        Original Article : The levels of liver enzymes and atypical lymphocytes are higher in youth patients with infectious mononucleosisthan in preschool children

        Background/Aims: Infectious mononucleosis (IM) is the clinical presentation of primary infection with Epstein-Barr virus. Although the literature contains a massive amount of information on IM, most of this is related specifically to only children or adults separately. In order to distinguish any differences between preschool children and youth patients, we retrospectively analyzed their demographic and clinical features. Methods: Records of patients hospitalized from December 2001 to September 2011 with a diagnosis of IM were retrieved from Peking University First Hospital, which is a tertiary teaching hospital in Beijing. The demographic data and clinical characteristics were collected. Results: IM was diagnosed in 287 patients during this 10-year period, with incidence peaks among preschool children (≤7 years old, 130/287, 45.3%) and youth patients (>15 and <24 years old, 101/287, 35.2%). Although the complaints at admission did not differ between these two patient groups, the incidence of clinical signs (tonsillopharyngitis, lymphadenopathy, hepatomegaly, and edema of the eyelids) was much higher in preschool children. The incidence of liver lesion and percentage of atypical lymphocytes were significantly higher in the youth group (P<0.001), and the average hospital stay was longer in this group. Pneumonia was the most common complication, and there was no case of mortality. Conclusions: The incidence of IM peaks among preschool children and youth patients in Beijing, China. The levels of liver enzymes and atypical lymphocytes increase with age.

        Characteristics, Controlled-release and Antimicrobial Properties of Tea Tree Oil Liposomes-incorporated Chitosan-based Electrospun Nanofiber Mats

        In this paper, a notable chitosan/poly(ethylene oxide) nanofiber mats containing tea tree oil liposomes (TOLCENs)were successfully fabricated using electrospinning process. The microstructures and morphology were characterizedby scanning electron microscopy. The porosity, fluid absorbability, water vapor permeability and mechanical properties ofnanofiber mats were also estimated by ethanol density method, gravimetric method, dish method and tensile test,respectively. Compared to the chitosan/poly(ethylene oxide) composite freeze-dried sponges containing tea tree oilliposomes, TOL-CENs had greater porosity, water absorption, breathability and better mechanical properties. In addition, thecontrolled-release properties and long-term bactericidal capability of the material were also assessed. From the analysis of therelease kinetics and mechanism, it was found that the significant decreased terpinen-4-ol concentration gradient fromliposomal surface to the outside of material was the key to the sustained terpinen-4-ol release in virtue of liposomalencapsulation. TOL-CENs exhibited long-term and more excellent microbicidal effects against Staphylococcus aureus,Escherichia coli and Candida albicans than chitosan/poly(ethylene oxide) nanofiber mats. The combination of tea tree oilliposomes and chitosan in nanofiber mats synergistically destroyed cell membrane, prevented cell adhesion and caused theirregular aggregation of cytoplasm, resulting in cell disintegration observed by transmission electron microscope. Insummary, TOL-CENs had potential application value as a long-term antimicrobial nonwoven materials.

      • Clinical Research on Albumin-Bound Paclitaxel-Based Chemotherapy for Advanced Esophageal Cancer

        Background: To evaluate the efficacy and safety of albumin-bound paclitaxel-based chemotherapy in treatment for patients with advanced esophageal cancer who failed in first-line chemotherapy. Materials and Methods: We collected29 advanced esophageal cancer patients who received albumin-bound paclitaxel-based chemotherapy fromJune 2009 to September 2013, and the efficacy and safety of the compound were evaluated. These patients were treated with $100-150mg/m^2$ nab-paclitaxel on days 1,8. The cycle was repeated every 3 weeks. Clinical efficacy was evaluated every two cycles. Results: Of the 29 patients, two persons interrupted treatment because of adverse reactions, failed to evaluate efficacy effect. The rest of 27 patients who could be evaluated for short-term response, 10 patients (37%) achieved partial response, 2 (7.4%) remained stable disease, and 15 (55.6%) had progressivedisease. The objective response rate was 37%, and the disease control rate was 44.4%.The median time to progression was 6.6 months.The major adverse reactions includedalopecia (62.07%), neutropenia (65.5%), gastrointestinalreaction (10.3%) andsensory neuropathy(6.8%). Conclusions: The albumin-bound paclitaxel-based chemotherapy is efficacy and safety in treatment for patients with advanced esophageal cancer who failed in first-line chemotherapy.

