        Modified Procedures for ALPPS Based on a Risk-Reduced Strategy: Paralleled Clinical Evaluation at Multiple Institutions

        Purpose: We compared the clinical outcomes of modified procedures for associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) based on a risk-reduced strategy with those of classic ALPPS procedures in treating large liver carcinoma. Materials and Methods: Short-term outcomes, increases in future liver remnant (FLR) and functional FLR (FFLR), and overall survival (OS) were compared between 45 consecutive patients treated with modified ALPPS procedures and 34 patients treated with classic ALPPS procedures. Results: Clinical outcomes after the 1st-stage operation markedly improved with the modified procedures. Although the proportions of liver cirrhosis and hepatocellular carcinoma were higher in the modified group, the mortality and incidence of severe complications did not increase. FLR and FFLR hypertrophy at 1 week after the 1st-stage operation were similar in both groups; however, kinetic growth rates in the modified group were lower. OS rates were similar. Conclusion: Modified ALPPS procedures could be safely applied to provide long-term survival for patients with liver cirrhosis without sufficient FLR.

        중국의 기술무역장벽이 한국의 수출성과에 미치는 영향에 관한 연구

        본 연구에서는 중국의 기술무역장벽이 한국의 수출성과에 미치는 영향을 분석하기 위해 선행연구의 분석을 통해 경제규모, 무역상대국의 거리, 인구수, 관세율, TBT, SPS통보건수, 무역 개방도, 보건분야지출, 이산화탄소 배출량에 관련된 가설을 세워 실증 분석 하고자 하였다. 각각의 데이터는 한국무역협회, World Bank, GlobeFeed, WITS(World Integrated Trade Solution), WTO/TBT 및 WTO/SPS, World Development Indicator 등에서 자료를 획득하였다. 실증 분석결과 경제규모, 인구규모, 관세율은 통계적으로 유의한 (+)의 상관관계가 있는 것으로 나타났다. 무역개방도가 높을수록 무역수출액도 증가함을 알 수 있으며, 보건 분야로의 지출이 증가할수록 대중국 수출액은 감소하는 경향이 나타나고 있음을 나타내고 있고, 이산화탄소 배출량이 증가할수록 대중국 수출이 활발하게 나타나는 양의 상관관계가 나타났다. 또한, 무역개방도, 보건수준, 환경수준은 통보건수()에 대한 모형별 분석결과는 유의하지 않는 것으로 나타났다. 따라서 국내에서는 TBT에 대한 관심과 이해의 제고가 필요하며, 관련 대응방안을 체계적으로 준비해야 한다. Purpose : The purpose of this study is to analyze the effects of variables such as GDP, population, tariff rate, trade openness, health level (expenditure in health sector), and environmental level (CO2 emission) on trade in both countries. Research design, data, methodology : In this paper, each data was obtained from the Korea International Trade Association, World Bank, GlobeFeed, WITS (World Integrated Trade Solution), WTO/TBT, WTO/SPS, and World Development Indicator. Results : As a result of empirical analysis, it was found that there was a statistically significant (+) correlation between economic size, population size, and tariff rate. It was found that the higher the degree of trade openness, the higher the amount of trade exports. This indicates that as expenditures in the health sector increase, exports to China tend to decrease. As the carbon dioxide emission increased, there was a positive correlation, which showed an active export to China. In terms of trade openness, health level, and environmental level, the analysis results of each model for the number of notifications were not significant. Conclusions : In Korea, it is necessary to increase interest and understanding of the technical trade barrier (TBT), and to systematically prepare related countermeasures.

