The purpose of this study were to understand the resident's attitudes toward Ulleung Island and to identify the residents' perception of recreation and the influence of socioeconomic characteristics on their response. In summer of 1999, 222 questioners were obtained from a convenience sample of households within Ulleung Island. The residents perceived both positive and negative environ-recreation and economic impacts, most of the residents supported additional recreation development and disagreed to limit the number of visitors. Their perception of recreation impacts were varied with age, education level, economic dependency on recreation place of residence, and length of residence.

목 적 : 경골 골절에서 골수정 고정술후 발생하는 교합나사 파단의 빈도와 발생 원인을 밝히고자한다. 대상 및 방법 : 1995년 3월부터 2000년 10월까지 골수정으로 치료하였던 경골 간부골절 82예중 내고정물 파단이 발생하거나 골유합이 이루질 때까지 추시 가능하였던 75예를 대상으로 하였다. 내고정물 파단 빈도와 위치, 골절의 양상, 술후 골절부 신연 및 골유합 이상 여부를 조사하였다. 결 과 : 교합나사 파단은 7예였고 (9.3%), 근위부 두 번째 나사의 파단이 가장 흔하였다. 교합나사으 ㅣ파단은 AO분류 B형과 C형 골절, 술후 골절부가 신연된 경우, 개발성골절, 자연유합 및 불유합의 경우 발생하였다. 결 론 : 교합나사 파단의 주된 요인은 골절부의 골과 골 접촉이 불안정하기 때문이며, 개방성 골절, 지연유합 및 불유합도 교합나사 파단의 위험인자라고 생각한다. 교합나사 파단을 예방하기 위해서는 수술시 골절부 신연을 피해야하며 골절부 골접촉이 불안정한 경우 완전 체중부하를 골유합시까지 제한하여야할 것으로 생각한다. Purpose : The aims of this study were to investigate the prevalence and the causes of crew breakage in tibia nailing. Materials and Methods : Between 1995 and 2000, eighty-two tibial diaphyseal fracture were treated with interlocking nails. The loss of follow-up was 7 cases. We retrospectively reviewed seventy-five cases. We investigated the rate and location of metal failure and evaluated the fracture pattern, the presence of distraction after nailing and union abnormality. Results : Screw breakage was identified in seven cases (9.3%) and most frequently occurred on the second proximal locking screw. Screw breakage occurred in AO type B or C type fractures, fracture site distraction after nailing, open fracture, delayed union and nonunion. Conclusion : The main cause of screw breakage is unstable bone to bone contact on the fracture site caused by comminution of distraction. The open fracture, delayed union, and nonunion also contributing factors for screw breakage. For preventing screw breakage, it is necessary to avoid fracture site distraction and delay full weight bearing in cases having unstable fracture site contact.

