        Columnar variant of papillary carcinoma in the thyroglossal duct cyst with progression to lung metastasis

        Thyroglossal duct cyst (TGDC) carcinoma generally shows a favorable prognosis. If metastasis is present latently, it may not threaten the patient’s life immediately. It has been shown, however, that larger than 1 cm papillary carcinoma (PC), level VI metastasis to the lymph node (LN), which is the nearest to the thyroid, independently predicts a worse prognosis. In the case presented herein, a 61-year-old female patient was diagnosed with an about 3 cm PC in the TGDC, particularly the columnar variant subtype, one of the aggressive variants. She had occult papillary thyroid microcarcinoma, but no LN metastasis. Even though she underwent the Sistrunk procedure and total thyroidectomy with central compartment neck dissection followed by high-dose radioactive iodine remnant ablation, however, the cancer cells spread to level IV neck LN, and finally to the lung. Therefore, when a patient is diagnosed with an aggressive histologic variant of PC in the TGDC, even without LN metastasis, the invasive surgical approach and close postoperative surveillance are necessary, with consideration of the risk of disease progression. Therefore, if it is possible to stratify the risk for patients, higher-risk patients can be offered a more invasive therapeutic approach.

        스마트폰 기반 마이크로블로그 학습활동이 사회적 실재감에 미치는 영향

        This study aimed to understand the enhancement of social presence through microblogging learning activities with smartphones. With the research contract and help of an IT company, the study conducted with 16 participants at a "Distance Learning" class in K University. The class lasted 12 weeks in a blended learning format. Triangulation methods were adopted for the analysis of the study: Paired T-tests, semi-structured interviews, and content analysis. As a result, all of the approaches showed that the participants' social presence enhanced in a significant way. Based on the results, it was suggested that (1) other variables such as academic performances, satisfaction should be dealt with to understand other effect of microblogging with smartphoens (2) discrepancies of social presence between online and offline classes should be addressed for other blended learning environments, and (3) more diverse mobile learning strategies should be developed for preparing for the ubiquitous learning society. 본 연구는 스마트폰 기반의 블렌디드 학습환경 속에서 마이크로블로그 활동이 사회적 실재감에 미치는 영향을 다양한 자료를 통해 파악하고자 하는데 목적이 있다. 이를 위하여 K대학교의 2010년 1학기 ‘원격교육활용론’ 강좌를 등록한 대학생 16명을 대상으로 12주 동안 수업과 직ㆍ간접적으로 관련된 다양한 형태의 의사소통이 마이크로블로그상에서 진행되었다. 연구문제에 대한 검증을 위해 사회적 실재감 측정치에 대한 사전-사후 양적결과 분석, 마이크로블로그 메시지에 대한 내용분석, 그리고 인터뷰 결과에 대한 분석을 실시하였다. 분석 결과 공통적으로 사회적 실재감이 향상된 것으로 나타났다. 이런 연구결과를 바탕으로 사회적 실재감에서 더 나아가 마이크로블로그의 학습 효과 등에 대한 연구, 온-오프라인 간의 사회적 실재감 격차를 줄이는 방안에 대한 연구, 나아가 향후 유비쿼터스 학습환경을 대비한 다양한 각도에서의 폭넓은 교육효과를 위한 지속적인 모바일 기반의 학습전략 접근 등이 요구됨을 제안하였다.