        한국 주요 수출 상품의 대 호주 및 뉴질랜드에 대한 불변시장점유율(CMS) 분석

        본 연구에서는 한국의 RCEP 발효 후, 한국의 주요 수출 상품 20개 품목에 대해 한·중·일의 대 호주 및 뉴질랜드에 대한 국제경쟁력을 분석하기 위해 품목별 시장점유율, 불변시장점유율을 분석하여 다음과 같은 시사점을 얻었다. 분석 결과, 한국의 대 호주 시장에서 시장점유율이 강세를 나타내고 있으며, 한국의 대 뉴질랜드 시장의 HS2710(석유와 역청유)품목과 HS7210(철이나 비합금강의 평판압연제품)의 시장점유율은 상대적으로 높은 추세로 나타났다. 그리고 한·중·일의 대 호주 불변시장점유율 변화를 살펴보면, 전반적으로 한국과 일본은 점차적으로 낮아지는 데 반해 중국은 몇몇 품목에서 상당히 높게 나타나고 있다. 또한 한국의 대 뉴질랜드 시장의 수출증감액 분해 결과를 보면, 대부분 상품의 수출 감소하는 이유는 모든 경쟁력 요인과 상품공급구조변동 요인 분석 결과는 마이너스(-)를 기록하여 대 뉴질랜드 수출 감소 요인으로 작용한 것을 확인하였다. Purpose: The purpose of this study is to compare and analyze the international competitiveness of Korea's 20 major export items to the Australian and New Zealand markets with those of China and Japan. Research design, data, and methodology: In this study, trade statistics are extracted from the UN Comtrade database and are analyzed using Constant Market Share (CMS) and Market Share models for each item. Results: Korea's share of the Australian market is strong, and Korea's share of HS2710 and HS7210 in the New Zealand market is relatively high, and appears to be trending up. Looking at the overall changes in CMS for Korea, China, and Japan in Australia, Korea and Japan show a gradual decrease, while China shows a significant increase in some items. Conclusions: Korea needs a strategy to strengthen competitiveness for items in which the market share of rival countries is increasing, and should use a business model that can utilize a RCEP.

        EBV-miR-BHRF1-1 Targets p53 Gene: Potential Role in Epstein-Barr Virus Associated Chronic Lymphocytic Leukemia

        Purpose The purpose of this study was to investigate the prognostic impact of Epstein-Barr virus (EBV)–microRNA (miRNA, miR)-BHRF1-1 with chronic lymphocytic leukemia (CLL) as well as role of EBV-miR-BHRF1-1 in p53 gene. Materials and Methods Quantitative reverse transcription–polymerase chain reaction and western blotting were used to quantify EBV-miR-BHRF1-1 and p53 expression in cultured CLL. Results p53 aberration was associated with the higher expression level of EBV-miR-BHRF1-1 (p < 0.001) which was also an independent prognostic marker for overall survival (p=0.028; hazard ratio, 5.335; 95% confidence interval, 1.193 to 23.846) in 97 newly-diagnosed CLL patients after adjusted with International Prognostic Index for patients with CLL. We identified EBV-miR-BHRF1-1 as a viral miRNA regulator of p53. EBV-miR-BHRF1-1 repressed luciferase reporter activity by specific interaction with the seed region within the p53 3- untranslated region. Discordance of p53 messenger RNA and protein expression was associated with high EBV-miR-BHRF1-1 levels in CLL patients and cell lines. EBV-miR-BHRF1- 1 inhibition upregulated p53 protein expression, induced cell cycle arrest and apoptosis and decreased cell proliferation in cell lines. EBV-miR-BHRF1-1 mimics downregulated p53 protein expression, decreased cell cycle arrest and apoptosis, and induced cell proliferation in cell lines. Conclusion This study supported the role of EBV-miR-BHRF1-1 in p53 regulation in vitro. Our results support the potential of EBV-miR-BHRF1-1 as a therapeutic target in EBV-associated CLL with p53 gene aberration.

        한.중 FTA 발효 이후 무역량 변화와 주요 품목의 경쟁력에 관한 연구

        본 연구에서는 한중 FTA가 체결 발효된 2015년 시점을 기준으로 한국의 주요 대중 수출 품목의 경쟁력이 어떻게 변화하였는지를 분석하는 것이다. 이를 위해 한국의 대중 20대 수출 품목을 HS 4단위로 분류하여 2011년부터 2020년까지 RCA와 SRCA 및 TSI를 산출하여 경쟁력을 분석하였다. 분석한 결과 한국 대 중국의 품목별 SRCA지수를 보면, 역시 2015년에는 HS8471 품목 하나만 음수(-)로 나타났으나, 2020년에는 4개 품목으로 나타나 한중 FTA이후 경쟁력이 약화된 것을 알 수 있다. 그리고 2015년 대비 경쟁력 약화 부분에서는 - 1 < TSI < 0, 사이에 위치한 품목은 7개 품목으로 나타났으며, 특히 한․중 FTA이후 수출특화에서 수입특화로 전환된 품목은 5개 품목으로 나타났다. 결론적으로 FTA발효 이후 경쟁력이 급격하게 낮아진 산업에 대해서는 정책 당국의 지원정책과 기업의 해외 거래선의 다변화 등의 내부적인 대응 전략이 필요하다고 본다. This study analyzes how the competitiveness of Korea s major export items to China has changed as of 2015, when the Korea-China FTA took effect. For this study, Korea s top 20 export items to China were classified into 4 HS units, and RCA, SRCA, and TSI were calculated from 2011 to 2020 to analyze competitiveness. As a result of the analysis, looking at the SRCA index for each item of Korea versus China, only one HS8471 item was negative (-) in 2015, but there were 4 items in 2020, indicating that competitiveness has weakened after the Korea-China FTA. And after the Korea-China FTA, 5 items were converted from export-specialized to import-specialized. In conclusion, for industries whose competitiveness has rapidly decreased after the entry into force of the FTA, it is necessary to develop a response strategy such as support policies from the policy authorities and diversification of overseas business partners.

