      • EFFECT OF TOP END CONDITION OF FUEL BED CONTAINER ON DOWNWARD SMOLDER SPREAD

        An experimental study was performed of natural-convection downward smolder spread across a sawdust bed peripherally enclosed with an insulating container, to examine the effect of the open- ing condition at the top end on downward smolder spread. Experiments were conducted by using relatively coarse sawdust and 25-cm-long cylindrical container The variations of temperature profiles along the bed axis with time were determined far different opening conditions and were com-pared with those in smolder spread from open top to open bottom. It was shown that the smolder zone initiated from open top toward closed bottom penetrates the bed with keeping high peak temperature like the case of open top to open bottom spread, although mean spread rate is smaller. This indicates that the downward smolder zone can be sustained stably if sufficient air or oxygen Is supplied from the back of it by natural convection even if upward draft entering from the bottom of the bed is absent. When the top end was partially closed by mounting a cover after stable smolder spread had begun from open top toward open bottom, the temperature at the peak decreased more than 200 K and the smolder zone became to spread with thickening residue. In this case, the shape of temperature profiles continuously changed or decayed until end-effect at the open bottom end enhanced the reaction. The temperature at the shrunk peak, free from the end-effect, was almost identical with the temperature at the exothermic oxidative-degradation zone in smolder spread from open top to open bottom. from these results, it can be inferred for natural-convection downward smolder spread that the oxidation reaction of the char is very sensitive to the oxygen supply by natural convection in the space above the smolder zone, and that the top end opening condition strongly alters the completeness of reactions, structure, and behavior of the smolder zone.

        Relative strength of phenotypic selection on the height and number of flowering-stalks in the rosette annual Cardamine hirsuta (Brassicaceae)

        We estimated phenotypic selection on the height and number of flowering-stalks in a rosette annual Cardamine hirsuta by applying path analysis to the data collected at three natural populations located in central Japan. The path from rosette size was positively connected with the fruit production through the both height and number of flowering-stalks. In the all three populations, the paths from the number of stalks were more strongly connected with the fruit production than from the height of stalks. The paths from the rosette size showed similar magnitude with the number of stalks and the height of stalks. The direct path from rosette size to the fruit production was detected only at one site. These results suggest stronger phenotypic selection on the rosette size through the number of stalks than the height of stalks. The lateral branching rather than increment of individual inflorescence size is the major response to control the fruit production for C. hirsuta growing in a natural habitat.

        Inhibiting Vascular Endothelial Growth Factor in Injured Intervertebral Discs Attenuates Pain-Related Neuropeptide Expression in Dorsal Root Ganglia in Rats

        Study Design: An experimental animal study. Purpose: To evaluate effects of anti-vascular endothelial growth factor (VEGF) on the content and distribution of the calcitonin generelated peptide (CGRP) in the dorsal ganglia in a rat model. Overview of Literature: Increased expression of VEGF in degenerative disc disease increases the levels of inflammatory cytokines and nerve ingrowth into the damaged discs. In animal models, increased levels of VEGF can persist for up to 2 weeks after an injury. Methods: Through abdominal surgery, the dorsal root ganglia (DRG) innervating L5/L6 intervertebral disc were labeled (FluoroGold neurotracer) in 24, 8-week old Sprague Dawley rats. The rats were randomly allocated to three groups of eight rats each. The anti- VEGF group underwent L5/6 intervertebral disc puncture using a 26-gauge needle, intradiscal injection of 33.3 μg of the pegaptanib sodium, a VEGF165 aptamer. The control-puncture group underwent disc puncture and intradiscal injection of 10 μL saline solution, and the sham-surgery group underwent labeling but no disc puncture. Two rats in each group were sacrificed on postoperative days 1, 7, 14, and 28 after surgery. L1–L6 DRGs were harvested, sectioned, and immunostained to detect the content and distribution of CGRP. Results: Compared with the control, the percentage of CGRP-positive cells was lower in the anti-VEGF group (p <0.05; 40.6% and 58.1% on postoperative day 1, 44.3% and 55.4% on day 7, and 42.4% and 59.3% on day 14). The percentage was higher in the control group compared with that of the sham group (p <0.05; sham group, 34.1%, 40.7%, and 33.7% on postoperative days 1, 7, and 14, respectively). Conclusions: Decreasing CGRP-positive cells using anti-VEGF therapy provides fundamental evidence for a possible therapeutic role of anti-VEGF in patients with discogenic lower back pain.

        [原文] 境界の不在、枠の存在 - 日本美術について私が知っている二、三の事柄

        First I claim that the attraction of Japanese art is often to be found in the coexistence of the representational and the abstract in one continuous space. The examples are Deep bowl (Kokubunji, Tokyo), Standing Yakushi Nyorai (Jingoji, Kyoto), Landscapes with Sun and Moon (Kongōji, Osaka) and Red and White Plum Trees (MOA Museum of Art, Shizuoka) by Ogata Kōrin. Before the 19th century, European and Chinese art seems to have disliked the coexistence or juxtaposition of the representational and the abstract. Secondly I argue that Japanese art is very conscious of the effect of cropping an object with the pictorial frame. The motifs breaking the frames have their greatest effect in Picture Scroll of the Legends of Shigisan (Chōgosonshiji, Nara), Wind and Thunder Gods (Kenninji, Kyoto) by Tawaraya Sōtatsu and ‘Gun’ Prostitute (Teppō) by Kitagawa Utamaro. To sum up, there is no boundary inside of the artwork, and at the same time, it is framed on the outside. Artworks possessing these characteristics generally intend to beautifully arrange planes or solids, and never try to thoroughly represent real nature or human beings. Japanese art possesses a sense of reality that captures the hearts even of us modern people, but that sense of reality is always limited to some parts. Abstraction, a very different mode of depiction, coexists with the representational mode in the same pictorial space. Japanese art concentrates its attention on the inner space within a frame, and finishes it off beautifully. Its ambiguity and refinement probably are a result of its derivative character, which is alien to European or Chinese art. But seeing these characteristics from another point of view and appreciating them positively, we might conclude that the fascination of Japanese art, as well as its power in creating contemporary visual culture, lies in this flexibility and in this high consciousness of arrangement, which permit the coexistence of contradictions.