목적: 경동맥의 내막-중간막 두께(carotid artery-intima-media thickness, cIMT)는 혈액 및 복막 투석을 받고 있는 만성 신부전 환자에서 심혈관 질환과 관련된 사망의 예측인자임은 이미 알려져 있으나 투석 전 만성신질환 환자에 대한 연구 결과는 부족하였다. 이에 만성신질환 환자들의 신기능 단계에 따른 경동맥의 내막-중간막 두께 변화를 확인하고자 본 연구를 계획하였다. 방법: 2005년 1월1일부터 2009년4월30일까지 경희대학교 부속병원에 내원한 사구체 여과율 60 ml/min/1.73m2 미만의 만성신질환 환자 중 고해상도 B모드 경동맥 초음파를 시행한 환자 88명 (남:여=49:39)을 대상으로 정상 신기능의 대조군 30명 (남:여=13:17)과 경동맥 내막-중간막 두께를 비교하였다. 결과: 경동맥의 평균 내막-중간막 두께는 사구체여과율의 감소에 따른 만성신질환의 단계가 증가될수록 통계적으로 유의하게 증가하였는데 그 두께는 만성신질환 단계가 3, 4, 5단계로 악화되면서 각각 0.82±0.19 mm, 0.93±0.13 mm, 그리고 1.04±0.27 mm로 증가하였다. 또한 정상 성인과 비교하여 모든 만성신질환 단계에서 경동맥 내막-중간막 두께가 통계적으로 유의하게 증가된 상태임을 확인하였다. (p=0.002). 당뇨성 신질환의 경우 비당뇨성 신질환에 비해 그 두께는 증가하였으나 통계적으로 유의하지 않았다. (0.96±0.32 vs. 0.92±0.20, p=0.142). 다중회귀분석에서는 나이와 사구체여과율, 만성신질환 단계가 경동맥 내막-중간막 두께와 유의한 상관성을 보였다. 결론: 만성신질환 환자에서 투석전 초기 신기능 저하 단계에서부터, 경동맥의 동맥경화는 진행되어 있었으며 경동맥의 내막-중간막 두께는 나이 및 사구체여과율 만성신질환 단계와 상관관계가 있었다. Purpose: Carotid artery intima-media thickness (cIMT) has been reported as the predictive factor of mortality of cardiovascular disease in dialysis patients but only a few reports are available on the patients with earlier stages. We compared cIMT according to the stage of chronic kidney disease, and analyzed the data in association with cardiovascular risk factors. Methods: Study subjects were 88 patients with chronic kidney disease less than 60 ml/min/1.73m2 of glomerular filtration rate. cIMT was measured by means of high- resolution B-mode ultrasonography. Cardiovascular risk factors and cIMT were analyzed and compared with 30 subjects with normal renal function. Results: cIMT was significantly increased with the stage of chronic kidney disease. When the stage was increased from 3 to 5, cIMT was increased (p=002). cIMT was further increased in all stages of chronic kidney disease than in patients with normal kidney function. But association of diabetic chronic kidney disease with non-diabetic chronic kidney disease was not significant (p=0.127). Multiple regression analysis showed that cIMT in patients with chronic kidney disease was significantly correlated to age, glomerular filtration rate, and the stage of chronic kidney disease. Conclusion: We suggest that carotid atherosclerosis could increase in no dialysis patients with early stage of chronic kidney disease. Carotid artery intima-media thickness was correlated with age, glomerular filtration rate, and the stage of chronic kidney disease.

Patients infected with human immunodeficiency virus (HIV) may develop various liver diseases, including viral hepatitis, granulomatous inflammation, malignancies, vascular disease, drug-induced hepatitis and, sometimes, diseases of unknown origin. In this case, a 35-year-old HIV-infected man presented with fatigue, myalgia, elevated liver enzymes, and multiple skin lesions for 3 months. Since the abdominal computed tomography (CT) and viral marker tests failed to explain the cause of his symptoms and signs, a liver biopsy was performed. This revealed a granuloma consisting of epithelioid cells without necrosis. His symptoms, skin lesions, and abnormal liver enzymes improved without changing his management, including the anti-retroviral agents. He was diagnosed with idiopathic granulomatous hepatitis based on the pathologic findings of the liver and clinical course. Aggressive diagnostic methods, such as a liver biopsy, should be considered for HIV-infected patients with liver disease. (Korean J Med 78:391-395, 2010)

Wegener`s granulomatosis is a systemic vasculitis of the medium and small arteries, as well as of the venules, arterioles, and occasionally large arteries, and primarily involves the upper and lower respiratory tracts and the kidneys. Renal symptoms of Wegener`s granulomatosis are indistinguishable from those of vasculitis such as Henoch-Schonlein purpura and microscopic polyangiitis. This case, though initially diagnosed as Henoch-Schonlein purpura, was confirmed as Wegener`s granulomatosis from a lung biopsy fifteen years after the initial diagnosis. <P> In this study, we have investigated application of piezoelectricity to actual commercially operating high-speed Korean train. We recorded and analyzed the vibrations of commercial Korea Train eXpress (KTX). We experimented with different cantilever beam thicknesses (0.25 mm, 0.6 mm, and 1.0 mm) and different piezoelectric material dimensions (length × width × thickness, 10.0 mm × 10.0 mm × 0.5 mm, 20.0 mm × 10.0 mm × 0.5 mm, and 30.0 mm × 10.0 mm × 0.5 mm) on real data from recorded random frequencies and train vibration amplitudes. The addition of tip masses on the cantilever beam decreased the resonance frequency range when the vibrations were constant but not when they were random. The optimal condition was experimentally found, to involve decreasing the piezoelectric substrate beam thickness and increasing the piezoelectric substrate beam area rather than merely increasing the tip mass. The most effective method to improve the operational sensitivity was combination of decreasing the resonance frequency by adding tip masses, decreasing beam thicknesses, and increasing beam areas. </P>
