significant in its treatment purpose. We reported on 43-year old male patient with allergic contact dermatitis. We treated him with herbal concoction, acupuncture, infra-red therapy and cupping therapy. We have received a good result in this case by using the combination of infra-red therapy. We reached a conclusion that using infra-red therapy can improve the facial flushing of patient with allergic contact dermatitis. However, further research on the effect of infra-red therapy for these symptoms is desired.