      • Effects of Secondary Left-sided Portal Hypertension on the Radical Operation Rate and Prognosis in Patients with Pancreatic Cancer

        Objective: To investigate the effects of secondary left-sided portal hypertension (LSPH) on the radical operation rate of patients with pancreatic cancer and systemically evaluate the prognosis of patients with LSPH secondary to pancreatic cancer after radical surgery. Materials and Methods: The data of patients with pancreatic cancer who underwent laparotomy over a 15-year period in Department of Hepatobiliary Surgery of Chinese PLA Air Force General Hospital from Jan. 1, 1997, to Jun. 30, 2012 was retrospectively reviewed. Results: A total of 362 patients with pancreatic cancer after laparotomy were selected, including 73 with LSPH and 289 without LSPH. Thirty-five patients with LSPH (47.9%) and 147 without non-LSPH (50.9%) respectively underwent radical operations. No significant difference was found between these two groups regarding the total resection rate and stratified radical resection rate according to different pathological types and cancer locations. The mean and median survival time of patients after radical operation in LSPH group were $13.9{\pm}1.3$ months and 14.8 months, respectively, while those in non-LSPH group were $22.6{\pm}1.4$ months and 18.4 months, respectively(P<0.05). Conclusions: Radical operations for pancreatic cancer and secondary LSPH are safe and effective. Because high-grade malignancy and poor prognosis are closely associated, the decision for radical surgery should be made more meticulously for the patients with pancreatic cancer.

        DISCOVERY OF AN X-RAY-EMITTING CONTACT BINARY SYSTEM 2MASS J11201034−2201340

        <P>We report the detection of orbital modulation, a model solution, and the X-ray properties of a newly discovered contact binary, Two Micron All Sky Survey (2MASS) J11201034-2201340. We serendipitously found this X-ray point source outside the error ellipse when searching for possible X-ray counterparts of 7-ray millisecond pulsars among the unidentified objects detected by the Fermi Gamma-ray Space Telescope. The optical counterpart of the X-ray source (unrelated to the 7-ray source) was then identified using archival databases. The long-term Catalina Real-Time Transient Survey detected a precise signal with a period of P = 0.28876208 (56) days. A follow-up observation made by the Super Light Telescope of Lulin Observatory revealed the binary nature of the object. Utilizing archived photometric data of multi-band surveys, we construct the spectral energy distribution (SED), which is well fit by a K2V spectral template. The fitting result of the orbital profile using the Wilson Devinney code suggests that 2MASS J11201034-2201340 is a short-period A-type contact binary and the more massive component has a cool spot. The X-ray emission was first noted in observations made by Swift, and then further confirmed and characterized by an XMM-Newton observation. The X-ray spectrum can be described by a power law or thermal Bremsstrahlung. Unfortunately, we could not observe significant X-ray orbital modulation. Finally, according to the SED, this system is estimated to be 690 pc from Earth with a calculated X-ray intensity of (0.7 - 1.5) x 10(30) erg s(-1), which is in the expected range of an X-ray emitting contact binary.</P>

        한국 전기자동차 산업의 국제경쟁력에 관한 연구

        In this study, to analyze the competitiveness of electric vehicles(HS 6 units), the RCA, SRCA and TSI for major countries including Korea were calculated using the UN Comtrade DB, and the export market share (MS) was examined. In the electric vehicle market, the US, Germany, Korea, and France have an export market share of over 60%, and the RCA, SRCA and TSI indexes are also good. The electric vehicle market is growing rapidly due to various support policies from around the world, and competition is intensifying. Although Korea’s electric vehicles seem to be increasingly competitive in the global market, the government and related industries should work together to find ways to strengthen competitiveness. To this end, the industry will need to strengthen its competitiveness by expanding electric vehicle-related parts, especially battery production facilities, developing next-generation batteries, and focusing on overseas marketing. In addition, the government will need to promote tax support, expansion of incentives, preemptive infrastructure construction, and regulation and system improvement.