      • e-Learning학습 시 학습자의 주의집중 향상에 관한 연구

        본 연구는 e-Learning 환경에서 동영상 강의를 들을 때, 강의 조작행위를 최소화해 학습자의 주의집중을 향상시키는 방안을 고안하는 것을 목적으로 진행했다. 학습자의 강의 조작행위는 주의집중을 방해하는 요소라고 간주했다. 강의 학습자의 고개 움직임에 따라 동영상의 속도를 변화시켜 학습자가 강의 조작을 하지 않도록 돕는 기능을 연구했다. 학습자의 고개의 상하 움직임을 분석해, 고개를 들고 숙이는 이유와 필기의 관계를 관찰했다. 필기를 하지 않는 고개 움직임의 최소 시간을 측정했고, 강의 속도 자동조절 기능 실험에 적용 했다. 이 시간을 강의 속도 자동조절 기능에 적용함으로써 짧은 고개 움직임에 기능이 반응하는 것을 방지할 수 있었다. 실험1 결과 학습자들의 고갯짓 중 2초 이내의 경우 필기와 관련이 없었으며, 2초에서 5초 사이는 짧은 필기, 5초 이상은 긴 필기를 하는 것으로 관찰되었다. 이에 따라 강의 속도 자동조절 방법은 고개가 내려간 후 2초간의 시간차를 두고 시행하는 것으로 실험2를 진행했는데, 필기를 방해하는 조작 횟수가 1/4로 줄어든 것을 확인할 수 있었다. 따라서 강의 속도 자동조절 방법은 필기에 도움을 줌으로써 주의집중 향상에 긍정적인 영향을 주는 것으로 결론지을 수 있었다. This study is to develop learner"s attention to help taking notes by minimizing function control and adjusting play speed with learner’s habit during studying e-Learning. we analyze learner’s head nodding and divide head shaking into two situation. One is nodding for note-taking and the others are nodding for nothing. We study on the relationship nodding with note-taking. Counted nodding time shows us minimum time for ignoring nod of head. This nodding time is applied to a function of auto-controlling of media speed for e-Learner. So we can prevent that the function unnecessarily changes a speed of e-learning lecture. As a result of the case 1, under 2 seconds of learner"s noding time is meaningless. Between 2 seconds to 5 seconds of learner"s noding time is note-taking for short words. Over 5seconds of learner"s noding time is note-taking for long sentences. We apply the auto-controlling of media speed to the case 2. In the case 2, learner" the number of function control and adjusting play speed decreased to 1/4. As a result of the study, it showed that the function of auto-controlling of media speed has a positive influence on note-taking for attention improvements.

        Efficient differentiation of human pluripotent stem cells into functional CD34+ progenitor cells by combined modulation of the MEK/ERK and BMP4 signaling pathways

        <B>Abstract</B><P>Differentiation of human pluripotent stem cells (hPSCs) into functional cell types is a crucial step in cell therapy. In the present study, we demonstrate that functional CD34+ progenitor cells can be efficiently produced from human embryonic stem cells (hESCs) and induced pluripotent stem cells (hiPSCs) by combined modulation of 2 signaling pathways. A higher proportion of CD34+ cells (∼ 20%) could be derived from hPSCs by inhibition of mitogen-activated protein kinase (MAPK) extracellular signal-regulated protein kinase (MEK)/extracellular signal-regulated kinase (ERK) signaling and activation of bone morphogenic protein-4 (BMP4) signaling. hPSC-derived CD34+ progenitor cells further developed to endothelial and smooth muscle cells with functionality. Moreover, they contributed directly to neovasculogenesis in ischemic mouse hind limbs, thereby resulting in improved blood perfusion and limb salvage. Our results suggest that combined modulation of signaling pathways may be an efficient means of differentiating hPSCs into functional CD34+ progenitor cells.</P>

        Pandemic Influenza (H1N1) and Mycobacterium tuberculosis Co-infection

        We hereby observe four co-infection cases of pandemic influenza H1N1 and Mycobacterium tuberculosis with various clinical presentations. It may be prudent to consider M. tuberculosis co-infections when patients with pandemic influenza reveal unusual clinical features that do not improve despite appropriate treatments against the influenza, especially in Korea, in the endemic areas of M. tuberculosis.

      • A Fully Closed Nano-Focus X-Ray Source With Carbon Nanotube Field Emitters

        <P>We developed a fully closed nano-focus X-ray source using carbon nanotube (CNT) field emitters. The nano-focus X-ray source includes a fully vacuum-sealed CNT Electron-gun (E-gun) tube and E-beam-focusing modules consisting of electrostatic and magnetic lenses, facilitating very compact and high-resolution X-ray imaging. The paste-printed CNT emitters were prepared in a small pattern as an electron source, and the entire components, including the CNT emitter module, Cu tubes, collimators, and transmissive anode target, were specially brazed at an elevated temperature, resulting in a very compact E-gun tube. The CNT emitter showed a stable and reliable emission current with its density of over 300 mA/cm<SUP>2</SUP> even in the vacuum-sealed E-gun tube. With the anode grounded and the CNT emitter module biased to negative high voltages, we achieved a considerably magnified x-ray image of a several hundred nanometer resolution. The fully vacuum-sealed CNT E-gun tube along with the magnetic lens module can offer much more compact, easily maintainable, high-resolution X-ray imaging compared with conventional open-type thermionic X-ray sources.</P>