        Oxidative stress has been postulated to contribute significantly to the accelerated accumulation of advanced glycoxidation endproducts (AGEs) in collagen, which is implicated in the process of skin aging. Effectiveness of Actinidia chinensis, commonly called gold kiwifruit, in counteracting skin aging was investigated. Firstly, primary crude 70% ethanolic extracts of whole A. chinensis, pulp, and rind were screened for their in vitro antioxidant activities and anti-glycation activity by using 1,1-diphenyl-2-picrylhydrazyl (DPPH), Ferric Reducing Antioxidant Power (FRAP) assay, and bovine serum albumin-derived glycation model. Result indicated that rind portion exhibited significantly (p<0.05) high antioxidant activity as well as high phenolic and flavonoid contents compared to those of pulp and whole A. chinensis. Thus, rind was selected for further fractionated with hexane, chloroform, ethyl acetate, and butanol. Among these solvent fractions, A. chinensis rind ethyl acetate (ACRE-E) had the greatest radical-scavenging activity and reducing power, comparable to standard antioxidant, vitamin C. Immunofluorescence staining was used to determine AGEs distribution in glycated collagen matrix. ACRE-E inhibited formation of 67% AGEs. High Performance Liquid Chromatography analysis revealed phenolic compound of ACRE-E as quercetin-3-rhamnoside. High antioxidant and anti-glycation activities of ACRE-E in glycated collagen model indicate its contribution to anti-aging process. A. chinensis rind, previously considered as a byproduct, may have potential as a low-cost raw material for cosmetic and pharmaceutical industries.

      • Efficacy and Safety of Neurokinin-1 Receptor Antagonists for Prevention of Chemotherapy-Induced Nausea and Vomiting: Systematic Review and Meta-analysis of Randomized Controlled Trials

        Objectives: Can addition of neurokinin-1 receptor antagonists (NK1-RAs) be considered as an ideal strategy for the prevention of chemotherapy-induced nausea and vomiting (CINV)? Researchers differ on this question. Materials and Methods: Electronic databases were searched for randomized control trials (RCTs) that evaluated the effectiveness and safety of NK1-RAs in preventing CINV. The primary end point was complete response (CR) in the acute, delayed, and overall phases after chemotherapy. Subgroup analyses evaluated the types of NK1-RAs, routines of administration, types of malignancies, regimens used in combination with NK1-RAs, and age of patients included in the studies. The incidences of different types of adverse events were also extracted to estimate the safety of NK1-RAs. Results: A total of 38 RCTs involving 13,923 patients were identified. The CR rate of patients receiving NK-RAs was significantly higher than patients in the control groups during overall phase (70.8% vs 56.0%, P<0.001), acute phase (85.1% vs 79.6%, P<0.001), and delayed phase (71.4% vs 58.2%, P<0.001). There were three studies including patients of children or adolescents, the CR rate was also significantly higher in the treatment group (overall phase: OR=2.807, P<0.001; acute phase: OR=2.863, P =0.012; delayed phase: OR=2.417, P<0.001). For all the other outcomes, patients in the NK1-RAs groups showed improvements compared to the control groups (incidence of nausea: 45.2% vs 45.9%, P<0.001; occurrence of vomiting: 22.6% vs 38.9%, P<0.001; usage of rescue drugs: 23.5% vs 34.1%, P<0.001). The pooled side effects from NK1-RAs did not significantly differ from previous reports and the toxicity rates in patients less than eighteen years old also did not diff between the two groups (P=0.497). However, we found that constipation and insomnia were more common in the patients of control groups, whereas diarrhea and hiccups were more frequently detected in patients receiving NK1-RAs. Conclusions: NK1-RAs improved the CR rate of CINV. They are effective for both adults and children. The use of NK1-RAs might be associated with the appearance of diarrhea and hiccups, while decreasing the possibility of constipation and insomnia.

        Rock mechanics and wellbore stability in Dongfang 1-1 Gas Field in South China Sea

        Thermal effect has great influence on wellbore stability in Dongfang 1-1 (DF 1-1) gas field, a reservoir with high-temperature and high-pressure. In order to analyze the wellbore stability in DF1-1 gas field, the variation of temperature field after drilling was analyzed. In addition, the effect of temperature changing on formation strength and the thermal expansion coefficients of formation were tested. On this basis, a wellbore stability model considering thermal effect was developed and the thermal effect on fracture pressure and collapse pressure was analyzed. One of the main challenges in this gas field is the decreasing temperature of the wellbore will reduce fracture pressure substantially, resulting in the drilling fluid leakage. If the drilling fluid density was reduced, kick or blowout may happen. Therefore, the key of safe drilling in DF1-1 gas field is to predict the fracture pressure accurately.

