Study Design: Experimental animal study. Purpose: We aimed to determine the optimal dose of a single direct injection of the tumor necrosis factor (TNF)-α inhibitor, etanercept, by using the rat model of degenerative intervertebral disc from injury. Overview of Literature: The pain-related peptide expression was suppressed in the etanercept (100 μg and 1,000 μg)-administered groups in a dose-dependent manner. Methods: The neurotracer FluoroGold (FG) was applied to the surfaces of L4/5 discs to label their innervating dorsal root ganglion (DRG) neurons (n=50). Ten rats were included in the nonpunctured disc sham surgery control group, whereas the other 40 were included in the experimental group in which intervertebral discs were punctured with a 23-gauge needle. Saline or etanercept (10 μg, 100 μg, or 1,000 μg) was injected into the punctured discs (n=10 for each treatment). After 14 days of surgery, DRGs from L1 to L6 were harvested, sectioned, and immunostained for calcitonin gene-related peptide (CGRP). The proportion of FG-labeled CGRPimmunoreactive DRG neurons was evaluated in all the groups. Results: There were no significant differences between the puncture+saline group and the puncture+10-μg etanercept group (p >0.05). However, a significant decrease in the percentage of FG and CGRP double-positive cells in FG-positive cells was observed in the etanercept (100 μg and 1,000 μg)-administered groups in a dose-dependent manner (p <0.05). Conclusions: When a low dose of the TNF-α inhibitor (10 μg of etanercept) was directly administered to the rat intervertebral disc in the rat model of degenerative intervertebral disc from injury, no suppressive effect on the pain-related peptide expression was observed. However, when a higher dose of etanercept (100 μg and 1,000 μg) was administered, the pain-related peptide expression was suppressed in a dose-dependent manner.

Study Design: Retrospective study. Purpose: To determine whether low-dose tramadol plus non-steroidal anti-inflammatory drug combination therapy could prevent the transition of acute low back pain to chronic low back pain. Overview of Literature: Inadequately treated early low back pain transitions to chronic low back pain occur in approximately 30% of affected individuals. The administration of non-steroidal anti-inflammatory drugs is effective for treatment of low back pain in the early stages. However, the treatment of low back pain that is resistant to non-steroidal anti-inflammatory drugs is challenging. Methods: Patients who presented with acute low back pain at our hospital were considered for inclusion in this study. After the diagnosis of acute low back pain, non-steroidal anti-inflammatory drug administration was started. Forty patients with a visual analog scale score of >5 for low back pain 1 month after treatment were finally enrolled. The first 20 patients were included in a non-steroidal anti-inflammatory drug group, and they continued non-steroidal anti-inflammatory drug therapy for 1 month. The next 20 patients were included in a combination group, and they received low-dose tramadol plus non-steroidal anti-inflammatory drug combination therapy for 1 month. The incidence of adverse events and the improvement in the visual analog scale score at 2 months after the start of treatment were analyzed. Results: No adverse events were observed in the non-steroidal anti-inflammatory drug group. In the combination group, administration was discontinued in 2 patients (10%) due to adverse events immediately following the start of tramadol administration. At 2 months, the improvement in the visual analog scale score was greater in the combination group than in the non-steroidal anti-inflammatory drug group (p <0.001). Conclusions: Low-dose tramadol plus non-steroidal anti-inflammatory drug combination therapy might decrease the incidence of adverse events and prevent the transition of acute low back pain to chronic low back pain.
Evaluation of landfilled waste stabilization by biological indicators

Since the landfill site has a large area, it is desirable that it can be reused as land resources promptly after the landfill is completed. However, these landfills which meet the closure standard with leachate quality, gas emission and etc., are often not reused as land resources in Japan. Because, the waste is not completely stabilized even if it meets the closure standard, and the possible risk of hazardous gas generation such as methane and water pollution by leachate is brought by reusing the land. These problems are caused by the activation of biodegradation of waste accompanying with the modification of the landfill such as excavation. Therefore, it is necessary to establish an index to predict the risk associated with change of landfill condition. The authors focused on biological indicator adopted as the standards for waste landfilling in the Europe etc. The landfilled wastes with different stabilization level such as the leachate quality, gas emission and ignition loss are prepared and evaluated biodegradable organic matter content by the BMP test and RA test. As a result, the percentage of biodegradable organic matter obtained by the BMP test is proportional to the degree of stabilization determined from the leachate and the gas quality, 0% for highly stabilized waste as H6-a and H6-b, and 97% for the waste with lower stabilization as H6-h(Figure 1). On the other hand, the ratios of biodegradable organic matter obtained by the RA test were slightly higher (about 10%) in case of wastes with a low level of stabilization than those of high degree of stabilization. However, the relationship between the percentage of biodegradable organic matter and the stabilization of waste shows less correlation in RA test than that in BMP test. The relationship between the percentage of biodegradable organic matter and the ignition loss also shows the high correlation coefficient as 0.52 in RA and 0.77 in BMP, although the BMP showed the stronger correlations than the RA (Figure 2). It was found that both BMP test and RA test can be applied as evaluation index of the pollutant and the gas generation risk due to the environmental change after landfill closure. However, the applicable evaluation methods are depending on the landfill conditions such as aerobic or anaerobic state. It can be proposed that the RA test is effective for the evaluation of the waste landfilled under an aerobic condition, and the BMP test for an anaerobic condition.

An infrared range finder using a self-mixing laser diode (SM-LD), which has been proposed and developed by the authors, can measure not only a range of a moving target but its velocity simultaneously. In this paper, described is that the precise mode-hop pulse train can be obtained by employing a new signal processing circuit even when the backscattered light returning into the SM-LD is much more weaker. As a result, the distance to a tilted square sheet made from aluminium or white paper, which is placed 10 ㎝ through 60 ㎝ from the SM-LD, is measured with accuracy of a few percent even when the tilting angle is less than 75 degrees or 85 degrees, respectively.<br/> And in this paper, described is the range-image recognition of a plane object under the condition of standstill. The output laser beam is scanned by scanning two plane mirrors equipped with each stepping motor. And we succeeded in the acquisition of the range-image of a plane object in a few tens of seconds. Furthermore, described is a feasibility study about the range-image recognition of a slowly moving plane object.
Mental workload of in-vehicle device operation by eye gazing for head-up display

When operating for information retrieval by in-vehicle device, we need to gaze at associative objects such as displays and in-vehicle control panels. This activity by gaze-based information retrieval may induce the risk of traffic accidents. In this study, a novel technique for reducing such risk using head-up display is introduced. The use of control panels shown in the head-up display can maintain the amount of attention toward traffic condition ahead of the vehicle. On the other hand, mental workload generated by using this novel technique has not been evaluated thus far. Therefore, this paper presents the results of evaluation for the mental workload generated by the proposed technique by comparing with the technique used for conventional manual operation, using NASA-TLX during performing peripheral detection task.

Research into first episode psychosis (FEP) is a growing field in psychiatry. In this paper, the authors provide an overview of neurocognitive findings and therapeutic approaches indicated for patients with FEP. Focus will be placed on evidence from neuropsychological assessment and brain imaging of electrophysiological activities, as well as the ability of some psychotropic drugs, such as second generation antipsychotics, to improve functional outcome. Efforts to identify biological signals characteristic of FEP are expected to facilitate early intervention into schizophrenia and related disorders.
