        류마티스관절염과 만성 호산구성 폐렴에 동반된 호산구성 흉막삼출

        We describe a 48-year-old man with family history of rheumatoid arthritis (RA) affected by chronic eosinophilic pneumonia (CEP) with severe peripheral eosinophilia. CEP might develop as a complication of longstanding active RA. The patient with 5 months history of seropositive RA and chronic respiratory symptoms, alveolar and blood eosinophilia, peripheral pulmonary infiltrates and pleural effusion on chest imaging. The lung may be involved as an extraarticular manifestation of RA. However, CEP is not recognized as a typical lung manifestation of RA, and the two diseases rarely coexist. The effusion was an eosinophil predominant exudates and was characterized by low pH, and glucose level and high lactic dehydrogenase. The patient responded rapidly to combination of steroids and disease modifying anti-rheumatic drugs.

        Stretchable, Skin-Attachable Electronics with Integrated Energy Storage Devices for Biosignal Monitoring

        <P><B>Conspectus</B></P><P>The demand for novel electronics that can monitor human health, for example, the physical conditions of individuals, during daily life using different techniques from those used in traditional clinic diagnostic facilities is increasing. These novel electronics include stretchable sensor devices that allow various biosignals to be directly measured on human skin without restricting routine activity. The thin, skin-like characteristics of these devices enable stable operation under various deformations, such as stretching, pressing, and rubbing, experienced while attached to skin. The mechanically engineered design of these devices also minimizes the inconvenience caused by long-term wear owing to conformal lamination on the skin. The final form of a skin-attachable device must be an integrated platform with an independent and complete system containing all components on a single, thin, lightweight, stretchable substrate. To fabricate fully integrated devices, various aspects, such as material design for deformable interconnection, fabrication of high-performance active devices, miniaturization, and dense arrangement of component devices, should be considered. In particular, a power supply system is critical and must be combined in an electromechanically stable and efficient manner with all devices, including sensors. Additionally, the biosignals obtained by these sensors should be wirelessly transmitted to external electronic devices for free daily activity.</P><P>This Account covers recent progress in developing fully integrated, stretchable, skin-attachable devices by presenting our strategies to achieve this goal. First, we introduce several integration methods used in this field to build stretchable systems with a special focus on the utilization of liquid gallium alloy. The unique characteristics and patterning process of liquid metal are summarized. Second, various skin-attachable sensors, including strain, pressure, with enhanced sensitivity and mechanical properties are discussed along with their applications for biosignal monitoring. Dual mode sensors that simultaneously detect temperature and pressure signals without interference are also introduced. Third, we emphasize supercapacitors as promising, efficient energy storage devices for power management systems in wearable devices. Supercapacitors for skin-attachable applications should have a high performance, such as high operation voltage, high energy and power densities, cyclic and air stability and water resistance. For this, strategies to select novel materials for electrode, electrolyte, and encapsulation are suggested. Several approaches to fabricate stretchable supercapacitor systems are also presented. Finally, we introduce recent examples of skin-attachable, stretchable electronics that integrate sensors, power management devices, and wireless data transfer functions on a single elastomer substrate. Conventional wireless technologies, such as near-field communications (NFC) and Bluetooth, are incorporated in miniaturized features on the devices.</P><P>To date, much research has been performed in this field, but there are still many technologies to develop. The performance of individual devices and mass fabrication techniques should be enhanced. We expect that future electronic devices with fully integrated functions will include advanced human-machine interaction capabilities and expand the overall abilities of the human body.</P> [FIG OMISSION]</BR>

        Electrochemical and Raman Spectroscopy Analysis for D- and L-Tryptophan-b-Cyclodextrin Inclusion Complexes

        An enantioselective recognition of D- and L-tryptophan (Trp)-b-cyclodextrin (CD) inclusion complex was performed using electrochemical and FT-Raman spectroscopic analysis. From the electrochemical analysis, the selectivity coefficient ($K_{DL}$) of b-CD inclusion complexes was found higher than that of the D- and L-Trp in phosphate buffered saline (PBS, pH=7.0) solution. The percentage of enantioselectivity ($I_{%{ee}}$) for peak current of D-Trp-b-CD inclusion complexes was observed higher than that of L-Trp-b-CD inclusion complexes in PBS solution. From Raman spectroscopy, chemical shift difference (D, $cm^{-1}$) for the C=C stretch, ring vibration, and ring breathing of D-Try-b-CD inclusion complex were observed higher than that of L-Trp-b-CD inclusion complex. The electrochemical and Raman spectroscopic analyses were found very useful for chiral detection of racemic amino acid in the presence of b-CD.