        Saponins from Panax japonicus ameliorate age-related renal fibrosis by inhibition of inflammation mediated by NF-κB and TGF-β1/Smad signaling and suppression of oxidative stress via activation of Nrf2-ARE signaling

        Background: The decreased renal function is known to be associated with biological aging, of which the main pathological features are chronic inflammation and renal interstitial fibrosis. In previous studies, we reported that total saponins from Panax japonicus (SPJs) can availably protect acute myocardial ischemia. We proposed that SPJs might have similar protective effects for aging-associated renal interstitial fibrosis. Thus, in the present study, we evaluated the overall effect of SPJs on renal fibrosis. Methods: Sprague-Dawley (SD) aging rats were given SPJs by gavage beginning from 18 months old, at 10 ㎎/㎏/d and 60 ㎎/㎏/d, up to 24 months old. After the experiment, changes in morphology, function and fibrosis of their kidneys were detected. The levels of serum uric acid (UA), β2-microglobulin (β2-MG) and cystatin C (Cys C) were assayed with ELISA kits. The levels of extracellular matrix (ECM), matrix metalloproteinases (MMPs), tissue inhibitors of metalloproteinases (TIMPs), inflammatory factors and changes of oxidative stress parameters were examined. Results: After SPJs treatment, SD rats showed significantly histopathological changes in kidneys accompanied by decreased renal fibrosis and increased renal function; As compared with those in 3-month group, the levels of serum UA, Cys C and β2-MG in 24-month group were significantly increased (p < 0.05). Compared with those in the 24-month group, the levels of serum UA, Cys C and β2-MG in the SPJ-H group were significantly decreased. While ECM was reduced and the levels of MMP-2 and MMP-9 were increased, the levels of TIMP-1, TIMP-2 and transforming growth factor-β1 (TGF-β1)/Smad signaling were decreased; the expression level of phosphorylated nuclear factor kappa-B (NF-κB) was down-regulated with reduced inflammatory factors; meanwhile, the expression of nuclear factor erythroid 2-related factor 2-antioxidant response element (Nrf2-ARE) signaling was aggrandized. Conclusion: These results suggest that SPJs treatment can improve age-associated renal fibrosis by inhibiting TGF-β1/Smad, NFκB signaling pathways and activating Nrf2-ARE signaling pathways and that SPJs can be a potentially valuable anti-renal fibrosis drug.

        Effect of a Novel Intracycle Motion Correction Algorithm on Dual-Energy Spectral Coronary CT Angiography: A Study with Pulsating Coronary Artery Phantom at High Heart Rates

        Objective: Using a pulsating coronary artery phantom at high heart rate settings, we investigated the efficacy of a motion correction algorithm (MCA) to improve the image quality in dual-energy spectral coronary CT angiography (CCTA). Materials and Methods: Coronary flow phantoms were scanned at heart rates of 60−100 beats/min at 10-beats/min increments, using dual-energy spectral CT mode. Virtual monochromatic images were reconstructed from 50 to 90 keV at 10-keV increments. Two blinded observers assessed image quality using a 4-point Likert Scale (1 = non-diagnostic, 4 = excellent) and the fraction of interpretable segments using MCA versus conventional algorithm (CA). Comparison of variables was performed with the Wilcoxon rank sum test and McNemar test. Results: At heart rates of 70, 80, 90, and 100 beats/min, images with MCA were rated as higher image scores compared to those with CA on monochromatic levels of 50, 60, and 70 keV (each p < 0.05). Meanwhile, at a heart rate of 90 beats/min, image interpretability was improved by MCA at a monochromatic level of 60 keV (p < 0.05) and 70 keV (p < 0.05). At a heart rate of 100 beats/min, image interpretability was improved by MCA at monochromatic levels of 50 keV (from 69.4% to 86.1%, p < 0.05), 60 keV (from 55.6% to 83.3%, p < 0.05) and 70 keV (from 33.3% to 69.3%, p < 0.05). Conclusion: Low-keV monochromatic images combined with MCA improves image quality and image interpretability in CCTAs at high heart rates.

        Analysis of the Baroreceptor and Vestibular Receptor Inputs in the Rostral Ventrolateral Medulla following Hypotension in Conscious Rats

        Input signals originating from baroreceptors and vestibular receptors are integrated in the rostralventrolateral medulla (RVLM) to maintain blood pressure during postural movement. The contributionof baroreceptors and vestibular receptors in the maintenance of blood pressure following hypotensionwere quantitatively analyzed by measuring phosphorylated extracellular regulated protein kinase(pERK) expression and glutamate release in the RVLM. The expression of pERK and glutamate releasein the RVLM were measured in conscious rats that had undergone bilateral labyrinthectomy (BL)and/or sinoaortic denervation (SAD) following hypotension induced by a sodium nitroprusside (SNP)infusion. The expression of pERK was significantly increased in the RVLM in the control groupfollowing SNP infusion, and expression peaked 10 min after SNP infusion. The number of pERKpositive neurons increased following SNP infusion in BL, SAD, and BL+SAD groups, although theincrease was smaller than seen in the control group. The SAD group showed a relatively higherreduction in pERK expression when compared with the BL group. The level of glutamate release wassignificantly increased in the RVLM in control, BL, SAD groups following SNP infusion, and thispeaked 10 min after SNP infusion. The SAD group showed a relatively higher reduction in glutamaterelease when compared with the BL group. These results suggest that the baroreceptors are morepowerful in pERK expression and glutamate release in the RVLM following hypotension than thevestibular receptors, but the vestibular receptors still have an important role in the RVLM.