        Freeze-Dried Human Platelet-Rich Plasma Retains Activation and Growth Factor Expression after an Eight-Week Preservation Period

        Study Design: Controlled laboratory study. Purpose: This study aimed to evaluate the efficacy of platelet-rich plasma (PRP) stored at room temperature (RT), frozen, or after freeze-drying. Overview of Literature: PRP enriches tissue repair and regeneration, and is a novel treatment option for musculoskeletal pathologies. However, whether biological activity is preserved during PRP storage remains uncertain. Methods: PRP was prepared from blood of 12 healthy human volunteers (200 mL/person) and stored using three methods: PRP was stored at RT with shaking, PRP was frozen and stored at –80°C, or PRP was freeze-dried and stored at RT. Platelet counts and growth factor content were examined immediately after preparation, as well as 2, 4, and 8 weeks after storage. Platelet activation rate was quantified by flow cytometry. Results: Platelet counts were impossible to determine in many RT samples after 2 weeks, but they remained at constant levels in frozen and freeze-dried samples, even after 8 weeks of storage. Flow cytometry showed approximately 80% activation of the platelets regardless of storage conditions. Almost no growth factors were detected in the RT samples after 8 weeks, while low but significant expression was detected in the frozen and freeze-dried PRP. Over time, the mean relative concentrations of various growth factors decreased significantly or disappeared in the RT group. In the frozen group, levels were maintained for 4 weeks, but decreased significantly by 8 weeks (p <0.05). The freeze-dried group maintained baseline levels of growth factors for the entire 8-week duration. Conclusions: Freeze-drying enables PRP storage while maintaining bioactivity and efficacy for extended periods.

        Microflora Profiling of Infected Root Canal before and after Treatment Using Culture-Independent Methods

        This study aimed to profile the microflora in infected root canals before and after root canal treatment using cultureindependent methods. Six infected root canals in singlerooted teeth with periapical lesions from five subjects were included. Quantification of total bacteria was performed by real-time PCR with primers targeting 16S rRNA genes. PCR products with universal 16S rRNA gene primers were cloned and partially sequenced, and bacterial identification at the species level was performed by comparative analysis with the GenBank database. The concentration of extracted DNA before treatment was higher than that after root canal treatment, although the difference was not statistically significant. Sequence analysis revealed that oral bacteria such as Fusobacterium, Streptococcus, Olsenella, and Pseudoramibacter detected in cases before root canal treatment disappeared after treatment. These results suggest that the root canal microflora are distinct before and after root canal treatment, and that treatment changes the microflora in both quantity and quality.

        Heartburn, Functional Dyspepsia, Anxiety/Depression, and Sleep Disturbances Are Associated With Clinically Significant Belching

        Background/Aims Belching is the act of expelling gas from the stomach or esophagus noisily through the oral cavity. Although it is a physiological phenomenon, belching may also be a symptom of upper gastrointestinal diseases such as reflux esophagitis and functional dyspepsia (FD). A detailed epidemiology of belching has not yet been reported. The aim of this study is to examine the prevalence and clinical characteristics of clinically significant belching (CSB) in adults. Methods We analyzed 1998 subjects who visited the hospital for annual health checkups. Belching was evaluated by a simple question “Do you burp a lot?” and scored as 0 (never), 1 (occasionally), 2 (sometimes), 3 (often), or 4 (always). Subjects with CSB were defined as having scores ≥ 3. We also collected the clinical parameters, endoscopic findings, and data according to the Athens Insomnia Scale, Rome IV questionnaire, and Hospital Anxiety and Depression Scale (HADS). Results Of the 1998 subjects, 121 (6.1%) had CSB. Subjects with CSB had FD more commonly than reflux esophagitis, but presence of heartburn was high (10.7% vs 3.1%). In addition, the HADS and Athens Insomnia Scale scores in subjects with CSB were significantly higher than those in subjects without CSB. Presence of heartburn (OR, 2.07; 95% CI, 1.05-4.09), presence of FD (OR, 2.12; 95% CI, 1.33-3.36), anxiety/depression (OR, 2.29; 95% CI 1.51-3.45), and sleep disturbances (OR, 1.73; 95% CI, 1.14-2.61) were significantly associated with CSB. Conclusion The detailed epidemiology of belching in the general adult population was clarified. (J Neurogastroenterol Motil 2021;27:581-587)

        Interventional management for postoperative arterial bleeding in gastrointestinal surgery

        Postoperative arterial bleeding after gastrointestinal surgery is a potentially fatal complication. Transcatheter arterial embolization is considered the first-line treatment because of efficacy and less invasiveness despite the risk of organ infarction. With the recent advances in endovascular devices, stent-graft placement, which can preserve arterial flow, has been an alternative treatment option in patients with extrahepatic artery hemorrhage. Moreover, clinical outcomes of stent-graft placement for pseudoaneurysms in relative long term have been reported recently. Herein, we review the techniques and clinical outcomes for interventional management for postoperative arterial bleeding.