      • A skin-attachable, stretchable integrated system based on liquid GaInSn for wireless human motion monitoring with multi-site sensing capabilities

        <P>This paper introduces a liquid-metal integrated system that combines soft electronics materials and engineering designs with advanced near-field-communication (NFC) functionality for human motion sensing. All of the active components, that is, strain sensor, antenna and interconnections, in this device are made of liquid metal, and the device has unique gel-like characteristics and stretchability. Patterning procedures based on selective wetting properties of the reduced GaInSn enable a skin-attachable, miniaturized layout, in which the diameter of the device is less than 2 cm. Electromechanical characterization of the strain sensor and antenna reveals their behaviors under large uniaxial tensile and compressive strains, as well as more complex modes of deformation. Demonstrations of these devices involve their use in monitoring various human motions in a purely wireless fashion; examples include wrist flexion, movements of the vocal cord and finger motion. This simple platform has potential for use in human-machine interfaces for prosthetic control and other applications.</P>

        Reuterin 및 이의 구조적 특징과 생물학적 활성에 대한 고찰

        Reuterin, one of the low-molecular organic compounds, contains an aldehyde and alcohol group with three carbons. It also possesses a certain degree of complexity in chemical structure because of the presence of aldehyde group. In an aqueous solution of reuterin, monomeric, hydrate, and dimeric forms coexist in equilibrium. Reuterin has diverse biological activities, including antibacterial, anticancer, antiinflammatory, and antifungal activities. Specifically, it displays stronger antibacterial activities and lower toxicities than the existing antimicrobial agents such as glutaraldehyde. Due to the beneficial biological attributes, reuterin can be a potential candidate for novel functional compounds in the food and pharmaceutical industry. Preparation methods for reuterin include biochemical conversion and chemical synthesis. Biochemical conversion approach employs bioconversion using glycerol and Lactobacillus reuteri and chemical synthesis utilizes acrolein or 1,2,4-butanetriol as a starting material. However, despite its exceptional biological activities, the mechanism of action is obscure. Collectively, reuterin has substantial potential as a functional compound in food and pharmaceutical domain.

        뇌졸중 환자의 인지기능과 한국판 수정바델지수 평가간의 상관성 연구

        This research aims to propose the relationships between the MMSE-K, MoCA-K, LICA, and K-MBI of stroke patients older than 60 and analyze the influence between each factor to investigate the effectiveness of those measures as clinical assessment tools for stroke patients. The research recruited sixteen participants who are hospitalized stroke patients aged over 60 from Y rehabilitation hospital located in Goyang city. As the inclusion crietra, first, the participant must have been diagnosed as the stroke for over six months. Second, the participant required to be able to communicate. Third, the participant had to be older than 60. Fourth, the participant had to be free from the hemi-neglect; fifth, the participant had to understand the purpose of the research and agreed upon participating it. The MoCA-K and K-MBI showed weak correlation with r=.560, and the LICA and K-MBI showed weak correlation with r=.463. However, MMSE-K and K-MBI did not show any correlation. The significance of this research is that it proposed the basis for an active utilization and effectiveness of MMSE-K, MoCA-K, LICA, and K-MBI as the clinical assessment tool for an accurate cognitive level selection and fast comeback to the society through investigating the correlations between the measures.

      • Robot-assisted myomectomy versus abdominal myomectomy for huge myoma

        Objective: To assess the feasibility and efficacy of robot-assisted myomectomy for huge myoma by comparing to abdominal myomectomy. Methods: This is a retrospective study for robot-assisted myomectomy(RAM) and abdominal myomectomy(AM) performed from April 2019 to June 2020 in a single tertiary hospital. Huge uterine myoma was defined as the longest diameter of dominant myoma equal to or larger than 10cm or total myomas weight over 250g. Results: 132 patients in RAM and 151 patients in AM were administered. There is no difference in the mean age(37.8 vs 39.1) and the mean BMI(23.0 vs 23.5). The operative time(160.8 vs 108.6) is longer in RAM compared with AM. The estimated blood loss(362.1 vs 297.2) and mean decrease in Hb(3.4 vs. 3.2) is comparable. The length of hospital stay(2.7 vs 4.1) is shoter, and the number of complications(25.0% vs 55.0%) is lower in RAM compared with AM. There is no intraoperative complications in both. Conclusion: Robot-assisted myomectomy is feasible and safe option for managing huge uterine myomas.
