17Beta-Estradiol Regulates NUCB2/ Nesfatin-1 Expression in Mouse Oviduct

NUCB2/nesfatin-1 known to regulate appetite and energy homeostasis is expressed not only in the hypothalamus, but also in various organs and tissues. Our previous reports also demonstrated that NUCB2/nesfatin-1 was expressed in the reproductive organs, including the ovaries, uterus, and testes of mice. However, it is yet known whether NUCB2/nesfatin-1 is expressed in the oviduct and how its expression is regulated. Therefore, we investigated the expression of NUCB2/nesfatin-1 in the oviduct and its expression is regulated by gonadotropin. Immunohistochemical staining results showed that nesfatin-1 protein was localized in epithelial cells of the oviduct. As a result of quantitative real-time PCR (qRT-PCR) and Western blot, NUCB2/nesfatin-1 was detected strongly in the oviducts. During the estrus cycle, NUCB2/nesfatin-1 expression in the oviducts was markedly higher in the proestrus stage than in other estrus stages. In order to elucidate whether the expression of NUCB2 mRNA is controlled by the gonadotropins, we injected PMSG and hCG and measured NUCB2 mRNA level in the oviduct after injection. Its level was increased in the oviduct after PMSG injection, but no significant change after hCG injection. In addition, NUCB2 mRNA levels were markedly reduced after ovariectomy, while recovered after 17β-estradiol (E2) injection, but not by progesterone (P4). This study demonstrated that NUCB2/nesfatin-1 is highly expressed in the oviduct of mouse and its expression is regulated by E2 secreted by the ovaries. These results suggest that NUCB2/nesfatin-1 expressed by the oviduct may affect the function of the oviduct regulated by the ovaries.
미국 노인 직업훈련 프로그램 및 노인 일자리 사업의 시사점

이 글에서는 미국의 고령 근로자와 실업자의 현황 및 노인 직업훈련 프로그램과 노인 일자리 사업을 개괄했다. 미국의 노인 직업훈련 프로그램과 노인 일자리 사업은 연방정부의 주도하에 각 주에 세워진 거점 센터를 통하여 운영되고 있으며, 고령 근로자의 재취업에 크게 도움이 되고 있다. 코로나19 팬데믹으로 인한 실업 위험이 고령 근로자에게 집중되어 있었다는 사실을 고려해 볼 때, 노인 직업훈련 프로그램과 노인 일자리 사업의 효과성은 한국의 유사 정책 및 프로그램에 중요한 시사점을 제공한다. 고령 근로자를 전담하는 정부 산하 프로그램 및 전문 인력의 개발, 프로그램 효과를 모니터링할 수 있는 국가 차원의 평가 시스템, 더욱 폭넓은 종류의 직업훈련 프로그램과 일자리 프로그램, 지속적인 인식 개선 노력 등을 통해 더 생산적이고 건강한 고령화를 도모하기 위한 국가와 지역사회의 노력이 함께 지속되어야 할 것이다.
Reliability Evaluation of Head Position Assessment Methods

Reliable assessment of forward head posture (FHP) has been an important issue for the efficient rehabilitation of head/neck disorders and pain symptoms, but no standardized evaluation methods have yet been developed. The purpose of this study was to determine a reliable head position assessment method by evaluating the test-retest reliability of various static and dynamic head position variables and assessment conditions. Twenty healthy subjects were recruited and their head position relative to the upper back were measured by three different dimensions (horizontal gap between the tragus and the 7th cervical vertebra, horizontal gap between the tragus and the acromion processes, angle between vertical and a line of tragus-7th cervical vertebra) three times per dimension in 5 different posture conditions. Study results show that the head position variables varied significantly between posture conditions, indicating the importance of a standardized test method. Test-retest reliability evaluation results show that the tragus-C7 horizontal gap dimension was more reliable than other head position variables specifically when it was measured while standing or walking. Based on the findings of this study, it can be concluded that the severity of FHP can be more reliably assessed if the position of the head is measured when the patient is in standing or comfortable walking.

Aims: In surgery for perihilar cholangiocarcinoma (PHCC), it is still controversial as to whether additional resection of the bile duct is needed on biliary intraepithelial neoplasia-3 (BilIN-3) margin. Methods: Patients who underwent surgery for PHCC with curative intent between 2001 and 2015 were stratified by resection margin, and were analyzed comparing the clinical outcomes. Results: Of the 306 study participants, 217 patients had negative margins (R0), 18 patients had BilIN-3, and 71 patients had positive margins (R1). The median overall survival of each group was 36.0 months in R0 group, 41.0 months in BilIN-3 group, and 25.0 months in R1 group while overall survival rates at 5 years were 34.5% in the R0 group, 44.4% in the BilIN-3 group, and 21.0% in R1 group. The median disease-free survival was 15.0 months in R0 group, 16.5 months in BilIN-3 group, and 12.0 months in R1 group. In the BilIN-3 group who had recurrence, 8 out of 9 patients had locoregional recurrence. Conclusions: Even if the BilIN-3 group has the potential to transform into malignancy, the survival and recurrence outcomes were comparable with R0 group, which suggests no additional resection is needed when maximal bile duct margin is BilIN-3 during PHCC surgery.
